I wake up to an empty bed and laughter from across the room. Confused, I rub at my eyes and brush my messy hair out of my face while I lift my head from the pillow to see what time it is. 
It’s 11:00. 
I fell asleep yesterday after we got to the hotel at 4 pm. How did I sleep so long? Why didn’t Calum wake me? Where is he? 
I slowly crawl out of the comfortable bed, looking around the room for my boyfriend, and find him leaning against the wall while Paula and Lucia sip from coffee cups on the couch. When I’m standing, I wave at the three of them. They all smile at me with genuine happiness that I can’t say I myself can feel yet today. How can I have slept for so long and still be so tired?
I’m always tired.
“Buenos! Oh my god, you’re here!” Paula says, standing from the couch and running up to hug me. 
Sleepily, I hug her back, trying to smile. 
“Hi, it’s so good to see you,” I say. I’m quieter than I probably should be. 
She lets go of me and lets Lucia give me a hug too. 
“Cómo estás?” Lucia asks.
I try not to wince. “Oh, uh, okay.” I smile with closed lips and nod, stepping back from the girls and heading to lean against Calum for support while we chat. 
“I’m sorry, I tried to wake you last night but you didn’t even flinch. I figured you could use the sleep,” Calum says, his arms wide open for me to walk into. 
Silently, I nod, melting into him and feeling better instantly. 
“Are you hungry? Maybe we could go get food at Puerta del Sol?” Paula asks, smiling her painfully perfect smile. 
“Sure, I could eat,” I lie. I’m not hungry at all. “Don’t guys mind if I shower really quick?” 
They all promise me it’s fine and that they’ll just keep chatting. I grab a pair of jeans and a sweater from my suitcase before I retreat to the bathroom. With everyone waiting, I do my best to be quick and not focus on the nausea that creeps in as I stand in the shower. Once I’m relatively clean, I dry off and throw the clothes on.
I haven’t worn jeans in months, and that’s very obvious with the way these should probably be worn with a belt that I do not have. There are inches of excess fabric at the waist. My baggy sweater thankfully covers the waistband, so I just hope that no one notices that my clothes don’t fit me anymore. I’m a fraction of who I once was.
I plaster on my smile before I head back out to face the three people whom I should feel okay to be myself around, but, these days, who I am isn’t someone anyone wants to be around. 
We head out and I don’t really pay attention to anything. Calum guides me while he holds my hand, and I robotically trod alongside him. My stomach is cramping and I feel like whatever minor remnants of food are left inside of me are right at the top of my throat. Any sudden movement and I’ll be throwing up all over the sidewalk.
Madrid looks the same as it used to, but it feels different. 
I’m different.
We sit down at a small metal table at one of the near-tourist-traps in Puerta del Sol. None of us expect the food to be great. What matters at this place is the location, surrounded by the bustle of the city and being in the heart of Madrid. Here, it feels like we’re in the center of the universe, with people from all over the world sharing this space that so many others have walked before.
Everyone orders coffee, including me, but when it comes to food, I skip out. Calum looks at me with immense concern and he squeezes my hand under the table, but I can’t look at him, even though I feel his eyes on me. I don’t trust my stomach to not reject any solid food I consume, and I really don’t want to vomit while I’m here with Paula and Lucia.
I don’t even feel like I’m here. My body is here, but my brain is just floating somewhere, and I don’t even know where. It’s an unsettling feeling. 
The others are talking about New York. Paula was accepted to the graduate program at NYU she was hoping for, so Calum is giving them a ton of recommendations of things to do and places to go and even remembers the name of some bartender at a bar that he goes to each time they visit.
“I’ll shoot him a message later, Lucia. I’m sure he’d be happy to have a bilingual bartender on his team,” Calum says. He sets a reminder for later today. 
“Really? You don’t have to, Cal, I feel bad asking,” Lucia tries to argue, but Calum just shrugs. 
“Happy to help,” he says with another shrug. “What’s the point of having all these connections if I don’t use them?” 
I then realize that Paula has been staring at me. I raise my eyebrows at her to silently ask ‘what?’
You okay? She mouths to me.
I try to smile and nod at her subtly, trying my best to not attract Calum or Lucia’s attention. 
“What’s the plan for after this?” I ask, avoiding Paula asking me more questions. 
“I was thinking the botanical garden? If you’re feeling up for walking,” Calum says, returning his attention to me and squeezing my leg under the table. 
“Sounds perfect,” I say. 
We finish drinking our coffees and the others eat what’s left of their food before paying the bill. While we walk, I’m back to wearing my mask and so are the others. I feel bad expecting the girls to do it, but they promised that it was no big deal if it meant they could spend more time with me. I find myself shivering in the January cold while we walk, so Calum takes the beanie off his head and slides it onto mine without me even asking. He also holds my hand inside the kangaroo pocket on his hoodie. 
My stomach continues to churn during the entire walk, and I’m grateful that the mask on my face helps hide my expression that, at this point, is probably something like a grimace. It takes a lot of self-control not to crush Calum’s hand in mine, but luckily I don’t think he notices. My hand that’s not holding his is balled into a fist, my fingernails digging into my palm. 
When we finally get to our destination, I excuse myself to the restroom while the others grab our tickets. I feel so much relief when I’m finally bent over the toilet bowl, and, on cue, my stomach contracts and empties what minimal contents were sitting in it. 
I look into the bowl and get the urge to vomit again once I see what’s in it. It’s mostly coffee, but not entirely. The only logical explanation for the bright red swirling in the toilet is blood. I just threw up blood. 
My head gets hazy and the room starts spinning. I have to rest my face against the coolness of the toilet seat to refrain from fainting. My breathing is loud, and the sounds of me gagging have no doubt caught the attention of anyone else in the bathroom. When I feel enough strength to stand again, I wipe my mouth with toilet paper and replace my mask over my face. 
There’s an older woman waiting for me outside the stall, concern written all over her face. “¿Estás bien? ¿Acabas de vomitar? ¿Necesitas algo, o alguien?” 
I stare at her. All I can do is stare. Why can’t I remember what half of those words mean? I don’t know what she said and I don’t know what to say. I speak Spanish. Maybe not perfectly, but I should know what this woman just said to me. 
“Bien,” I end up saying. It’s the one word other than gracias that pops up in my brain. 
Why can’t I remember Spanish? 
In a daze, I stumble out of the bathroom and end up running straight into another person that’s walking in.
“Seriously,” I mumble to myself, trying to remain standing, but I’m wobbly.
The other person grabs my arms and helps steady me, and when I can lift my head enough to see their face, I realize it’s Paula. 
“Woah, O, are you okay? You’re so pale,” she says, still supporting me while I try to regain my balance and strength. 
“Yeah, yeah, I’m fine,” I breathe out. It takes everything in me not to lean into her and relax, but I manage to mostly stand on my own. “May need to sit down for a bit.” 
“Cal!” She yells, and I sigh. I have to pass this off as just being tired. I can’t ruin our day. I can’t ruin our trip. 
Calum appears behind her in seconds, and before I know it, he’s throwing my arm over his shoulders and holding up half my weight himself. 
“I got you, baby,” he whispers quietly enough that no one else would be able to hear. 
“There’s a bench over here,” Paula says.
Calum guides me to the bench and helps me take a seat. He crouches down in front of me, and I hate the look on his face. He’s terrified. He’s nearly holding back tears. I can’t tell him I just threw up. I can’t tell him I just threw up blood.
“Here, I brought you a protein shake in case anything like this happened.” Lucia holds a carton in front of me that looks like some kind of chocolate concoction.
I want to push it away. I want to say I don’t need it. I don’t want to need to drink a protein shake. 
But I do need it, so I take it. 
“Do you need anything? Water? What can I do?” Calum asks gently. He’s still kneeling in front of me and looking at me with concern and care.
I shake my head, pulling my mask down and unscrewing the lid on the drink. I gulp it down quickly, doing my best to fight the urge to gag while I drink. The acidic burn of whatever was in my stomach is still clawing at my throat and makes this drink taste disgusting. “All good. Just need a sec.” 
Calum stays in front of me and watches me closely while I drink. Lucia is standing a few feet off to the side, giving me space, but I’m not sure where Paula went. 
When the carton is empty, I set it next to me and wipe my mouth with the back of my hand. I force a smile. 
“Did you get the tickets?” I ask. I look back and forth between Calum and Lucia. 
Cal nods. He reaches into his hoodie pocket and pulls out four tickets to show me proof. “Are you feeling okay enough to go in? We don’t have to, if you need to go back to the hotel.” 
I make my fake smile bigger. “No, I’m fine. We should go in.” 
“We can come back tomorrow,” Paula says, reappearing with a bottle of water in her hands. She sits next to me and swaps it for my empty protein shake. “I just asked.”
I just shake my head quickly. “Guys, no. It’s okay, just give me a few minutes. I’m fine.”
None of them seem convinced and start communicating nonverbally. They don’t believe me, but I don’t think any of them want to be the one to say we should leave. They don’t want that title of being the one who makes me rest like I need to. 
