Stamp Duty and Registration Revenue Falls Short of Target for FY 2024-25
The state's revenue from stamp duty and registration has fallen below expectations for the fiscal year 2024-25, raising concerns over meeting the annual target. As of December 31, revenue stood at ₹39,767 crore, or 72.3% of the ₹55,000 crore target, significantly lower compared to the previous two years.
Stamp duty and registration contribute to the state’s third-largest source of income after GST and sales tax. Despite strong performance in the past three years, the revenue growth has slowed this year. A total of 19.90 lakh documents, including sale and lease agreements, were registered, contributing to this year’s revenue.
In comparison, the revenue at the same time in 2023 was ₹35,167 crore, or 78.15% of the ₹45,000 crore target, while in 2022, it was ₹29,866 crore, 93.33% of the ₹32,000 crore target. The dip in collections has raised concerns over achieving the ₹55,000 crore goal for FY 2024-25.
Government officials acknowledge that the remaining months will need to see a significant increase in collection to meet the target. "Two months, August and October, saw collections of over ₹5,000 crore each, but the recent months have seen a dip. A ₹15,000 crore collection in the last three months may be difficult, especially with the rising fiscal deficit. The government may increase the target during the March budget, but it will make it more challenging to achieve," said an official.
However, the department remains optimistic about the coming months. "Revenue collections typically rise in March due to a surge in registrations ahead of a potential hike in ready reckoner rates. With no revision in rates for three years, a revision is expected to be implemented this year, leading to a significant increase in registrations. We are hopeful that this will help us meet and potentially exceed the target," said a revenue department official.

yes i'm talking about otw/ao3's finances again, sorry not sorry
the director of the nonprofit i work for in the u.s. just announced that we have $1 million usd in our reserves at the start of 2025, and that that is a big amount for us to have. we have 35+ full-time employees and several contractors, all of whom are paid good salaries, and we have numerous other operating expenses, including but not limited to running a website, advertising, employee travel, conference registrations, and paying legal costs (we're an advocacy org and often get involved in litigation).
and all i could think about was how the last time i checked the finances of the organization for transformative works (@transformativeworks), which runs ao3, they had almost three times that amount - $2.8 million usd - in their reserves, and zero paid employees, contractors or otherwise. Z E R O.
but that's just normal nonprofit math, right?
and to be clear when i say reserves, i mean money that is not allocated for any specific purpose in the yearly budget. this is just the extra. my org invests that extra so that we can generate additional revenue from it; the last time i checked, otw had only put $10,000 thousand usd of it in an interest-bearing account, which meant they were only earning about $150/year in interest on it. no, i didn't miss any zeroes there. only $150 interest on TWO POINT EIGHT MILLION DOLLARS
anyway i am not going to go check the more recent numbers because any time i try to put any effort into this kind of research, like @manogirl and i did in 2023 and i updated in early 2024, we get so much shit that it hardly feels worth it. but anyone is welcome to follow the process outlined in our previous posts to find the latest numbers yourself. and if you do please tag me! i'm happy to share
but bottom line: remember this when the next otw/ao3 fundraising drive comes around! they don't need your money, and they don't even know how to manage it properly when they get it
(oh, and for anyone who's been following along, no, i still have not received a reply from the otw finance team in response to the one-line question i asked them about their reserves in may 2023. 🙃)

Do you want to amend the Missouri Constitution to: State governmental entities estimate onetime costs of $660,000, ongoing annual costs of at least $5.2 million, and initial license fee revenue of $11.75 million. Because the proposal allows for deductions against sports gaming revenues, they estimate unknown tax revenue ranging from $0 to $28.9 million annually. Local governments estimate unknown revenue.
Fair Ballot Language:
A “yes” vote will amend the Missouri Constitution to permit licensed sports wagering regulated by the Missouri Gaming Commission and restrict sports betting to individuals physically located in the state and over the age of 21. The amendment includes a 10% wagering tax on revenues received to be appropriated for educational institutions in Missouri. Luthor-Danvers - A Reckless Endevour
Part 1: Sunday
Sunday evenings in the Luthor-Danvers household are pleasantly predictable, well at least as predictable as a home with a kryptonian, genius CEO, and half-kryptonian genius teenager can be. As usual Lena sits in her dimly lit office wearing yoga pants, an old MIT hoodie, and fuzzy slippers that her wife gifted her last christmas. The CEO was pouring over the quarterly L Corp. revenue reports in preparation for Monday’s jam-packed schedule of meetings--first research and development, then two separate board subcommittees, and finally ending with video calls with potential overseas investors.
