Toxic blood effects Clotting disorders in Covid patients were spotted by researchers in China in early 2020, but the true extent of the risk only became clear when even patients with mild respiratory symptoms began experiencing strokes.

At first, doctors suspected these clots might result from a “cytokine storm” — an intense immune response releasing a surge of inflammation-signaling proteins. Others noted that the virus could directly damage blood vessel linings.

But Katerina Akassoglou, a neurovascular brain immunologist at the Gladstone Institutes and UC San Francisco, wasn’t convinced that the virus itself wasn’t a cause.

Navigating social distancing requirements that complicated lab work, Akassoglou and her collaborators conducted a series of experiments in mice to explore the pernicious role of the coronavirus’s spike protein.

They discovered that beyond serving as the virus’s “key” to enter cells, spike binds with a blood clotting factor called fibrinogen, creating structurally abnormal, inflammation-promoting clumps of fibrin — the insoluble material that forms the mesh-like structures essential for wound healing.

High levels of these abnormal clots not only push the body’s clotting system into overdrive, increasing clot formation and inflammation, but also suppress natural killer cells — the immune system’s virus-clearing soldiers.

When this damaging cycle occurs alongside a breakdown in the protective layer of cells around the brain’s blood vessels, toxins and bloodborne proteins, including fibrin, can seep into the body’s most vital organ. Once there, these substances activate microglia — the brain’s immune cells —which begin attacking healthy brain cells, contributing to the neurological symptoms of long Covid.

Akassoglou had been studying this damaging cascade for decades in patients with Alzheimer’s disease and multiple sclerosis. Still, until SARS-CoV-2 came along, she had no idea it could be triggered by a viral infection.

“For some reason, this virus has evolved to interfere with the coagulation system in a way that other viruses do not,” she told me. Fibrin’s role in driving toxic inflammation is common in many diseases, but “in the presence of spike, it gets a lot worse.”

In experiments with mice lacking fibrinogen, Covid leads to much less inflammation, and the infection clears faster. “Studies suggest that if you deplete fibrin, inflammation improves, no matter what initially triggers it.”

Although Akassoglou’s focus has been on the brain, she’s hopeful this research will be expanded to understand the effects on the heart, liver, kidneys, and gastrointestinal tract.

In the meantime, she’s developing a way to halt this damaging reaction. A first-in-class antibody treatment designed to specifically block fibrin’s toxic effects entered early-stage patient studies in May, with no reported safety concerns so far.

Results are expected next year and could lead to more advanced clinical trials to test the immunotherapy’s potential to treat not only long Covid but also other serious diseases like multiple sclerosis and Alzheimer’s.

OOC: Does Doc carry a phone with him? Nope! Thinks it's something redundant because of his sonic being able to act as a communicator at a rudimentary level. Granted, you're not going to get stellar audio or video quality from something like that, but it works throughout the multiverse so Doc doesn't bother. If you plan on having an OC or other character meet him through this route, it could be as a transmission to his sonic!

Doc's Appearance:

Since the time he received the repair box, Doc has been biologically frozen in a state where his physical body remains in its 40s.

Doc is standing tall at 6'2", no different from most iterations of his canon counterparts.

His coat the the same as the one he already uses in post-canon gravity falls, that hasn't changed aesthetically.

The doctor sports a suit because if he's going to kick butt might as well do it in style, he also switches between neck-ties and bow-ties because bow-ties are cool.

He wears sneakers instead of his boots since those had worn down and broken sometime during his travels and sneakers are just generally easier to replace.

Underneath all his dress-up though he's covered in scars from past battles and his old tattoos that he'd never had the time to get rid of.

His glasses still have a crack in them, mostly because he couldn't be bothered to get a new pair of bifocals anyway.

Always clean shaven, yes he still shaves his face with fire that has never changed.

Healthcare in Piltover depends entirely on your status. If you are wealthy or well-connected, the finest medical advancements are at your disposal. But if you are poor, disabled, or otherwise deemed "lesser" by Piltover’s elite, then you will find yourself struggling for care.

