Flint, Michigan, has one of the [United States]'s highest rates of child poverty — something that got a lot of attention during the city's lead water crisis a decade ago. And a pediatrician who helped expose that lead problem has now launched a first-of-its-kind move to tackle poverty: giving every new mother $7,500 in cash aid over a year.
A baby's first year is crucial for development. It's also a time of peak poverty.
Flint's new cash transfer program, Rx Kids, starts during pregnancy. The first payment is $1,500 to encourage prenatal care. After delivery, mothers will get $500 a month over the baby's first year.
"What happens in that first year of life can really portend your entire life course trajectory. Your brain literally doubles in size in the first 12 months," says Hanna-Attisha, who's also a public health professor at Michigan State University.
A baby's birth is also a peak time for poverty. Being pregnant can force women to cut back hours or even lose a job. Then comes the double whammy cost of child care.
Research has found that stress from childhood poverty can harm a person's physical and mental health, brain development and performance in school. Infants and toddlers are more likely than older children to be put into foster care, for reasons that advocates say conflate neglect with poverty.
In Flint, where the child poverty rate is more than 50%, Hanna-Attisha says new moms are in a bind. "We just had a baby miss their 4-day-old appointment because mom had to go back to work at four days," she says...
Benefits of Cash Aid
Studies have found such payments reduce financial hardship and food insecurity and improve mental and physical health for both mothers and children.
The U.S. got a short-lived taste of that in 2021. Congress temporarily expanded the child tax credit, boosting payments and also sending them to the poorest families who had been excluded because they didn't make enough to qualify for the credit. Research found that families mostly spent the money on basic needs. The bigger tax credit improved families' finances and briefly cut the country's child poverty rate nearly in half.
"We saw food hardship dropped to the lowest level ever," Shaefer says. "And we saw credit scores actually go to the highest that they'd ever been in at the end of 2021."
Critics worried that the expanded credit would lead people to work less, but there was little evidence of that. Some said they used the extra money for child care so they could go to work.
As cash assistance in Flint ramps up, Shaefer will be tracking not just its impact on financial well-being, but how it affects the roughly 1,200 babies born in the city each year.
"We're going to see if expectant moms route into prenatal care earlier," he says. "Are they able to go more? And then we'll be able to look at birth outcomes," including birth weight and neonatal intensive care unit (NICU) admissions.
Since the pandemic, dozens of cash aid pilots have popped up across the nation. But unlike them, Rx Kids is not limited to lower-income households. It's universal, which means every new mom will get the same amount of money. "You pit people against each other when you draw that line in the sand and say, 'You don't need this, and you do,' " Shaefer says. It can also stigmatize families who get the aid, he says, as happened with traditional welfare...
So far, there's more than $43 million to keep the program going for three years. Funders include foundations, health insurance companies and the state of Michigan, which allocated a small part of its federal cash aid, known as Temporary Assistance for Needy Families.
Money can buy more time for bonding with a baby
Alana Turner can't believe her luck with Flint's new cash benefits. "I was just shocked because of the timing of it all," she says.
Turner is due soon with her second child, a girl. She lives with her aunt and her 4-year-old son, Ace. After he was born, her car broke down and she was seriously cash-strapped, negotiating over bill payments. This time, she hopes she won't have to choose between basic needs.
"Like, I shouldn't have to think about choosing between are the lights going to be on or am I going to make sure the car brakes are good," she says...
But since she'll be getting an unexpected $7,500 over the next year, Turner has a new goal. With her first child, she was back on the job in less than six weeks. Now, she hopes she'll be able to slow down and spend more time with her daughter.
"I don't want to sacrifice the time with my newborn like I had to for my son, if I don't have to," she says."

A trans-identified male is set to compete in a national women’s water polo championship this weekend, shortly after becoming a women’s chess champion in Michigan. Alicia, formerly Johnny, Paans, is a star member of the University of Michigan’s Women’s Water Polo club team.
Paans, 31, is expected to appear at the 2024 National Collegiate Water Polo Championships (CWPA), which will be held from May 3 to 5 at Texas A&M University. The competition includes 16 US-based teams vying for the women’s national title in their sport. Paans’ first game will be against Texas State University on May 3 at 9:00 GMT. 
Paans is expected to compete on the University of Michigan’s Women’s Club Water Polo team. The club’s official Instagram account recently named Paans as “player of the week,” boasting that he is also a state chess champion in the women’s category, having won second place at the Michigan Chess Association women’s open in 2022.

