Imagine lying back in an enclosed chamber where you bask for 90 minutes in a sea of pure oxygen at pressures two to three times that felt at sea level. This is the world of hyperbaric oxygen therapy (HBOT), a technology that’s been around for decades and is now being explored as a possible treatment for Long COVID.
"The silence on the inside is deafening at first,” says John M.,* who has undergone dozens of HBOT treatments for his persistent Long COVID symptoms. Fortunately, there is a television outside the chamber in view, and it is easy to communicate with the provider if needed. While the potential protocol is still being refined, patients may undergo up to 40 HBOT sessions to address some of the most problematic, lingering symptoms of this complex condition.
HBOT is a therapeutic process that has been widely used to treat such conditions as decompression sickness in scuba divers, carbon monoxide poisoning, and diabetic foot ulcers. In HBOT, the body is exposed to 100% oxygen, a significant increase from the 21% oxygen concentration we typically breathe. The therapy takes place in an enclosed chamber where the air pressure is elevated above normal levels. The combination of high-pressure and high-oxygen conditions enhances the amount of oxygen that can reach the body's tissues. The hope is that this therapy can provide the same relief and healing to people with Long COVID that it does for those with other conditions.
According to John M., HBOT was the first treatment that helped with his sleep and reduced his heart palpitations. “At one point after hospitalization, my Long COVID symptoms were so bad that I could barely walk or talk. HBOT was a great tool that really assisted with my recovery,” he said. John added that he hopes the medical community will achieve a better understanding of how HBOT can help relieve suffering for patients with Long COVID and that more research will increase access to this innovative therapy.
Does HBOT improve Long COVID symptoms? One key observation from the work of Inderjit Singh, MBChB, an assistant professor at Yale School of Medicine (YSM) specializing in pulmonary, critical care, and sleep medicine, is that Long COVID patients often experience debilitating fatigue. Based on Dr. Singh’s previous Long COVID research, the exhaustion is thought to be linked to the muscles’ inability to efficiently extract and utilize oxygen.
To picture how HBOT might work, you can think of your muscles as engines sputtering, struggling to get the fuel they need. If oxygen is the gas that fuels the muscles, it’s as if you are trying to complete your daily routine while the gas tank is running on “empty.” By aiming to directly address this oxygen utilization impairment, HBOT may be a potential solution.
A systematic review by researchers at the China Medical University Hospital noted that HBOT could tackle another major factor in the Long COVID puzzle: oxidative stress. This relates to the body's struggle to maintain balance when harmful molecules, known as free radicals, run amok, causing chronic inflammation.
Research co-authored by Sandra K. Wainwright, MD, medical director of the Center for Hyperbaric Medicine and Wound Healing at Greenwich Hospital in Connecticut, suggests that HBOT, with its high-oxygen environment, might dampen this chronic inflammation by improving mitochondrial activity and decreasing production of harmful molecules. Other potential benefits of HBOT in the treatment of Long COVID may include restoration of oxygen to oxygen-starved tissues, reduced production of inflammatory cytokines, and increased mobilization of hematopoietic stem cells—primary cells that transform into red blood cells, white blood cells, and platelets.
HBOT for Long COVID: Current and ongoing research Several small-scale reports have indicated that HBOT is safe for patients with Long COVID.
To address this question, a trial that followed the gold standard of modern medical research—a randomized, placebo-controlled, double-blind design—assigned 73 Long COVID patients to either receive 40 sessions of HBOT or a placebo of only 21% oxygen. The study observed positive changes in attention, sleep quality, pain symptoms, and energy levels among participants receiving HBOT. In a longitudinal follow-up study published in Scientific Reports, the authors at the Tel Aviv University found that clinical improvements persisted even one year after the last HBOT session was concluded. In a second study, the same authors focused on heart function, measured by an echocardiogram, and found a significant reduction in heart strain, known as global longitudinal strain, in patients who received HBOT.
In another study, 10 patients with Long COVID underwent 10 HBOT treatments over 12 consecutive days. Testing showed statistically significant improvement in fatigue and cognitive function. Meanwhile, an ongoing trial at the Karolinska Institute in Sweden has reported interim safety results wherein almost half of the Long COVID patients in the trial reported cough or chest discomfort during treatment. However, it was unclear whether HBOT exacerbated this symptom or if this adverse effect was due to the effort of participation by patients suffering from more severe Long COVID symptoms.
Is HBOT currently available as a treatment for Long COVID? For HBOT to become a mainstream treatment option for Long COVID, several critical priorities must be addressed. First, there is currently no established method for tailoring HBOT dosages to individual patients, so researchers must learn more about the specific features or symptoms that indicate potential benefits from HBOT. At the same time, we need to identify factors that may be associated with any adverse outcomes of HBOT. And finally, it’s important to determine how long these potentially beneficial effects last in a larger cohort. Will just a few HBOT trials be enough to restore patients to their baseline health, or will HBOT become a recurring component of their annual treatment regimen?
For now, HBOT remains an experimental therapy—and as such is not covered by insurance. This is a huge issue for patients because the therapy is expensive. According to Dr. Wainwright, a six-week course of therapy can run around $60,000. That’s a lot to pay for a therapy that’s still being studied. In the current completed studies, different treatment frequencies and intensities have been used, but it’s unclear how the treatment conditions affect the patient’s outcome.
“I have had some patients notice improvements after only 10 or 15 treatments, whereas some others need up to 45 treatments before they notice a difference,” notes Dr. Wainwright. “I think that HBOT is offering some promising results in many patients, but it is probably a strong adjunctive treatment to the other spectrum of things Long COVID patients should be doing, like participating in an exercise, rehab, and nutritional program.”
Dr. Singh notes that “a major challenge for research is the heterogeneity of Long COVID. It is hard to determine which symptoms to treat and enroll patients into trials based on them.”
Perhaps treatments that target multiple issues caused by Long COVID, like HBOT, may help overcome this challenge.

