My Upcoming Research Study: The Effects of Yogic Breathing on Chronic Sinus Symptoms
This week I have been putting together a presentation that I will be giving about my new research project. It's a study of a yogic breathing technique called Bhramari Pranayama as an adjunctive treatment for people with chronic nose and sinus issues.
Essentially the premise is this:
Our sinuses produce nitric oxide, which then in turn effects our nasal airway and our lungs. The nitric oxide can improve nasal airflow, up-regulate mucociliary clearance, and enhance anti-viral immune activity. Additionally, humming actually temporarily increases the amount of nasal nitric oxide released by about 15 fold. Therefore my study is intended to examine the effects of this pranayama technique that utilizes humming on patients with chronic nose and sinus symptoms.
Typically for patients with these symptoms, we start them out on a steroid nasal spray (flonase) as well as sinus irrigations (something like a neti pot).
I always hear from people outside the medical field about how no one studies these things--non-pharmacologic interventions, the beneficial effects of non-proprietary supplements, or other alternative medical options. People often think that if big pharma (or someone expecting to profit) isn't paying for a study, it cannot happen.
This really isn't true likely 90-99% of the time. The problem isn't funding. I'm conducting a prospective randomized control study with human subjects in order to evaluate the benefits of yogic breathing for patients--something that if found to be helpful will bring in no additional monetary profit for anyone. How much will my study cost? $0.
But do you know what it did require?
Two things: interest and opportunity.
Firstly I, a resident physician, had an idea. I learned about nasal nitric oxide and thought it was cool. I read about how humming has a bolusing effect by transiently increasing nitric oxide output by 15x. And then I recalled that there are pranayama techniques that utilize humming.
With my interest piqued, I spoke with one of my bosses, an attending physician at an academic medical center. He's the head of our Rhinology and Skull Base Neurosurgery division and he is cool as hell. He's all about healthy lifestyle and benefiting patients as much as possible. He loved the idea immediately.
And lastly we roped in a medical student. Med students are very helpful with doing the grunt work of collecting the data into spreadsheets, running the statistical analyses and such. Sometimes they bring some excellent ideas of their own as well. In return for their work, med students are often given a significant portion credit upon publication of the study and this allows them the opportunity to add some scholarly publications to their CV. I don't really need more publications under my name, but they do.
My point with sharing all of this is that people often claim there are health benefits to doing or imbibing certain things but that they'll never be studied because there's no money to be made. And it may be true that private companies such as those in the pharmaceutical industry may not have such interest; their existence in a capitalistic economy relies on profitability. But this is part of why academic institutions are so important--because learning and discovery is part of the essential mission there. Profit doesn't dictate their avenues of research.
When it comes to the study and validation of alternative/complimentary medicine, the focus really needs to be on raising awareness and interest. Talk to your doctors, nurses, physician assistants, etc. The good ones listen. The younger they are, the more likely they are to be open-minded about it too (the older ones are hit or miss--some are so cool and some are very old school).