Meanwhile, at the kitchen island, Kara furrows her brow in concentration as she scrolls through an email from a new source concerning an ongoing story about proposed alien protection legislation. The legislation goes before Congress in a few days and, if passed, would provide greater housing and employment protections for second generation aliens and hybrid-aliens born on earth. Even though Kara has a duty as a journalist to be neutral, she can’t help but be invested, after all her daughter is a second generation hybrid-alien. Kryptonians blend into human society easily, but there are still calls for alien registration and alien detection technology.
Kara enjoys her nights like this, being home with her wife and daughter. She has been off duty from the DEO for the past two weeks, ever since she was struck with a concentrated form of kryptonite that left her in a coma for three days. When Alex initially ordered her sister to take two weeks off to recuperate, Kara was constantly fidgeting, wanting to get back to helping her city. But now she’s gotten used to the domesticity of it all. She enjoys the time with her little family of three, leisurely sipping on her coffee in the morning and family dinners every night. But, that's all about to change, come Monday morning Kara is back on Supergirl duty.
Down the hall behind closed doors, El sits in her swivel chair at her desk with notes strewn about and multiple windows open on her dual screen computer. While normal highschoolers are rushing to finish their homework for Monday, El, who had already completed the credit requirements to graduate and her homework for the entire semester, kept herself busy with independent learning and experiments. Kara and Lena were aware of El’s genius and even encouraged El to engage in independent learning, but the couple insisted the teen go to a normal highschool and have time to be a normal kid, something the couple had been denied.
Tonight though, El was working on a secret project. Usually El freely shared her ideas with her moms, especially Lena, who saw a lot of herself in the curious teen, but not tonight. El started working on her secret, and potentially dangerous, project right after Kara woke up from her coma.
El still remembers that dreaded night. Lena woke El up at 3 am with a shaky voice and unshed tears in her eyes, telling the teen that Jeju was hurt and they had to go to the DEO now. Lena and El drove silently but frantically to the DEO and were immediately led to the med bay. Kara’s lifeless body lying prone on a hospital bed below sunlamps, the medical team rushing about to save the fallen hero. El was numb, she stayed silent as Aunt Alex explained what happened and that they would do everything possible. El had never seen mom look so lost, so shattered. El didn’t shed a tear, not in front of her mom anyway, she had to be strong for her family now. Her powers were far from fully developing yet, but El knew she needed to step up for her mom and National City if anything happened to Supergirl. El felt the weight of the world on her shoulders for three whole days, until finally Kara woke. Even now, the teen has trouble sleeping through the night without the images tormenting her mind.
El vowed to prevent anything like that from happening again. She had to protect her family, and had to protect her jeju. The only way El saw to protect her family was to fully develop her powers, train like crazy, and... eliminate kryptonite as a weakness. El was disheartened when her initial research showed that it was 99% impossible, without full body armor, to eliminate the effects of kryptonite on an adult Kryptonian. But, that didn’t mean that El couldn’t try to eliminate or at least lessen her own future kryptonite weakness, after all El was a hybrid whose powers had only just started presenting and she still had time considering she had only just turned 15. Of course, El had never been exposed to the substance but Alex guessed that once El’s powers were fully developed, kryptonite would take effect too.
As she continually researched, El developed and refined her theory for eliminating kryptonite weakness. Just like Rasputin and others throughout history had practiced mithridatism, El hypothesized that she could develop an immunity to kryptonite by self-administering non-lethal amounts to ultimately protect herself from kryptonite poisoning in the future. The “how” was El’s biggest obstacle. How was El going to get a supply of kryptonite and process it into liquid form that she could then inject into herself on a regular basis at increased volumes. El figured that kryptonite was similar to cyanide, in that even a small exposure could devastate her system - she would have to be extremely precise on how she determined the dose scale and the process would likely take months, if not years.
