Writing Notes: Antipsychiatry
The term “antipsychiatry” originated in the 1960s to describe a broad-based movement that questioned the legitimacy of standard psychiatric theory and practice.
The movement specifically challenged the validity of psychiatric categories, diagnostic practices, and common forms of treatment.
The antipsychiatry movement was motivated by anger at the perceived arbitrariness of psychiatric diagnostic practice as well as outrage at the apparent inhumanity of certain treatments, such as electroconvulsive therapy and long-term involuntary hospitalization.
The original antipsychiatry movement was led by psychiatrists, many of whom resented the label “antipsychiatry” and insisted they wanted reform rather than revolution within the discipline.
The movement began as an understandable reaction to various missteps of psychiatry, such as the over-medicalization of mental health, the inhumane management of asylum care, and the inappropriate pathologizing of minority groups.
In the early days of the movement, it might be said that "anti-psychiatry" helped psychiatry to self-correct in a way that moved the discipline towards a more scientific endeavor reliant on empiric evidence, while maintaining a sharp focus on the interaction between biological, psychological, and social contributions to mental health and illness.
More recently, however, the anti-psychiatry movement has lost its way.
It has transformed from a predominantly academic and political movement to one of consumer groups, akin to the anti-vaccine saga.
In its current form, anti-psychiatry exists as a disorganized entity outside of mainstream medicine, largely propagated on social media and in non-peer-reviewed sources like newspaper opinion articles, books, and blogs that evade scientific dialogue and critique.
Modern anti-psychiatry is not a monolith, yet there are common themes that could be harmful to patients.
Different from helpful scientific criticism, it often takes the dangerous form of disinformation that aims to tear down the discipline and deter treatment-seeking.
Those who espouse anti-psychiatry ideology often lay charges against the very existence of psychiatric disorders and the wholescale efficacy of psychiatric medication.
While the late psychiatrists Thomas Szasz and R.D. Laing remain heroes of the movement, academic critics of psychiatry today are often psychologists with limited experience treating severe mental illness, involvement in medication management, or collaborating with psychiatrists.
A tragedy of anti-psychiatry is that its merit is lost in its extremism.
Refuting Common Tropes of Anti-Psychiatry
"Psychiatry still promotes the 'chemical imbalance' hypothesis to explain mental illness."
Not really.
The "chemical imbalance" explanation was an early and incorrect way to make sense of the effectiveness of medications that alter neurotransmitters like dopamine or serotonin in the brain, whereas in reality the biochemical mechanisms of mental illness and how treatments work are still being researched.
"It is not ethical to use a medication if one doesn't know the mechanism of action."
This has never been a principle of medicine.
Efficacy and safety are routinely established long before full mechanisms are understood and they guide future understanding of pathophysiology.
"Psychiatric medications are harmful."
All medical interventions can cause harm, including surgery, medications, psychotherapy, and recommendations to exercise.
In medicine, interventions must be understood and explained to patients in terms of "risk-benefit" relative to prognosis without treatment.
This is the foundation of informed consent.
"Psychiatric medications don't work."
There are a range of different psychiatric medications used to treat different conditions, each with varying levels of efficacy compared to placebo.
"Psychiatric diagnoses are made by checklist."
Sound diagnosis requires thorough investigation, interview, collateral history, objective examination, and sometimes "ruling out" medical disorders and other psychiatric issues.
Bonus side trope debunking: the DSM (Diagnostic and Statistical Manual of Mental Disorders) is not the "Bible" of psychiatry -- it is more of an imperfect "rough guide" that continues to be revised.
"Psychiatrists practice 'biological psychiatry.'"
Modern psychiatry is a medicine subspecialty that seeks to understand mental illness based on the integration of biological, psychological, and social aspects of one's life.
In this respect, psychiatry was ahead of most medical fields in understanding the impact of psychology and social factors on health.
"Psychiatrists seek to 'medicalize normal.'"
The boundary between health and disease is "fuzzy" across all of medicine, where the treatment of disease overlaps with health promotion.
Generally speaking, psychiatry focuses on emotional suffering and impairments to functioning, whether it is an "expected reaction" or not.
"Mental illness doesn't exist."
The definitions and boundaries of particular mental disorders fluctuate over time, reflecting our shifting understanding of the biological, psychological, and social phenomena that account for them.
Mental disorders can best be understood as symptom clusters that co-occur in patterns that can be recognized and identified with good "inter-rater reliability" in order to guide evidence-based treatment approaches.
Psychiatry is far from a perfect science and must continue to evolve and improve over time and in response to criticism.

