People honestly portray Tuvok as far too "rolling his eyes, reluctantly going along with Janeway's silly little shenanigans" - he's literally so serious about being right there with her on every decision she makes. Janeway's like "I'm going to stay behind if the ship blows up" and Tuvok's like "I'm staying with you." Janeway's like "I'm going to deliver every member of the Equinox crew into the jaws of death via an alien revenge massacre" and Tuvok protests a grand total of one time before being fully on the bridge assisting her. He was the only one with her when she made the decision to honor the caretaker's wishes and save the Ocampa, dooming them all. He was willing to get court marshalled in order to fulfill a wish she couldn't grant by her own hand: Get them home [no matter what happens to me] <- wherein 'me' is Tuvok. This was the same wish that spurred him forward when he had to leave her on that planet and everyone left thought him cold for trying to fulfill it without her when in his mind it was akin to a dying wish, the last thing she'd ever express to him: Get them home [no matter what happens to me.] <- wherein 'me' is Janeway. He told Seven that the golden rule to follow is that the captain is "ALWAYS RIGHT" <- (His ACTUAL words) and when Seven asks if the captain should be followed even if someone KNOWS she's wrong he says "Perhaps." This man is perhaps the most ride or die dude in the universe about Janeway. Despite her labeling him her 'moral compass' he is by NO means impartial or unbiased. He'd defend her to his last breath. He canonically makes detailed psychological observations about her and has for years. He accounts for her luck when calculating the success of certain plans. It's implied in 'Twisted' that Janeway typically listens to Tuvok's suggestions and follows them nearly without fail - to the point that he's surprised and obviously irritated when Chakotay doesn't. Despite this they've been inside one another's quarters so infrequently that Tuvok can remember each instance. They call each other "Captain" and "Mr. Tuvok" even though they've known each other for twenty years. There's something wrong with them.

Okay so the vision for this sketch is, Tuvok and Kathryn have been hanging out in Kathryn's quarters. They've been in the Delta Quadrant for a while now. Tuvok is of course still all official in his uniform, but Kathryn is just wearing her undershirt and some sweatpants or whatever. She doesn't care, it's Tuvok. He lets her complain to him about everything petty that's bothering her (something she never gets to do because she's everyone's boss). She gets a bit tipsy and he has to hold her on her way to bed. She's still complaining.

Summary:

An Injured Janeway is found by Tuvok and Chakotay.

Preview of story:

Tuvok finds Janeway in the woods, leaning against a tree. Her eyes are half-lidded, and not quite there. She’s putting most of her weight on the single arm braced against the large tree beside her. Its dark bark digs into her skin, leaving her arm red and black. Her clothes are torn in several places, a large chunk of her shirt hangs, barely against her side. Even if he could not see all the blood on her, Tuvok would have smelled it, but alas the image of her, drenched in blood, sweat, and other materials he can only guess at without a full scan makes his stomach harden as his heart beats furiously at the flesh separating the two organs.

He’s beside her before Chakotay can even get her name out of his mouth. He loops one hand around the small of her back, as he cradles the other beneath her arm, guiding her away from the tree and keeping her feet in front of his. Janeway all but falls into his hold. Her head leans back. She grins as their eyes meet. “Tuvok.” She whispers, clearly relieved.

“Kathryn!” Chakotay calls out again, and Tuvok’s hold on her tightens ever so slightly. She does not like to be referred to by her name when in command, but clearly she and the first officer had reached some sort of agreement. That must be the reason why she smiles at Chakotay instead of reprimanding him. Janeway likes her rules, and agreements. Her clearly set codes, and guides to life, and relationships. Tuvok understands the ease of having clearly set expectations to meet.

There are rules in place for this scenario too, so Tuvok follows them. He doesn’t flinch away as Chakotay moves to help him get Captain Janeway back to the shuttle. Nor does he argue when Chakotay chooses to lift Janeway and rests her on a bench seat in the back of the shuttle, before moving to the pilot’s seat. Tuvok returns to work.