The last word from Lisa Sanders, MD: Hyperbaric oxygen therapy (HBOT) is just one of the many existing treatments that are being looked at to treat Long COVID. We see this with many new diseases—trying to use a treatment that is effective in one set of diseases to treat another. And there is reason for optimism: We know that HBOT can deliver high levels of oxygen to tissues in need of oxygen. That’s why it’s used to treat soft tissue wounds. If reduced oxygen uptake is the cause of the devastating fatigue caused by Long COVID, as is suggested by many studies, then perhaps a better delivery system will help at least some patients.

saw a bunch of libertarians citing the recent and from what i can tell very awful case of indi gregory, a small child with a rare mitochondrial illness taken off nhs life support whose parents were denied the right to transfer her to an italian hospital that agreed to see to her medical needs. which was ofc trotted out as an illustration of the evils of statist health care, death panels, etc
the obvious retort here ofc is that the main effect of privatising healthcare on cases like these is to multiply them, but what particularly interested me was the judge who issued that ruling, robert roger peel. turns out peel is currently the lead judge of the uk's financial remedies court (for determining financial disputes between divorced or separated couples), and has published an interesting couple of articles on his (very positive) assessment of the court. a lot of his focus is trained on time/cost minimising, settlement:trial ratio maximising measures for the court to implement or that the court has implemented, including (from the 2nd one)
the extensive use of "private fdr's," a sort of privatised dispute resolution in place of the court itself, in which an ex-couple hires a "private fdr judge" (could be a solicitor, barrister or retired judge) to rule on their case without recourse to the actual judicial system. ("So too the widespread use of Private FDRs. Judges need little persuasion to permit parties to attend a Private FDR and return to court thereafter for, as the case may be, a mention hearing to endorse the consent order, or a directions hearing to timetable to trial. The use of Private FDRs has in turn relieved pressure on the courts.")

in short, while this is definitely not an area where i have domain knowledge, my first impression is that the judge responsible directly for this decision is sort of a miserly ghoul happy to undermine the rights of individual brits in the service of shrinking and cost-cutting that portion of the govt over which he exercises authority. (to his credit he does at least declare the lack of public funding for legal representation to be "iniquitous", tho ofc that bit is out of his hands.) many such cases!

High-intensity physical exercise is one of the five key and foundational inputs I advocate for inclusion in a lifestyle which seeks optimal health outcomes and here’s why

Human beings, as hypercarnivores in the context of physiology and evolutionary history, are biologically primed for high levels of physical activity. Throughout our history physical exertion has played a key and central role in not just our survival, whether through hunting, gathering, or migration, but also in the informing of our very physiology and how our bodies work too. As such, our physiology has adapted to engage in complex metabolic processes that support not only efficient energy production but also cellular resilience and systemic coordination. In contemporary society, the benefits of regular physical activity, especially high-intensity exercise, are widely recognized, particularly in terms of maintaining optimal energy balance and body composition. However, to understand the full significance of high-intensity exercise, it is crucial to examine its effects from a biochemical and multisystem perspective, focusing on how it serves as a modulator for the endocrine, vascular, nervous, and musculoskeletal systems – yes, physical activity serves a far more complex purpose for our health and well-being than simply helping sort our body fat levels

I advocate strongly that regular high intensity physical exercise, whether as part of a sport / game or as dedicated training in a gym or similar setting, is a MUST HAVE for optimal health outcomes because of its physiological impacts. It is crucial for people to develop an appreciation of the role of high-intensity exercise in optimizing cellular health and overall metabolic function - approaching these mechanisms from a standpoint of mitochondrial energy metabolism and system-wide adaptations, rather than simply through the lens of energy expenditure!

