Digital technologies, such as smartphones and machine learning, have revolutionized education. At the McGovern Institute for Brain Research’s 2024 Spring Symposium, “Transformational Strategies in Mental Health,” experts from across the sciences — including psychiatry, psychology, neuroscience, computer science, and others — agreed that these technologies could also play a significant role in advancing the diagnosis and treatment of mental health disorders and neurological conditions.
Co-hosted by the McGovern Institute, MIT Open Learning, McClean Hospital, the Poitras Center for Psychiatric Disorders Research at MIT, and the Wellcome Trust, the symposium raised the alarm about the rise in mental health challenges and showcased the potential for novel diagnostic and treatment methods.
John Gabrieli, the Grover Hermann Professor of Health Sciences and Technology at MIT, kicked off the symposium with a call for an effort on par with the Manhattan Project, which in the 1940s saw leading scientists collaborate to do what seemed impossible. While the challenge of mental health is quite different, Gabrieli stressed, the complexity and urgency of the issue are similar. In his later talk, “How can science serve psychiatry to enhance mental health?,” he noted a 35 percent rise in teen suicide deaths between 1999 and 2000 and, between 2007 and 2015, a 100 percent increase in emergency room visits for youths ages 5 to 18 who experienced a suicide attempt or suicidal ideation.
“We have no moral ambiguity, but all of us speaking today are having this meeting in part because we feel this urgency,” said Gabrieli, who is also a professor of brain and cognitive sciences, the director of the Integrated Learning Initiative (MITili) at MIT Open Learning, and a member of the McGovern Institute. “We have to do something together as a community of scientists and partners of all kinds to make a difference.”
An urgent problem
In 2021, U.S. Surgeon General Vivek Murthy issued an advisory on the increase in mental health challenges in youth; in 2023, he issued another, warning of the effects of social media on youth mental health. At the symposium, Susan Whitfield-Gabrieli, a research affiliate at the McGovern Institute and a professor of psychology and director of the Biomedical Imaging Center at Northeastern University, cited these recent advisories, saying they underscore the need to “innovate new methods of intervention.”
Other symposium speakers also highlighted evidence of growing mental health challenges for youth and adolescents. Christian Webb, associate professor of psychology at Harvard Medical School, stated that by the end of adolescence, 15-20 percent of teens will have experienced at least one episode of clinical depression, with girls facing the highest risk. Most teens who experience depression receive no treatment, he added.
Adults who experience mental health challenges need new interventions, too. John Krystal, the Robert L. McNeil Jr. Professor of Translational Research and chair of the Department of Psychiatry at Yale University School of Medicine, pointed to the limited efficacy of antidepressants, which typically take about two months to have an effect on the patient. Patients with treatment-resistant depression face a 75 percent likelihood of relapse within a year of starting antidepressants. Treatments for other mental health disorders, including bipolar and psychotic disorders, have serious side effects that can deter patients from adherence, said Virginie-Anne Chouinard, director of research at McLean OnTrackTM, a program for first episode psychosis at McLean Hospital.
New treatments, new technologies
Emerging technologies, including smartphone technology and artificial intelligence, are key to the interventions that symposium speakers shared.
In a talk on AI and the brain, Dina Katabi, the Thuan and Nicole Pham Professor of Electrical Engineering and Computer Science at MIT, discussed novel ways to detect Parkinson’s and Alzheimer’s, among other diseases. Early-stage research involved developing devices that can analyze how movement within a space impacts the surrounding electromagnetic field, as well as how wireless signals can detect breathing and sleep stages.
“I realize this may sound like la-la land,” Katabi said. “But it’s not! This device is used today by real patients, enabled by a revolution in neural networks and AI.”
Parkinson’s disease often cannot be diagnosed until significant impairment has already occurred. In a set of studies, Katabi’s team collected data on nocturnal breathing and trained a custom neural network to detect occurrences of Parkinson’s. They found the network was over 90 percent accurate in its detection. Next, the team used AI to analyze two sets of breathing data collected from patients at a six-year interval. Could their custom neural network identify patients who did not have a Parkinson’s diagnosis on the first visit, but subsequently received one? The answer was largely yes: Machine learning identified 75 percent of patients who would go on to receive a diagnosis.
Detecting high-risk patients at an early stage could make a substantial difference for intervention and treatment. Similarly, research by Jordan Smoller, professor of psychiatry at Harvard Medical School and director of the Center for Precision Psychiatry at Massachusetts General Hospital, demonstrated that AI-aided suicide risk prediction model could detect 45 percent of suicide attempts or deaths with 90 percent specificity, about two to three years in advance.
Other presentations, including a series of lightning talks, shared new and emerging treatments, such as the use of ketamine to treat depression; the use of smartphones, including daily text surveys and mindfulness apps, in treating depression in adolescents; metabolic interventions for psychotic disorders; the use of machine learning to detect impairment from THC intoxication; and family-focused treatment, rather than individual therapy, for youth depression.
Advancing understanding
The frequency and severity of adverse mental health events for children, adolescents, and adults demonstrate the necessity of funding for mental health research — and the open sharing of these findings.
Niall Boyce, head of mental health field building at the Wellcome Trust — a global charitable foundation dedicated to using science to solve urgent health challenges — outlined the foundation’s funding philosophy of supporting research that is “collaborative, coherent, and focused” and centers on “What is most important to those most affected?” Wellcome research managers Anum Farid and Tayla McCloud stressed the importance of projects that involve people with lived experience of mental health challenges and “blue sky thinking” that takes risks and can advance understanding in innovative ways. Wellcome requires that all published research resulting from its funding be open and accessible in order to maximize their benefits. 
Whether through therapeutic models, pharmaceutical treatments, or machine learning, symposium speakers agreed that transformative approaches to mental health call for collaboration and innovation.
“Understanding mental health requires us to understand the unbelievable diversity of humans,” Gabrieli said. “We have to use all the tools we have now to develop new treatments that will work for people for whom our conventional treatments don’t.”