El was painfully aware that Kara would return to the DEO on Monday, and felt an urgency to find a way to start the immunization process soon. She became so desperate that she even hacked into Lena’s personal files. El had hacked Lena’s files before for purposes of a birthday surprise and hadn’t been detected, so it was relatively simple to do it again. Her mom had various reports and notes on kryptonite but nothing helpful for El’s purposes. Instead El delved deeper and accessed the research notes of the one person she knew who had the greatest expertise on kryptonite, her notorious Uncle Lex. El downloaded all of Lex’s notes onto a thumb drive and cleared any trace of herself from Lena’s system.
El had never met Lex. Sure, she knew he was a brilliantly insane scientist who hated aliens, but there had to be more to him, right? A few years ago, when El started taking an interest in experimental chemistry and biology she was naturally drawn to cutting edge studies, almost all of which had their origins with Luthor Corp. She became extremely curious about the Luthor side of her family much to Lena’s chagrin. El read everything she could on the Luthors’ work, and eventually asked her moms about it. Lena was slightly nervous by her daughter’s request but Kara insisted they be open with El about who her family is. Lena answered all of El’s questions, putting particular emphasis on the fact that El is her own person and not bound to the Luthor name as well as the importance of ethical experimentation. The old articles and interviews, and stories from her moms were helpful but never quite satisfied her curiosity about the Luthors. And now, with her project in full swing, El’s curiosity about Lex’s work peaked again.
For the past few nights El read and reread Lex’s notes, until she came across something that looked promising. The notes were from Lex’s early years at Luthor Corp, around the time he and Clark were friends. Lex was working on a cloning theory, which included sub-research on kryptonite effectiveness elimination. The theory was not based on use of armor or a suit, instead Lex’s notes were focused on measured exposures to kryptonite, similar to El’s theory. But, Lex’s notes were incomplete and left El with the same question she started with “how?”.
In order to test her theory, El would have to find kryptonite and figure out how to expose herself just enough to develop an immunity while avoiding irreversible damage, or worse, death. Lena had long since destroyed her supply of kryptonite as well as any supply L-Corp or its affiliates possessed. El could always try synthetic kryptonite, but red kryptonite had its own dangerous side effects and may not provide the immunity El needs. No, for the best possible chance, El needed pure kryptonite and a way to expose herself in measured increments. Once again the only person El could think of asking was her Uncle Lex, which is why, at almost 10:30pm on Sunday, El was figuring out how to get to Lex’s prison and ask the source himself for more information and, she hated to admit it, help.
Back in the kitchen, Kara decided to call it a night. The reporter closed her laptop and threw the empty bag of extra butter popcorn into the trash can, before stepping into Lena’s office to try and pull her workaholic wife away for some beauty rest. Kara stood in the CEO’s doorway watching as her intense and adorable wife worked away in total concentration.
“I can feel you staring.” Lena softly spoke, not even looking up from the papers in front of her.
“I can’t help myself.” Kara smirked and went around Lena’s desk to wrap her in a relaxed hug, resting her chin on Lena’s shoulder. “Time for bed babe.”
“Just a few more minutes, and I’ll be right in.” Lena replied.
Kara protested, “it's my last night free of Supergirl duties . . . thought you might want to take advantage while you still can.” Lena’s head perked up at the mention of their last uninterrupted night together. Spinning her chair to look at the blonde, Lena leaned upward and placed a soft kiss to Kara, just breezing over her lips teasingly before pulling away and getting up.
“Well I can’t possibly say no to that now can I.” Lena stood from her chair and wrapped her arms around her wife’s neck. Kara moved her hands to her wife’s waist, pulling her in close. She could stay in this moment forever, but she also couldn’t help but see from the corner of her eye the light on down the hall coming from El’s room.
“I hate to end this but I think El has developed your nighttime work habits.” Lena gazed down the hallway and sighed, she couldn’t deny it, El was certainly her daughter.
El heard two pairs of soft footsteps coming her way and panic immediately set in. She wasn’t used to keeping things from her parents and this was a particularly big thing. El quickly collected her notes, throwing them in a hidden desk drawer, closed the windows on her computer, and pulled the USB out of the export and threw it safely into her backpack. Just as a lite knock came at the door, El pulled out a book and opened it to a random page.