Skin - emergency (graphic)
My hands were pretty dry all of today. At night after washing them I got pretty itchy and almost developed a flareup. Thankfully I was able to calm it down and relieve it by washing my hands under hot water (bad habit but it was an emergency). I applied Cerave healing ointment on my problem areas (hands, wrist, neck, mouth, and ears) and went to bed at around 11:30.
Then I woke up at around 12:30 with some itchiness on my mouth and ears, mostly my mouth as I felt some oozing on the corners.
I applied zinc oxide cream to relieve the discomfort, but I felt like I needed something stronger. I did end up itching my ears and they started to bleed and ooze.
It felt like ants crawling on my skin. Anytime there's a sensation of that, along with oozing upon itching, is a sign of staph. So I applied the tacrolimus and antibiotic on it. I know I'm treating the staph internally, but I'll probably benefit from doing so externally.
I did feel very uncomfortable, and the oozing causes even more itchiness. I applied this mix on my right thumb which has a lot of cuts. But I know I'll feel better after this. Thankfully it's not a case where I felt a sudden need to slather the tacrolimus on my hands. In fact I didn't, just a bit with the antibiotic mixed in right on my thumb that has cuts.
I have a red patch on my wrist and neck. These are no longer itchy and there is no open skin or oozing/bleeding. So I'll just let those heal on their own, and I did apply the Cerave healing ointment onto it. As long as those don't ooze then I'm happy.
I know healing is not linear and will flare up here and there. As long as there is a net positive - I will get a flareup here and there, but after applying some topicals it will recover quickly and won't be as inflamed as it was in the winter.
I've looked at pictures of my skin from last year. I remember having inflammation, but it wasn't this bad. I really don't know what triggered it to be this bad. I really do not want to live the rest of my life like this either. This is no way to live.
People who have healed themselves said it took 6-12 weeks, so really 2-3 months. So I'm not going to expect everything to heal by week 4 (next week) even though some of the supplements are a 4 week (~30 day) supply.
So yeah I had some flaring, it's okay. It's been worse in the past. With topical application it should recover by tomorrow. The fact that there is inflammation and infection externally means that I can benefit from sort of anti-inflammatory (not steroids) and anti-bacterial topical. I might even consider applying more antibotic to my ears because it looks and feels like impetigo. Kind of like when I had a (sorry tmi) skin fungal infection and kept on applying ketoconazole. So I'll see how the topical antibiotic can help here and there, even if the anti-inflammatory isn't extremely necessary. Afterall my condition is due to bacterial imbalances/overgrowth, which is what's driving the inflammation. I'm just a bit worried about longterm antibiotic use because it can cause resistance and kill good bacteria. But in my case it can be helpful.
So like I said I'm on week 3, almost a month. I'm really going to need more like 6-12 weeks on this, but I'll see with my doctor 2 weeks from now. Overall there is a net positive where my skin is clearing up, and the inflammation and flaring is occasional. It's not perfect but it's much better than January-March.
Update: my ears stopped itching, my mouth is still itchy but I'm not sure what ultimately caused it as my mouth is usually not this inflamed. I'll even consider applying more tacrolimus and antibiotic to these spots when I wake up tomorrow.

Nortriptyline is an anti-depressant, if you're already on an anti-depressant then don't take this
Topamax is an anti-seizure med also used to prevent migraines. it can make birth control pills less effective
Emgality is administered once a month with a self-administered shot (something like an epi pen). this is what my doctor recommended I take if the supplements don't work. it specifically targets the migraine receptor, so it has very few side effects

Side effects: Nausea, bloating, and mouth ulcers. Feverfew can cause uterine contractions and is not recommended during pregnancy.
Coenzyme Q10 (CoQ10)
Mechanism: Involved in energy production and helps maintain mitochondrial integrity.
Evidence: Two randomized controlled trials showed benefit over placebo.
Dose: 300mg daily (can be split into 150mg twice a day)
Side effects: Insomnia, fatigue, nausea, or diarrhea.
Other Supplements Butterbur demonstrated benefit for migraine in two large randomized controlled studies; however; preparations may contain pyrrolizidine alkaloids which are toxic to the liver. Due to safety concerns, butterbur was removed from the market in Europe and is currently not recommended. Melatonin and the combination of folic acid/B6/B12 have mixed results for migraine prevention.

My skin is looking much better. I know stress relief, eating more anti-inflammatory foods and even the spirulina/wheatgrass are helping. I'm applying Vaseline to my skin as well as zinc oxide to my neck. Allergies have been annoying, but I'll find a way to deal with them (an anti-inflammatory diet is my top priority).
I'm pretty good with limiting inflammatory sugary foods. I need to do the same with savory foods like pasta, pizza, and processed breads because those are also inflammatory. Again I don't want to eliminate and develop another ED. But I do want to follow an anti-inflammatory lifestyle/diet to the best of my abilities.
I've been taking a shot of spirulina and wheatgrass every morning as my aunt swears by it. They are both very anti-inflammatory and remove toxins/heavy metals from the body and are good for gut health and are rich in vitamins/minerals. Only problem is that in my opinion, the wheatgrass tastes horrible. Like salty grass. I don't like it. The spirulina doesn't taste good either, but I can tolerate it more. But I want to add some sort of supplement that has the previously mentioned benefits to further reduce inflammation, spirulina or something else. I'll eventually figure it out.