Olfactory Dysfunction is Associated with Mild Cognitive Impairment (MCI)
This article is written based on the presentation done by Prof Wang De Yun at the NUS Psychiatry Symposium on Ageing-in-Place held on 27 Apr 2023. Olfactory impairment, or more commonly referred to as sense of smell, is ageing related. The ability to detect and identify smell has also long been linked with cognitive decline in older adults. However, there remains questions on its association with mild cognitive impairment (MCI) and by extension, dementia.

As part of the Yeo Boon Khim Mind Science Centre’s Community Health and Intergenerational (CHI) Study, Prof Wang De Yun from NUS Department of Otolaryngology, conducted a study to assess the olfactory function of community-dwelling older adults in Singapore and its link with their cognitive function. For this study, researchers at NUS developed a localised version of the nine-item smell test to better screen the population. 
Smell ability as a predictive tool for Cognitive Impairment
When it comes to modifiable risks of dementia, the focus is often on vision and hearing impairments rather than smell impairment. Nonetheless, there are studies indicating a potential link between smell identification ability and MCI.

A meta-analysis found that smell identification is impaired in those with MCI compared with healthy older adults. Longitudinal studies also showed that cognitively healthy individuals with smell impairments at baseline subsequently developed MCI in the follow-up.
The evidence suggests that smell identification has the potential to detect and predict the risks of developing dementia in later life.
To the best of our knowledge, there has yet to be any study examining smell function in Singapore’s healthy and/or cognitively impaired community-dwelling older adult population. Mind Science Centre’s study aimed to assess their smell identification ability and associate the outcomes with their cognitive status.
Culture specific smell test developed in NUS
The original nine-item smell test developed by the University of Pennsylvania (UPSIT) included items as liquorice, dill pickle, and skunk. These are not common in Singapore and participants often struggle to recognise and identify them.

To better screen and evaluate smell function in Singapore’s older adult population, our researchers recognised a need to create a localised version of the smell test that is culture-specific to our population. Researchers in NUS developed the Singapore version of the 9-items Smell Test Panel based on a local smell familiarity survey. This includes almond, banana, cinnamon, coconut, lemon, orange, pineapple, rose, and mushroom.
Beyond this study, other clinicians and researchers could also benefit from this locally developed smell test to better screen their patients or research participants.
Participants aged 60 years and above were recruited from a larger epidemiological study titled Community Health and Intergenerational (CHI) study. The smell test and neuropsychological testing are part of a 10-hour assessment done over five separate visits. More details on that can be found in the CHI study protocol.
Smell Tests For Cognitive Impairment
The locally developed nine-item smell test was used to assess smell detection and identification. The test was conducted in an interview format with each odour sample (in total 9) given only once. Between each odour, there was at least a 30-second time interval to prevent sensory fatigue.

For the smell detection task, participants were asked to respond yes or no based on their perception of the presence of an odour. Each successful detection was scored a point.
For the smell identification task, participants were asked to identify the same test odour before choosing through 4 narrated answer choices. If they thought none of the answer choices matched the test odour, they were allowed to describe the odour instead. Each successful identification was scored a point.
Cognitive Function Tests - Testing for Cognitive Impairment
Participants’ cognition was measured with the modified mini-mental state examination (MMSE) scale, clinical dementia rating (CDR) scale, and a validated battery of neurocognitive tests. The protocol is described in more details here.
Diagnosis of the participants’ cognitive status was made at consensus review meetings involving two psychiatrists and a neuropsychologist. Participants were categorised into normal active ageing, MCI, or dementia and further subtyped as amnestic or non-amnestic, and single or multiple domain.

