Introduction

Almost 1 in 5 adults (44 million) in the US experiences mental illness and distress in a given year, according to the NAMI. A nearly 10 million experience a debilitating mental illness that substantially interferes with their QoL [1,2]. Mental and addictive disorders affected more than 1 billion people globally in 2016. They caused 7% of all global burden of disease as measured in DALYs and 19% of all years lived with disability [3]. Depression was the leading cause of disability in the world, and suicide was the 10th leading cause of death in 2015 [2]. Major depressive disorder (MDD) is the fourth cause of disability around the world and is estimated to be the second leading cause of disability by 2020 [4]. Over the past 20 years the prevalence of child and adolescent mental disorders in high-income countries has not changed despite increased investment in MH services. Insufficient contact with MH services may be a contributing factor [5]. In EU, factors that had the strongest association with depression were chronic diseases, pain, limitations in daily living, grip strength and cognitive impairment. The gap in MH service use was nearly 80% [6]. The treatment gap in developing countries was 76%-85%, according to WHO. According NMHS, it is 83% in India for mental disorder and 86% for alcohol use disorders [7]. stated that mental morbidity above the age of 18 years is 10.6% with a lifetime prevalence of 13.7%. This means that 150 million Indians need active intervention [8]. Canadian Mental Health Association estimated that 500,000 Canadians miss work every week due to MH issues, costing the Canadian economy approximately $51 billion dollars per year, as reported by [9]. According to Australian Bureau of Statistics (ABS) National Survey of Mental Health and Wellbeing (NSMHWB) 45% of Australians had experienced a mental disorder in their lifetime, with 20% experiencing a mental disorder in the previous year [10]. Refugees and asylum seekers often have increased MH needs, yet may face barriers in accessing MHPSS services in destination countries [11].

