Hello, I appreciate your medical posts very much and having seen a post the other day where you said migraine was in your areas of special interest, I'd love to ask a related question. You talk about bodies and medicine and patient experience etc in a way that makes a lot of sense to me and I'd trust your take.
I have chronic migraine. I'm currently at 100% pain days, with varying severity. Very hard to pin down what is prodrome, the main event, and postdrome as it's all blurred into one. My migraine team want me to reduce painkiller usage (currently dihydrocodeine and paracetamol daily, and ibuprofen maybe every other day on top) due to rebound headache. I want to cut down because they're fucking expensive and I'm scared for my liver and kidneys. But I literally can't cope with life without them. I went off them for four months a few years ago and the pain was so severe and so debilitating I was the most suicidal I've been in my life. Without painkillers I can't get to the toilet unaided, rarely leave bed, even more rare to leave the house. It's hell. And that's not even considering the effects on everyone around me who has to pick up to care for me.
So what do I do? The way I see things, I need something to help the pain improve before I can use less painkillers, but the longer I go on trying to find something that works and not getting there, the more I think maybe I'm wrong in that. I know a bit about how codeine based painkillers can reduce your pain tolerance / pain baseline. I don't think it's an addiction issue because I've been at the same (over the counter) dosages for 4 years now. I just want to do all that I can to be better, but I also need to be alive to be better. I am stuck.
TL;DR - If you have any thoughts on the relationship between chronic migraine, painkiller use, preserving quality of life while finding a treatment, and increasing the chances of a treatment working, and where on earth the balance between all that lies, I'd really like to hear them.
Again, I absolutely appreciate if you can't answer this, don't want to etc. Giving advice online is notoriously tricky and all that. I won't speak to your case directly, since I'm not your doctor, but here is my personal algorithm for escalating treatments for migraine (note that "abortives" in this case means something you take after a migraine starts to try to end it, while "prophylactic" means a daily treatment you take to reduce likelihood of developing a migraine):
-OTC combination of magnesium, feverfew, and butterbur, taken daily
-Triptans (insurance will usually demand patients fail at least 3 to cover a more expensive treatment)
-High-dose NSAIDs (as abortive treatment given risk of rebound headaches if used daily)
-Daily topiramate (insurance will always demand this is either failed or there's a clear contraindication)
-Daily calcium channel blockers
-Daily beta blockers (higher dose than used for anxiety or low-grade arrhythmias)
-Daily anti-epileptic medications (such as Lamictal)
-Monthly anti-CGRP monoclonal antibody injections (Aimovig or Ajovy; expensive so insurance will demand you've failed some or all of the previous meds)
-Abortive anti-CGRP orals (Nurtec or Ubrelvy)
-Abortive ergotamine, usually Migranal, a nasal spray (very expensive and must be repeated 15 minutes after initial dose regardless of whether symptoms are improving or not)
-Prophylactic Botox (I believe this is every 3 months, must be done in the office of a trained and licensed professional, usually but not always a Neurology provider)
-Sphenopalatine ganglion blocks (done by dripping lidocaine far back into the sinuses to reach the sphenopalatine ganglion, again in the office of a trained and licensed professional)
-Cephaly (transcranial magnetic stimulation at-home device), expensive so insurance hates covering it
Now, one of my newer tools, and my current personal favorite, is a greater occipital nerve block--easy and fast, low risk, and I've had about 90% success with my patients in aborting current headaches. Effects seem to last 3-4 weeks in most cases and since it's straight lidocaine (you don't have to include steroids, though you can) you can do it as often as needed. I generally do this in my office, but I did train one patient's spouse to do it at home given how frequent their headaches. The pharmacy lost their fucking mind about letting an outpatient have lidocaine. I don't know why.
I currently manage my pretty awful chronic migraines with a combination of monthly Aimovig, as-needed Excedrin (the combination of caffeine, Tylenol aka paracetamol, and aspirin is effective for many people but is a real risk for causing medication overuse headaches, the more official term for bounce-back), as-needed Ubrelvy (I can sleep after taking Ubrelvy but not Excedrin so it's a good option), and roughly monthly greater occipital nerve blocks (I teach my trainees to do it using myself as a subject). I wouldn't mind trying the Botox but it's a PITA to get in to see our only local Neurology provider and since my migraines are relatively well-controlled (probably 1-2 headache days a week right now) I don't think it's worth the effort.
I also really got a lot out of this lecture, so give it a try.

Just accidentally fell down a rabbit hole of Reddit DIY cosmetic procedures community and I’m sick to my stomach😟 these women inject Botox against wrinkles, fat dissolvers, lip fillers and etc all by themselves at home without any training, ordering the materials from the internet. Quite a few posts are coming from women who are afraid they did something wrong because they feel unwell and it just makes me genuinely sad to see the lengths women are forced to go to fit in with society standards of forever youth, forever skinny, always pretty, it’s hard to blame anyone nowadays for choosing plastic surgery and cosmetic procedures because the pressure both direct and indirect is unreal and it’s so normalised and even though I wish everyone would feel truly happy and comfortable as they are without getting procedures solely for aesthetic purposes done, if they would still do it at least go to the professionals because this do it yourself thing is horrifying…
some examples of that subreddit.
I mean I get it, getting this stuff done by a professional is extremely expensive, but why do these people give each other tips instead of trying to accept the fact that they're aging or that their features don't fit the beauty standard?
I actually think that in a couple of hundred of years, we will see these excessive plastic surgeries as similar to foot binding in ancient China or people elongating their babies' skull for aesthetic reasons.