“Come in,” El let out a bit more breathless than intended. She tried to calm her heart rate which was still beating excessively fast. El knew her Jeju’s super hearing would pick up on her heart beat if she hadn’t already.
The door creaked open fully allowing both of her parents entrance. “What are you working on kiddo?” Kara looked at the open book on the desk as she placed both hands on El’s shoulders and bent to kiss El on the head. Lena leaned on the doorway, looking on with a soft loving expression.
El pushed up her glasses and replied, “nothing really, just some light reading before bed.” This time El’s body didn’t betray her and she was able to keep her heart at a normal rate. Given that she grew up with one mom who hid her identity from the world and another who grew up in the Luthor household, El shouldn’t be so surprised that she is somewhat of a natural at lying. She’ll have to be good given her plans for tomorrow and the coming weeks.
Kara looked down at the sheer girth of the book and in a mock suspicious voice said “Right, light reading about . . .” Kara peered down at the book as Lena walked over to join the pair “magnetohydrodynamics.”
“You know if you let me take some college courses I wouldn’t be so bored out of my mind that I have to read about MHD.” El gave a slight eyebrow raise, reminiscent of Lena’s signature facial expression.
Lena, leaning her hip against the desk, nodded approvingly, a slight smirk on her face, “Well, it's never too early to develop a well rounded understanding of physics. But being a kid is just as important.”
“I know, I know. Blah blah blah” El grumbled. “Make friends, go to football games, drink beer and be a normal teen.” El rolled her pale green eyes.
“We want you to do all of that, minus the beer,” Kara stated. “But seriously, are you at least trying to be social El?” Kara and Lena have had this conversation with El before. On several occasions El’s school had notified them that, while El excelled in academics, she didn’t engage with her classmates. It was only last month that Lena and Kara started considering putting El in therapy, thinking El’s lack of social engagement was due to social anxiety or bullying. Now, the couple, who had never taken therapy off the table, looked at their daughter with concern.
This was delving into a conversation El did not want to have tonight, or ever really. Every time her parents brought up the importance of El having a normal life, El had to hold herself back from screaming the obvious--she’s a Luthor and a half Kryptonian, nothing about her life will ever be normal. Instead El went for deflection. “Ever considered that I’m just too far advanced for such shenanigans?” El jokingly inquired.
Kara knew this method of deflection all too well from years spent with Lena. Something was clearly going on whether or not El wanted to discuss it. “Sweety, is it your powers? Anything new coming in?” Kara asked gently, sitting on El’s bed facing the teen. Kara couldn’t help but think back to a month earlier when she found the teen pale and in pain, curled up in the fetal position after an 'accidental' kryptonite exposure in the DEO’s classified weapons locker, which El had somehow managed to just 'wander' into. The scene broke Kara’s heart, and she wanted nothing more than to help El though this.
El didn’t answer. This was yet another conversation she didn’t want to have. “I know it can be overwhelming and confusing when they start to develop and a lot is changing.” Kara remembered the panic she experienced in her teenage years from over stimulation and fear of accidentally hurting people. But, El just looked down at her hands, still silent.
“And you know that no matter what, we’re here to help.” Lena said, rubbing El’s shoulder. “Is the watch working ok?” Lena had worked nonstop the day after the incident in the weapons locker to develop the watch to prevent the teen from becoming too overwhelmed. Still no answer from El. The teen hated being reminded that she was going through what she referred to as “powers puberty.” Sure, the idea of powers was cool, and she already started showing signs of super speed and strength, but constantly being watched by her parents, Alex, Eliza, J’onn, and everyone else like she’s a ticking time bomb was a bit much sometimes.
“Yeah, it's fine.” El raised her wrist to show the small blue watch which Lena designed to dim El’s powers and make them easier for El to control when the time came. El hated the idea of being out of control but equally hated having to rely on technology to avoid being dangerous to others.
Her parents were patiently looking down at her, clearly not letting up. So, El used yet another method of avoidance she referred to as the “tired child” and gave a big yawn with droopy eyes.
Just as El hoped, Lena’s protective mom instincts gave into her tired child. “Alright sweetheart, I think it's time for you to get to bed.” Lena said as she brushed a stray piece of strawberry blond hair behind El’s ear. “We love you so much and you can always talk to us about anything.” Lena pressed a soft kiss to El’s forehead. “Good night my genius girl.”