“Seriously, I’m fine. I should’ve just eaten some tortilla at the restaurant. Sorry to be such a pain in the ass,” I say as convincingly as I can. I sip from the water Paula brought over. 
“O, you just—” Paula starts.
“Paula, I’m fine.”
I stand up then, and take a gamble on possibly passing out, but thankfully, I remain steady. I take another long drink from the water bottle before I put my mask back on. Calum protectively follows suit and stands with me, reaching a hand out to help me stay balanced, but I don’t need it. At least, I’m trying my hardest not to need it. 
“If you’re sure, Orion, we can go in,” Calum says. His eyes search my face for any sign of uncertainty. Even though my mouth is covered now, I smile and hope it reaches my eyes. 
I take his hand in mine once more and drag him toward the entrance, once more doing my best to ignore my immense fatigue and resurfacing nausea.

vi. Backstory
Juliet was born and raised in Queens, New York. She was raised in a middle class family, and lived an ultimately normal life until her mothers death when she was 14. Juliet's mother, Monica was killed in a tragic car accident on a snowy night in December. This changed Juliet forever. The once bright and happy young girl became very reserved and closed off.
She remained that way throughout high school and college, and was eventually dubbed as being boring. Juliet was a smart girl, but she lacked the knowledge of how to cope with trauma.
Juliet worked closely with Evan, he was her mentor during her time at Rolling Stone. When the opportunity arose to be in the middle of a warzone for the sake of a book, Juliet saw this as her chance to get out of her shell and prove she was not boring.

The New York Times covered this in 2018. It is not novel and is only newer to the medical literature. (Side Note: I wasn't even aware of this in 2018, hence me resharing it now for those in a similar boat)
Here's a highlight from the NYT article:
When a transgender woman told doctors at the Mount Sinai Hospital that she wanted to breast feed her pregnant partner's baby, they put her on a regimen of drugs that included an anti-nausea medication licensed in Britain and Canada but banned in the United States. Within a month, according to the journal of Transgender Health, the woman was producing droplets of milk.
With three months - two weeks before the baby's due date - she had increased her production to eight ounces of milk a day.
In the end, the study showed, "she was able to achieve sufficient breast milk volume to be the sole source of nourishment for her child for six weeks," according to the journal.

"Sabrina was one of those people who always made me smile even in the darkest times. Loosing her feels like loosing a part of myself. She was the sun herself. Always a smile on the lips and a sass retort. Man she was a stubborn girl but always right. She'd gone places. We'd read her book or visit one of her championship. I haven't been on the beach since too many memories connected to It. Sometimes I look at the night sky and it feeds like she's looking down on me. She was never mine, but losing her broke my heart. But she was such a cheerful girl she'd be heated we're crying right now," he laughed.
He walked over to the music station and pressed play.
From the speaker Freddie Mercury began to sing who want to live forever.
"Let us keep her alive by hearing her music, by surfing these huge waves she'd loved, by writing down our thought and emotions, she'll be amazed what all of you are thinking."
Suddenly the music changes midways and Every breath you take echoed through the chapel.
"Okay that's strange" he mumbles to himself.
"Just don't forget about this incredible girl." He ends his thought and went to the front.
Alex mom leans over to her son and whispers "he'd loved her right?" Alex just nodded.
He stopped at her coffin and taped a sticker atop Sunset curve. Luke stumbles back to his seat.
Alex raised from his seat and walked to the lectern.
"First I wanna thank everyone for coming. And now let's start" his words fading at the end.
"Will is.." he stopped thinking about his words.
"Will was the best sister a guy could wished for-- She was a best friend, a sister and kind of a mother in one person, even with her sweet 16 years. Her jokes were the worst and her humor just horrible" the blonde let a sad laughter out, his friends joined this gesture.
"But truth to be told, she radiated happiness and calmness. Sabrina had an extraordinary mind which I'm going to miss"
As her name left his mouth the blonde looked to the front her eyes meeting his but he wasn't able to see her, this she learned the hard way.
Days after days she laid down beside her sleeping brother brushing strands of hair out of his face or sitting down at the table during dinner, her hand always brushing over his; maybe he didn't noticed or played it down. She didn't know she had to die to hear her real name from him.
"The last days I felt her presence but I must have imagined it. There was always a hand laying on my during dinner or a feeling someone's laying in my bed besides me"
he noticed
"I met Robin and Grace telling them what happened. I am still sorry for you Luke..."
Oh no stop Lexi.
She heard the conversation about this, his words still ringing in her head.
She rushed to him and laid her arms around his waist holding him tight. Feeling his warmth, feels like home. His muscles tensed as he felt the touch.
Alex stopped in tracks and shuts his mouth. "I'm mostly sorry that everyone here lost this unique girl, friend, sister and loved one." His teeth grinding. A feeling of nausea floating his body.
He wanders back to his seat sighing as his butt hits the wood.
After hours of sobbing and constant crying the immediate family and friends gathered in the Mercer residence eating Sabrina's favourite cake and converse about all and sundry. The girl herself eavesdrop her brother and his friends in her room.
Since Alex and Luke exchanged their intense words the other day in there, Alex doesn't think of sleeping in his bed ever again.
In this moment the named boy laid on her bed with Luke and Reggie rested in her chair facing the boys.
"I wish I could lay here forever" Alex's voice low, no power left in his body. The other guys just sighed.
Sabrina winced at the sight of them. She moved to her skateboard and knocked it over. Their graze falling on the board. Alex's face went blank and pale, his jaw droped and Reggie even screamed in horror. Luke being himself his mouth curved into a smile.
"You know what this means"
"Someone's haunting us" Reggie’s voice shaking.
"I'm.. I'm not driving her boards" Alex argues.
"It's one thing sleeping in her bed but her boards. They were her babies" he added.
"It's going to be one time." Luke get off of her bed and opened her closet, taking her favourite shirts out.
"I'm not wearing her clothes neither." His laugh was shaking. As the words left his mouth he had a Queen shirt in his face.
This was my favourite thought the blonds.
"I want Billy" Reggie suggests Luke as he searched for the shirt.
The shaggy haired boy himself chose the shirt he bought her on one of the concerts they'd visited; AC/DC.
He undressed from his red and black squared jacket the blonde girl gifted him years ago.
Due to her passion she owns more than one skateboard which helped in this situation. Everyone had his own and Sabrina didn't wanted to miss this so she steps behind Luke on the board, her arms resting around his waist and her head on his back; he smelled like always, her favourite scent apple.
Luke felt the light pressure on his body, he turns his head slightly hoping to see her standing right there.
Focusing back on the friends they start to ride down the streets, passing various people which were annoyed by the teenagers but they didn't cared, this was her moment.
They reached the beach and for every single one it was the first time to step on the sand again. She saw their sadden expression; the boys slouch their shoulders.
"I think I'm not ready," Alex stated.
"After all we did?" Luke remarked. Alex sighed and took the first step. His feet sinks into the sand.
"You're coming?" His friends amazed by his action but soon they joined him.
"She'd be proud of you" Reggie laid his hand on his shoulder and squeezed it.
Minutes, even hours passed the group in silence. Soon the sun began to set and they were astonished by its beauty. Luke's view raised up and fell on the bright star which adorned the sky.
Sabrina sat next to him all the time and followed his graze. A tear slipped from her eye as she thought about it. Her head now resting in his shoulder which made Luke cringe at the pressure. His eyes searching for her green one without any luck.
"We'd head back" Reggie was the first back on his feet.
"I'm grateful to have you as friends" Alex said and taking the boys in a warm embrace. Sabrina's arms were also laid around Alex and Luke, they exchanged knowing glares and broke the hug.
"Let's go" they grabbed their boards and made their way home.

(Not so) Random considerations on birth control methods and menstrual cycle

Although absolutely nobody fucking asked, I wanted to talk about my personal experience with birth control pills and menstrual cycle. First of all, let's catch up on how did I get here.
I started taking oral contraceptives (OC) since my mother took me to a gynecologist for the first time. The doctor made me a prescrition because I told her I suffered with cramps during my period. I was about 13 years old.
I kept taking OC every single day for the following 11 years, until I reached 24. Several doctors I passed by along these years changed the dosage and combinations of hormones I took, because each of them gave me a different bunch of adverse effects. Headache, nausea, menstrual cramps, recurrent urinary tract infections, candidiasis, vaginal bleedings... the list goes on.
During my teenage years I found out some women from my mother's family have circulatory problems, from varicose veins to venous thrombosis. There are also cases of cancer possibly induced by sexual hormones. That is: conditions that make OC, especially the combined ones, contraindicated for me. I got worried and decided to come back to the doctor and talk about another options available. The only one that was presented to me was the so called minipills, which are OC made with a single hormone instead of a combination of two. I took it for the following 5 years straight, and it seemed a good idea at the time because I've spent all my life struggling with underweight and anemia. Since the OC completelly suspended my period, I was supposed to be fine.