Maybe two days later, around the same time as the last answer, Sunny got pinged again.
"Are you ready to say sorry to her?"
"Is–"
Sunny ended the contact and didn't touch it when she tried again.

"Guardian."
The way the Titan froze made Thomas angry. She never told him the complete story, but he knew it was ugly, and it took ages for her to finally stop sneaking around the apartment just because she felt too big and obtrusive.
And here this asshole was starting her back up again. It was the first time in a very long time Thomas wanted to fight with an authority figure.
The speaker made a choking electronic noise then stopped and the room fell silent.
"I do not appreciate this," said Rex through the intercom. "I have allowed it enough, and I am disallowing it now."
And with that, the intercom turned off entirely.
Thomas watched it briefly, then settled into his seat with a small smile. "He's such an asshole." He glanced up at Freija, who was moving her lips slightly as she recited a poem. Her eyes flooded with tears but when she blinked them free, they were gone. "You okay?"
"I just wasn't ready," she replied, voice steady and low. "I'm okay."

"That is my Guardian!"
Caiatl didn't realize the Ghosts could transmat themselves so far, nor did she realize that Sunny could so easily trace the signal back to the smaller cruiser she traveled in.
She also didn't know what to do now because she had been counting on the incorrect information.
"I know it–"
"Stay away from her!"
The little Ghost's petals flailed with rage and the sharp jumping motions worried Caiatl for the ceiling.
"I did not–"
"It wasn't an apology! Stay away!'
Caiatl watched Sunny silently, hardly breathing. The Ghost stopped moving to stare back, petals still going like mad.
"I–"
"I don't want to hear any more excuses. Say it right," snapped Sunny. "I shouldn't even have to have this conversation with you."
Caiatl opened her mouth, but took several seconds to say, "I have been a bad friend. I hurt your Guardian far worse than intended. I have failed you."
"That's. Still. Not. Right," snapped Sunny. "So you can just pout up here and tell yourself that my safety is worth my Guardian, because it isn't."
"I understand you feel–"
"She is my purpose! You asshole! My entire existence has always been dedicated to my Guardian! Before I knew her! Before she was born! Before her people existed! I live for her and I would die for her! In fact– I did! I died for everyone! And my last thought was of her! I love her more than anything else in my entire life! I missed her when I was dead! And you hurt her!"
"Sunny…"
"She gave me that name! You have no right to use it!"
Caiatl leaned back as if struck.
"So you can tell her you're sorry or you can never call my name again!"
Sunny gave no more chance for argument, disappearing in an instant with a small shower of Light.

Sunny 'matted so close that Freija didn't have to move to grab her out of the air, and the Ghost let the warm hand press her over the pounding heart.
"Rex call you?" Thomas asked.
"I called him," she said. "I needed to find her and I knew he was already tuned in from spying on you."
"He handled it in true Rex fashion."
"Declaring what Is True as deemed by his highness?" Sunny asked. "I mean that in the most appreciative way."
"Even better, he made it arbitrary. No logic, no explanations. Just, okay, had enough. So you went to confront her?"
"I told her she's not allowed to call my name until she says she's sorry," growled Sunny. "I'm not okay with this, this wasn't okay, and she will not treat my Guardian this way."
Freija hadn't moved the entire conversation, except for her lips repeating the poem.
"Freija."
"I'm okay," she lied, giving Sunny a light squeeze. "I just wasn't ready. I'm getting there. It's okay. I'm okay. I'm at home, looking at a magazine, looking for the best… solar… scout…" she mumbled, slowly finding her way back to the thing on her lap. "The newest one handles nice but the single reload," she mumbled aloud, obviously trying to distract herself.
"I think you can find Dual Loader on that one," supplied Thomas. "Does that interfere with Incandescent? That's what you were looking for, right?"

