Empowering Diabetes Management: The Role of DSMES in Improving Health Outcomes
Diabetes Self-Management Education (DSMES) helps people with diabetes improve their blood sugar levels and prevent or delay complications. DSMES programs are typically covered by insurance and can be provided in person or online.
Physicians can help increase access to DSMES by encouraging patients to use the Medicare DSMT benefit, and connecting them with ADA-recognized programs and resources.
Topics
Many individuals with diabetes can have a dramatic impact on their disease and prevent long term complications by taking an active role in their care. This involves a complex set of behaviors, including meal planning and eating healthier foods; engaging in regular physical activity; adherence to medication, monitoring blood glucose and foot care.
Studies have shown that these seven essential self-care activities are highly correlated with lowering glycosylated hemoglobin levels. Healthcare providers should assess perceived patient barriers to these behaviors and make realistic recommendations. It is also important to evaluate how much responsibility a patient is willing to assume for their self-care management. This will determine how successful a treatment regimen is. A good way to measure this is by evaluating patient adherence.
Assessment
A person-centered approach to DSMES requires an initial assessment of the person with diabetes to determine their individual needs and interests. It should include a comprehensive review of the person’s life situation, including food access and affordability, comorbid chronic conditions, housing and transportation, health beliefs and attitudes, family support and social networks.
Credentialed DSMES programs as well as individual diabetes care and education specialists conduct such assessments. They also assess the person’s health literacy and numeracy, coping, problem-solving, decision-making, and interpersonal skills to identify and address barriers to effective self-management.
Health care professionals can encourage and promote DSMES by discussing its benefits and value to all persons with diabetes. They can advocate for DSMES as part of quality diabetes care, and they can make referrals to and facilitate participation in DSMES at the four critical times: at diagnosis, annually and/or when not meeting treatment targets, when complicating factors develop, and when significant transitions in life and/or health care occur.
Curriculum
A comprehensive DSMES program has a curriculum, a plan for providing education and evaluation. The curriculum should include topics related to diabetes self-management, including how to collaborate with the healthcare team, make informed decisions, solve problems, develop personal goals and action plans, and coping with emotions and life stresses.
DSMES programs should include discussion of the natural history of diabetes and that many people with diabetes struggle to reach their health targets (7,31).
Individuals need access to a variety of culturally appropriate services that address a range of barriers to participation in DSME such as transportation, literacy, numeracy, scheduling challenges, language, culture, physical disabilities, and socioeconomic status (5,56). DSMES also requires the use of person-centered approaches that are responsive to the unique needs and goals of PWD.
Individualization
Individualization refers to a person-centered approach to care that considers each person’s values and preferences. It involves identifying and addressing barriers to participation, including logistical factors such as cost, transportation, and scheduling issues. It also includes a person-centered approach that allows the diabetes educator to customize the curriculum and/or delivery of education to meet a person’s needs.
A reevaluation of diabetes self-management needs should be done at critical transition times, such as the initial diagnosis of diabetes, hospitalization, moving to an assisted living facility or nursing home, changes in health insurance coverage, and life events (e.g., marriage, death of a loved one).
Diabetes self-management education is available through many community programs, such as the Association of Diabetes Care and Education Specialists and the American Diabetes Association. It is also covered by Medicare and other insurance plans.
Support
As a person with diabetes (PWD) makes decisions and engages in self-management, they are challenged by various barriers and emotions. DSMES has been shown to improve glycemic and cardiometabolic outcomes by supporting PWD in making effective decisions, solving problems, and developing goals and strategies to cope with emotions and life stresses, thus improving quality of life (3).
Credentialed DSMES programs and individual diabetes care and education specialists use a person-centered approach beginning at diagnosis to assess the needs of each participant including social determinants of health such as food access, financial means, and health literacy. These assessments identify what needs to be addressed, including when to refer for DSMES. The National Standards recommend four critical times to refer for DSMES: (1) new diagnosis of diabetes, (2) changes in the treatment plan, (3) major life change, and (4) new barriers identified during an assessment for DSME (3,4).
