I am not an expert on data security or data integration, but great minds are working to solve those challenges. With respect to trusting AI-generated predictions, we need efficient and credible ways to validate those predictions.

Certis has taken a two-pronged approach to this: in silico validation via internal, cross-validation studies, and in vivo validation—performing studies in clinically relevant mouse models to evaluate the accuracy of our platform’s predictions. Over time, these tools will also be validated clinically in human patients—but of course, that will take a great deal of time and money, as well as the willingness to change the current cancer treatment paradigm. The medical and regulatory community will have to stop relying on how things have always been done and embrace the power of computational analyses to inform decisions.

Modern medicine doesn’t yet have a great way to match patients to the ideal treatments. I'm going to get a lot more personal on here than I usually do:

Baulder's Gate 3, and especially Karlach, make me feel seen in a way no price of media I've ever engaged with before has. It's a big reason why I love it and why it's sticking with me like it is. I too am a big, loud, enthusiastic woman who says out of pocket shit. I have a heavy internal dialogue with myself. I have a temper, I'm bisexual, I love cute shit. I endured years of being utterly touch starved. I love sex and food and being alive...

I also have cancer; it's Follicular B-Cell Lymphoma. I was diagnosed in 2020 when it had reached stage 3A. It's currently under control after chemo* and I'm living my life normally. But while it's a very treatable kind of cancer, it's not fully curable**. There's nothing in me they can remove (it's my whole lymphatic system that's broken) and no drug currently that can wholly knock it out. It's cause is not known (genetically or environmentally), and its unusual for this type of cancer to affect someone as young as I am (I was diagnosed at 35 - it's much more common 65+***). So every three months I go into oncology and they check my blood and symptoms and see if I'm doing ok. I get imaging and biopsies occasionally too. This will go on until I'm not ok (hopefully a long time yet). I'll have to have still manageable but more invasive treatment the next time around. It's a well researched kind of cancer, and my oncologist hopes that in the next decade there may be some more permanent cure for it.

So you can see where I'm going with this. When you hit Karlach's monologue after killing Gortash, I've never felt better understood. I have no one to blame for my condition, no revenge arc of course, but the very same fear, frustration, grief, and anger are all things my husband has heard from me. I've never seen a more beautiful and moving and real exploration of the topic that resonates with me so meaningfully out of a piece of fiction that I love. Béart's performance is amazing - capturing a whole spectrum of emotion.

Karlach's story is absolutely not incomplete as written, confirmed by both the devs and Sam Béart - a terminal condition is just that. You don't get to fix or save her, you get to go with her through tremendous trial and difficulty. If you want to continue that story in your mind that's great (I know I'm eager to do so, for my own personal encouragement if nothing else) but on it's own it's a whole story about coming to terms with something that doesn't have a quick and easy little fetch-quest resolution.

So - every time I see another whinging post/essay/bitchfest about how her story feels "unfinished" or "incomplete" or "has too much cut content" you can see why I might take it a bit personally - knowing that the life I have, the easy solutions I don't get, and the frustration and grief I live with is seen as incorrect and wrong and bad writing makes me a little pissed off. If you think the story is incomplete as told and think a 'third engine upgrade' is missing you completely missed the point.

Karlach doesn't get a simple easy ending because people like me**** don't get that either. And like my husband and the others who love me you can choose to follow that story anyway. And that's fucking beautiful.

This may be an over-generalization but I think a lot of misguided home remedy type people are interested in options that work (or appear to work). And by that assumption, showing them things that actually work or showing them how their current methods lead to the opposite desired outcome can sometimes open the curious door to better resources. Idk, I don't think people are stupid. I think people can do stupid things, especially when they're scared. And when you're scared of a health issue, you may end up doing stupid shit like offering special tea to a loved one bc you think it'll cure their cancer. Arguably you would like better for this person, and the only tool you have is tea that doesn't work at best and at worst nullifies medicines. In my experience, meeting someone where they're at can look like asking what they know about their remedy and what they want from it.

