Female Athlete Triad
A condition observed in physically active females due to energy imbalances.
Energy imbalances may be a result of inadequate nutrient intake and excessive exercise energy expenditure.
The effects of this condition may occur even if one does not experience all components of this triad simultaneously.
Consequences may not be completely reversible.
Athletes most susceptible are those participating in sports emphasizing a low body weight and lean physique (ie, gymnastics, runners and dancers).
Lowe Energy Availability
Disordered eating has been defined as abnormal eating behaviours which range from restrictive dieting to clinical eating disorders.
Restrictive eating may be a result of:
Inadequate knowledge of proper nutrition
Not setting time aside to ensure caloric needs are met
Not having an adequate appetite to meet energy expenditure
Intentional restriction in attempts to be leaner
Menstrual Disturbance
Disturbances can range from infrequent menstrual cycles to primary or secondary amenorrhea.
Primary amenorrhea is the absence of menstruation past the age of 15.
Secondary amenorrhea is when an individual misses their menstrual cycle for 3 or more months or have menstruation less than 3 times a year.
Anovulatory (no ovulation/release of the egg) bleeding patterns (ie. irregular or absent bleeding) are important components of the disorder.
Low Bone Mineral Density
Hypothalamic amenorrhea is the absence of menstruation for more than several months, as the result of abnormal releases of the gonadotropin-releasing hormone.
Can lead to anovulation and infertility.
Can result in osteoporosis due to reduced circulating estrogen levels.
Estrogen plays a critical role in bone formation and resorption.

New podcast appearance with Emma and Mackenzie @abcsofmatrescence ♥️♥️♥️ . We start with our highs and lows of the week (from before the pandemic sooooo... Life as usual) and then chat about #hypothalamicamenorrhea (#HA) and #recovery ... And then I didn't get a chance to post when the episode came out because all of a sudden I have three children home with me.... Not complaining, it's actually been easier than I anticipated, but me time is practically non-existent (especially with my 7yo not going to sleep until 11 freaking 15 tonight!!!)... . Anyway, both Emma and Mackenzie are in my FB support groups, so that was a nice connecting point... . Anyway, hope you listen and enjoy.. and if you're a new mom or trying to get pregnant, check out some of their other episodes! . #pregnantafterha #HArecovery #newmom #motherhood #HAwarrior #getyourperiodback #amenorrhea #amenorrhearecovery #npnw #npnw_podcast #noperiod #noperiodnowwhat #femaleathletetriad #triad #femaleathletetriadrecovery #relativeenergydeficiencyinsport #REDS #redsrecovery https://www.instagram.com/p/B93PcZ5FdXP/?igshid=1u4ut8ffeih33

I find I keep referring people to this, so I thought I'd post it again!♥️ #Repost @noperiodnowwhat (@get_repost) ・・・ Evidence to support why cutting out high intensity exercise for the time being can be beneficial to restoring menstrual cycles is provided in No Period. Now What? Chapter 12, "Exercise Changes". . I have gotten some pushback from people on this question, however, and they point in particular to a very elegant and well designed study by Anne Loucks et al. that showed no alteration in reproductive hormones based on exercise stress alone. **However one large caveat to this study is it was performed in normally menstruating women and therefore, IMHO, does not adequately represent the effect of exercise on someone who already has HA.** . My view is that high intensity exercise increases cortisol (that is documented in the medical literature) which may not be sufficient to shut down the hypothalamus or affect reproductive hormones on its own, but I suspect that for a hypothalamus that is already suppressed, the cortisol is then sufficient to continue the suppression even in the absence of underfueling. . I have done two surveys now (albeit retrospective and self reported) where I find a strong correlation between period recovery and cessation of high intensity exercise. . A study by Hill et al. looked at cortisol and other stress hormone levels in people at various levels of exercise - none, 40% of max, 60% of max, and 80% of max. They found no changes in stress hormones with no exercise or at 40% of max intensity. Increases in cortisol were seen in some people at 60% of max intensity (average increase in cortisol of 40%), and in every person at 80% of max (average increase in cortisol of 83%). . Therefore, 🌹I recommend keeping your heartrate at around 100bpm or less while working to recover from HA🌹. This is about 50% of max in most people, which should not lead to  much in the way of stress hormone changes. I used to say 140bpm, but when wearing a heart rate tracker recently, I realized that this is about 70% of max which I strongly suspect is too much. . Thoughts? . #hypothalamicamenorrhea #femaleathletetriad #RelativeEnergyDeficiencyInSport #HA https://www.instagram.com/p/BzL0cAnF9hI/?igshid=vddsudvgpwf7

