Cleft Palate Repair in Child with Pierre Robin Syndrome This child was born with Pierre Robin syndrome along with an accompanying isolated wide cleft palate deformity. Children with Pierre Robin syndrome developed obstructive sleep apnea due to their mandibular micrognathia. His parents were extensively counseled regarding his condition and it was decided to perform cleft palate repair at 11 months of age. The Wardill-Kilner palatal pushback technique was used to surgically repair this. As his cleft palate was very wide, greater palatine osteotomy was performed for better mobilization of the greater palatine artery to enable better closure of the cleft defect. Using Delaire’s principle, the abnormal attachment of the levator palati and tensor palati were detached and a hammock was form with the muscle fibers. This recreated normal palatal structure. There was also perfect recreation of the uvula. Completely Collapsed Columella of the Nose – Open Rhinoplasty with Reconstruction of Prolabium This young girl was born with a bilateral cleft lip and palate deformity and had undergone surgical repair as an infant. She now has a collapsed columella and a shortened prolabium that has led to a whistling deformity with exposure of her anterior teeth. The bridge of her nose is also extremely flattened. Costal rib grafts were harvested and used to augment the bridge of her nose through an open rhinoplasty. A double-decker graft was used due to the extreme flatness of her nasal bridge. This gave her a raised and esthetic nasal form. She then underwent a prolabium flap repositioning to reconstruct the shortened columella. An Abbe flap reconstruction of the residual prolabium defect was then performed. This resulted in complete correction of her deformities. Her entire facial appearance was transformed by her surgery. Closed Rhinoplasty and Lip Scar Revision Surgery This gentleman had been born with a unilateral cleft lip and palate deformity, which had been surgically corrected as an infant. He now presented to our hospital complaining of nasal asymmetry and an unsightly lip scar. A closed rhinoplasty was performed with augmentation of his nasal bridge and correction of his nasal deformity with a costal rib graft harvested at the time of surgery. Nasal Speech Correction Surgery – Pharyngoplasty with Positive Suction Test This teenaged girl was born with a bilateral cleft lip and palate deformity. Bilateral cleft lip repair was performed at 3 months of age and cleft palate repair at 11 months of age. She had however always been troubled with improper pronunciation of certain sounds along with a pronounced hypernasality. Of late, her teachers in school had been complaining about inability to understand her speech in class and she was referred to our hospital for management by her speech therapist who diagnosed her with velopharyngeal incompetence. An Orticochea dynamic sphincter pharyngoplasty was performed with creation of a small central port to facilitate normal nasal breathing. A suction test performed at the end of surgery demonstrated proper action of the soft palate, thus indicating complete correction of her velopharyngeal insufficiency. The floor of her nose and nasal sill were reconstructed using a C-flap. This was followed by reapproximation of the vermillion border. Premaxillary Setback Procedure This little girl presented to us with a bilateral cleft lip and palate deformity. She underwent cleft lip repair at 3 months and cleft palate repair at 11 months of age. Her parents brought her back for premaxillary setback surgery. It was decided to proceed with this surgery even though she was only 4 years old as she was facing a lot of teasing at school. Her premaxillary segment was completely mobile. It was decided to harvest a graft from the ribs as her hip bone would be predominantly cartilaginous at this age. Excess vomerine bone was removed and also utilized for the graft. Her premaxillary segment was aligned with the maxilla on either side. The space between the maxillary segments and the premaxillary segment was closed with bone grafts. Abbe flap Correction of Short Upper Lip The patient is a case of repaired bilateral cleft lip and palate with a residual whistling deformity. There was no mustache growth on the prolabium. He now presents for correction of flat nose with collapsed columella and stunted prolabium with a short upper lip. The prolabium was dissected and used to reconstruct the collapsed columella and raise his flat nose. The resultant upper lip defect was reconstructed with an Abbe flap harvested from the lower lip. Bilateral Cleft Lip Surgery This little boy had a congenital bilateral cleft lip and palate deformity. His parents were very upset at this. They had been counseled regarding cleft deformities and sent to our hospital for corrective surgery. His bilateral cleft lip deformity was addressed first. At the age of 3 months, he underwent a single stage repair utilizing Paul Black’s technique. No cross incision was made at the prolabium. Available prolabium and columella were utilized for reconstructing the philtral column and Cupid’s bow. This was followed by creation of the nasal floor and vermillion. Bilateral C-flaps were utilized for this. This was followed by meticulous three layer closure of the cleft lip under loupe magnification. Orbicularis oris muscle was approximated at the midline. Cleft Lip, Palate and Premaxillary Setback Surgery for a Hypertelorism Child This little boy was born with a bilateral cleft lip and palate deformity as well as hypertelorism.