Hello you! (I say specifically hello to you, as I know I've had a few in my inbox from you, and while I'll probably not go back and reply to them all, I want you to know I appreciate your check-ins!) but also hello to anyone else checking in on me.
Yes, I will be coming back. I'm aware it's been a hot minute, and trust me, I hate that as much as anyone else. Life is just...hectic as life can be. But I'm here, I'm still plugging away and I will be coming back. Last night I wrapped up in like 4 layers because I can't afford gas prices to heat my house and wrote like another 1k of chapter 11 and then I nearly cried because it's not done yet.
I don't know if that's positive news or not, but at least it means I'm not going anywhere until it's bloody done.

Thomas nods and hmms and considers until Adams throws out an offhanded better to be spending time on this than on your poor, sinking French ship anyway and Thomas hates his own instinctive reaction to the obvious bait, but he just can’t help it. “Sinking?” he asks, too-sharp and too-defensive even to his own ears. “I don’t know what you’ve heard, but that ship is sailing perfectly well, and damn near watertight, actually. Hamilton’s even on board.” Adams laughs, except it’s neither genial nor kind; it’s pitying and patronizing and it sets Thomas’s teeth on edge, and the way he leans forward eagerly, something flashing suddenly in his eyes feels like this is the crux of the matter; like like he’s started this entire damn conversation with the sole purpose of delivering this information like a prize in a cereal box full of bullshit that Thomas has had to sift through to get to. “Thomas, my boy,” he snorts, smugness dripping from every word. “I’ve heard directly from a reliable source that Hamilton’s already put the rusty nail in that coffin. He’s told George directly that it’s not viable. You’ve already been sold down the river.” Thomas is shaking his head before John is even finished, denial on his tongue, because he’s fucking sick of being spoken to like he doesn’t have a clue and that’s not how it is. “No. I don’t know where you heard that, but your source is incorrect. I’ve spoken to Hamilton.” Adams blinks at him for a second before his eyes narrow slightly, and he scoffs. “You can’t be serious, Thomas. You know fine well that you can’t trust him-”

Not as many as I'd like between bookclub and finally having access to the Tristamp dub! I do try to remember (and often forget) to bookmark my favs on AO3, but tonight's lineup has been really fun so let me grab them for you:
kinder, gentler by skittidyne (VW, WIP, been my favorite WIP since Tristamp was airing, can't recommend enough)
Vash goes through another life. He's done it enough to be pretty good at it, these days. They're getting easier, even if "easier" means a string of murders while trying to make friends anew. Not his usual bullet-ridden trouble, but this is meant to be a kinder, gentler life, after all.

I (30M) keep getting into fights with a cook at Waffle House. --- "Uh, hey, 'scuse me?" Nick calls out toward the kitchen. "Sorry?" the cook asks. He comes over, and Nick gets a good look at spiky blonde hair and fathomless blue eyes framed by lashes like clock springs. "Was there something wrong?" And okay. So Nick might have a thing for a tall blonde with a dancer's figure. A man likes what he likes. With an effort he usually reserves for life-and-death scenarios, he forces himself to smile and says, "Is this your first day?" "No? Why do you ask?" "I asked for my eggs over easy." The cook glances at Nick's table, now holding six different eggs cooked three different, incorrect ways, and Nick can see in his eyes the exact moment he decides the next thing he'll say. His brow furrows into a faux quizzical expression. "I don't see the problem," he answers.