The time when I wanted desperately to be pregnant but was not able to get there was one of the darkest of my life. Before we had started trying, I always thought I would be okay if I didn't have kids, I would focus on other things - but that was before I realized that might be a reality. When I was staring a childless existence in the face due to my missing menstrual cycle and four failed rounds of injections, it became a whole different story. I realized how many of my plans for the future revolved around kids, grandkids, and growing old enjoying my family. While those four BIG FAT NEGATIVE failed cycles sucked, I was lucky enough to get pregnant shortly therafter, while waiting to do IVF. That experience led me to where I am today, with my life's work in helping other women to recover their missing periods and get pregnant. I have helped thousands of women get their periods back, and played a part in the births of hundreds of babies. It is so much more rewarding than anything else I've done in my life. For #nationalinfertilityawareness week, please know that if you're struggling to get pregnant, you are NOT alone. And I would love for you to reach out and chat with me about how I can help YOU achieve the family you want. This is a great depiction of what i see as the best part of recovering from hypothalamic amenorrhea... Not just getting your period back, but the freedom that comes along with learning that you do not have to micromanage your food intake to be healthy and happy, you do not have to exercise for hours a day, that there is so much more to you than what you look like, how fast you can run, or how much you can bench. ♥️ #Repost @sunnythymesblog (@get_repost) via @food_freedom_coach ・・・ there's better ways to spend your life than trying to have the perfect body or perfect diet👏🏼 what are you passionate about? what lights up your soul 💫? is obsessing over food and your body living your best life or do you think there's a greater purpose for you in this world 🌎? what else would you have space for if you let go of trying to have the perfect body or diet 🤔? #hypothalamicamenorrhearecovery #amenorrhea #hypothalamicamenorrhea #noperiod #npnw #femaleathletetriad #RelativeEnergyDeficiencyInSport #dietitian #nondiet #intuitiveeating #orthorexia #missingcycle https://www.instagram.com/p/Bv-MZLGlP_5/?utm_source=ig_tumblr_share&igshid=9pqgd11x75lp

There aren't many things around recovery of missing periods that I will give a black and white, yes/no answers on. But intermittent fasting is one of them. It is NOT helpful for recovering missing periods, and could very well be a reason you lost your period, even while consuming overall what seems to be enough calories. . A study done in Sweden a few years ago (lots more info in this blog post, http://noperiod.info/energy-balance) found that among elite athletes all consuming about 3500 calories per day and burning ~1000 calories, some had their periods and others didn't. When they looked more closely at how food and exercise were distributed through the day, the researchers found that the athletes without periods were in an energy deficit for on average four hours more per day than those who had periods. . Where energy deficit is the major driver of hypothalamic amenorrhea for most of us, if you're not spreading your food through the day, you are still not giving your hypothalamus the signals it needs that there is adequate energy. . Think about it. Your body is burning calories all day and all night even when you're just lying down.Your heart is beating, you're breathing, brain is working, all your cells are busy making protein and DNA... so while you sleep you're using up energy. If you don't eat when you first get up in the morning, you're going further into deficit. Even more so if you then do purposeful movement without eating. THIS IS INTERMITTENT FASTING. It means that even if you are eating enough overall calories to support your body's needs, your brain is still registering a deficit, and your hypothalamus is not getting the signals of caloric surfeit or surplus that it needs to turn back on. . If you need more advice about how to recover your periods, consider scheduling time to work with me directly; http://noperiod.info/appointments. . #amenorrhea #noperiod #hypothalamicamenorrhea #femaleathletetriad #RelativeEnergyDeficiencyInSport #missingperiod #amenorrhearecovery #periodrecovery #hypothalamicamenorrhearecovery #HA #exercise #sport #fitness #crossfit #running #weightlifting #noperiodnowwhat #eatingdisorder #normalperiod #intermittentfasting https://www.instagram.com/p/BvzZRTOlk-O/?utm_source=ig_tumblr_share&igshid=1nybu09u6z0pp

