Common Challenges with the Pelvic Floor During Pregnancy
Though pregnancy is a natural and miraculous experience, it can also lead to various pelvic floor challenges due to the physiological transformations that occur within the body. Some of the prevalent issues encountered during pregnancy include:
1. Urinary Incontinence: The increased pressure on the bladder and pelvic floor muscles often results in urinary leakage, particularly stress urinary incontinence. Additionally, some expectant mothers may face urge incontinence, characterized by sudden and intense urges to urinate.
2. Pelvic Organ Prolapse: Pelvic organ prolapse arises when the pelvic floor muscles and connective tissues weaken, causing pelvic organs to descend into the vaginal canal. Symptoms may include sensations of heaviness or pressure in the pelvic area, vaginal bulging or protrusion, urinary challenges, and discomfort during sexual intercourse.
3. Pelvic Discomfort: Pelvic pain is a frequent concern during pregnancy, stemming from various factors such as altered posture, increased pressure on pelvic structures, hormonal fluctuations, and musculoskeletal imbalances. Conditions like symphysis pubis dysfunction (SPD), sacroiliac joint dysfunction (SIJD), and round ligament pain commonly contribute to pelvic discomfort during pregnancy.
4. Sexual Challenges: Changes in hormone levels, pelvic floor function, and body image during pregnancy can lead to sexual dysfunction for some women. Symptoms may include discomfort or pain during intercourse, reduced libido, and difficulty achieving orgasm. Addressing pelvic floor issues and maintaining open communication with healthcare providers or pelvic floor specialists can aid in enhancing sexual function during pregnancy and postpartum.
You can get in touch with Dr Firuza Parikh, who is India' s best gynecologist and currently practices at HN Reliance Hospital in Mumbai, to help you have a smooth pregnancy journey and also manage any problems associated with the pelvic floor health.

Autodumping and Self-Emptying with Menstrual Discs
If you’ve heard the term autodumping (or self-emptying) when talking about menstrual discs, you might be wondering what it means. Simply put, self-emptying happens when a menstrual disc partially empties itself while using the bathroom.

Unlike menstrual cups, discs sit in the vaginal fornix without suction, allowing them to shift slightly when you relax your muscles to use the restroom. This movement can release some fluid into the toilet, a natural and often helpful feature that can extend wear time without needing to remove the disc.
Not everyone experiences auto-dumping. Factors like pelvic floor strength, disc design, and disc sizing can affect whether it happens. Some discs stay firmly in place, while others shift more easily.
Is it normal? Absolutely! Autodumping doesn’t mean your disc is leaking, it’s just adjusting with your body’s movements. Many users appreciate it as a way to avoid overflow on heavier days.
As long as your disc only self-empties when using the restroom, this is perfectly fine! If you prefer a product that doesn’t self-empty, you may prefer a period cup. If you want some extra protection from leaks, consider pairing your disc or cup with leakproof period underwear. Welcome to a more sustainable period!

All of these, obvs, but the shoe one forgot to mention one of the most pertinent for women: the pelvic floor. I learned a lot of this from Katy Bowman, absolute kickass biomechanics who uses a combination of mechanics and fucking common sense to illustrate how tilting the pelvis at an unnatural angle distributes the rest of the load (i.e. your body weight) incorrectly.
Yes shoes damage knees, feet, and back, but super importantly, that amazing pelvic floor. When your average woman gets pregnant, and the incredible process of reforming the body, growing an entirely new organ, shifting and stretching, begins, the pelvic floor takes a lot of that change. During pregnancy and after birth, she's told the damage to her pelvic floor is just part of the process. Oh you pee when you sneeze? Welcome to motherhood!
Wrong! Welcome to wearing shoes with ANY heel (and all mainstream/normal shoes, including men's, have a heel, sorry) that causes pelvic floor weakness and instability which is the, of course, blamed on women. Tada!

Someone posted about women and incontinence recently and that reminded me more women should be aware of pelvic floor therapy and how it can help. This is just one exercise that is really beneficial if you have incontinence. I personally used it to help my bladder symptoms with endometriosis. I've also shared it with several new moms and older women who say it has helped them a lot, especially with incontinence with sneezing or physical activity. I wish it was more common knowledge and hopefully it can help someone here too. Make sure you fully void first before you do the exercise! That is really important. Don't work against yourself