Communicatie in tijden van coronacrisis
De afgelopen drie maanden is er weer een hoop werk verzet op de communicatieafdeling bij Zeeuwse Gronden. Naast de nodige projecten en andere werkzaamheden kregen we allemaal te maken met de coronacrisis. Die heeft natuurlijk overal zijn invloed!
Na de nieuwe pagina ‘Jeugd’ op onze website, heeft ook Omroep Zeeland aandacht besteed aan onze unieke aanpak voor de jeugdzorg op de Kraaghoeve. Verderop in Terneuzen werd onze vernieuwde werkplaats geopend. De PZC heeft hier ook een bericht over geschreven.
Coronacrisis Tot zover was alles ‘normaal’ de afgelopen maanden. Maar toen raakten we met zijn allen in de Coronacrisis. Dat werd werken vanuit huis. Voor ons van communicatie betekende dat ook crisiscommunicatie! Er zijn verschillende brieven naar cliënten, familie en collega’s gegaan. Ook hebben we de eerste informatie op het intranet gezet en zijn er de nodige e-mails gestuurd.

Om iedereen toch up to date te houden, hebben we onder andere een speciale Facebookpagina (enkel voor cliënten) gemaakt over de dieren op de Kraaghoeve en hebben we het ‘Zeeuwse Gronden Journaal’ in het leven geroepen.
Ook is de Cursus geluk van onze directeur gedeeld, dat kon iedereen wel een beetje gebruiken zo aan het begin van vreemde periode waarin iedereen te maken krijgt met opnieuw balans vinden. Iedereen droeg zijn/haar steentje bij: de één verzorgde yogalessen online, de ander zette een speurtocht op, maakte kaartjes voor eenzame cliënten en externen hadden een lekkere verrassing voor onze cliënten. Zo brengt zo’n crisis iedereen dan toch ook samen.
Congres verzet Helaas waren ook wij genoodzaakt ons Congres Mens & Natuur te verzetten naar 2021. Samen met Cows and Stars hebben we de nodige communicatie en andere werkzaamheden hiervoor verzet.

Ondertussen gingen een hoop ‘reguliere’ werkzaamheden gewoon door. Er verscheen een artikel in BIT Magazine over de Equitherapie op de Kraaghoeve. We hebben vanuit communicatie onze bijdrage geleverd aan het Jaarverslag van 2019. Er werd een Verkoophoekje ingericht op de website met spullen die onze cliënten in dagbestedingsplek De Werkplaats maken of bijzondere items die in de Kringloopwinkel te koop zijn (wanneer die weer open is).
Bezigheden
Momenteel ben ik druk bezig een ‘Belisboom’ in te richten op ons intranet. Dit zodat collega’s zelf via een soort telefoonboom kunnen zien wie van de collega’s van kantoor ze voor welk onderwerp het beste kunnen aanspreken. Super handig, vanuit je werk op de woonlocatie.
Alle readers die onze directeur heeft geschreven, komen binnenkort op de website te staan. Bergen informatie over allerlei onderwerpen die te maken hebben met geestelijke gezondheid. Fijn als naslagwerk voor bijvoorbeeld begeleiders op een van de woonlocaties.
Ook starten we binnenkort met het nieuwe Zeeuwse Gronden Magazine, is er een Zeeuwse Gronden Kookboek in de maak en help ik de Ondernemingsraad met de verkiezingen.
En zaken als het up-to-date houden van het intranet, de website, Facebook, LinkedIn en allerlei flyers en ander printmateriaal verzorgen blijft natuurlijk onverminderd doorgaan.

