Hemangioma Removal through Closed Rhinoplasty and Scar Revision Surgery This teenage girl has been plagued by a nasal hemangioma for as long as she can remember. Completely Collapsed Columella of the Nose – Open Rhinoplasty with Reconstruction of Prolabium This young girl was born with a bilateral cleft lip and palate deformity and had undergone surgical repair as an infant. She now has a collapsed columella and a shortened prolabium that has led to a whistling deformity with exposure of her anterior teeth. The bridge of her nose is also extremely flattened. Costal rib grafts were harvested and used to augment the bridge of her nose through an open rhinoplasty. A double-decker graft was used due to the extreme flatness of her nasal bridge. Closed Rhinoplasty, Vermillion Correction and Lip Bulking Surgery This young lady had undergone unilateral cleft lip and palate deformity correction as an infant. She now presented with complaints of an asymmetrical upper lip and an asymmetrical nose. Treatment planning was explained to her and she consented to surgery. She first underwent vermillion correction along with excision of excess scar tissue from her lip. Broad Alar Base Correction with Weir Excision This patient has always felt that he had a very broad alar base and that this detracted from the overall appearance of his face. Secondary Orbital Blowout and Diplopia (Double Vision) Correction and Cosmetic Rhinoplasty This gentleman had been involved in a motor vehicle accident with resultant facial fractures. He had suffered right-sided zygomatic bone, orbital rim and orbital floor blowout fractures. He had undergone surgery elsewhere without any improvement in his diplopia caused by a lowered right orbit, excessive scleral show or enophthalmos. He was also unable to open his mouth fully due to impingement of the coronoid process with the zygomatic bone. A local surgeon had subsequently referred him to our hospital for corrective surgery. The orbital and zygomatic fractures were approached through intraoral and lateral canthal incisions. Plates from the previous surgery were removed. A mesh was used to repair the orbital blowout fracture after releasing entrapped orbital contents. The lateral orbital rim, supraorbital rim and zygomatic fractures were set back in proper anatomical alignment and stabilized with plates and screws. There was complete correction of his diplopia with normalization of his facial esthetics. Cosmetic rhinoplasty was also performed using a rib graft harvested from the patient to augment his nasal bridge. Cosmetic Rhinoplasty and Augmentation of Alveolar Defect with Bone Graft This young man had been born with a unilateral cleft lip and palate defect, which had been corrected when he was an infant. He now presents with a depressed right alar base arising from a maxillary alveolar bone defect and fistula. Treatment planning was explained to him and he consented to surgery. Lyophilized bone grafts were utilized to augment his alveolar bone defect and fistula. The bone graft was then utilized to augment the bridge of his nose through an intercartilaginous incision after excision of the lower part of his medial nasal cartilage.