Paans also competed on the University of Michigan team in 2023, during which time they won the national women’s title against the University of California-Santa Barbara. In the same competition, Paans was noted to have scored goals in a 17 to 5 defeat against the University of Virginia, and in a 17 to 6 defeat against the University of Washington.
Outside of the sporting world, Paans has a history of involving himself in women’s issues, including volunteering as a counselor at a “hotline specialized in topics relating to female health and pregnancy.”
While it is unclear when Paans began to identify as a “woman,” a 2013 Facebook post reveals that friends still referred to him by his birth name while congratulating him for beginning his secondary education in his home nation of the Netherlands. Paans appears to have immigrated from Europe to California in 2017 when he began an internship for Volo San Diego — a social sports club — and has remained in the country for education ever since. In 2023, he began a social work degree at the University of Michigan.
According to the Independent Council on Women’s Sports (ICONS), the parents of Paans’ opponents have confirmed that he uses the female locker rooms and facilities when he competes.
Speaking to Reduxx, Jennifer, the mother of a female NCAA water polo athlete who participated in the 2023 national championship with Paans, explained it had been “obvious” that Paans was male from the moment she had seen him. Jennifer’s full name is being withheld for the protection of her daughter.
“My husband and I saw him first, standing aside the pool before [University of Michigan’s] first game. We looked at each other in surprise, and immediately knew it was a man,” she says, adding: “I observed Alicia quite a bit over the tournament, partly out of concern for the women, and also wondering how his teammates acted around him.”
Jennifer says she had been distressed by the thought that her daughter might have been forced to participate against a male, and was “relieved” when the match-up did not come to fruition.
“None of these women were initially aware a man had been allowed to play,” Jennifer noted. “Many of the player’s on my daughter’s team were almost in shock at the situation, and spoke to one another about discomfort with it. But to my knowledge, no one spoke to the organizers of the tournament. They felt that nothing would be done given that it was allowed based on [CWPA] policy.”
In January of 2024, the CWPA released a transgender athlete policy which explicitly allowed males to compete in women’s categories. The policy reads: “Transgender athletes who identify as female are eligible to compete in both the Co-ed League and the Women’s League.”
Trans identified males are also required to provide a “letter of confirmation” regarding their gender identity, either from the athlete or, if underage, a parent or guardian; proof of testosterone levels below 10 nmol/L within 60 days of competition; and a physician’s note to prove that the athlete is starting “physical gender transitioning/testosterone suppression.” The association purports to ensure “fairness, respect, and equal opportunities for all.”
The policy has been slammed by ICONS as a “pathway for men” to enter women’s water polo.

In an official statement, ICONS condemned the CWPA, highlighting the safety risks associated with allowing males to self-identify into contact sports.
“The Collegiate Water Polo Association has made it clear in their policy that they do not prioritize safe or fair competition for women in water polo,” said ICONS co-founder Marshi Smith. “Their discriminatory policy announcement came after they had crowned the University of Michigan team with a national title, despite utilizing a male player – a secret weapon not accessible to other women’s teams in the tournament.”
Smith continues that despite water polo being a high-contact sport, female athletes were thrown into the water with no disclosure of the added risks of competing against a male.
“This year’s national championships are shaping up to be a tragic deja vu of domination for the female opponents who simply seek a fair chance to succeed.”

How physical is water polo?
A: Very. As said before, water polo is a full-contact sport. This means that players are nearly always in contact with one another. In order to defend an offensive player, the defender keeps at least one hand on their player at all times. Players jostle each other the water in order to get control of the ball. Pushes and kicks, while not strictly legal in all situations, are common.
However, the physicality of the game is controlled by the referees. Kicking off of other players as well as holding or pushing them under water is against the rules.

A professor has been slammed over a scientific journal in which she said trans man should be allowed to take testosterone while pregnant, despite warnings it could trigger severe health issues.
Dr. Carla Pfeffer, who is not a medical doctor, penned a new paper alongside five other academics titled 'Medical uncertainty and reproduction of the 'normal': Decision-making around testosterone therapy in transgender pregnancy.
Pfeffer, who is a sociology professor at Michigan State University, argues that pregnancy care is too focused on helping women have healthy babies, and that trans men might be fine to take testosterone while pregnant.
The authors , who are sociologists, have since been accused of choosing to disregard the safety of a developing fetus 'in the name of trans inclusion'.
In a scathing response to the paper, two nurses have criticized the article saying if abiding by their take 'would land us in a vacuum devoid of medical ethics'.

Authors Jennifer Lahl and Kallie Fell, who is the Executive Director at The Center for Bioethics and Culture, published a response to the paper titled: 'Is There a Doctor in the House?'
In it, they said: 'The authors argue that 'gendered' pregnancy care is too focused on helping women have healthy babies, and that it might be okay for transmen to continue taking testosterone during pregnancy despite the known health risks to the fetus and effects on its normal development. 
'This is, quite frankly, insane.'
In the paper, Pfeffer and her fellow colleagues had studied 70 international trans individuals and 22 health care providers who focus on trans people. 
Their paper says: 'We argue that in the context of lacking and uncertain medical evidence (HRT with testosterone during pregnancy and chest feeding) in a highly gendered treatment context (pregnancy and lactation care), both patients and providers tend to pursue precautionary, offspring-focused treatment approaches.'
In response, Lahl and Fell say the findings are flawed as only biologically fertile human females possess the attributes for pregnancy and childbirth. 
The two label this 'a simple biological reality', saying that pregnancy care isn't 'gendered' at all. 
According to the Mayo Clinic, testosterone may cause birth defects if a pregnant woman comes into contact with it. 
Lahl and Fell also added: 'The concerns raised by Pfeffer and colleagues focus on the modern treatment approach physicians take, which they deem excessively 'precautionary' and 'offspring-focused.'

'If a woman chooses to continue a pregnancy, doesn't the developing fetus also have a right to the four principles of medical ethics?'
'In such cases, the physician is duty-bound to care for both the child and the mother.'
One health care provider also told researchers: 'I think if you choose to have a pregnancy and your female hormone levels would be already so high that testosterone probably wouldn't even mentally help.
'If you're producing breast milk and you couldn't be without testosterone for mental health, if you couldn't deal without testosterone, then you probably shouldn't be pregnant.'
In concluding their piece, Lahl and Fell said: 'The authors' suggestion that medical providers should deviate from the principle of 'do no harm' to follow paths where the evidence indicates harm is quite shocking.
