Residual Diplopia Correction for Blowout Fracture This young man had suffered a blowout fracture to his left eye from a two wheeler accident, which had resulted in enophthalmos and double vision. There was also slight lowering of his left orbit along with a bony depression lateral to the orbit. He had been operated at a multispecialty hospital, but there was no improvement in his diplopia and facial asymmetry. Treatment planning was explained to him and he consented to surgery. Herniated fat and other entrapped orbital contents were released and the blowout fracture was repaired with a titanium mesh. He also underwent superior placement of his lateral canthal ligament and augmentation of the bony depression with a bone graft. There was complete correction of his diplopia and facial asymmetry following surgery. Resurgery of Facial Fractures with Blowout Fracture Correction of Right Orbit This young man had been involved in a four wheeler accident with resultant right-sided facial fractures involving the orbital rim and zygomatic bone. He also had a blowout fracture of the right orbital floor with entrapment of orbital contents and herniation of fat. This had resulted a lowered right orbit and diplopia. Surgery had been performed at a multispecialty hospital, which had left him with facial asymmetry as well as a lowered right orbit due to the blowout fracture not being addressed. The diplopia was hence not rectified. He had met with a local surgeon who had referred him to our hospital for management. The plates from the previous surgery were removed. The zygomatic bone and supraorbital rim fractures were accessed through a lateral canthal incision as well as intraoral incision. They were realigned and stabilized with plates and screws in normal anatomic position. Superior repositioning of the lateral canthal ligament was performed through the lateral canthal incision. A titanium mesh was utilized to correct the right orbital blowout fracture after release of entrapped orbital contents. This resulted in resolution of his diplopia as well as reestablishment of facial symmetry.