However, last January I had a major vaginal bleeding, even though I didn't stop taking my OC. I had terrible abdominal pains, and the bleeding continued for almost 10 days straight. Like I said, being underweight didn't improve the situation and my immune system shut down very quickly. Besides, I was having a hard time to keep up with my bills and wasn't covered by any health insurance at that time (I live in Brazil, and for those who are not familiar, things are a little bit different here. Theoretically we do have a public health system, but in real life we can't barely count on it and the access to the private system is kinda surreal for those living with minimum wage).
Well, as soon as I could, I saved enough money to go see a private doctor. I paid for the appointment and a several exams to find out that my bleeding was possibly caused by multiple ovarian cysts. Both of my ovaries were 3 times bigger than the normal size, and the doctor hypothesized that a big one of them (or a few) must have simply ruptured, and that the whole shit was probably induced by the fucking OC.
In summary, the doctor said I had polycistic ovary syndrome (PCOS). Plus, I should stop taking my actual OC and go back to the combined ones. Yeah, those same I was not supposed to take both because of my family history and the previously described adverse effects. He emphasized that was the only treatment available, and that my condition actually had no cure, so I should just take it for the next 30-40 years until I’d reach menopause, while praying for not having cancer or thrombosis or embolia and... well, to die of something else not related with OC.
So, well... I quit. I smiled and waved to the doctor and left the office. I was about to turn 25 and I decided I wasn’t going to take it that way. Now that you’re up to date in the story, let’s move on to where I was really trying to get with this post.
Please note: I ain't no gynecologist nor physician, but nowadays I’m a post-graduate health professional with a couple years of clinical practice. And I think I’m allowed to apply the little knowledge I acquired during 7 years (so far, still counting) of higher education to see through this situation with a tad of criticism. Not only regarding my own case, but regarding the doctors’ position when it comes to women’s reprodutive health - at least in my country. Therefore, let’s consider some key points:
Is there a real need to prescribe OC to young girls aged 13 years or less just because they come to the office complaining about menstrual cramps? During the period the lining of the unfertilized womb is being shed through the vagina. It involves muscular contractions, so of course it might get painful. There’s nothing abnormal about it, so why purging it like a plague instead of teaching them that’s a physiological process and how to relieve the pain in case it happens? Nutritional counseling, physical exercises, simply using a hot-water bottle or even taking an occasional painkiller can totally solve the problem.
The primary aim when taking OC is expected to be, should be, birth control. Yet, they’re frequently prescribed to girls that don’t even have an active sex life because of light acne, oily skin, menstrual cramps and/or intense menstrual flow without any further clinical complications... or just because. You might take it as some conspiracy theory, but you know what it looks like to me? Creating a very profitable market for pharmaceuticals. And nothing more. If women get sick and end up developing cancer or whatever, even better, so more drugs (way more expensive ones) will be sold.
In fact, there are another treatments available for PCOS. But it seems doctors are too lazy, or too comfortable in their position of filling a single standard prescription, that they completely ignore any alternatives. Can you wonder why? Maybe because it requires a minimum of health and sex education, and that takes time. How are they going to be able to attend people in less than 5 minutes if they’ll have to talk to their patients, right? Simply doesn’t worth it. Anyways, again, alternatives include acupunture, homeopathy, phitoteraphy, dietotherapy throught nutritional counseling and regular physical activity. Each case is different, but keep in mind: OC aren’t the only way, indeed, literally speaking they’re not even a treatment because they don’t treat it.
Opening a parenthesis: of course there probably are exceptions and good doctors no matter where. But doctors at public health system are in general unsatisfied with their working conditions and environment, while doctors at the private system usually are anything but well paid by insurance companies. In overall terms, the more academically qualified the doctors get, the less prepared for attending real life demandings in developing countries they are. Also, the less willing to work in such places they are. (If you’d wish to read more about it, I highly recommend seeing Chapter 5 - An example of a paradigm and its social conditions: scientific medicine of La construction de sciences, by Gérard Fourez.)
Still on PCOS topic: first of all, having multiple cyst on one or both ovaries doesn’t necessarily mean PCOS. PCOS, as a syndrome, means there are multiple criteria that need to be fulfilled for closing the diagnostic. In this case, criteria involve imaging exams, symptomatology, clinical and biochemical evaluation. In my case, for instance, PCOS is a diagnosis that simply doesn’t suit my medical history, but no doctor has ever bothered making an anamnesis. I’m not trying to say anybody should go to Dr. Google’s opinion (seriously, don’t), but look out for more information than it’s given to you at the office, even because often none is given.
I know suspending the menstrual cycle can make life much more easier. No worries about pads, unexpected leaks, cramps, PMS etc. But take it from a different perspective for a second. There seems to be a lot of content over the internet nowadays about body positivity, empowerment and tons of so called movements of deconstruction of established paradigms in our society about feminility and feminism. I’ve seen a lot of girls online sharing their experiences on stopping taking OC etc. I don’t know how far it’s good or not, but there’s a point that can be taken from all of it: the menstrual cycle is a natural part of every woman’s reprodutory phase in life. It’s not disgusting, embarrasing or whatever nonsense we’ve been told. And it can be a good way for us to conect with ourselves, to listen to our bodies. Observing symptoms such as pain, fatigue, cravings, emotions, sex drive; checking on cervical mucus, body temperature, hours of sleep... all of this can be part of a daily self-care routine and, moreover, be useful to birth control.
Talking about birth control: I’m genuinely surprised on how much the doctors whom I interacted during my life underrate condoms as a method against unwanted pregnancy. They say out loud that it’s not safe and, unless the conspiracy theory about selling drugs is real, I simply don’t get the reason why they do that. In first place, this is bullshit because condoms are a very effective fisical barrier that prevent even a single spermatozoid from swimming along the vaginal canal and straight up to the womb. Second, there’s no 100% safe method except for sexual abstinence; not even OC + condoms (theoretically not even tubal ligation) are 100% safe, since the human body isn’t a static machine and everything is prone to error. So, yes, opting for non-pharmacological methods of birth control instead of synthetic hormones can be valid.
Obs: condoms work as long as they’re properly stored, used and discarded. But the same can be said about OC and any other contraceptive methods. And, important: choosing a contraceptive method involves not only statistical data on the margin of error of condoms and pills, but also individual phychossocial aspects. In other words: a determined method might not be the doctors’ first option and they might not personally like it, but they can suck it up and use their fucking knowledges to find the best alternative for you.
Again, I’m not trying to encourage you anybody else to contradict their doctors. However, I think that questioning is part of a healthy and constructive process. First because doctors are human beings, therefore they’re as prone to error as anybody else (or even more due to long working hours). Second, because they’re supposed to be the primary source of information for any questions you might have about your own health. Third, because I believe with all my heart that the relationship between health professionals and their patients must include, if not be based in, trust.

8. What is your OC’s theme song?
For this one, I think “Painkiller” by Three Days Grace
I feel that somehow, after everything that’s been happening in her life, now that she’s back to U.A. and has Dabi in her life, in a very disfunctional way, she leans on him and Hawks for support, since she’s in the grey area of Villain and Hero, and needing to find some way to erase the pain she has in her heart that she had to bottle down so many years.
31. What is their most traumatic memory/experience? What is their favorite memory?
Her most traumatic memory is when her sister died in her arms.
They were doing a Hero training, with All Might and Aizawa supervising, and the twins were in the HERO side, having to rescue a “hostage” from the “villains”, but as her sister was the one to fight, being the more powerful one, and Kitsune having to rescue the hostage, as she was the sneaky and fast one, her sister, Kasai, was caught in a crossfire real Villain attack, and when Kitsune returned with the “hostage” to show her they won the training, she saw her sister on the floor, bleeding, barely able to utter a word.
On the other hand, her favourite memory is when her and her sister got into U.A., but she wasn’t happy that she got into U.A., since she never cared much about becoming a hero - She was happy she was in the same school and class as her sister, since she never wanted to be apart from her, no matter what.
50. If your character was presented with imminent and unavoidable death/fatality, how would they react? Would they try to avoid death anyways? Would they try to make their last days count?
It depends on the time she has left and on the thing that’s causing the unavoidable death.
She will talk to everyone in power, seeing if they know anything on the matter and if they can help, and if there is a chance of hope, she’ll try to take it.
If not, she’ll go have a private word with Hawks, Aizawa and Toshinori, since they were the most influencial people in her life, then she’d go to Dabi and tell him everything there is to be told, lowkey forcing him to accompany her on a trip around the world - Or well, whatever place she wants to visit - Wanting her last days to be with the person who gets her and showed her that there’s nothing bad about having feelings. ( How ironic/hypocritical of him. )

A year and a half flew by like the speed of light, and things were starting to come together for every one of us. 
I put school on hold, which Nikki didn't like because he felt it was his fault, even though he was aware I would've put school on hold anyway for Tommy. I didn't even have to tell my parents I was dating Nikki because as soon as mom found withdraw forms from Julliard in the mail, she called me dishonest, accused me of disobeying my parents, and threw me out as soon as I let her know I found somewhere else to stay. I figured she would find out about us eventually, anyway, so there was no reason for me to go out of my way to tell her why I was putting school off. After that happened, I left my house and was living with Nikki once everyone saved up enough money to move out of the crappy apartment. To Vince and Tommy we were just room mates. We knew Mick didn't fall for it because, like Tansy, he also just knew things but he never mentioned a word of it Tommy or Vince, or even confirmed to either me or Nikki that he knew what was really going on. He minded his business, like he always did.
