Completely Collapsed Columella of the Nose – Open Rhinoplasty with Reconstruction of Prolabium This young girl was born with a bilateral cleft lip and palate deformity and had undergone surgical repair as an infant. She now has a collapsed columella and a shortened prolabium that has led to a whistling deformity with exposure of her anterior teeth. The bridge of her nose is also extremely flattened. Costal rib grafts were harvested and used to augment the bridge of her nose through an open rhinoplasty. A double-decker graft was used due to the extreme flatness of her nasal bridge. Finally! (ﾉ◕ヮ◕)ﾉ*:･ﾟ✧

It’s been weeks, maybe months that I’ve been practicing lucid dreaming techniques and it barely worked, lots of failures and losing control over the conscious state... until this early morning! I still need to improve and master this type of skill, but I finally got to have a lucid dream early this morning and I can’t help but share and celebrate ヾ(＠＾▽＾＠)ﾉ Now I can finally confirm, practice is the key, and patience, of course, ‘cause it doesn’t work the first night, I’m not that kind of lucky ^ ^’

Let’s get straight to the dream, which of course includes my Wolf Prince ｡◕ ‿ ◕｡ (yes, I’m going to use a lot of kawaii faces for this occasion!)

So, I was kind of laying a very long table before lunch and there was the maid from the previous apartment where I lived speaking to her husband on the phone about the pandemic lockdown or something. I started realizing it was a dream and that I could have total control over it (and this only comes after a lot of practice unfortunately).

BUT, as the tutorials recommend, I didn’t immediately change things the way I wanted in the blink of an eye, I took it easy and slowly looked at my phone as there was a message displayed asking me to get outside the building.

I walked out and HE was there waiting for me...

I can’t say exactly what he was wearing, but it was something dark, so maybe the outfit I call “the foam and flames coat” (from the mission where he wears that).

The next part has been so romatic (and a little bit erotic, ok, let me admit it!) He lifted me up in his arms (as always happens in dreams with him, I don’t know why!) and pinned me sweetly against the wall as we kissed softly... I wrapped my arms around his shoulders and my legs around his waist; he kissed my neck on the right and I closed my eyes taking deep breaths...

Then kissing each other again... I clearly felt his tongue in my mouth and saw his carved prolabium, above that perfect Cupid’s bow, so big and sensual.

His lips were irresistible and had a pleasant taste, I can’t describe nothing like that in reality. Maybe something between eucalyptus honey and caramel, I don’t know... (~￣▽￣)~

Then, some people walked through the building main hall as we were there initmately kissing and stared at us like shocked!! xD I started feeling unconrtable and took his hand as I headed outside in a sort of garden to have more privacy. The garden turns out to be... a graveyard!

“Nobody will disturb us here” I said and led him to a great abandoned chapel. It was raining and I felt the raindrops hitting my shoulders, face and head, but I couldn’t stop.

We entered the chapel and there were a couple of makeshift beds as I knew once in a while homeless people slept there and used the chapel as a shelter.

I locked the door and turned off the lights, so the graveyard’s guardians thought the chapel was empty and walked away.

Then, we were finally alone and... well, I won’t go on... Sorry, that’s it! ≖‿≖

Abbe flap Correction of Short Upper Lip and Dental Implant Surgery The patient is a case of repaired bilateral cleft lip and palate with a residual whistling deformity. There was no mustache growth on the prolabium. He now presents for correction of flat nose with collapsed columella and stunted prolabium with a short upper lip. His upper incisor teeth were also missing. He and his parents requested rehabilitation of the missing teeth with implants. The prolabium was dissected and used to reconstruct the collapsed columella and raise his flat nose. The resultant upper lip defect was reconstructed with an Abbe flap harvested from the lower lip. Premium quality dental implants were also fixed at the site of the missing teeth. There was very good lip apposition following surgery with resolution of the whistling deformity. The ceramic crowns that were fabricated by our inhouse dental laboratory were shade matched to perfection with his natural teeth. Bilateral Cleft Lip Surgery This little boy had a congenital bilateral cleft lip and palate deformity. His parents were very upset at this. They had been counseled regarding cleft deformities and sent to our hospital for corrective surgery. His bilateral cleft lip deformity was addressed first. At the age of 3 months, he underwent a single stage repair utilizing Paul Black’s technique. No cross incision was made at the prolabium. Available prolabium and columella were utilized for reconstructing the philtral column and Cupid’s bow. This was followed by creation of the nasal floor and vermillion. Bilateral C-flaps were utilized for this. This was followed by meticulous three layer closure of the cleft lip under loupe magnification. Orbicularis oris muscle was approximated at the midline. Premaxillary Setback and Abbe Flap This little boy had been born with bilateral cleft lip and palate deformity. He had undergone cleft lip and palate surgeries at 3 months and 9 months respectively. His parents bring him back for a premaxillary setback now. The trapdoor technique was utilized to harvest iliac cancellous graft. An osteotomy was performed and the premaxillary segment was set back to align with the two halves of the palatal maxillary bones. The graft was used to fill in the bony defects on either side of the premaxillary segment. He returned in one year’s time for an Abbe flap correction. He had a flat nose deformity due to a collapsed columella and stunted prolabium with a short upper lip. His prolabium was dissected and used to reconstruct the collapsed columella and raise his flat nose. An Abbe flap harvested from his lower lip was used to reconstruct the resultant upper lip defect. The flap was detached in three weeks from the lower lip once collateral blood supply had been established from the upper lip. The boy and his parents were very happy with the results of the surgery. Unilateral cleft lip and alveolus defect This little girl was born with a left-sided cleft lip and alveolus defect. She was eight months old at the time of her cleft lip repair. A flap raised from the prolabium was used to recreate the labial sulcus. A three layer closure was then utilized to approximate the orbicularis oris, subcutaneous tissue and skin.