Death of skin peeling patient raises concerns about aesthetic procedures in Brazil
There are thousands of Brazilians seeking aesthetic treatments to correct what they consider flaws in their appearance. Brazil counts the second-highest number of plastic surgeries in the world behind the United States, according to data from the International Society of Aesthetic Plastic Surgery (ISAPS).
At least 1.5 million plastic surgeries are made annually in Brazil, with 57% being aesthetic and 43% reconstructive, according to the Brazilian Society of Plastic Surgery (SBCP). The Brazilian market is so active that hundreds of foreigners come to the country only for plastic surgeries: at least 7% of annual procedures (more than 100,000) are made on foreigners.
Regarding non-surgical aesthetic treatments, the growth is even higher: 56% between 2020 and 2022, which also makes Brazil the second country in the world ranking of non-invasive treatments. Among the most sought-after procedures, according to ISAPS, are botox, hyaluronic acid-based facial fillers, laser hair removal, collagen bio-stimulators, and fat reduction treatments through the freezing of adipose tissue cells.
But with so much demand, the risks to patients are also increasing.
The death of a young man following an aesthetic procedure earlier this month in São Paulo reignited discussions about booking treatments with unqualified professionals, often without medical training, or in clinics that lack the structure to handle emergency situations.

Is it me or did anyone think the Hilarie book launch event was the weakest turnout?
This was literally her LAUNCH event.
—Held in Tribeca — a mecca for art and culture and urban center.
—Promoted over a month on national broadcast media and social media.
—Hilarie selected a friend, fellow actress and “witch”, that happens to have a million+ huge platform in a fandom that believes in the supernatural.
…and only 30-40 people showed up??
The Tribeca B&N probably had that size of a crowd on a normal Wednesday evening.
What happened?
If I were the publicist (cough Jeffrey Chassen) I would be kinda embarrassed, or, really disappointed in the lack of pull, participation and support Danneel showed her friend. Not a post to promote the event. Not a post after the event. Not a post recognizing what she liked most about the book. Nothing.
Watching the panel, Elta brings absolutely no energy or emotion, and didn’t seem to have even read her book, at least not enough to have an engaging conversation with her friend about it on stage. She looked bored up there and looked and acted out of place.
Or is her face so full of Botox that she’s just unable to express emotions??

Hilarie ended up having to take over as she got out of her chair to engage the crowd. JDM seemed to pick up on the low energy as well and knows how to read his wife to help lift up the room. Those two are an in-sync couple.
I was just really surprised at how few people stumbled in to attend the event and be able to interact in an intimate setting with either Hilarie or Danneel.
I was pleasantly surprised that JDM was even there. Such a supportive husband and partner. Maybe they know how to work the optics, but dayum they got great energy together and both light up when they are near one another. That’s hard to fake.
I wonder if Danneel thought the bookstore wouldn’t be able to handle her (Jensen’s) swarm of fans if she promoted the event on her own platform, and why she chose to not support Hilarie AT ALL publicly (*sigh of sarcasm*)
I’m watching Hilarie on the morning show circuits and it’s clear Kelly Ripa, folks from the Today show and View have all read the book and are asking really deep and detailed questions and now I really want to read her book. I wasn’t even remotely interested after the launch event panel.
Sophia Bush is the moderator for Hilarie’s panel for the last event on the tour and she’s already posted about it and made comments on book details on Hilarie’s social. Along with so many others. (Kids, that’s how it should be when asked to be a panel host).
Her publicist is getting her great press and setting up a wonderful tour. It’s too bad the kick off event seemed to be the most lack luster part of what seems to be a really good publicity book tour.
Beautifully written and spot on. Danneel looked like she felt above the crowd and was unable to discuss the theme deeply. She's always faking being into whatever the crowd expects, like she did when she pretended Misha was her favorite and that she knew all his lines only to have Jensen out her later on and say she's only ever watched a handful of episodes. 🤭 Comments she made later on showed us even more how ignorant she is about the Supernatural legacy, lore and plot.
Not taking the time to thank her friend for all the free promotion and for involving her in the tour speaks volumes about how entitled Danneel is. Once again, she did not prepare and did not do any research beforehand. She's an amateur and it shows. She could have turned this into a good pr opportunity to create some buzz around her too, her friend at least tried to promote her (out of interest but still).
For the record, any trained professional actor should plan soundbites beforehand and promote the event in time. I guess Danneel counted once more on Jensen's fans showing up and her putting in zero effort. This event sadly showed she has very few fans which is no surprise given her lack of depth.