Depression and anxiety in more common chronic physical conditions such as CHD or diabetes can be associated with increased mortality, as reported. Additionally, depression is associated with an increase of about 50% in costs of chronic medical illness [16]. 2019 reported that anxiety and depression lead to sexual dysfunction is between 30% and 70% in sexually active men and women in high-income countries [17]. Despite psychiatry’s current status as the sixth largest medical specialty, the supply of available clinicians has not kept up with demand [18]. Outside of the diagnosis and treatment of depression, PCPs indicate a lack of comfort in treating PCMH patients with MH disorders [19]. Efforts to improve the outcomes of patients with mental illness often have involved incorporating the skills of a variety of health care professionals into collaborative care models. For over 40 years, clinical pharmacists have contributed to these care models in capacities ranging from educator to consultant to provider [20]. Medicines are a major treatment modality of management for many mental illnesses and pharmacists are therefore well positioned to enhance MH services with the potential to reduce the associated burden of mental disorders [21]. A study conducted by the CDC estimated that 22% of American adults had filled a prescription for anxiolytics, hypnotics, anticonvulsants, and antidepressants in the past 30 days [22]. According to epidemiological studies, about 70% of patients with depression and anxiety are treated in PHC, and about 75% of all antidepressants are prescribed by general practitioners (GPs), reported by 2019. In 2014 depression was the main diagnosis in about 35% of all cases of prolonged sick leave (≥60 days) [23]. Antidepressant drug treatment (ADT), alone or in combination with psychotherapy, is recommended by the CANMAT for a minimum duration of 8 months. However, a large proportion of individuals show suboptimal adherence to ADT. In previous studies, more than 35% to 70% ceased treatment within 6 months, with up to 25% to 40% of patients having ceased their treatment within the first month [24,25]. Also, reported non-adherence to antidepressants is high among older patients with depression in primary care settings [26]. Even after achieving remission, depression has higher rates of recurrence in up to 80% of all MDD patients with odds of becoming chronic in 20% of patients. The onset of each new major depressive episode increases the chances of relapse, chronicity, and treatment-resistant depression [27]. 10%-30% of MDD patients do not improve or show a partial response coupled with functional impairment, poor quality of life, suicide ideation and attempts, self-injurious behavior, and a high relapse rate [28]. Pharmacists are highly accessible health care professionals, trusted by the public and have regular interactions with consumers that suffer acute mental illness [29,30].
Medication counselling provided by community pharmacists is an important source of medication information for patients and their caregivers. Community-pharmacy based medication counselling interventions have resulted in improved patient adherence to antidepressant and antipsychotic medications [31]. Transitions in care have the potential to be destabilizing periods for many patients and is an area where pharmacist-performed medication therapy management (MTM) has been found to be beneficial [32]. The pharmacist interim prescriber clinic was associated with a significant decrease in mean number of patients seen per month in PES [18]. The clinical pharmacist can make an impact by improving mild-to-moderate MH conditions, promoting interdisciplinary collaboration, and increasing documentation and follow-up that align with published treatment guidelines [2]. Reported that pharmacists improved rate of patient interest in behavioral health counseling during the consult and recommending counseling directly to the patient or even initiating the referral themselves [33]. Highlighted the value of the pharmacist’s involvement, suggesting the potential for improved nutrition, physical activity, and sleep for patients with MH conditions, at least in the short term [34]. Throughout the United States, pharmacists have crafted interventions designed to prevent, identify, and manage opioid misuse and abuse [35]. Also, Reported that physicians and nurses have mostly positive perceptions and expectations from clinical pharmacists at the psychiatric hospital [36]. In Australia, The Pharmaceutical Society’s Mental Healthcare Framework recognizes pharmacists as primary health care professionals who have an important role to play within MH care. Globally, the International Pharmaceutical Federation has urged members to include pharmacists as part of their “human resource development policy” so that “an increase by 20% of service coverage for severe mental disorders can be achieved” [37]. Says pharmacists are not practicing to their full scope of practice in mental illness and addictions care for several reasons including limitations within the work environment and lack of structures and processes in place to be fully engaged as health care professionals [38]. Sexual dysfunction is an underdiscussed adverse effect to antidepressants and may increase the risk for discontinuation and nonadherence to antidepressant pharmacotherapy [39-42]. Sildenafil effectively improved erectile function and other aspects of sexual function in men with sexual dysfunction associated with the use of SRI antidepressants [43]. Clinical pharmacists increased their skillset for treating depression and anxiety and enhanced their ability to make interventions with patients who are referred to them for other chronic disease state management (e.g. hypertension, chronic pain) [44]. Though pharmacists are trained in psychopharmacology, they lack formal MH intervention skills [29]. A solution to address this gap is to up-skill pharmacists in mental health first aid MHFA. It is an educational program geared towards educating MHFA responders to assist those who may be experiencing a MH condition or disorder [45]. Latzman Reported that approximately 20% to 50% of adults with SMI did not receive past-year MH services [46]. Douglass Reported that stigma has a significant impact on the treatment of MH, with substantial implications on patient quality of life. To provide professional, culturally sensitive care, pharmacists should reflect on their skills, attitudes, and beliefs of MH treatment and actively participate in changing the stigma of mental illnesses [47].
Evidence suggests that socioeconomic factors can have the greatest effect on health and wellbeing, accounting for 40% of all influences on the individual. Taylor recommended inclusion of pharmacy team in social prescribing pathways would widen the ability to support people with psychosocial needs arising from non-medical determinants and reach people who are unable to access general practice health services [48]. The complexity of psychotropic drug therapy would be expected to increase, and the challenges inherent to the safe pharmacological treatment of mental disorders will expand. The merits of a partnership approach with collaborative work involving psychiatrists and pharmacists have been established [49]. With provider education and appropriate physician champions, pharmacists are able to work collaboratively with psychiatrists in an MH clinic [50]. A Primary Care Mental Health Integration (PCMHI) clinical pharmacy specialist (CPS) successfully manages and maintains patients with uncomplicated MH conditions in primary care through evidencebased pharmacotherapy, as evidenced by symptom improvement, medication adherence, and low rate of specialty MH referrals [51].

CARING FOR OLDER ADULTS: WHAT SHOULD YOU KNOW?

The fastest growing age group in Ontario are seniors who are 65 years and older and it is projected that by the year 2041, 25% of Ontario’s population will be over the age of 65 (Government of Ontario, 2017) The provincial government launched a plan recognizing the need to enhance the lives of Ontario seniors so that they can remain independent, healthy and socially connected at all stages of their lives (ibid). This in keeping with the activity theory, which posits that the way to age optimally is to remain as active as possible in three areas: physical, mental and social (Palmore, 2017). Even though this plan was launched under the previous provincial government, the current government also seems to have a focus on improving and supporting the lives of older adults as outlined below..

