Wound Care For Adults

So the wife was on backpacking reddit and found that a lot of people can't tell the difference between wounds you need some vaseline and a bandaid for, wounds that need a little home TLC, wounds you need to go to an urgent care for, and wounds you actually need to go to the emergency department for, so we're gonna talk about that here.

Wounds that need some vaseline and a bandaid:

A blister that popped

A non-gaping cut

A skinned knee (small amount of fresh, shallow road rash with nothing embedded)

Keep in mind that you should NOT use rubbing alcohol, iodine, mercurochrome, or hydrogen peroxide on any of these. It will just hurt and potentially kill healthy cells in the wound. Neosporin or other antibiotic ointment is okay if you happen to have it, but the antibiotics themselves don't last long and are generally not worth the extra money.

Wash the wound with plain tap water, pat it dry with a clean cloth or piece of gauze, dab on a little petroleum jelly (Vaseline) and slap a bandage on that beby.

Wounds that need some TLC at home:

A small, shallow burn with nothing stuck to it

A slightly infected open blister or non-gaping cut

Slightly infected road rash or shallow road rash with something embedded in it

Cut gaping less than 1/4 inch (1/2 cm)

Small, shallow burn: Right after you get the burn, run it under cool tap water for 5-10 minutes, even if you think it's already cool. This will help clean the wound and stop the burn from getting any deeper. Do not ice. Do not put oil or butter or vaseline on the wound. Use an over the counter burn gel and a bandaid to hold it in place.

Slightly infected small wound/road rash: You'll know it's slightly infected if there's redness and swelling around the edges (up to 2cm), if there is drainage, and if it smells bad. It will also probably hurt more than you think it should. For this you'll want to do hot compresses about 4 times a day for 20 minutes per time until the infection goes away. To do this, get a pot and get water hot enough that it is uncomfortable to touch. Then put a wash cloth in that water, pull it out, wring it out, and hold it against the wound. It should be uncomfortably hot and just a little painful. When it cools down, dip it back in the pot, wring it out, and do it again. At the end of 20 minutes the whole area around the wound should be pink.

Road rash with something embedded: If there's a tiny stone or pieces of visible dirt on this section of road rash, you'll need to clean it with a moderately forceful stream of water. You can do this with an irrigation syringe you can get from the pharmacy, or you can make your own using a plastic zipper bag. Fill a bag with water, then cut a teeny tiny hole in one of the corners. Squeeze the bag to make a stream of water, then direct that stream at the wound. This will take potentially a lot of water. Keep at it until there is nothing visible in the wound, then treat with vaseline and a bandaid.

A cut gaping less than 1/4 inch: If this is on your face, genitals, or hands and you care about scarring, go to an emergency department. If this is on another part of your body and you're okay with a scar, keep reading. Stop bleeding with pressure. Clean the wound by running clean tap water through it and pat the edges dry. Make some butterfly bandages out of strong tape- I recommend silk medical tape, but in a pinch you could use duct tape or similar.

Starting on one end, tape down one side of a butterfly bandage, pull it across the cut, and tape it to the other side. Move a 1/4 inch down the cut, and tape another one down, then another, until you have taped the length of the cut. Put some kind of breathable bandage on top of this.

Wounds that need to go to an urgent care:

Cut gaping more than 1/4 inch but that you can still stop bleeding with pressure.

Open blister, cut, burn, or road rash that is draining thick, yellow-or-white drainage and is not getting better with hot compresses, but you don't have a fever

Stop any bleeding, clean by running water over the injury, and go to an urgent care or your family doctor if you happen to be able to get in for a same-day appointment.

Wounds that need to go to an emergency department:

Any cut that gapes on the face, palm of the hand, or genitals

Infection with streaks or with which you have a fever/chills, or for which the red area grows by more than a cm in an hour

Burn larger than the palm of your hand or that is more than skin deep

Any wound that was spurting blood or that needed a tourniquet to stop bleeding

Go to the emergency department as soon as possible, they'll take care of it.

Ratchet who was neutral in the war.

He was neither Autobot nor Decepticon, but a medic for those who were caught between fire fights or who couldn’t fight. Sparklings, injured civilians, lower class citizens who couldn’t get shelter from either side for whatever reason.

Ratchet would go out himself and find them, brining them back to one of several hidden infirmaries he set up with volunteers and past students of his.

First Aid always stayed in the biggest one, which held all the sparklings as it was the safest.

Fixit would sneak between them all to make sure that all the set false safes were in order. He helped show the little ones where to gin in the case of an attack.

He believed Optimus Prime wanted to help, but the mech was too busy fighting the fight to be able to spare much time on the broken bots that had a chance.

They knew each other and Prime had worked to keep their bases a secret even as Ratchet resumed to let them sway him into fighting with them.

Megatron’s men were allowed only in one base where other soldiers were kept, guarded by Pharma and Remedy both. Autobots and Decepticon both had an understanding that it was neutral ground and no fights would be had, the two keeping in seperate wings of the fragile clinic.

