Week 1 after surgery

Very swollen and numb face, oozing puss in my mouth, constant nose bleeds. The swelling managed to get even bigger after I started waning steroid dosages and transitioning from ice to heat mid week (as instructed). My face is enormous! Can’t emphasize this enough. Significantly bigger than post-wisdom tooth extraction which was a surgical pre-requisite 18 months ago (so the jaw can heal). I look OTHERWORLDLY levels of uggo.

Clear liquid diet from a squeeze bottle with a long nozzle. Eating a lot of broth, juice, jello (which needed to be slurped into my mouth like I was a fat sea creature by my loving boyfriend). I graduated to adding watered down nut milk mid-week. Surprisingly not hungry, but probably because my mouth/face is so uncomfortable it’s too busy to think about eating. Eating normally is just unimaginable, so out of the question, that it isn’t even desirable.

I’m drooly, ugly, kind of gross. I’m not minding it that much, but that’s simply the truth of it. I have become comfortable with my inevitable grossness.

I didn’t shit for five days (combination of prescribed narcotics and anesthesia). Lots of strange, uncomfortable bloating. Laxatives come in handy, but still took two days to work.

Heavy duty hygiene routines- salt water and prescription oral rinses, multiple nasal sprays, etc. My mouth opens more at the end of the week than it did on day one, but still I only have pretty minimal available access. It’s one big gooey mess inside and out- unexplained sores all around my lips despite my rigorous aquaphor and chapstick application. Even with the recommended baby toothbrush, I couldn’t brush at all for the first few days. Now, I can barely brush the midsection of my four front teeth (thankfully, because seeing nasty shit build up there was really bothering me).

Pain is a constant. The drugs help, but the only way out is through. Discomfort even more of a constant. It’s wild to feel so much numbness and so much pain simultaneously.

It’s hard to describe how strange the numbness is- you literally can’t feel most of your face. Touching it feels foreign. When I adventured into jello without the help of my boyfriend (jello was the only thing I ate this week that wasn’t from a long nozzle squeeze bottle), I had to do it in front of a mirror, and even then I struggled and made a mess.

Sensation is starting to return to my upper lip! My surgeon did a kickass job and believes my lower lip/face sensation will come back eventually (this can take up to a year, is sometimes permanent which is just a risk for jaw surgery). I feel occasional tingles or twitches in my lower lip which, from what I’ve read, is a good sign that the nerve damage isn’t permanent.

Painful stares when I take short walks. I do look really, really fucking strange, to be fair.

Hardcore general routines- I feel like I’m in active care mode around the clock. Between...

1) needing to take meds every 3 hours (makes for a great night’s sleep, right?) to stay on top of the 10+ medications (prescribed and over the counter, all instructed by my surgeon) and their various schedules

2) required hygiene rituals like nasal sprays and multiple oral rinses

3) meal prep and clean up with all the special tools- bibs, syringes, squeeze bottles so they don’t taste like the last thing in them (i.e- milk to broth: gross!), etc. I bought more so I can dedicate bottles to flavor profiles (one for milk/sweet stuff, one for broth/soup, one for water), but they won’t arrive until week two. The bottles I got are dishwasher friendly, but the dishwasher doesn’t clean out the taste effectively, so they take tedious elbow grease to maintain every time I want to consume something.

4) heat and cold therapy schedules (at first, ice packs 30min of every waking hour, later in the week heat packs or pads 20 mins every two hours)

5) making sure I take a short walk/move my body in some way to make sure I don’t go crazy and circulate blood

6) generally staying comfortable and clean in this bizarre state of being

7) restocking on all the relevant goods to make this magic happen (reminder to self- add this as a tip to stock in bulk when pre-surgery shopping)

It’s legitimately a full time job. Even when I’m relaxing watching TV or reading with my boyfriend, we’re juggling a task or two the meanwhile, someone gets up because I need something what feels like every few minutes.

It’s important to have an incredible support system and a parent or partner that can give you full support week one. No joke!

Tip #1: Buy adult bibs

If you’re having jaw surgery, you’re going to be messy for a while.  Check your dignity at the door to spare your clothes and minimize clean up after meals and hygiene routines by wearing a bib - to get an adult size, you’ll need to shop “elderly” *sigh*.

You won’t be have full oral motor functions or sensations - you’ll probably be numb and you won’t be able to open your mouth fully, so spills are inevitable.

The food you’ll be able to eat in early recovery will be liquid or gelatinous - both of which have a way of getting *everywhere*.  You’re also going to have a long oral and nasal hygiene process (lots of rinses, nasal lavage and sprays) and those get messy, too.  

I recommend getting one with a pocket at the bottom to catch spills.  Your comfy recovery pants will thank you!