One issue that was not addressed in Ontario’s action plan for seniors was improving timely access to psychiatric care. For seniors who are living with mental illness, it is important that they have timely access to care as poor outcomes have been linked to delayed treatment (Loebach & Ayoubzadeh, 2017). The government of Ontario has been successful in implementing mandatory reporting wait times for many surgical and medical services which has led to great improvements in wait times for cataract surgery (61% reduction), hip replacements (53% reduction), coronary angiography (51% reduction) and cancer surgery (22% reduction) (ibid). Psychiatry services for mental health care in Ontario have lengthy wait times and there are no mandatory reporting wait times for these services (Loebach & Ayoubzadeh, 2017). This will be an important issue that the government will have to address as our population continues to age and as they move to integrating the healthcare system.

Polypharmacy is classified as the use of five or more prescription medications and is common among seniors and can lead to decreased compliance with medication routines and problems with drug interactions (Reason, Terner, Moses McKeag, Tipper, & Webster, 2012). The use of prescription medication increases with age partly due to multiple illnesses in older adults and it is therefore crucial that doctors effectively manage prescription medication (Reason et al., 2012). Due to physiological changes that occur as we age that change the way the body responds to and processes medication, older adults are at increased risk of adverse drug reactions (ADR) such as unsteadiness, confusion, delirium, depression and dependence (Bernier, 2017). It is essential to complete medication reviews and have discussions about side effects with older adults and because of the physiological changes associated with aging, there may be a need to change medication dosages and routines to decrease the risk of ADRs (Reason et al., 2012).
Here are some startling numbers on medication use in older adults
66.1% of seniors took 5 or more drugs; 28.0% took 10 or more drugs; and 9.3% took 15 or more drugs (2014).
39% of Canadians in long-term-care received at least one antipsychotic (2014).
Only 4% of seniors in British Columbia in residential care have a diagnosis of a psychiatric disorder, BUT 34% are on antipsychotic medications (2015).
Over 27,000 Canadian seniors had an adverse drug related reaction leading to hospitalization (2011).
The biomedical model presumes that mental illnesses like schizophrenia, major depressive disorder, attention deficit/hyperactivity disorder (ADHD), and substance use disorders are biologically based brain disorders (Deacon, 2013). Treatment mainly involves prescribing psychotropic drugs to modify/improve bio-chemical imbalances (Horgan, n.d., Module 4). When medication is prescribed to at risk populations, supervising them can be very complicated and physicians may need access to electronic medical records and to consult with a clinical pharmacist who can play an important role of completing medication reviews and educating patients and other team members about safer medication usage (Bernier, 2017). Having team members working as part of an interdisciplinary team can improve the management of patient medication. Having a trusting multidisciplinary team relationship is important for effective collaboration, communication and knowledge sharing (Kirst et al., 2017). It is also important that when individuals visit different care providers that there is a coordination of information regarding drug prescriptions otherwise there is a risk of over prescribing. An integrated care system would minimize this risk an decrease the risk of ADR for older adults with normal and abnormal mental health issues.

The leading cause of injury and death among older adults and a significant public health issue is falls which affect one in three adults over the age of 65 every year and 50% percent of adults over the age of 80 (Ambrose, Paul, & Hausdorff, 2013). Experiencing a fall can be devastating both physical and psychologically and could result in disability, chronic pain, loss of independence, reduced quality of life, and even death (Public Health Agency of Canada, 2014).

In 2018, the province of Ontario introduced the falls prevention equipment funding policy with the purpose of providing additional funding to Long-Term Care (LTC) homes to acquire equipment including devices and assistive aids that will help prevent residents from falling or suffering injuries from falls (Ministry of health and long-term care, 2018) .With this funding comes the expectations that LTC home will maintain or improve their performance measures on residents who are falling or being transferred to the hospital for fall related incidents (ibid). With the prevalence of falls among seniors, the focus of health care providers should also be in assessing for falls risk factors and address modifiable factors. Implementing a checklist such as one below as part of a healthcare providers assessment could be a first step in decreasing the risk of falls in older adults with normal or abnormal mental health issues.