Tansy and her mom had moved out to New York the summer after we graduated because she had been approached for a contract with some modeling agency out there and they were paying for a nice apartment for them to stay in. I was proud of her because she had always wanted to model, however, I was skeptical. She was 5'3" and back then, you had no chance of hitting a runway if you were too short. So Editorials were her only option, but I knew she would be just as happy posing in Vogue as she would be walking down a catwalk.
So with the new album, Tansy modeling, everyone moving out of the hell-hole apartment and Nikki and I managing to make it over a year tolerating each other in a relationship, everything was falling in to place. And it was only getting better.
"'Jesus is Satan?' I raise a brow as I ask, seeing Nikki and Vince smiling and laughing while explaining why one of their sound engineers fled their recording session.
"It's only backtracking, it's not like we're throwing it out there. You gotta look for it." Vince explains.
"Aren't you two just every church girl's dream?" I ask sarcastically.
"Maybe you should start cleansing my soul before I come inside the apartment. It might help exorcise whatever demon you're convinced has possessed me." He says it genuinely, but I know he's full of it from the smirk he gives me. I'm tempted to threaten to close my legs anytime he wants any, but Vince is here and he'd tell Tommy if he knew about us, so I save the threat for later, narrowing my eyes at Nikki.
"Ohh, if looks could kill you'd be a dead man." Vince hits Nikki's arm as Nikki takes a swig of his Jack.
"Whatever, I'm taking a nap." I tell them, stepping to the guest bedroom which is supposed to be my room.
"You just woke up a couple hours ago," Nikki argues.
"I don't feel good." I reply, honestly, the budding feeling of nausea resting in the pit of my stomach.
Vince leans over to Nikki, whispering something in to his ear and Nikki shakes his head, grinning.
"No, not for another week." He says to Vince, the two smiling innocently at me and I ignore them and shut the door of the bedroom, crawling in to the bed.
After a few minutes of laying on my stomach, the nausea subsides and I'm able to go back to sleep for an hour and a half before the door of the bedroom door creaks open. I already know it's Nikki coming to harass me, and the feeling of his lips touching the bare skin between my shoulder blades that's been left exposed by the tank top I'm in, just confirms it.
Another kiss is pressed to the back of my shoulder, then to my cheek and I turn over to face him, enjoying the feeling of him over me.
"I gotta go to the studio." He tells me. "I'll see you later."
"Alright." I nod, sighing as he leans down to press his lips to mine for a second, before going to leave. It's not enough for me, though, and I'm scrambling to the foot of the bed to catch his wrist in my hand and pull him back to me, reminding him some two second peck on the lips is basically an insult to me, and usually is an insult to him unless it's to avoid being late to get on stage or going to the studio.
He chuckles at my desperate effort and pulls himself closer to me by my hips as our lips, tongues and teeth meet. Before I can reach for his belt buckle, he's groaning a little and putting his hands on my arms to push me away a slightly.
"I gotta go, Viv." He breathes out with a wide smile, brushing a few strands of red hair from my face. "We can fool around when I get back." He assures me.
"You'll be wasted when you get back." I argue.
"I won't drink that much." He presses a last kiss to my cheek, patting my hip before he leaves.
One of the many things to know about Motley Crue: "we won't drink that much" = "we'll try not to drink that much...but we will drink that much, and then some, and not remember any of it". Same rule applies to drugs and sex.
He gets back twelve hours later at four in the morning, swinging the front door open and stumbling back in to it with his hand still gripping the door knob to keep him from face planting. I look up from my bible to see him do this, and set it aside to go help him.
"Hey, Viv," He greets me with a slur and I give a sympathetic smile. "I think I drank that much."
"I kind of knew you would." I reply, helping him keep his balance as he steps to our bedroom. He plops down on the edge of the bed, tugging his boots off, nearly falling off the bed as he throws them across the room.
"C'mere." He says to me while I'm standing against the door frame with my arms crossed, watching him.
I do as he says, his hands grabbing at me at pulling me to straddle him.
"You smell like a bar," I tell him, laughing as he nips and licks at my neck.
"You smell good." He replies dopily and I rest on of my hands at the back of is neck, my fingers lacing through his hair.
"It's called a shower," I say. "You should try one some time."
"Fuck you." He falls back on the bed, his arms splayed out beside his head as he closes his eyes. "You know somethin'?" He asks me and I lean forward, my body against his as I rest my elbows on his chest.
"What?"
"I wanna fuckin' tell people we're together." His words surprise me a little.
"If we do that, Tommy will find out. And he will kill you."
"If he wants to kick my ass for fuckin' his hot friend, then fine, because I'm tired of lying and telling them we're 'room mates'." He explains sloppily. "Room mates don't sleep in the same bed naked and suck each others dicks in the shower."
My brows furrow at "dicks" and even drunk, he realizes his error.
"That's not right." He complains, rubbing his tired eyes.
"I don't have a penis, Nikki." I remind him and he gawks.
"I hope not cause I've been seein' pussy anytime I look down there." He says, looking directly at me with wide eyes and I have to try my hardest not to start cackling.
"You need to go to sleep." I move off of him, gently tugging at his hand to get him to move to his side of the bed.
"I got it." He waves me off, dragging himself to his spot, not bothering with the covers before laying down.
He's asleep within a couple minutes and I exhale, getting on my side of the bed before cutting the lamp on my bedside table out and going to sleep.
The next few days are spent scrambling to put together decent Christmas plans, which include picking Tansy up from the airport after she calls and gives us a heads up she's coming to visit.
"What do you mean you won't have time, Vince?" I snap, smearing icing on a cake. "You've had since January of this year to save December 23!"
"Beth doesn't wanna come." He finally says after minutes of going back and forth over the phone about why he can't come to dinner tonight.
"Then leave Beth at home! We all agreed last year to celebrate Thanksgiving and Christmas together every year."
"If Beth doesn't want to come, I can't come." He explains as Nikki and Mick come in with the beer and more Vodka and Jack.
"Well, no one told you to marry the uppity bitch, Vince." I state rudely, Nikki raising his brows at my language.
"Look, I love you, but I can't make it. I'm sorry, I promise I'll make it up to you." He sighs out and I stay silent for a second.
"Did I mention it's also my birthday today? Are you really gonna ditch me, you're favorite red head, on my birthday?" I ask him, in a tone that hopefully sends him on a guilt trip. He doesn't answer me for a good two minutes before groaning.
"You are fuckin’ killing me, Viv." He tells me and I smile, knowing I'm getting my way. "Fine. I'll be there."
"Thank you, Vince." I say before he hangs up. "He is way too easy." Just as I finish saying it, Nikki's digging his finger into the tub of icing and smearing it down my face.
"Nikki!" I scold loudly, grabbing a paper towel to wipe it off my face but before I can, his tongue sweeps across my cheek and I scrunch my face up, not able to hold back the laughter in my throat as he licks the mess off my face, finally giving me a paper towel when he's done. "Beth wasn't gonna let Vince come tonight."
"Oh, no. How awful." Mick pipes sarcastically.
"First of all, it's my birthday. Second of all, it's tradition." I point out o him.
"Not really. We only spent celebrated your birthday and Christmas in one night last year because we were too broke for two different occasions." Nikki says, opening a beer and I give him a look, causing him to pause, look at me, look at Mick, then back to me, and say: "That fucking bitch was about break our tradition."
"Smooth." Mick mumbles to him.
"I need someone here who actually cares about sentiment. Where's Tommy?" I ask, continuing to glop and spread icing on the cake I've just made.
"Tommy's getting Tansy." Nikki tells me. "Like you told him to do, then Tansy's gonna grab all the stuff you'll need to make dinner. It will be fine. Calm down."
"I'm calm." I argue in a not-so-calm tone.
"Yeah, I'm gonna get started on this so I can endure your bitching all day." He opens the bottle of Jack and takes a big gulp.
"Then you'll be drunk tonight and you can't be drunk tonight." I take the bottle out of his hand.
"Then I'll get stoned." He states, stepping to our bedroom. After a moment, he's coming back in to the kitchen, seeming to be pissed off. "Where the fuck is my--"
"I hid it." I say without hesitation, looking over my shoulder at his expression. "It's my birthday."
"I haven't spent one night sober since I can remember. You're not changing that all because it's your fucking birthday. Give me my stuff back, Saint Vivian."
"Mick, tell the Devil's Spawn to 'man up' because one night sober will not kill him."
"Mick doesn't get a say in it because you're still letting him drink!" Nikki exclaims, motioning to the silent man on the couch who's nursing from his vodka.
"Mick has special privileges because he's the most responsible one out of you four any other time of the year." I finally finish icing the cake, turning to face Nikki, who's staring at me.
"Mick's fuckin' stayin' outta this fight and every fight after it." Mick states, leaving no room for discussion.
