Secondary Orbital Blowout and Diplopia (Double Vision) Correction and Cosmetic Rhinoplasty This gentleman had been involved in a motor vehicle accident with resultant facial fractures. He had suffered right-sided zygomatic bone, orbital rim and orbital floor blowout fractures. He had undergone surgery elsewhere without any improvement in his diplopia caused by a lowered right orbit, excessive scleral show or enophthalmos. He was also unable to open his mouth fully due to impingement of the coronoid process with the zygomatic bone. A local surgeon had subsequently referred him to our hospital for corrective surgery. The orbital and zygomatic fractures were approached through intraoral and lateral canthal incisions. Plates from the previous surgery were removed. A mesh was used to repair the orbital blowout fracture after releasing entrapped orbital contents. The lateral orbital rim, supraorbital rim and zygomatic fractures were set back in proper anatomical alignment and stabilized with plates and screws. There was complete correction of his diplopia with normalization of his facial esthetics. Cosmetic rhinoplasty was also performed using a rib graft harvested from the patient to augment his nasal bridge. Rhinoplasty and Orbital Blowout Repair This young man was involved in a motor vehicle accident and suffered fractures to his nasal and orbital bones. Having left it untreated, he now has a residual crooked nose deformity and a right eye that is slightly lower than the left eye. Wishing to get this corrected, he presented to our hospital. He had an untreated right orbital floor blowout fracture with entrapment of musculature. This had resulted in the lower position of the right eye. The blowout fracture was repaired using a titanium mesh placed through a transconjunctival incision. A fat graft was placed in the infraorbital soft tissue through a lateral canthal incision to augment the fat loss in that region. This was followed by rhinoplasty with excision of the lower crus of the nasal cartilages with augmentation of the bridge of the nose using a costal cartilage harvested from the patient. The patient was involved in a grievous car accident while holidaying recently. He was rushed to the emergency where he was diagnosed with a right-sided blowout comminuted zygomatico-orbital complex fracture. His family immediately airlifted him to Chennai for surgery at our hospital Dr SM Balaji, facial trauma surgeon, examined him and obtained imaging studies including a 3D CT scan. He had a depressed right side of the face and complained of double vision. There was also a midpalatine suture split with herniation of the right orbital contents into the maxillary sinus. Treatment plan was explained to the patient and he consented to surgery. The dentoalveolar and midpalatine split were fixed first utilizing titanium plates and screws. A maxillary vestibular incision was then made to access the zygomatic fracture. A lateral canthal incision was then made to reduce the lateral orbital wall fracture, which was then stabilized with titanium plates and screws. The comminuted zygomatic segments were then stabilized, reduced and fixed with titanium plates and screws. The blowout fracture of the right orbital floor was then accessed through a transconjunctival incision. Orbital contents were elevated from the maxillary sinus with care taken to release all entrapped orbital contents from the fracture. The floor of the orbit was then reconstructed using a Titan Medpor implant. All incisions were closed using resorbable sutures internally and nonresorbable sutures externally. The patient was very happy with the results of the surgery. There was reestablishment of facial symmetry along with complete normalization of his vision.
