This. This was houses love confession.
Throughout the whole show there have been themes of characters reflecting their problems onto patients and talking about the patient when they're actually talking about themselves.
House - the one uncomfortable with human emotions like love - making a conversation that was heading to talking about Wilsons feelings, into a joke that is even more steered towards love and whatnot.
Maybe, house just randomly decided to make a gay joke in a very tender moment before starting a treatment that might kill his best friend, you know, a moment where you might... confess something.
Or he finally said i love you without actually having to. In this essay i will-

The C-Word//The Adventure Of The Dying Detective
Rewatching the magnificent Brett made me reread the Dying Detective and that reminded me of the C-Word situation and I realized how similar they are in some scenes and got all emotional...
("I'd feel like I deserved it" paralleled to "Let it be someone in whom I have confidence" aren't too similar but they are two quite hurtful things the sick one says in anger so yeah)

I just watched this episode last night, and when I tell you that so many parts of it making me fucking feral
House to Wilson: “Spring break. I’m on vacation. Although, looking at you now, I can tell the wet t-shirt contest is probably gonna suck.”
Wilson: “Go away. I have a headache.”
House: “We don’t have to have sex. Sometimes it’s nice to just cuddle and talk.”
House: “Can we tone down the bromance a few notches? My leg is killing me.”
Spoilers for the episode, but this is the one where Wilson decides he wants to take really intense chemotherapy to fight his cancer, and the chemo has a good chance of killing him. House first tries to convince him not to go through with it, and then when he realizes Wilson is dead set on it makes Wilson do it at House’s apartment. The way House took care of Wilson was he was very sick was *so tender*. Sure, he had the same sarcastic bedside manner that he always has, but every time Wilson woke up and needed something, House was already there, ready with whatever it was. He let Wilson go through his entire personal stock of morphine and a lot of his Vicodin. House, a man with a long standing Vicodin addiction, chose not to take Vicodin for a little while just to make sure Wilson would have enough to get him through the chemo.
House also took a bunch of stupid picture of Wilson while Wilson was passed the fuck out because of the cancer drugs he’s on (Wilson is multiple different pairs of ridiculous sunglasses, hookers posing with wilson, etc.), editing the pictures to the beat/music of “anyway you want it” and had it ready on Wilson’s desk for him to play when he came back, absolutely cracking Wilson up.
Like, this episode really said “House may be a complete ass and a miserable bastard 99% of the time, but when it comes to Wilson, his care for him knows no bounds”
would you believe me if i told you “If you're gonna say that you've always been secretly gay for me, everyone always just kind of assumed it” is a real line that house says to wilson. this episode also happens to be the c-word (an episode hugh laurie directed)