"We're not fighting." I insist. "We're having a slight disagreement."
"Which will be resolved the second Mother Mary gives me back my freedom." Nikki says.
"Since when does alcohol and psychoactive drugs have anything to do with freedom?"
"Since I was able to do them freely until I started having to put up with you." He argues.
"Put up with me? You say that like you just tolerate me." I cross my arms.
"Anytime I do something you don't think is good enough for you, you give me this look like I just shot a puppy. I can't breathe the wrong way without you looking down on me."
"Probably because when you 'breathe the wrong way', you're actually breathing wrong, do to all the depressants and stimulants you load into your body simultaneously!" I outburst. "I don't look at you that way because I'm judging you, I look at you that way because I'm hoping you won't over do it and croak over, so forgive me for not wanting you snorting and drinking everything in sight tonight because I would like my boyfriend to not risk dying on my birthday but apparently that's too much to ask for, so go for it!"
I'm expecting him to give me the look he's always given me when I try to tell him not to drink, pop pills or snort anything, but he doesn't. I guess it's because it's my birthday.
Before he can answer, someone's clearing their throat, and we snap our attention to the front door, seeing Tansy.
"Welcome to hell." Mick tells her, and she looks at me and Nikki hesitantly, as if she's thinking about turning back around and leaving. I'm sure she is until Tommy pops up behind her, arms full of groceries and a wide, excited smile on his face. Completely oblivious.
"Happy Birthday!" He pipes, walking in to set the bags on the counter and hug me tightly, picking me up. Nikki and I continue to glare at each other while I'm looking over Tommy's shoulder, and when he puts me back down, the two of us put on fake smile so he won't know we're fighting.
"Hey, babe, where do I need to put this?" A man that has to be at least 30 years old is walking in to our apartment, talking to Tansy while holding a Tupperware container.
"What is it?" I ask him, thinking it's food, taking the container.
"Oh, Smack." He says casually and I'm dropping the container in a matter of milliseconds, taking a step back and looking at Nikki with raised brows.
"Vivian!" Tansy complains, picking the container up, going to put it in the fridge but I block her way. "Oh, so, what? All the things these idiots pump themselves full of, you draw the line at heroin?" She asks me.
"I am the line." I inform her. "I am not keeping some stranger's drugs in my fridge."
"It's not some random stranger, it's Sparkie." She explains to me as if that makes me and the man best friends. "My boyfriend." She continues.
"I figured that." I reply.
"Why do they call you Sparkie?" Nikki asks him, skeptically.
Sparkie was an absolute idiot. Think of what strung out, worn down, one shoot up away from being dragged to hell, looks like. He looked like a walking corpse with a peculiar hue of blue to his skin and had sleeves of tattoos to try to mask his track marks, to no avail. A few of his back teeth had rotted out and it was evident anytime he opened his mouth to speak. His bleached blonde hair had grown out to his chest and his pitch black roots were greasy due to lack of bathing. He had frown lines, crows feet, smile lines and greyed facial hair along his jaw. Tansy claimed they had met on the set of one of her photoshoots and he was the photographer's assistant and said that he told her he fell madly in love after laying eyes on her for the first time. Well, Tansy was gorgeous, but she was also naked and posing for Playboy at the photoshoot where they met, so he might've confused wanting to have sex with her with wanting to date her. I prayed he'd only be temporary, but he managed to hang around for five more years until the two of them broke up during the "Girls" tour. I was stunned when she went from dating someone like Sparkie, to dating someone like Axl Rose.
"Freebasing accident. Ether's easy to catch on fire, man." He tells him with a chuckle and I feel multiple brain cells die.
"Fuckin' awesome, right, dude?" Tommy asks as he hits Nikki's shoulder and Nikki looks at me with a smug expression, knowing it's killing me to keep my mouth shut.
"We have room in the fridge, right, Viv?" Nikki asks me innocently and I roll my jaw, looking at Tansy's perfect smile, as if she's silently begging us to like her boyfriend.
"Absolutely." I say, taking the container from Tansy and putting it in the fridge before me and her start cooking dinner.
By the time Seven o'clock is rolling around, dinner's done and Vince is waltzing through the front door.
"Hey, Viv," He comes up behind me in the kitchen while I'm washing my hands, kissing my cheek as he puts a present on the counter.
"Hey," I greet him, seeing Beth occupied with talking to Tansy.
"Who's...?" He nods in Sparkie's direction and I turn the sink off and get in his ear.
"Tansy's boyfriend, Sparkie. His White China is in my fridge and he got his name after a freebasing accident." I educate him and he raises his brows, an amused smile pulling at his lips.
"Tansy's dating him?" He whispers and I nod.
"Jesus," He mumbles, watching as she comes over to us to hug him.
"Vince!" She greets him eagerly, throwing her arms around him and he pulls her to him tightly, catching an evil look from his wife.
"Hey, Tans," He says when she pulls away.
"C'mon and meet my boyfriend." She grabs his hand and pulls him to the living room.
"Um, hey, Devil-Spawn, I need your help for a second." I tell Nikki and he looks up from his conversation with Sparkie and rolls his eyes, stepping to the kitchen.
"What?" He asks me and I drag him to the bathroom, shutting the door and locking it.
"Tansy's dating a loser." I voice out immediately and he gives me a look.
”I thought you wanted a quickie. Had I known you were just gonna bit--"
"He's a moron. 'Ether's flammable'? 'Put my smack in the fridge'?" I mock his voice.
"He didn't say the last one."
"Tansy is too naïve for someone like him." I ignore him. "She doesn't know what she's getting herself in to being with someone like that."
"I'm someone like that." Nikki reminds me and I scoff.
"Um, you don't carry a Tupperware of Smack with you, you're not burning yourself in ether explosions, you're not taking advantage of a beautiful blonde girl who just wants to make everybody happy..." I list a few things Sparkie's doing. "Look, I'm not worried about me doing anything because I have the will power of God when it comes to drugs and alcohol but Tansy will do the most ridiculous, disgusting, debasing things to make someone happy. Trust me, she's told me some of the things she felt like she had to do in order to keep some of her past boyfriends and I'm scared he's gonna get her hooked on some of his mess because he'll figure out if he asks her to do anything, she'll do it." I explain to him and he sighs.
"Maybe she's not as naïve as you think she is. I mean, it's been over a year since you last saw each other, so it's possible she's changed and knows exactly what she's doing." He tries to reassure me.
"I'd rather her be oblivious and naïve to the severity of the situation than knowingly be waltzing in to her own self-destruction." I state blankly.
"I wonder who all's gonna be saying that about you when they find out you're with me." Nikki says pointedly and I open my mouth to speak, but close it again.
"You're right. She's a big girl who can take care of herself and I'm being judgmental of Sparkie." I admit.
"You're admitting you're wrong?" He asks me in pretend shock, leaning closer to me, putting the palm of his hand flat on the door beside my head and I cross my arms and avoid looking at him.
"I guess." I mumble, not liking the fact I have to admit I'm wrong and he knows it, basking in the glory.
"Viv?" He asks and I look at the bathroom floor, still not acknowledging him. "Hey," He grabs my jaw gently, forcing me to look at his darkly lined eyes. "You have such a big heart, you worm your way in to other people's lives to try to get them to not make decisions that you wouldn't make, to protect them. But just because you wouldn't choose to do something, doesn't mean it's a wrong choice for someone else."
"Snorting coke and throwing back shots until you can't stand up, isn't a good choice for anyone." I already know what he's getting at and he sighs, stomping his foot.
"If I hear one more 'dude' from Sparkie without some kind of buffer in my system, I'm gonna knock his head into the fuckin' counter, Viv." He states, pointing his finger in the direction of the living room.
"You're doing great." I tell him, taking the hand he's pointing his finger with and hold it in both of my hands. "We only have to deal with him for a few more minutes and then everyone will be gone, and we'll have the place to ourselves," I say, taking one of the chains of his few necklaces around his neck and running it threw my fingers, pulling him to me. "And I'll let you do whatever you want."
His expression darkens at my promise, his lips pulling upward in to a crooked smile as his hands move to my hips and he guides me to the sink, picking me up and putting me on the edge of it,
He's putting himself between my open legs, his finger tips gripping at the flesh of my thigh as his other hand is hiking my maroon dress up, his tongue dancing with mine. I push the thin straps of my dress of my shoulders and his lips leave mine when he yanks the top of my dress down to expose my breasts. Just as his teeth are biting a trail down my skin, the door swings open, causing me and Nikki to scramble to get ourselves off of each other, but it's too late.

Non-Operatively Managed Primary Small Bowel Volvulus: A Case Report by Ewnte B*

Abstract
Background: Primary small intestinal volvulus is one of the common causes of intestinal obstruction in various localities of the developing world. Although operative intervention has been the usual mode of treatment; this case report depicts meticulous follow-up & care, there is a possibility for relief of obstruction with non-operative management.