“Night” El said in a drowsy voice, giving another small but genuine yawn this time.
“Love you Ellie. Sleep tight.” Kara said as she closed the door behind her and Lena.
Even though El was tired, she tossed and turned in her bed, debating tomorrow. Should she take the dive and talk to Lex? She’d have to keep her head level and not let him get to her. On top of that, should she really risk ruining her relationship with her parents? Sure El had kept secrets before, but only minor secrets, nothing as large as seeing her murderous uncle who certainly hated her half-kryptonian guts. El didn’t know about what tomorrow would bring as she drifted into yet another night of restless sleep.

yeah these are different paradigms! but you made me thinky thoughts so i'm expanding on this
corporations are already primed to think about negative space as space they could have claimed but have not, eg "we lost $15 million to piracy!" when there's no guarantee any of those eyeballs would have subscribed to netflix if not for torrenting, but presumptively they think you should be giving them your money. So if you tell a company "I'm not giving you my money because I disagree with your political donations to X" for example, that makes sense to them, their reputation and public perception of their viewpoints is affecting their bottom line
because for a corporation, bottom line--profit--is the biggest consideration. Market share is important but fundamentally if you don't generate more revenue than costs, you (eventually) have to go under. Individual sales do matter.

like i understand the temptation to view your vote as currency, but it's not. You can't save it up for later. You can't exchange it for goods or services. And most crucially, a candidate doesn't need an amount of votes. They need a proportion of votes.
They don't want more votes like a corporation wants more money; for the corporation, more money is the final goal. For a candidate, more votes only matters in the context of more than the other guy. The competition is more important than the scale. If only 10 people vote but 6 of them vote for you, who cares about voter apathy, you won! A candidate is not threatened when you withhold your vote, because that doesn't change the proportion of votes they receive. A candidate is only threatened when you vote for the other candidate in an election, because that changes the proportion of votes.
the fact that different parties practice voter suppression or "rock the vote" stuff muddies people's understanding of this, I think. the thing you have to understand is that demographically, the bloc of voters affected most by these efforts will tend to vote Democrat. So when Republicans try to make it more difficult for a predominantly black district to vote, they're trying to shift the proportion of votes. There may have been Republican votes in there suppressed too, but they're outnumbered by the Democratic votes prevented. And when a liberal org runs a voter registration drive to help more people vote, they don't ask who you're going to vote for. They understand they're registering some Republicans voters too, but statistically they're registering more Democrats, so the effort is overall shifting the proportion of available votes. These efforts seem like they're about the amount of votes but it's never about the amount, only the proportion.

Peter Thiel also has Vice President JD Vance as a protege. Thiel is a US government contractor, conservative and was famous for bankrupting Gawker media.
two top security officials at USAID were put on administrative leave for trying to stop Musk's team from access. USAID is the humanitarian arm of the us government formed in 1961 by President John F Kennedy giving billions of dollars to fight poverty, disease, natural disasters. It also promotes democracy independent media and social initiatives.
Photos lining the halls of USAID showing humanitarian aid work over the decades have suddenly been removed, Trump and Musk both state they will destroy the agency, Musk has also said that he wants to sell off public buildings once the federal employees have been removed.
Republicans in Congress are pushing the SAVE voter suppression act which requires citizens to show birth certificates or passports to vote, disenfranchsing millions.
Estimates say that Kamala Harris would have received at least 3 million more votes if voter suppression bills instigated by Republicans had not been implemented.
55 Employees at the Education Department were placed on paid administrative leave - according to the federal employees union, these employees include civil rights attorneys, persons who work on grants and an employee assisting AI development for education.
An official noted that some of those put on leave had attended a two-day diversity training seminar in the last two years of the first Trump administration. The goal was for 400 employees to attend and now it's being used as a pretext to remove people.
Trump won the vote by less than 1.5% yet people seem to believe there is now a mandate to dismantle the federal government.
These bills included, closing drop boxes and cutting the number of polling places and voting machines in black majority districts, allowing random people to challenge hundreds of thousands of votes by people they didn't know. Halting registration to vote in various places, blocking same day registration, active military serving overseas were not allowed to vote due to imposed deadlines, vote by mail was blocked, etc.