His tongue stumbles on a metal buckle, but his teeth feel only the satisfying give of worn leather. And, if he presses deeper, skin. ______________ or, Wolfwood goes a bit too feral and gets telepathically poetry blasted

How right he had been, but also oh so wrong. The other man had never spoken to him in such a manner before; he would never have tolerated it. His own tongue would have cut back with sharp barbs. But none come. He stands there, his life dissected and spat on. He’d been a boy of fifteen when he’d ran from his village, swearing he’d lie, cheat, and steal so he’d never have to live that simpleton life. He could not live like that, to forsake his potential so, not even in his fury; thus he had become a diplomat for the enemy. Years of his life that had felt like an eternity, swallowed in grief and spite. Days and nights that he'd hid what he was from the world. But he could never hide from him, could he? The other’s back was turned, but it did little to soften the onslaught, or to change the gravity of the secret now revealed to him. That, of everything, was the most concerning. Like a wolf who had finally shown himself. But this victory, of being right, did not bring what he thought it would. He had gained no upper hand, but instead felt even more behind. He flinches slightly when the other took a step forward towards him. Whatever he thought he knew of the other was incorrect; or the small pieces of this lifetime were inconsequential. I would not have chosen you. It felt as painful as when his father had kicked him while he was the ground. How he had to tell himself: get up. He should have the right to yell at the other that he'd lied to him, but all he was left with was: "I gained the truth," he finally spoke, wondering how long the other would have kept this from him if he'd not been so resistant. Would he ever have told him? The light in his hand flickered, and then died. He takes a step forward, towards a man who he is now sure has told him more lies than truths. But this criticism now makes him feel shattered, as if the jagged edges of a mirror have stabbed him. He reaches out, hand placed on the other's chest, where a heart should be: is it black, he wonders? Does it beat like everyone else's? "If we are one and the same, then you are as much a coward as I am," he says softly, but not harshly. Did he understand, truly, as much as he said he did? That trust came hard, and that he abhorred liars above all? Thomas had been on the defensive, but he had been honest in his push-back. While the other had kept so much from him. "If you wish my trust, you must give me yours too. You know who I am, but I do not know who you are."
“ How hypocritical. You look down on me like we are not one and the same. ” A sneer as he turns away unable to look at the other man and hear the same insults over and over again. They had been over this, he could not let him lead his secluded little life. “ And that would be what you want? To live forever in that hut of yours? Visiting a grave for all eternity? ” A laugh falls out of him, it’s cold and jarring, there is no humor or joy there, but accusations. “ I’ve spent centuries, life after life making sure that people like us can exist, have a place in this country. I have been fighting, all by myself, and waiting for someone like you. There is no other life for you sun summoner. ”
Spits the words out, back still turned. “ Thomas, you wish to be left alone, let the world continue spinning, to hell with it all. Because if you don’t, if you have to make choices, if you have to make decisions. You might get your own hands dirty. And then what will you do? How will you be able to judge others so easily from that pedestal you’ve placed yourself on? Do not deny it! ” Aleksander moves a step towards him but keeps his distance. At this point, he wishes nothing but to turn around and walk away. Why did he have to, on top of all that lies on his shoulder, also beg him? It’s not like he’s asking for much. Just to accept the role that life had given him.
“ You will not die of age, I’ve told you this. I have lived for centuries, that is not our path. And this king…needs to be the last. This family has done nothing but destroy Ravka, deplete the country of its resources, spend recklessly, and make horrid political moves. The king is useless and so are his sons. Even the peasants in the villages know this, I’d like to think that you are smarter than they are. ” Taking a breath. “ I am not sure what you gain with your little pretense. To paint me as this monster, like I’ve trapped you. As if I had the power to make you into what you are. Believe me, if I did. I would not have chosen you. I have no interest in your saints and gods, those have died a million times over in the camps and trenches of our borders, along the soldiers. ”