Case presentation: this is a case report of a 20-year-old male patient presented with crampy abdominal pain and frequent bilious vomiting. Plain abdominal film showed multiple distended small bowel loops with air fluid level, consistent with small bowel obstruction. Ruling out other etiologies primary small bowel volvulus was entertained and naso-gastric tube inserted, patient catheterized and kept nil per oral. After 48 hours of admission all symptoms resolved the patient resumed feeding and was discharged home.
Conclusions: The reported case shows evidence in which the patient’s primary small bowel volvulus was relieved non-operatively with insertion of naso gastric tube keeping nil per oral.

Introduction
Small bowel volvulus is a condition in which there is a torsion of all or a segment of the small bowel and its mesentery: this can lead to bowel obstruction, ischemia, infarction, or perforation. The typical patient with the primary volvulus of the small intestine was found to be a young adult, male, muscular, farmer, from a rural area whose diet was bulky and mainly made of cereals [1,2].
Case Presentation
A 20-year-old male patient presented to Nefas Mewcha primary hospital Emergency department in January 2020 with the main complaint of crampy abdominal pain and distention of 14 hours duration. Associated with this, he also had nausea and frequent bilious vomiting eight times. He had passed feces 24 hours ago. He had no fever, cough, chest pain or night sweating. He had no history of similar illness before, no history of previous abdominal surgery.
He is not married and claims to be not sexually active. Lives with his parents and has three sisters and two brothers. He makes a living as a farmer. There are no medical illnesses that run in the family. There was no history of tobacco smoking or substance abuse. He consumes a local alcohol made of sorghum occasionally.
At presentation, his blood pressure was 105/60 mm Hg, pulse rate was 68 Beats Per Minute (BPM), respiratory rate was 18 per minute and temperature was 36.2 oC axillary. Physical examination of the patient at presentation, the patient was acutely sick looking in pain; not in cardio respiratory distress. He had a dry tongue and buccal mucosa. No palpable lymph adenopathy in all accessible areas. Chest was clear and resonant. S1 and S2 cardiac sounds were well heard and there were no added cardiac sounds. Abdomen was slightly distended, moves with respiration, flanks were full, there were no scars, no distended veins and hernia sites were free. Palpation revealed a tense abdomen with no area of tenderness, no shifting dullness, hyperactive tympanic percussion note, bowel sounds were 35 per minute. There is scanty stool on the examining finger, with no blood on it from digital rectal examination, no palpable mass was detected. The patient was conscious and neurological examination was intact.
A complete blood count of our patient showed: White Blood Cells (WBC) 12500 mcL, Red Blood Cells (RBC) 4.6 mcL, Hemoglobin (HGB) 16 gm/dL, Hematocrit (HCT) 48%, Mean Corpuscular Volume (MCV) 89.1fL, platelets 470×103, creatinine 0.6, Blood Urea Nitrogen (BUN) 30, Alanine Aminotransferase (ALT) 28, Aspartate Aminotransferase (AST) 24, Alkaline Phosphatase (ALP) 48, albumin 4.3, total bilirubin 1.1 and direct bilirubin 0.4.
Plain abdominal X-Ray showed centrally distributed, distended small bowel loops and rectal gas shadow (Figure 1). CT scan is not available at this setup so it was not possible to do one.
Management and Outcomes
The diagnosis of acute abdomen secondary to small bowel obstruction secondary to primary small bowel volvulus plus stage I shock was entertained, Double intravenous line was inserted and Trans-urethral catheter inserted, Naso-gastric tube was inserted. Three Liters of normal saline was given over a course of 2 hours, at emergency department. The patient was admitted to the ward and was advised on the possible options of management, consented on conservative management, associated risks and the possibility of surgical intervention at any time in the course of the management. The patient was kept Nil per oral, put on maintenance fluid and replacement of ongoing losses. Nasogastric tubes produced 600 ml of bilious content during the first 6 hours; which was replaced with an equal amount of ringer lactate. The abdominal cramp subsided after 4 hours of inpatient admission. After 12 hours of admission, the Blood pressure was 100/70 mmHg, pulse rate 68 per minute and the abdominal distension decreased significantly and the bowel sounds were 26 per minute, there was no area of tenderness and the patient passed flatus.
Following 24 hours of admission, the patient passed feces and vital signs were within normal range. Naso-gastric was removed and the patient was initiated with sips. The patient tolerated sips very well and was observed for 24 more hours and discharged on the next day. He was appointed to the surgical referral clinic after a week.
In subsequent weeks, the patient was seen at a referral clinic; he had no change in bowel habit or any other complaint. His vital signs were stable and physical examination was detected with no abnormality. He has been followed every month for 3 consecutive months and has reported no recurrence of symptoms.
Discussion
Volvulus is the Latin word for rolled up or twisted and is derived from the verb ‘volvere’, meaning to roll or turnabout. By definition, volvulus is an abnormal twisting of the intestine, which can impair the blood flow to the intestine. Volvulus can lead to gangrene and death of that segment of the gastrointestinal tract, intestinal obstruction, perforation of the intestine and peritonitis [3]. Small intestinal volvulus in adults can be classified as primary or secondary. In the former there is no obvious anatomical cause involving the mesentery or the small bowel, whereas in the latter there is an abnormal fixation due to adhesions or bands leading to the twisting of the mesentery. The primary type is often seen in Africa and Asia [4]. It is a significant cause of primary bowel obstruction in sub-Saharan Africa [5]. It is the leading cause of small bowel obstruction in Ethiopia [6].It is a rare entity in Western adults [7].
Clinical signs & symptoms were unspecific & resembled intestinal obstruction [8]. The most frequent symptom was observed to be sudden abdominal pain [9]. Vomiting was also observed in most of the patients while abdominal distention and constipation were reported less frequently [10]. Clinical examination reveals abdominal distension and/or diffuse tenderness with or without signs of peritonitis [8]. Small bowel volvulus is a rare but life-threatening surgical emergency. Owing to its rarity, it is seldom entertained as a differential for small bowel obstruction [11].
One of the challenges in managing primary small bowel volvulus operatively has been the risk of subsequent adhesion obstructions. The risk of occurrence of adhesion obstructions among patients that underwent laparotomy in general was reported to be 4.6% [12]. This gives rise to the endless circle of obstruction and subsequent operation, which further increase the risk more.
This case report presents a case of primary small bowel volvulus causing small bowel obstruction; which was managed non-operatively. Treatment of primary volvulus has mostly been via surgical intervention. This report depicts with close follow-up and Naso-gastric tube decompression, primary small bowel volvulus can also be treated without surgical intervention.
In the course of managing patients with primary small bowel volvulus, spontaneous resolutions has been observed [3]. This is because of natural de-rotation of the volvulus segment and relief of the obstruction. The case reported presented with symptoms and signs of small bowel obstruction. The patient has frequent vomiting with severe abdominal cramp associated with mild abdominal distension. The vital signs were within normal range supporting the diagnosis of non-ischemic obstruction. Abdominal x-ray showed multiple air fluid levels, which confirmed the diagnosis of small bowel obstruction. Ruling out other causes and considering the epidemiological prevalence, primary small bowel volvulus was entertained as a cause of obstruction.
As per the request of the patient to be followed conservatively, the patient was managed non-operatively with insertion of naso gastric tube and keeping nil per oral. The patient responded well for the management and were discharged subsequently. Showed no recurrence during the follow-up period.
Conclusion
Primary Small bowel volvulus is a rare cause of small bowel obstruction. The reported case shows evidence in which the patient’s primary small bowel volvulus was relieved non-operatively with insertion of naso gastric tube keeping nil per oral.

