The only time a SUSPECTED Izumi was mentioned in the MANGA, was OBITO speaking of a LOVER, no gender or name. "Koibito" can be any gender. And that was before Izumi was created as a character.
Izumi was in the novels, and whether or not they're canon is debatable, but other than the courtesy dream when he *checks notes* readily murdered her, Itachi never did anything romantic with her. All he ever did was make her cry. If you call that boyfriend material, I feel sorry for you😂 He explicitly said at the coffee shop that she's just a friend, making her cry AGAIN 😂 Yeah he liked her, but she was canonically less important than Shisui and their relationship was not Itachi's priority. He got his MS losing Shisui, not murdering Izumi, just saying. You get the MS from losing your closest person, so that clearly wasn't her.
But both the novels and Izumi's anime debut came after Obito mentioning a lover. Lover, not girlfriend.
And it's also a canon fact that Itachi interacted more with, and spoke more of, Shisui. He never once mentioned Izumi in the manga or anime. Why is that, if she canonically meant so much to him? Shit they even made him sad about KISAME, but Ishikawa never got a line with Izumi's name in his entire script😂😂😂🤌🏼 She was insignificant 🙃
