Lip Reconstruction with Abbe Flap The patient was born with bilateral cleft lip and palate, which was repaired when he was an infant. He has had a residual whistling deformity. Mustache growth was also absent on the prolabium. He wishes to undergo flat nose correction. He has a collapsed columella and stunted prolabium with a short upper lip. His prolabium was dissected and used to reconstruct the collapsed columella and raise his flat nose. An Abbe flap harvested from his lower lip was used to reconstruct the resultant upper lip defect. Bilateral Cleft Lip Surgery This little boy had a congenital bilateral cleft lip and palate deformity. His parents were very upset at this. They had been counseled regarding cleft deformities and sent to our hospital for corrective surgery. His bilateral cleft lip deformity was addressed first. At the age of 3 months, he underwent a single stage repair utilizing Paul Black’s technique. No cross incision was made at the prolabium. Available prolabium and columella were utilized for reconstructing the philtral column and Cupid’s bow. This was followed by creation of the nasal floor and vermillion. Bilateral C-flaps were utilized for this. This was followed by meticulous three layer closure of the cleft lip under loupe magnification. Orbicularis oris muscle was approximated at the midline. This was followed by approximation of the subcutaneous tissue and skin. Bilateral cleft lip repair This little girl was born with a bilateral cleft lip and palate deformity. Her distressed parents were referred to our hospital for surgical management. It was explained to them that the first step of rehabilitation involved uniting the bilateral cleft lip. This would be done as a single stage surgery. Using Paul Black’s technique and without making a cross incision on the prolabium, the available prolabium and stunted columella were utilized to recreate the philtral column and Cupid’s bow of the lip. Bilateral C-flaps were then raised to reconstruct the nasal floor and the vermillion. This was followed by meticulous three-layer closure under loupe magnification. The orbicularis oris was approximated at the midline followed by the subcutaneous tissue and skin.