As an outsider and someone with a chronic condition, I have faced skepticism and condescension from so-called "experts."

So, is healthcare in Piltover a pain? If you are not one of them, then yes: it is an uphill battle against both illness and the arrogance of those who think they know your body better than you do.

I have received decent treatment since becoming Heimerdinger's assistant. Jayce's brace prototypes have been instrumental in maintaining my mobility. And I am grateful for the truly good-hearted doctors and surgeons I have been lucky enough to find over the years. Certainly, I am quite lucky to no longer reside in the Undercity, where continued exposure to toxins would have exponentially worsened my condition.

I do think troll medicine is likely to be much less interested in interventions than ours is, and a lot more focused on a) can this troll be fighting fit again, or do we cull them here and now? and if they're salvageable, b) how fast can we make that happen? Day 2: Visiting a hospital

After a lot of infighting and straight up civil war drama rivalling the Mahabharata, our new group rose like a phoenix out of the ashes and we changed our space to Sri Krishna AYUSH. Ayurvedic Hospital. We visited the facility. It was the only medical college in Kurukshetra.

Context of the place

Shri Krishna AYUSH University, established by the Haryana Government under Act No. 25 of 2017, began its operations in the academic year 2018-19. Headquartered in Sector 8, Kurukshetra, Haryana, it is the first university of its kind in the world to offer all AYUSH courses. This university plays a pivotal role in the affiliation and regulation of Ayurveda courses across government and private colleges in Haryana.

Tour of the Hospital

We walked in through the emergency block and tried to soak everything in and get a feel for the place and later found out about the main entrance. There, we spoke to the medical superintendant, Dr. Rajender Singh who introduced us to Dr. Priya, an intern who is doing PG. She gave us a tour of the place, meticulously explaining everything in great detail.

Dr. Priya started with the OPDs and explained what the purpose of each ward was. She guided us through the entire hospital starting with the lobby where patients are instructed to get a slip with their name, age and sex.

They get assigned to specific departments based on your ailment represented by a number of the slip. Later, she walked us through all the outpatient departments on the ground floor and the accompanying waiting areas. A general sense of uncleanness was observed with the washrooms and water coolers.

Panchkarma

We then proceeded to go one floor upwards where the inpatient wards were located. Separate wards existed for male and female. The wards were mostly not occupied with a lot of empty beds. However, there was a long line which extended far beyond the waiting area outside the Panchkarma OPD. There were two people stationed on a bench managing the people and clearing any doubts they had.

Dr. Priya explained what Panchkarma was. It means to detox. It is an Ayurvedic treatment that helps cleanse and heal the body. It involves five main methods:

Vamana (Therapeutic Vomiting): Removes toxins from the stomach.

Virechana (Purgation): Cleanses the intestines using herbal laxatives.

Later, we proceeded towards the pathology lab. There was a general lack of organisation. Urine samples were placed on the table out in the open. We were introduced to advanced machinery like the centrifuge. Syringes and other meds were stored in the refrigerator.

OT Block

Then we went to the Operation Theatre Block. This part, the Major OT was still under construction with exposed concrete walls, but the minor OT was functional with surgeries being conducted. Dr. Priya showed us the various instruments. There was a labor room which was not occupied.

Agnikarma

Finally, we were in the agnikarma department where Dr. Priya introduced us to various instruments to treat joint pain without the need for painkillers. The shalaka was one such tool. The pointed end is heated and then briefly touched to the skin to treat various ailments like joint pain, musculoskeletal issues, and certain skin conditions. Aloe vera and turmeric were common treatment methods. Bowls were placed for cupping therapy. To our astonishment, Dr. Priya picked up a container from the shelves and showed us leeches inside. The saliva of leeches contain anticoagulants. While feeding on the blood, these substances are released into the bloodstream thereby helping in improving blood flow and reducing symptoms of varicose veins.

After the tour was over, we decided to split up and observe different areas all over the hospital. We made a list of all the equipments, items, and other stuff kept inside every ward. Then we started observing the people around us.