For the first time since the civil rights movement there were more bills to disenfranchise voters than there were to expand voting
Trump has told the US to 'prepare for pain' due to the tarrifs he is imposing on Canada, Mexico and China.
A goal of the tarrifs is to raise revenue so that Trump and Republicans can extend 2 trillion in taxes to the 1% imposed during Trump's first term.
Trump is threatening Panama and Greenland, but these might be distractions as Trump dismantles the federal government using illegal methods.

Philadelphia City Commissioners' Office Tells O'Keefe Non-Citizens Can Vote as Long as They Are Philadelphia Residents
In a recent conversation with an undercover O’Keefe Media Group journalist, Milton Jamerson, Election And Voter Registration Clerk from the Philadelphia City Commissioners Office, stated that non-citizens can vote in local elections as long as they are residents of Philadelphia.
This claim is echoed by the nongovernmental organization (NGO) “Ceiba,” (@CeibaPhiladelp1) located just across the street, which asserts that individuals with an Individual Taxpayer Identification Number (ITIN) are eligible to vote.
An ITIN is a 9-digit number issued by the Internal Revenue Service (IRS) to individuals who are required to file a tax return but do not qualify for a Social Security number. While the ITIN serves as a means for tax reporting for those who are not U.S. citizens, possessing an ITIN does not equate to legal voting rights, raising questions about Ceiba's claim that ITIN's deem individuals eligible for participation in the electoral process.

The cognitive dissonance of listening to someone bitch about the lack of services in Ontario in one breath and spend the next one lauding our current premier.
I truly cannot understand how you do not see that this man has stripped our province for parts in order to pay off his friends and cronies. And he will continue doing it as long as ppl keep voting for him because he promises beer for a buck (that never happened btw) and canceled our license plate registration fees (this costs the province millions of dollars in lost revenue btw, all of which could have been used to help fund necessary repairs to schools, hire nurses, and the social welfare that many ppl in this province rely on).
That you have been on a waiting list for three years for a funded round of IVF is a direct result of the fact that Ford has completely obliterated Healthcare spending in this province in favour of building a highway to nowhere (so he can make sure his developer buddies get lotsa prime greenbelt real estate) and spending billions of dollars to demolish the lcbo ahead of schedule (the beer store is the actual shitty monopoly), among other things.
"Do better, Ontario!" you say, as if you and your votes for Doug Ford are not why we are in this fucking mess in the first place.
I cannot stand the voting public in this province. Why did none of you pay attention for one (1) hour of civics classes?

J'ai pensé que ce serait dur les premiers jours. J'ai été naïve : ça a été très déstabilisant. Au lendemain, j'étais comme hors de mon corps, cela m'a un peu rappelé la façon dont Tante Rose endort ses patients (elle est chirurgien et elle adore parler de son métier). J'étais hors de moi, mais consciente, comme si j'étais une autre personne, et que ce problème n'était pas vraiment le mien. Comme si c'était arrivé à quelqu'un d'autre et que j'étais désolée pour lui. Cet état étrange a duré un bon moment. Oncle Adelphe a été très inquiet, mais il m'a dit que c'était le choc, que certains gars revenus de la guerre avaient un peu le même regard, et que ça finissait toujours par passer. Puis, il s'est passé une chose vraiment étrange.
Un jour où nous étions lui et moi dans le bureau à faire la comptabilité, quelques jours avant le mariage d'Anne, j'ai réalise quelque chose de grave. Et je l'ai réalisé à voix haute. Once Adelphe en a fait tomber les registres de l'étagère sous le saisissement. Puis je suis allée voir ma Grand-Mère, car il n'y avait qu'elle qui pouvait régler ce nouveau problème.