The difference between having a harmonious passion and an obsessive passion
If, like many, you are searching for your calling in life – perhaps you are still unsure which profession aligns with what you most care about – here are five recent research findings worth taking into consideration.
First, there’s a difference between having a harmonious passion and an obsessive passion. If you can find a career path or occupational goal that fires you up, you are more likely to succeed and find happiness through your work – that much we know from the deep research literature. But beware – since a seminal paper published in 2003 by the Canadian psychologist Robert Vallerand and colleagues, researchers have made an important distinction between having a harmonious passion and an obsessive one. If you feel that your passion or calling is out of control, and that your mood and self-esteem depend on it, then this is the obsessive variety, and such passions, while they are energising, are also associated with negative outcomes such as burnout and anxiety. In contrast, if your passion feels in control, reflects qualities that you like about yourself, and complements other important activities in your life, then this is the harmonious version, which is associated with positive outcomes, such as vitality, better work performance, experiencing flow, and positive mood.
Secondly, having an unanswered calling in life is worse than having no calling at all. If you already have a burning ambition or purpose, do not leave it to languish. A few years ago, researchers at the University of South Florida surveyed hundreds of people and grouped them according to whether they felt like they had no calling in life, that they had a calling they’d answered, or they had a calling but had never done anything about it. In terms of their work engagement, career commitment, life satisfaction, health and stress, the stand-out finding was that the participants who had a calling they hadn’t answered scored the worst across all these measures. The researchers said that this puts a different spin on the presumed benefits of having a calling in life. They concluded: ‘having a calling is only a benefit if it is met, but can be a detriment when it is not as compared to having no calling at all’.
The third finding to bear in mind is that, without passion, grit is ‘merely a grind’. The idea that ‘grit’ is vital for career success was advanced by the psychologist Angela Duckworth of the University of Pennsylvania, who argued that highly successful, ‘gritty’ people have impressive persistence. ‘To be gritty,’ Duckworth writes in her 2016 book on the subject, ‘is to fall down seven times, and rise eight.’ Many studies certainly show that being more conscientious – more self-disciplined and industrious – is associated with more career success. But is that all that being gritty means? Duckworth has always emphasised that it has another vital component that brings us back to passion again – alongside persistence, she says that gritty people also have an ‘ultimate concern’ (another way of describing having a passion or calling).
However, according to a paper published last year, the standard measure of grit has failed to assess passion (or more specifically, ‘passion attainment’) – and Jon Jachimowicz at Columbia Business School in New York and colleagues believe this could explain why the research on grit has been so inconsistent (leading to claims that it is an overhyped concept and simply conscientiousness repackaged). Jachimowicz’s team found that when they explicitly measured passion attainment (how much people feel they have adequate passion for their work) and combined this with a measure of perseverance (a consistency of interests and the ability to overcome setbacks), then the two together did predict superior performance among tech-company employees and university students. ‘Our findings suggest that perseverance without passion attainment is mere drudgery, but perseverance with passion attainment propels individuals forward,’ they said.
Another finding is that, when you invest enough effort, you might find that your work becomes your passion. It’s all very well reading about the benefits of having a passion or calling in life but, if you haven’t got one, where to find it? Duckworth says it’s a mistake to think that in a moment of revelation one will land in your lap, or simply occur to you through quiet contemplation – rather, you need to explore different activities and pursuits, and expose yourself to the different challenges and needs confronting society. If you still draw a blank, then perhaps it’s worth heeding the advice of others who say that it is not always the case that energy and determination flow from finding your passion – sometimes it can be the other way around and, if you put enough energy into your work, then passion will follow. Consider, for instance, an eight-week repeated survey of German entrepreneurs published in 2014 that found a clear pattern – their passion for their ventures increased after they’d invested more effort into them the week before. A follow-up study qualified this, suggesting that the energising effect of investing effort arises only when the project is freely chosen and there is a sense of progress. ‘Entrepreneurs increase their passion when they make significant progress in their venture and when they invest effort out of their own free choice,’ the researchers said.
Finally, if you think that passion comes from doing a job you enjoy, you’re likely to be disappointed. Consider where you think passion comes from. In a preprint paper released at PsyArXiv, Jachimowicz and his team draw a distinction between people who believe that passion comes from doing what you enjoy (which they say is encapsulated by Oprah Winfrey’s commencement address in 2008 in which she said passions ‘bloom when we’re doing what we love’), and those who see it as arising from doing what you believe in or value in life (as reflected in the words of former Mexican president Felipe Calderón who in his own commencement address in 2011 said ‘you have to embrace with passion the things that you believe in, and that you are fighting for’).
The researchers found that people who believe that passion comes from pleasurable work were less likely to feel that they had found their passion (and were more likely to want to leave their job) as compared with people who believe that passion comes from doing what you feel matters. Perhaps this is because there is a superficiality and ephemerality to working for sheer pleasure – what fits the bill one month or year might not do so for long – whereas working towards what you care about is a timeless endeavour that is likely to stretch and sustain you indefinitely. The researchers conclude that their results show ‘the extent to which individuals attain their desired level of work passion may have less to do with their actual jobs and more to do with their beliefs about how work passion is pursued’.