Wearing an adult bib isn’t sexy.  But it can be a little cuter with some print!  I recommend these ($24.99 on Amazon) and these ($22.85 on Amazon).  Both have snap pockets to catch “crumbs” (in this case, getaway jello and soup).  They come in 3 pack sets which is useful so you don’t need to clean after every meal - you’ll be busy enough!

I had wiggle room around the neck, so using a towel underneath during particularly messy activities (like warm salt water rinses) helps with the mess!

Bony TMJ Ankylosis Release through Submandibular Approach This young boy had suffered an injury to his jaw as a child and had subsequently developed ankylosis of his right TMJ. His mouth opening had slowly decreased to the point where he is now able to open his mouth by only 0.5 cm resulting in difficulty with eating. His upper central incisors had been extracted to facilitate feeding, but he still continued to have difficulties with both speech and eating. A social worker had subsequently referred him to our hospital. He was bronchoscopically intubated as he had inability to open his mouth. The ankylosis was released through a submandibular incision. His postsurgical mouth opening was 4.5 cm. It was explained to his parents that he needed to do regular mouth opening exercises everyday to ensure that the joint did not get reankylosed. TMJ Ankylosis Release with Interpositioning of Temporalis Flap This young lady had suffered an injury to her jaw as a child, which had not been addressed at that time. She had subsequently developed ankylosis of her left temporomandibular joint. She now has nil mouth opening and has lost many teeth due to dental caries arising from years of inability to perform oral prophylaxis.. Treatment planning was explained to her and she consented to surgery. Bronchoscopic intubation was performed because of the inability to open her mouth. The ankylosed left TMJ was first exposed through a preauricular endaural approach using a modified Alkayat-Bramley incision. Extreme care was taken to ensure that there was no damage to the facial nerve. Correction of Chronic TMJ Dislocation This young man has been suffering from chronic dislocation of his left temporomandibular joint for many years now. He has been terribly inconvenienced by this and had developed a phobia for social occasions. This has progressed to the point where he has turned reclusive. His family was referred to our hospital by a surgeon in his hometown for management of his condition. Imaging studies and examination were performed in our hospital following which a treatment plan was formulated. He was in agreement and consented to the treatment plan. A modified Alkayat-Bramley incision was used to access the left TMJ. Extreme care was exercised throughout to ensure that there was no damage to the facial nerve. An L shaped plate was fixed to the zygomatic arch on the anterior glenoid tubercle in front of the joint. This will serve as a mechanical blockade thus preventing further dislocation of the TMJ. On follow up in a month’s time, he reported that his dislocation never recurred again. TM Joint Ankylosis Release and Facial Asymmetry Correction This young lady had suffered a fall as an infant resulting in left sided TMJ ankylosis. She had subsequently developed a retruded mandible with deviation to the left side. As her mouth opening worsened and she began developing sleep apnea with episodes of sleepiness at school, her parents brought her to our hospital for treatment. She underwent release of her left TMJ ankylosis with gap arthroplasty and interpositioning of her temporalis muscle. Her obstructive sleep apnea resolved shortly after her surgery. It was explained to her parents that she would need to undergo distraction osteogenesis at a later date for correction of her facial asymmetry. They now present for her scheduled surgery. Imaging studies including a 3D CT scan were obtained and revealed that she had a left sided deviation of the mandible with an obvious occlusal cant. Treatment planning was explained to them and they consented to surgery. She underwent distraction osteogenesis of her left ramus with an advancement of 18 mm. A Le Fort I maxillary osteotomy was also performed for correction of her occlusal cant. There was establishment of facial asymmetry along with correction of her occlusal cant. TMJ Ankylosis Release with Gap Arthroplasty for Sleep Apnea Correction This young lady had been involved in a road traffic accident many years ago. An undiagnosed minor injury to the right TMJ had resulted in ankylosis with subsequent retarded growth of the mandible. There was also deviation of the mandible to the right side. It worsened to the point where she could not open her mouth. She had subsequently developed obstructive sleep apnea and snoring because of the retruded mandible. A surgeon in her hometown had referred her to our hospital for treatment. Imaging studies including a 3D CT scan revealed an ankylosed right TMJ and an extremely retruded mandible. Sleep studies revealed that she had obstructive sleep apnea with very poor oxygen saturation levels. Treatment planning was explained to her and she consented to surgery. She underwent an ankylosis release of the right TMJ with gap arthroplasty and interpositioning of the temporalis muscle. This was followed by a left coronoidectomy. Postoperative sleep studies revealed complete resolution of her sleep apnea and she had a postoperative mouth opening of 4.5 cm within a week’s time. Bilateral Pediatric Mandibular Distraction Osteogenesis This little boy suffered a chin injury as an infant, which had resulted in bilateral temporomandibular joint ankylosis and subsequent micrognathia. He had been having obstructive sleep apnea ever since with very disturbed sleep. Teachers had complained that he fell asleep in school and had even snored on occasion. He underwent bilateral pediatric distraction osteogenesis at our hospital with an advancement of 25 mm.