Dr Ellie Staple x fem!reader 
It was hard to believe that it all was happening for real. You graduated in psychiatry. You got a job about which you had been dreaming since high school. You not only met, but also could work with the greatest specialist - Dr Ellie Staple. 
As a student you had heard about her a lot. She's a genius. She was the only woman in that field, who was better than any man could ever be. No wonder why you wanted to succeed as well. Even though you hadn't met her, she became your inspiration and motivation to continue the studies. You wanted to be just like her - a strong, independent woman with a brilliant mind. 
You're working hard to be a top student and it was worth it. All the sleepless night, all breakdowns and other burdens that you had had to face on your way to succeed were worth it. Because at the end, as a top student, you got the opportunity to work with your goddess. It's like your biggest dream came true. It seemed so unrealistic. You felt as if you're on top of the world. At the same time you knew that working with her was probably the biggest challenge in your life. You needed... You wanted to be the best. You wanted her to notice you. To let you be the part of her team. 
Though you had never thought you may developed a crush on her. You fell harder that you knew it's possible. But there was nothing you could do. The desire was choking you like an illness. It felt as every time you saw her she was lighting you up and you wanted nothing more, but her to smoke you like Marlboro. 
You didn't really know how it happened that she chose you to be her assistant. You're a mess whenever she was around. You're tripping over your own tongue and couldn't even focus on anything, for all you could do was rolling down into the wildest dreams you had about her. She was both your curse and your blessing. 
As her assistant you're spending with Dr Staple quite a lot of time. You're with her basically from early morning to the evening, sometimes even night. You're always bringing her coffee, that you had prepared at home, simply because you wanted her to have what's the best. You loved watching her drink it and praise you for being the best assistant she could ever have. Besides participating in sessions with her patients - individuals, who believed they had superpowers - Staple let you stay with her at her office. She was telling you, more like teaching, about her theories and a way of psychosomatic treatment she devised. Listening to it only confirmed your belief that she was the genius. The one of her kind. Her compassion, dedication... Her brain. She truly was extraordinary. 
She was better than any high. It felt almost as if she were a type of drug that was affecting you and only you. And that was fascinating. When she asked you to go with her to the symposium you couldn't help, but be excited as a little kid. It's not only because you had never participated in such an event, but it also meant you're going there with her alone. She was picking you up in the morning, so as always you had a coffee ready for her. 
"Thank you, you're a doll" she said smiling and watching how your cheeks turn crimson, because of the nickname 
It was hard for you to stay composed, but you loved every minute of it. At your destination you're like her shadow. You're following her almost everywhere. And that was your possibility of watching all her moves. Before you knew you found yourself starring at her legs - again. Her skirts were giving you a hard time, since the very begging of your work at the hospital, making it impossible for you to focus. You literally loved and hated them at the same time. 
Luckily for you Dr Staple didn't pay much attention to your incompetent behaviour. She was polite, as always, even introduced you to a few other doctors. That's when it happened - for a while her hand was resting on your lower back. If you hadn't know better you'd say that she was moving it lower and lower to your ass. But well, she was your boss and you're sure she didn't give a damn about you. Though, for some unknown reason you're second guessing it for the whole day. You weren't going to complain. You enjoyed it all - the nicknames, brushes of hands and accidental touches - way too much to do it. But we're confused. And it didn't get any better when Ellie asked you to come to her room in the evening. You kept on telling yourself that it definitely was only about work, but your brain couldn't get over the thoughts of Dr Staple acting different than usually. She wasn't reserved and didn't act as if work was the only connection between the two of you. 
 Your intuition made no mistake. One glass of wine that she offered you led you to her bedroom. You're pretty sure you're dreaming when Staple told you she had known about your crush on her since the beginning.
"Oh sweetheart, do you really think you can fool me? I'm a genius, remember? I know you're always starring at my legs and ass. Why do you think I chose you to be my assistant?" 
 That night was intense. That night was wonderful. Though it left you confused even more than before. You didn't know whether it was lovely or just a sort of tender curiosity. Something that was giving both of you a thrill. And so there you were sitting in an armchair in the corner of her room. Ellie was still asleep, but you apparently couldn't sleep. All the thoughts that were bothering you made it unable. Instead of overthinking and regretting all that had happened you prepared a coffee. Only then did you approach Ellie. She looked beautiful even when she was asleep with her honey curls all over her face. You smiled fondly placing the cup of coffee on the nightstand. You gently brushed Ellie's hair away from her face and smiled when she opened her eyes. 
"Good morning" she said sleepily 
 "Morning. I brought you some coffee. I've thought you may need it, considering that you didn't get much sleep last night" 
 "Thank you, sweetheart" she said sitting up 
You watched how she took the cup in her delicate hands and then how her soft lips touched  it. You didn't dare to speak, only look at her. 
"What's going on?" she asked 
"Nothing" you said biting your lip, what you did always when you're nervous "Come on, just tell me" she took another sip 
"I... Well, I was just wondering... can I stay a bit longer?" 
"Oh, sweetheart if you keep on making me such amazing coffee you can stay forever" she winked at you You looked at Ellie not sure whether she was serious or not. 
"Well, I hope it's not only coffee that makes you want me to stay" 
"Of course not. You have a lot of assets. Speaking of which, shall we continue from where we've finished yesterday?"

He is continuing the work of Ian Stevenson at the UVA Division of Perceptual Studies with children who report memories of previous lives. His overview of the research, Life Before Life: A Scientific Investigation of Children’s Memories of Previous Lives, was published in 2005 and has been translated into ten languages. His latest book, Return to Life: Extraordinary Cases of Children Who Remember Past Lives, is a collection of recent American cases he has studied.