I wrote this to a woman in the life after recovery support group (link at the end) and thought others might find it useful... . It's okay not to be perfect all the time, and that is true for every aspect of our lives. . You don't have to be perfect at recovery either. Good enough on most days, especially with 6+ periods under your belt, is good enough. You're doing great. Let go of the worry about your cycles and trust your body. Yes, you may miss a cycle from time to time, you might not ovulate this coming cycle because this does sound like a lot of stress, but that's not abnormal - 8.3% of cycles in normal women (or something around that) are anovulatory... IT HAPPENS. Very few people have completely spot on normal cycles all the time. . Life is ups and downs and in betweens and some of it is messy and untidy and some of it is neat and perfect... give yourself grace when you mess up, try and do your best to fix things when you can, let go of them when you can't... believe in your body. Believe that when you treat it well most of the time, it will respond in kind. . It sounds like you've been doing a good job of letting go of your mom's judgement... keep doing that as much as you can because sometimes there are people who can't and won't get it because they have been so indoctrinated by diet culture they cannot see their way out. She can't see that you're happier and healthier because she doesn't know how to process that. She's projecting her negative feelings about her body on you which is totally unfair, but it will be a lot of work for you to change that and maybe it's not worth it.  . 🤯😤I am so tired of hearing , "My doctor said my weight is normal so I can't have hypothalamic amenorrhea"... yet if you examine the medical literature for information about diagnosis of amenorrhea, absolute weight IS NOT mentioned as a criterion. For example, 👩‍🏫Klein and Poth (Am Fam Phys 2013) specifically say, " Functional hypothalamic amenorrhea occurs when the hypothalamic-pituitary-ovarian axis is suppressed due to an energy deficit stemming from stress, weight loss (independent of original weight), excessive exercise, or disordered eating." They do NOT say, "if you are in a small body" or "if starting BMI is less than X", in fact they specifically say **INDEPENDENT OF ORIGINAL WEIGHT**. . 👩‍🏫The American Society of Reproductive Medicine, in their 2008 paper "Current Evaluation of Amenorrhea" mentions weight LOSS multiple times, but absolute weight not once. They state, " Psychogenic stress, weight changes, undernutrition, and excessive exercise are frequently associated with functional hypothalamic amenorrhea" . 🤯It is not medically sound to fail to diagnose someone with hypothalamic amenorrhea based on body size alone🤯.  . 👩‍🏫Please check out my resources on HA at http://noperiod.info/HA for more information. And for help with distinguishing between HA and PCOS, http://noperiod.info/HAvsPCOS. . #amenorrheadiagnosis #amenorrhea #medicaldoctor #obgyn #reproductiveendocrinologist #doctor #noperiod #hypothalamicamenorrhea #npnw #femaleathletetriad #relativeenergydeficiencyinsport #missingperiod #missingcycle #periodhealth #menstrualcycle #truthtime https://www.instagram.com/p/BvhXzXUHmlH/?utm_source=ig_tumblr_share&igshid=qjnazcvivnxr

Laura in my FB group (http://noperiod.info/Support) wrote: "I went to my 10 year olds parents evening last night.Teacher commented,“I don’t know what’s happened but since Christmas your son is a changed child, he is so much more confident and self assured, he puts his hand up in class and is so much fun to have around” My husband last night said “ I am so happy you have come back to us, you went away so slowly we didn’t notice you going until you had gone and it was too late” Both these things broke my heart. The personality difference in me is huge. When I was restricting I was a ghost of my former self. I may be bigger, but my capacity to laugh and enjoy life and love people and experiences is so much bigger than my larger frame. #positivesofrecovery #hypothalamicamenorrhea #amenorrhea #noperiod #periodhealth #femaleathletetriad #relativeenergydeficiencyinsport #npnw #noperiodnowwhat #recovery #riotsnotdiets #diet #HAES #amenorrhearecovery #hypothalamicamenorrhearecovery #2fab4ana #disorderedeating https://www.instagram.com/p/BvSM9VtFepz/?utm_source=ig_tumblr_share&igshid=4yk2cqpqeno3

There are general recommendations that have been demonstrated to help with recovering missing periods: eating more in terms of caloric intake and types of foods, probably gaining some weight, cutting high intensity exercise, reducing stress... And if you have HA because of one of those factors, chances are you will need to work on those issues. . But it is so important to remember that each of us had a unique path to HA and we will each have a unique path out the other side. There are many other factors at play... What is your current distribution of food and movement through the day? How does your body size related to the past (weight loss can often lead to ha)? How much and what type of exercise do you do and how does that compare to past times? Do you have any food allergies? How quickly do you want to restore cycles? What is your reason for working on this??? . It is so very easy to compare ourselves with others, but so rarely is there benefit in doing so. Perhaps if you aren't yet "all in", looking at changes made by those who did restore cycles might be helpful. Barring that, however, comparing your food intake, types of foods, weight gain, movement with anyone else is unlikely to be beneficial because you aren't in their body, don't have their hormonal system, don't have their food, menstrual, exercise history... . Figure out what works for you and your body and your life, and do that. . If you are lost as to what steps to take or how far to go, I have been guiding women for many years and can help you figure out what changes will have the most impact, and how to design a path to recovery that will work FOR YOU. http://noperiod.info/appointments link in bio. ❤️ #noperiod #npnw #amenorrhearecovery #amenorrhea #nondiet #allbodiesaregoodbodies #exercise #sport #hypothalamicamenorrhearecovery #hypothalamicamenorrhea #functionalhypothalamicamenorrhea #femaleathlete #femaleathletetriad #RelativeEnergyDeficiencyInSport #missingperiod #periodhealth #postpillamenorrhea https://www.instagram.com/p/BvPStCYFyWo/?utm_source=ig_tumblr_share&igshid=x62ciiezhxq2
