Vial of Archfey Blood
Consumable, very rare (requires attunement)
“A crystalline vial containing a dose of golden swirling Archfey blood. This blood was willingly given by the Archfey, and thus, it holds greater magical properties than it would otherwise.”
Consuming this potion grants you a permanent boon, allowing you to cast the Misty Step spell without expending a spell slot. Once you cast this spell, you cannot cast it again until you finish a Long Rest.
Keepsake. A vial of Archfey blood is a powerful item and radiates magic. If you attune to and keep this vial on you, you are Immune to Divination magic such as the Scrying spell. Additionally, if you fail a saving throw against a spell or magical effect, you can choose to succeed instead. Once you have used this property of the vial, you can’t use it again until the next dawn.
Whether you consume, or attune to this vial your creature type registers as Fey to spells and other effects that detect the presence of the Fey creature type.

Putting the Omnitrix in DnD

The original purpose of the Omnitrix was to give the user access to the DNA of aliens from various worlds to preserve the DNA of a species in the event of a genocide or mass extinction. The watch fused to one person, was extremely hard to remove, and its default setting gave the user access to the DNA of 10 different species they could choose between. Aside from Humanoid, there are 13 total creature types: Aberrations, Beasts, Celestials, Constructs, Dragons, Elementals, Fey, Fiends, Giants, Monstrosities, Oozes, Plants, and Undead. In the spirit of Ben 10, I’ll make a list of 1 creature from each creature type that the watch can turn a player into. Playable races vary a little. Kenku, Grung, and Drow are all CR 1/4, but Changeling, Satyr, and Hobgoblin are 1/2, and Bugbears are CR 1 creatures. So, I’ll be considering everything within that scope. Some do go beyond that, such as Centaurs and Minotaurs, but most are within the window of CR 1/4 and CR 1. To give the DM and Player some variety, I’ll pick one option from each, if applicable. Celestials had to be eliminated because the lowest CR Celestial is CR 2 Pegasus. In-Universe, it was likely created by Mordenkainen.
ITEM:
BRACELET OF POLYMORPH
VERY RARE | ATTUNEMENT REQUIRED
The DM chooses any 10 possible transformations from the list below, only choosing 1 from each creature type. As a bonus action, you can activate the bracelet, becoming one of the creatures chosen by the DM. You use that creature’s stat block instead of your own, except you retain all languages you know, and your INT, WIS, and CHA are not affected unless the creature’s are higher than your own. If the creature you turn into does not speak any languages, you cannot communicate verbally while polymorphed by this item. Depending on the CR of the creature you turn into, the recharge rate of this item varies. For a CR 1/4 creature, it recharges on a short rest, on a CR 1/2, it recharges at dawn, and for a CR 1 creature, it recharges after 1d4 days. You can stay in the form of a CR 1/4 creature for up to 8 hours, a CR 1/2 creature for up to 4 hours, and a CR 1 creature for up to 1 hour. If your hit points in this form are reduced to 0, you return to your original form with your current hit points at the moment you transformed. The DM decides what the bracelet polymorphs the player into, and may ask the player for a contested INT check against a DC 15 to coax the bracelet into cooperation. Otherwise, the bracelet may choose to polymorph the attuned into a different creature than the one they wanted to turn into.

Magic Item: Ring of Turn into Muppet
Upon equipping and attuning to this item, you become a Small felt puppet. While a puppet, you have the following attributes:
-Immune to Bludgeoning, Force, and Poison damage (but vulnerable to Fire damage)
-Ability to summon useful items from Hammerspace
-Mechanic similar to Wild Magic Surges, during which you can cause any effect you want as long as you can convince the DM that it would be funny.

A glamored dress that appears as one of two designs depending on who observes it. The first time a creature views the dress, flip a coin. On a heads, the dress appears to be black and blue; on a tails, the dress appears to be white and gold. Once a creature has viewed the dress once, it will always appear to be the selected colors whenever that creature views the dress in the future. The effect is not illusory such that truesight or any spell would reveal any “true” color; it has two simultaneous true color combinations that differ by observer.

Caia's Thorn (rapier, made for the same thorn-themed melee ranger as above)
Fyren's Quarry (longbow, made for a gloomstalker ranger.)
Moriata (pepperbox/revolver, made for a gunslinger who also has one or two levels in warlock.)

the arcane gate spell really made me think of portal (the game) and i want to make a portal gun that basically casts arcane gate 4 times a day (and every times you do use it you roll a d4 and if it rolls on a 4 it recharges one charge) and all charges are recharged the next dawn. also the change with this gun is that instead of you having to cast it on the ground, as long as the surface is flat, or a round surface is large enough that would be close to round like the side of a large circular tower for example as long as whatever curved circle's diameter is equal to or greater than 100 ft, you can cast that version of arcane gate to create a gate there. basically the portal gun from portal and you can do the whole manipulating inertia and stuff like that like in the game
the question is should this be a rare item or a very rare item?

It contains her many winged patron and it can change shape according to her needs, although the actual form it tends to take is the first one. We really, really love this design. It was made along with an illustration for our Sky Knight Class (which you can find on our Patreon!). We let the stream name it, and the Moonlight Lance was born.

Moonlight Lance
Weapon (lance), very rare
“This long, thin lance is made of an alloy rumored to be from the moon. It was forged and crafted by a sect of druids that have never worked with metal, except for this one weapon. It’s dressed in druidic runes that harness the energy of the moon.”
When you hit a target with this weapon, if the moonlight property has been used and is no longer active, you deal an additional 1d8 radiant damage.
Moonlight. When you hit a creature with this magic weapon you can summon a silvery beam of pale moonlight that shines down in a 5-foot-diameter, 20-foot-high cylinder centered on the creature, dealing 2d10 radiant damage. Until this effect ends, dim light fills the cylinder. You can maintain this moonlight for 1 minute by concentrating on it as if it were a spell. When a creature enters the moonlight for the first time on a turn or starts its turn there it takes 2d10 radiant damage. You can use a bonus action to move the moonlight up to 20 feet in any direction. Once you have used this property of the weapon, it cannot be used again until you finish a long rest. Mastercraft Weapon. Add an additional half of your proficiency bonus (rounded down) to attack and damage rolls made with this magic weapon.

D&D Concept: The Sword in the Tarrasque
Absolute Monarchies can be tricky things, unless you happen to have a One True King, preferably with a magic sword that’s a good judge of character. Fortunately, our kingdom has one of those. Or, well. Had. The King’s Sword turns things to stone. It’ll turn you to stone too, if you try to use it without being the One True King. Most folks let go of it fast enough that it’s reversible. And yes, Wizards have been debating for decades about how exactly it knows, with everything from reincarnation to recessive traits to sentient metal being thrown around. The result is that we’ve had centuries of great kings, with the occasional bad apple when someone hides the sword or drops it in a lake. For the record, we didn’t lose the sword this time, we know exactly where it is.
A stone Tarrasque.
The last One True King used it to fight a Tarrasque that was threatening the capitol, about sixty years ago. See that hill over there? It’s not a hill. We tried all the obvious things to get it back. But it turns out that stone Tarrasques are just as durable as regular ones, so trying to break it up will just break your hammer. And, here’s the thing: the Wizards are pretty sure the Tarrasque is still alive in there.
So yeah, we're going to be stuck with normal rulers for a while.
One more thing, in case you get any bright ideas: the Sword is stabbed into the roof of the Tarrasque’s mouth.