That understanding was crushed after one fateful cycle.

Ratchet had responded to First Aids distress signal as quick as he could, travelling over half of Cybertron to get to the infirmary as quick as he could.

He arrived just in time to see Shockwave leaving.

With dread in his spark he had rushed through a secret exit and into the main room and instantly fell to his knees.

Over two hundred sparklings lay desecrated in the entire place.

Most of them were missing some part of their body, some burnt into nothing.

Ratchet only made a noise, a wailing sound of pure grief, when he saw the remains of First Aid clutching three little ones.

All dead.

His screams had only grown in intensity as he looked around the room and found that Pharma and Remedy had come to aid and fallen as well.

Ratchet could have stayed there and rusted over if he hadn’t notice some of his patients weren’t there, hope burning his spark as I rushed to the feeds to try and figure out where they had gone.

He watches the footage with grime determination even as he feels his spark shattering into pieces.

It falters for several moments when he watches Shockwave order some of his men to take some of the sparklings. He hears the disturbing mech say something about ‘suitable test subjects’ and feels the energon in his systems freeze. Ratchet’s heard of what Shockwave has done, how the unfeeling monster doesn’t care for the notion of ethical conduct and onto for results.

Eventually Optimus as some of his most trusted come by the hidden location in the hopes of stopping a massacre, only to find Ratchet sitting on the floor with the body of a sparkling in his hands.

He had tried to bring the femme back after seeing a slight flux in her tiny spark, but it was useless.

Bumblebee is hurt the most as it was the same clinic he had been raised in before he became a scout, seeing Ratchet work for years and being the main reason they had even known something was wrong.

He falls to his knees as Elita moves to check for Decepticons, if only to distract herself.

Optimus approaches Ratchet with grief in his spark, carefully removing the deceased sparkling from his hold.

“I… I am truely sorry, old friend.”

Ratchet looks up and sees Orion Pax, the young mechling who had once asked him for an autograph.

When he speaks it’s distant, like his mainframe as gone on autopilot, “He took some of the sparklings. We need to find them.”

Optimus nods, helping him stand on unsteady pedes, “We will, I swear we will it stop until we have.”

Ratchet looks at Optimus with a fire in his eyes, “I know. But Optimus, I don’t care about you code. I don’t care about your morals or war crimes, Shockwave will pay for this.”

For the first time since the war began, the Prime looked around the room and nodded with a darkness over his optics.

“You are right. They have gone too far to deserve honour.”

As someone who used to work at a nursing home, I'm getting really tired of nursing staff turning to each other and going, "if I ever end up like THAT, just fucking kill me" like motherfucker the quality of life of these residents is your SOLE responsibility and if they're not living their best lives that is YOUR FAULT.

These residents are here because this is their best option. They're here because if they weren't, their quality of life would be worse.

These residents make friends with their roommates, the people they eat with, and the people who go to activities with them.

A disabled elder's quality of life is not measured by their ability to walk.

A disabled elder's quality of life is not measured by their ability to breathe without oxygen.

A disabled elder's quality of life is not measured by whether or not they're on a feeding tube.

A disabled elder's quality of life is not measured by level of continence.

A disabled elder's quality of life is not measured by whether or not they can chew their food.

A disabled elder's quality of life is entirely measured on whether or not their needs are met.

If their quality of life is worse at a nursing home than it would be if they had just stayed home, then their needs are not being met. If you are not meeting their needs, that is YOUR FAULT and you have FAILED your residents as a medical professional.

Vampire!Ingo & Ansy are back for a little PSA: don’t be afraid to tell the nurse if you’re feeling off after / during getting your blood drawn. From my experience, they usually respond quickly, giving you water / juice or whatever else you need.

As an aside, if anyone wants any writing references for what almost fainting during one is like, I have a fresh memory from doing labs earlier lol XDXD

- I see Asclepius in the brave and selfless doctors and nurses who risk their own health to help and protect the sick and vulnerable, and to nurse those who are suffering back to health.

- I see Asclepius in the blood that seeps from a cut or scrape on my flesh as I apply a bandage to it in its gentle state.

- I see Asclepius when I see an ambulance race by at breakneck speeds to rush to the aid of those facing medical emergencies, and i say a prayer to him as I watch them go by.

- I see Asclepius in the medicine I take to comfort a stomach ache, a headache or a migraine, and I thank him for giving it to Me.

- I see Asclepius in the humanitarian aid workers, the battlefield medics, and the emergency surgeons who undergo some of the most horrific and devastating situations of mankind all to save just one single life if they can. Truly the bravest men and women there are.

Accommodations do not 'limit' what a person is able to do.

It means that it eliminates a point of stress/grief in their daily life, so they can do *more*.

It is not 'laziness' to want to do more, but needing accommodations to make that happen.