Eugénie « Cela peut-il attendre, jeune fille ? Le livre de ta sœur est passionnant et j’aimerais au moins conclure ce chapitre. »

Eugénie « Et bien voyons… Quelle malédiction pèse donc sur ma descendance pour que nous soyons tous des sots à même de se marier avec le premier laborieux venu ou la première souillon dont on croise le regard ? Je note cependant que tu utilises le passé. Tu ne le vois plus ? »
Arsinoé « Non Grand-Mère. Je ne veux plus le voir, et Oncle Adelphe y veille. »
Eugénie « Dieu merci ! Ce garçon est de loin le membre de cette famille qui a toujours le mieux veillé à nos intérêts. Et qui a fait le plus raisonnable des mariages ! »
Arsinoé « Il a épouse sa cousine ! »
Eugénie « Oui, c’est bien ce que je dis. »

How it works on the inside - billing inside hospitals and clinics
Years in this field. There are not enough words to explain my anger at the system. The primary point is that it is broken, everyone on the "inside" has known for ages, COVID was the death knell.
I could make post after post on why it is broken but I think it is easier to explain how it is intended to work for the first one. Because I will be real with you people on the inside that WORK ON THESE TEAMS do not even fully get it.
This example is for a hospital (think ortho, cancer centers, surgery, etc) - clinics, like your primary care clinic use a very simplified version of this.
The first people you usually speak to are called "Patient Registrars" the team is usually called PAS (Patient Access Services). These are the front line staff that get you scheduled, take your insurance, get your authorizations. It is the doctor that SENDS the referral who is responsible for getting your initial authorization. Usually this is for a consult. The authorizations after are done by the folks above.
Side bar - what is an authorization? it is a request by your doctor for something you need that the payor/insurance company agrees that they will pay for. These can be for a one time service like a MRI, or something that is based upon time/dosage - like chemo. The latter must have re-auths done by the team mentioned above.
Lets say this all goes according to plan, which it does for the most part. What happens after you get your first visit?
The doctor goes into their medical record (sometimes called an ELECTRONIC MEDICAL/HEALTH RECORD -EMR/EHR; these used to not be electronic and be on paper, some facilities are still on paper. The medical field is the last bastion of faxing in this nation) and documents what was found/discussed - their diagnosis, prescriptions, next steps like treatment or routine meetings etc.
That information (regardless of format) flows to a team called the Medical Records Team. They do a QA, make sure the registration folks didn't miss any forms, the nursing staff got all the stuff they need and then ship it over to coding/charging.
Now this next step does involve traditionally 2 teams. It can deviate by facility, some are merged, some are not. Lets talk about a bigggg ass facility with all the proper teams and staff.
Charging - this is a sub team of either 3 different departments: Health Information Management (HIM), Patient Financial Services (PFS), or Revenue Integrity (RI). Regardless these folks sit down, review what the doctor wrote and generate charge codes based upon it. There is a dictionary (several truly) in every hospital with every price for every service. These prices are set based upon a few factors but I will delve into that in another post called Charge Master or CDM.
From there it goes to coding. Coding reviews these records and documents in extreme depth. They assign codes (which are dictated by CMS - the gov office that oversees Medicare) that is digestible to insurance companies. If anything looks off they send it back to the correct team to fix - most of the time their communication is directly with the doctors themselves. These communications are part of your EHR and available to you if you request them.
Once coding is done then it goes to billing/PFS. PFS then does another QA (sometimes this is done via the tools not a human) and a claiml is generated and sent to a clearing house.
A clearing house is not a bank - it is like a transit center for facilities and insurance companies and their banks (Change Healthcare is owned by UHC and is the biggest clearinghouse in the nation). So a claim goes to the clearing house, it is scrubbed AGAIN for errors, and then sent to the respective insurance company. The insurance company then goes through a fun little circus which is again another post. Eventually they tell the facility (via the clearing house) the claim is either denied, partially denied or fully approved and sends the $ to the clearing house. Sometimes that money goes directly to the bank of the facility ymmv. Regardless the PFS team goes and pulls this data from the clearing house and updates the patients record accordingly.
They will then begin the denial process of fighting the insurance companies. There are hundreds of denial types, but generally they are called technical denials or medical necessity denials. Another post.
This usually takes months of arguing, sending information, resubmitting, rinse and repeat. At the very very end of it the patient (only if the insurance agrees) gets a bill. From THERE the patients will then reconnect with the hospital billing team and maybe the financial assistance team.
This lovely system is called the REVENUE CYCLE. It is sometimes a division, sometimes its left as standalone departments, each functioning on their own. Sometimes the facility is small enough there is only 1 person responsible for a function instead of teams.