Image caption Officials think the Comando Vermelho (known by its initials CV) gang may be behind the attacks Police in the north-eastern Brazilian city of Fortaleza on Monday escorted public buses after a series of attacks on public buildings and vehicles over the weekend. Buses and buildings were firebombed in the city and the surrounding state of Ceará in at least 10 incidents. The attacks may be in retaliation for a move to block mobile phones in prisons, local media report. But officials blamed a war between rival gangs for the attacks. In the state capital of Fortaleza, gunmen shot at the building housing Ceará's ministry of justice. Police shot three of the suspects dead but four others escaped, officials said. Shots were also fired at a special criminal court building in the state capital. Dozens of vehicles were set alight, including at least five buses in Fortaleza. In the town of Cascavel, south of Fotaleza, 50 vehicles were burned in a bus depot belonging to the infrastructure ministry. Six people have been arrested in connection with the attacks. Map of Brazil Local media said that the series of attacks started on Thursday when a post office was targeted. A threatening note was left in the post office saying that more attacks would follow unless state governor Camilo Santana dropped plans to block mobile signals in prisons. Prisons in Brazil are notoriously overcrowded and gang members serving time often not only control the prison itself but plan criminal actions on the outside via their mobile phones. Brazil prison riots: What's the cause? Moves to block signals completely in prisons have been suggested by a number of politicians to get the situation under control. In February, the Brazilian Senate approved the use of signal blocking devices in prisons across Brazil. But some officials said rivalry between two powerful gangs, Guardiðes do Estado (Guardians of the state) and Comando Vermelho (Red Command) was to blame for the attacks. Announcing the move, Home Affairs Minister Peter Dutton described Prakash as "a very dangerous individual". Intelligence officials say Melbourne-born Prakash was a recruiter for the Islamic State (IS) group and encouraged terrorist plots in Australia. He is currently being held in Turkey where he faces trial on terror-related charges. In July, a Turkish court ruled against extraditing him to face terrorism charges in his home country. In Australia, he faces charges of being member of a terrorist organisation as well as supporting and promoting IS. In a televised new conference, Mr Dutton said Prakash had been central to IS's efforts in the Middle East. He said he had been forced to make IS propaganda videos and that he had fled the group after seeing its "true face". Prakash, 27, left Australia for Syria in 2013, taking the name Abu Khaled al-Cambodi. He was mistakenly reported to have been killed in a US air strike in Mosul, Iraq, in 2015. Through his father, Prakash had joint Australian and Fijian citizenship. Under Australian law, a dual national can be stripped of their citizenship if they are convicted or suspected of terror offences. They just don’t want to be one. So when word got around this week about a petition asking that the scenic Northwest Angle be given back to Canada, it didn’t go over well. “We’re proud to be neighbors, but that is where we draw the line — figuratively and literally,” said Lisa Goulet, a fifth-generation “Angleite” who runs the Angle Outpost Resort with her husband and eight children near the Minnesota-Canada border. “We love being neighbors with Canada,” she said. “We have a U.S. and a Canadian flag on our porch. We started celebrating Boxing Day this year. “If they gave us to Canada, it would be very personal.” A petition was launched by an anonymous user Dec. 30 on the White House website, where a section called “We the People” offers citizens a chance to bring issues to the president’s attention. If a petition gets 100,000 signatures, the White House promises to respond. That doesn’t mean any action will be taken, only that the White House will pay attention. And with fewer than 1,000 signatures as of Wednesday, the Angle petition has a long way to go. Goulet, for one, is skeptical. “Have they really considered all the hoops we would have to jump through?” she said. “It just seems really unfeasible. On the other hand, anything that gets us in the news and spotlights our tourism is great.” The Northwest Angle, the little chunk of land that sticks out atop Minnesota’s northern border, comprises about 600 square miles, most of it — about 80 percent — water. Except in winter, when Lake of the Woods freezes over, the Angle can’t be reached on land without driving through Canada. About 120 people live there year-round, many of them involved in tourism. This isn’t the first time there’s been talk of making the Angle part of Canada. In the late 1990s, Angleites were upset when a fishing war broke out with their neighbors across the border. The province of Ontario declared that anglers fishing the Canadian portion of Lake of the Woods — comprising about two-thirds of the lake — couldn’t keep any fish unless they were staying at Canadian resorts. Minnesota resort owners howled, and U.S. Rep. Collin Peterson, a Democrat who represents the area, proposed a constitutional amendment giving Angleites the right to vote on secession. The issue was eventually settled when Ontario backed off its catch restriction. The recent petition claims that the Angle was created in error by a surveyor’s mistake more than 200 years ago, leaving it as the only land in the contiguous United States above the 49th parallel. “Make America great by correcting this critical survey error,” the petition reads. That proposal doesn’t sit well with the Red Lake Nation, which owns 80 percent of the Angle, held in trust by the federal government. “It belongs to our tribe. We don’t support that idea,” said tribal Chairman Darrell Seki Sr. Judy Risser, an Angle resident for 46 years, doesn’t think much of it, either. “I have nothing against [Canadians],” she said. “And who wants to go through a fight? “I believe I’m a U.S. citizen and would like to stay that way.” Still, love of their Canadian neighbors runs strong in the Angle, and at least one resident is willing to entertain the notion of swapping the Stars and Stripes for a maple leaf. “As long as I don’t have to move, I guess I wouldn’t mind being Canadian,” said Samantha Palmquist, who grew up in Anoka but moved to Oak Island four years ago to manage the Sunset Lodge. “There’s a lot of Canadians up here, and they’re stereotypically nice, mellow. He was also stripped of his refugee status. The government was supposed to deport Hamdan “as soon as possible,” but his Jordanian passport has expired and he has refused to fill out the paperwork required to obtain a new one. The latest attempt to elicit his co-operation occurred on Dec. 6, when a CBSA removals officer visited Hamdan at the Fraser Regional Correctional Centre, where he is being detained. The Fraser Regional Correctional Centre in Maple Ridge, B.C., where Othman Ayed Hamdan is being held. THE CANADIAN PRESS/Jonathan Hayward After speaking to his lawyer on the phone, Hamdan read a statement to the CBSA officer saying that contacting the Jordanian authorities was “premature and ill-advised.” As a result, the CBSA has asked its liaison officer in Amman to see if the Jordanian government will allow a deportation to take place using an expired Jordanian passport and national identity card. The CBSA has Hamdan’s identity card and two of his expired passports. The Nipigon River Bridge, which the Immigration and Refugee Board said Othman Ayed Hamdan had identified as an infrastructure target for his Facebook followers. Story continues below Hamdan arrived at the B.C. border from Washington State in 2002, and made a refugee claim based on what the CBSA now calls a fictitious story about converting to Christianity. In 2014, while living in Fort St. John, B.C., he began posting messages on Facebook that “identified infrastructure in Canada which could be targets for attack,” according to the IRB. He also “encouraged lone wolves in the West who could not travel abroad to carry out attacks at home,” the IRB said in its ruling, which found Hamdan had promoted the “social media agenda” of ISIS. While Hamdan was acquitted of terrorism charges, the Refugee Board ruled he was a danger to security and ordered his deportation. Hamdan has appealed the decision to the Federal Court. Laura Pedersen/National Post Email Twitter Facebook Pinterest Google+ Linkedin A Canadian man arrested for trespassing at the White House, telling police he wanted to give U.S. President Donald Trump two bottles of Crown Royal whisky and ask his help finding a wife, is being held in jail for a mental health evaluation. While the bizarre circumstances of the arrest of Yianny Georgopoulos ignited a wave of merriment and mockery on social media, a darker side emerges in court documents alleging a history of domestic violence, stalking and mental health issues. Georgopoulos made “spontaneous statements” and did not seem able to follow his attorney’s or the judge’s instructions during his brief court appearance following his bizarre arrest, causing concern for prosecutors. “Without understanding the defendant’s mental health, it is impossible to determine whether the defendant intends any actual harm to the President of the United States or any other (U.S. Secret Service) protectee,” Assistant U.S. Attorney Jolie Zimmerman wrote in a prosecutors’ request for a delay in releasing Georgopoulos prior to trial, filed Thursday afternoon. “Little is known about the defendant’s background and criminal history in Canada,” Zimmerman wrote. Little is known about the defendant’s background and criminal history in Canada “It’s certainly troubling that the defendant took the unusual steps of traveling from Canada to the District of Columbia, walking to the White House complex, physically moving a security barrier at an entrance to the White House, and attempting to gain access to the President.” Georgopoulos, whose age and hometown in Canada were not released, was arrested shortly after midnight — at 12:14 a.m., on Jan. 2 — making his case the first federal criminal complaint of the year in Washington, D.C. The location on the complaint makes it even more distinctive: 1600 Pennsylvania Ave. NW. Georgopoulos was confronted by a Secret Service agent at the White House after he approached a bicycle rack used as a security barrier to help protect the north fence line of the official residence and workplace of the U.S. president. The rack is marked with signs saying “No Trespassing Authorized Personnel Only” and “Restricted Area Do Not Enter,” according to Secret Service agent Brenton Brown’s sworn statement, filed in court. Ignoring the signs, Georgopoulos physically moved the barrier and stepped into the restricted area, according to the statement. Brown called for him to step back out of the White House grounds. He wanted help from the President to find a wife During a post-arrest interview with Secret Service agents, Georgopoulos claimed he traveled to Washington, D.C., from Canada “to speak with the President of the United States because he wanted help from the President to find a wife,” according to the Secret Service. He admitted he saw the no trespassing signs but ignored them and continued on his mission. He “admitted to having been arrested recently for ‘threatening family members,’” according to the sworn statement, and “admitted to having stalked his cousin.” He also said he didn’t know when he will return to his home in Canada, saying he “can’t go home,” but declined to reveal why, according to the agent’s statement. He was charged with entering a restricted building or ground, namely “the temporary residence of the President,” and unlawful entry to public property. Georgopoulos appeared in court in Washington, D.C., later that day. U.S. prosecutors requested Georgopoulos be held in custody without bond pending a forensic psychological evaluation. The motion was opposed by a public defender appointed for him. Georgopoulos’ behaviour in court made prosecutors even more concerned, noting that the hours between the alleged offence and the hearing meant his unusual actions were unlikely to be attributable to alcohol or drug use. Court filings say Georgopoulos self-reported being “bipolar.” It’s certainly troubling that the defendant took the unusual steps of traveling from Canada to the District of Columbia, walking to the White House complex, physically moving a security barrier at an entrance to the White House, and attempting to gain access to the President “This court needs to understand the defendant’s current mental state to identify whether there are any conditions of release that can reasonably assure the safety of the community and the defendant’s appearance in court,” prosecutors argued. Judge Deborah Robinson had previously agreed Georgopoulos should undergo a mental health evaluation by the District of Colombia Department of Behavioral Health. She ordered that Georgopoulos remain in custody until Friday for the evaluation. However, the earliest appointment for a psychiatric evaluation is Monday, prompting the prosecutor’s request for the delay. Georgopoulos’s lawyer has not yet had a chance to respond to the government’s delay motion. Consular notice to the Canadian embassy was given by the U.S. government, according to court records. Richard Walker, a spokesman for Global Affairs Canada, said the department “is aware of a case involving a Canadian citizen detained in Washington, D.C. Non-Operatively Managed Primary Small Bowel Volvulus: A Case Report by Ewnte B*

Abstract
Background: Primary small intestinal volvulus is one of the common causes of intestinal obstruction in various localities of the developing world. Although operative intervention has been the usual mode of treatment; this case report depicts meticulous follow-up & care, there is a possibility for relief of obstruction with non-operative management.