Maybe they were on the right research track and got pulled into a snake oil rabbit hole. Kill the snake oil salesman but help the person tricked into buying snake oil. Did you know the USA's breadth of cancer knowledge is published publicly for anyone to access? We also have our international knowledge pool available. There are patient verbiage versions so you don't have to learn a quarter of oncology to follow their explanations. Did you know snake oil sells best when the salesman knows you are scared bc you haven't heard the medical GOOD news? Scientists have a shotgun in cancer's mouth, girl. My grandmother was able to beat cancer in 7 MONTHS bc of trust in her awesome doctors. Any "doctor" trying to make a sale with a treatment may as well be Dr. Kellogg to me.

Idk! I live in the south and work with the Affordable Care Act patient population so I spend lots of time demystifying and encouraging (very conservative) people to explore their options. In my experience, I discover most times that while conservatives are not more or less doctor-averse than others, they ARE more likely to have been misinformed about a health remedy (often by someone close to them who didn't even know the remedy was fake!). You cannot treat gout with cayenne and apple cider vinegar. The temporary relief may make you confident that your money saving solution works, but the disease process will continue to worsen and you will find yourself looking down the barrel of necrotic tissue + amputation. You cannot lose weight by injecting pregnancy hormones. A doctor charging you $1500 a month for a disproved "treatment" does not care if you gain weight or get sick from it. You do not have secret intestine parasites. You do not have secret toxins.

You are working so hard to ensure good health, but you are securing an early decline. At the very least, let me leave you with a list of trusted, peer-reviewed resources. You have the investigative initiative and the drive to make healthy decisions, so try applying those traits in a sphere that does not have anything to gain from lying to you about treatments.

People very rarely change their minds in front of you. Let me leave an objectively helpful foothold for you to explore other options. I try not to get hung up on timelines but I swear Wilson's timeline makes no damn sense to me 😭 I don't believe we are ever given a precise birth date for him, so assuming he's about the same age as his actor (who already looked incredibly young for his age) he would've had to be a child prodigy as well for the timeline to make sense.

RSL was born in 1969, so we'll use that year for Wilson too bc I just cannot imagine him being any older than that. He was 35 when the show aired and could've easily passed for his 20s. Aging him up more than 35 feels incredibly unrealistic with how young he looked.

Being born in 1969 means he likely graduated high school in 1987ish. Four years of undergraduate school already puts him at 1991 before he even starts med school. Another 4 years in med school puts him at 1995, 3 years after he meets House. And that doesn't even include his residency years after he graduated.

But wasn't Wilson already a doctor when they met? He said him and Sam split because he was working two jobs while doing his residency in order to put her through med school. So we can extrapolate that when he met House, he had graduated but was still going through his residency in 1992.

For shits and giggles, let's say he graduated as a prodigy at 16 in 1985, 4 yrs of undergrad- 1989, and let's say he did the accelerated program that allowed him to graduate med school in 3 years, 1992. That would make sense if Wilson had only just started his residency when he met House. But according to his lore, he must've been doing his residency for a while in order for it to affect his marriage so much that they divorced. They'd already been split up for a while before he was served divorce papers.

Even if that timeline makes a little more sense, what doesn't make sense is that he would've finished his residency between 1995 and 1998 (even later if he didn't graduate early) and then sometime between then and before the show aired, he became the HEAD of the oncology department. I'm pretty sure he had been department head for a while before the show aired, but even if he'd only been promoted to department head just before the show started, that means he was made department head 5-9 years after finishing his residency. I don't know how ranking in a hospital works, but less than a decade out of his residency feels like a very short amount of time to become head of a huge department in a big teaching hospital— one of the best in the country.

I guess the solution that makes the most sense is to consider him to be closer to 40 at the beginning of the show, but you cannot look at that young sweet face in s1 and tell me that man is anywhere close to 40. He doesn't even start looking close to 40 until s6 imo.