Case presentation: this is a case report of a 20-year-old male patient presented with crampy abdominal pain and frequent bilious vomiting. Plain abdominal film showed multiple distended small bowel loops with air fluid level, consistent with small bowel obstruction. Ruling out other etiologies primary small bowel volvulus was entertained and naso-gastric tube inserted, patient catheterized and kept nil per oral. After 48 hours of admission all symptoms resolved the patient resumed feeding and was discharged home.
Conclusions: The reported case shows evidence in which the patient’s primary small bowel volvulus was relieved non-operatively with insertion of naso gastric tube keeping nil per oral.

Introduction
Small bowel volvulus is a condition in which there is a torsion of all or a segment of the small bowel and its mesentery: this can lead to bowel obstruction, ischemia, infarction, or perforation. The typical patient with the primary volvulus of the small intestine was found to be a young adult, male, muscular, farmer, from a rural area whose diet was bulky and mainly made of cereals [1,2].
Case Presentation
A 20-year-old male patient presented to Nefas Mewcha primary hospital Emergency department in January 2020 with the main complaint of crampy abdominal pain and distention of 14 hours duration. Associated with this, he also had nausea and frequent bilious vomiting eight times. He had passed feces 24 hours ago. He had no fever, cough, chest pain or night sweating. He had no history of similar illness before, no history of previous abdominal surgery.
He is not married and claims to be not sexually active. Lives with his parents and has three sisters and two brothers. He makes a living as a farmer. There are no medical illnesses that run in the family. There was no history of tobacco smoking or substance abuse. He consumes a local alcohol made of sorghum occasionally.
At presentation, his blood pressure was 105/60 mm Hg, pulse rate was 68 Beats Per Minute (BPM), respiratory rate was 18 per minute and temperature was 36.2 oC axillary. Physical examination of the patient at presentation, the patient was acutely sick looking in pain; not in cardio respiratory distress. He had a dry tongue and buccal mucosa. No palpable lymph adenopathy in all accessible areas. Chest was clear and resonant. S1 and S2 cardiac sounds were well heard and there were no added cardiac sounds. Abdomen was slightly distended, moves with respiration, flanks were full, there were no scars, no distended veins and hernia sites were free. Palpation revealed a tense abdomen with no area of tenderness, no shifting dullness, hyperactive tympanic percussion note, bowel sounds were 35 per minute. There is scanty stool on the examining finger, with no blood on it from digital rectal examination, no palpable mass was detected. The patient was conscious and neurological examination was intact.
A complete blood count of our patient showed: White Blood Cells (WBC) 12500 mcL, Red Blood Cells (RBC) 4.6 mcL, Hemoglobin (HGB) 16 gm/dL, Hematocrit (HCT) 48%, Mean Corpuscular Volume (MCV) 89.1fL, platelets 470×103, creatinine 0.6, Blood Urea Nitrogen (BUN) 30, Alanine Aminotransferase (ALT) 28, Aspartate Aminotransferase (AST) 24, Alkaline Phosphatase (ALP) 48, albumin 4.3, total bilirubin 1.1 and direct bilirubin 0.4.
Plain abdominal X-Ray showed centrally distributed, distended small bowel loops and rectal gas shadow (Figure 1). CT scan is not available at this setup so it was not possible to do one.
Management and Outcomes
The diagnosis of acute abdomen secondary to small bowel obstruction secondary to primary small bowel volvulus plus stage I shock was entertained, Double intravenous line was inserted and Trans-urethral catheter inserted, Naso-gastric tube was inserted. Three Liters of normal saline was given over a course of 2 hours, at emergency department. The patient was admitted to the ward and was advised on the possible options of management, consented on conservative management, associated risks and the possibility of surgical intervention at any time in the course of the management. The patient was kept Nil per oral, put on maintenance fluid and replacement of ongoing losses. Nasogastric tubes produced 600 ml of bilious content during the first 6 hours; which was replaced with an equal amount of ringer lactate. The abdominal cramp subsided after 4 hours of inpatient admission. After 12 hours of admission, the Blood pressure was 100/70 mmHg, pulse rate 68 per minute and the abdominal distension decreased significantly and the bowel sounds were 26 per minute, there was no area of tenderness and the patient passed flatus.
Following 24 hours of admission, the patient passed feces and vital signs were within normal range. Naso-gastric was removed and the patient was initiated with sips. The patient tolerated sips very well and was observed for 24 more hours and discharged on the next day. He was appointed to the surgical referral clinic after a week.
In subsequent weeks, the patient was seen at a referral clinic; he had no change in bowel habit or any other complaint. His vital signs were stable and physical examination was detected with no abnormality. He has been followed every month for 3 consecutive months and has reported no recurrence of symptoms.
Discussion
Volvulus is the Latin word for rolled up or twisted and is derived from the verb ‘volvere’, meaning to roll or turnabout. By definition, volvulus is an abnormal twisting of the intestine, which can impair the blood flow to the intestine. Volvulus can lead to gangrene and death of that segment of the gastrointestinal tract, intestinal obstruction, perforation of the intestine and peritonitis [3]. Small intestinal volvulus in adults can be classified as primary or secondary. In the former there is no obvious anatomical cause involving the mesentery or the small bowel, whereas in the latter there is an abnormal fixation due to adhesions or bands leading to the twisting of the mesentery. The primary type is often seen in Africa and Asia [4]. It is a significant cause of primary bowel obstruction in sub-Saharan Africa [5]. It is the leading cause of small bowel obstruction in Ethiopia [6].It is a rare entity in Western adults [7].
Clinical signs & symptoms were unspecific & resembled intestinal obstruction [8]. The most frequent symptom was observed to be sudden abdominal pain [9]. Vomiting was also observed in most of the patients while abdominal distention and constipation were reported less frequently [10]. Clinical examination reveals abdominal distension and/or diffuse tenderness with or without signs of peritonitis [8]. Small bowel volvulus is a rare but life-threatening surgical emergency. Owing to its rarity, it is seldom entertained as a differential for small bowel obstruction [11].
One of the challenges in managing primary small bowel volvulus operatively has been the risk of subsequent adhesion obstructions. The risk of occurrence of adhesion obstructions among patients that underwent laparotomy in general was reported to be 4.6% [12]. This gives rise to the endless circle of obstruction and subsequent operation, which further increase the risk more.
This case report presents a case of primary small bowel volvulus causing small bowel obstruction; which was managed non-operatively. Treatment of primary volvulus has mostly been via surgical intervention. This report depicts with close follow-up and Naso-gastric tube decompression, primary small bowel volvulus can also be treated without surgical intervention.
In the course of managing patients with primary small bowel volvulus, spontaneous resolutions has been observed [3]. This is because of natural de-rotation of the volvulus segment and relief of the obstruction. The case reported presented with symptoms and signs of small bowel obstruction. The patient has frequent vomiting with severe abdominal cramp associated with mild abdominal distension. The vital signs were within normal range supporting the diagnosis of non-ischemic obstruction. Abdominal x-ray showed multiple air fluid levels, which confirmed the diagnosis of small bowel obstruction. Ruling out other causes and considering the epidemiological prevalence, primary small bowel volvulus was entertained as a cause of obstruction.
As per the request of the patient to be followed conservatively, the patient was managed non-operatively with insertion of naso gastric tube and keeping nil per oral. The patient responded well for the management and were discharged subsequently. Showed no recurrence during the follow-up period.
Conclusion
Primary Small bowel volvulus is a rare cause of small bowel obstruction. The reported case shows evidence in which the patient’s primary small bowel volvulus was relieved non-operatively with insertion of naso gastric tube keeping nil per oral.

Summary:  The compromised wormhole was ripping apart at the seams, sending everyone spiraling away in completely different directions. Lance could feel himself being pulled and bent in ways he was definitely not supposed to be. -- Stitching the team back together after everyone is separated is difficult, and for one Paladin rescue wouldn't be coming for a very long time.
Comments:  Ouch my heart. Klangst, feat. the Blue Lion. This one does not go where you expect it to, and I love this fic for that unpredictable-ness.