I want to highlight a very important paper published in January in Nature Communications by a group from Amsterdam. They looked at muscle abnormalities worsening after exercise in patients with Long COVID. This symptom is very common in chronic fatigue syndrome, and we see it in over 80% of patients with Long COVID. In this study, they included 25 patients with Long COVID and 21 age- and sex-matched controls who had fully recovered from COVID-19. They collected blood and muscle biopsies before and after exercise. Both groups were healthy and socially active. None of the participants were hospitalized due to COVID-19 infections. Fatigue questionnaires and exercise data confirmed the impact of Long COVID on daily lives. The study found that people with Long COVID have lower VO2 max, indicating decreased oxygen absorption and reduced peak power output. They also had lower gas exchange (CO2 and oxygen) levels, suggesting that their muscles were affected by physical activities. Structural changes in the muscle were observed, with a shift from type 1 fibers (aerobic) to type 2X and 2A fibers (anaerobic). This indicates a shift towards glycolytic metabolism, which is less efficient for sustained energy production. Further, mitochondrial function was compromised in patients with Long COVID, showing lower oxidative phosphorylation capacity. This means their cells were less efficient at producing energy. After exercise, these changes were more pronounced. Mitochondrial dysfunction was evident, as seen by decreased oxygen transport and increased markers of mitochondrial stress. Interestingly, the study found no amyloid depositions in healthy controls or fully recovered individuals, refuting the theory of microclots as a cause of Long COVID symptoms. However, in Long COVID patients, amyloid deposition was found around the vascular endothelium, indicating a possible biomarker for this condition. Muscle atrophy was also observed in Long COVID patients, with increased damage and necrosis after exercise. This suggests that physical activity can exacerbate muscle damage in these patients. Additionally, signs of muscle regeneration were noted, indicating ongoing muscle repair processes. Metabolic abnormalities were also highlighted, including reduced TCA cycle activity and lower levels of creatine and ATP synthesis. This contributes to the reduced oxidative phosphorylation capacity in Long COVID patients. In summary, this study confirms mitochondrial dysfunction and metabolic abnormalities in Long COVID patients, with a shift towards anaerobic pathways. Physical activity exacerbates muscle damage and inflammatory responses, leading to further complications. The presence of viral proteins in both healthy and Long COVID patients suggests ongoing immune activation, but not active viral replication.

Biology:
Witches are humanoids whose origins sprung from the bridge between Velsaid and the mortal world. Though their physical traits do not differ from the variations displayed in the human genome, their lifespans are much longer and they possess greatly versatile magic. The average witch is expected to live around 200 years.
Contrary to common connotations, "witches" refer to the entire species and both sexes, rather than strictly female witches. However, since some magic is passed matrilineally (through mitochondrial DNA), the most commonly-spotted witches in the earlier centuries tended to be female, partially because they were more likely to leave their covens and venture into human settlements.
Magic:
The two main categories of magic are Common and Clan magic. Clan magic is a unique form of magic that manifests differently in each family. As previously stated, Clan magic is matrilineal, as it is passed through the mitochondrial DNA. This means that while all witches possess magic, witches that are male in sex are unable to pass clan magic onto their offspring. Clan magic cannot be learned or obtained through other means beyond genetic inheritance, though spells or potions with similar affects might exist. These substitutes are typically much weaker and less efficient than the inherited version. Clan magic typically manifests as traits or qualities, but those that manifest as abilities exist as well.
Within the subset of Clan magic, the two branches are Active and Inactive. "Active" Clan magic are traits and qualities that can't be "turned off", and are therefore constantly "active" such as enhanced durability or rapid healing. Those that are "inactive" must be intentionally used, such as telepathy. Clan magic is often the most instinctive and first to manifest in witches. However, it has its limits. Inactive Clan magic has a tendency to be more dangerous and difficult to control. However, it also tends to be more powerful than Common magic. Active Clan magic is safer and doesn't require control. It doesn't "drain" a witch the way Common or Inactive magic can. However, it tends to be weaker than even Common magic alternatives of the same ability (though not always).
Common magic is the magic that all witches are able to wield. This can come in a range of forms, from elemental to utility to combat-based. Most witches use Common magic in their daily lives, from healing injuries to making their tea stay warmer for just a bit longer.
Magic, being so untameable a force, requires much practice, skill, and control. Therefore, in order to learn how to properly control their magic, young witches are taught using wands, which serve as conduits that focus and strengthen their magic control, much like a metal rod for electricity.
Once strong enough, witches will abandon wands and perform their magic on their own. However, there are still occasions in which they will opt to use a wand, typically when casting magic above their own caliber, as the wand will serve to amplify their own magic and lessen the burnout. If performing a particularly powerful and/or dangerous ritual or spell, they might choose to use a staff, which is far stronger than a simple wand.

The brain is unique in that it houses our cognitive and emotional capacities in the form of the mind. It is a 'cognitive' organ that hungers for stimulation from new experiences and challenges. Many brain fitness programs strive to satisfy this need. Yet the brain is also a physical organ that plays by many of the same rules as the heart, lungs, liver and kidneys. To stay healthy and perform optimally it requires quality nutrition, physical activity and optimal sleep. The brain, especially, relies on a healthy vascular system to efficiently deliver oxygen and key nutrients and remove waste. In fact, the brain uses approximately 20% of the oxygen we breathe to satisfy its high-energy demands. Given that the brain only weighs about 2% of the body, we can consider it an energy hog and we must cater to its needs very carefully.
Nutrients play key roles in brain function. Several have shown efficacy in clinical trials treating cases of mood disorders, cognitive decline and of course benefiting the physical health of the brain. Nutrients are both the raw materials employed in creating new neural connections and important components in regulating the activity of genes involved in these processes. Specific nutrients involved in mitochondrial efficiency, the energy factories of brain and body cells, are particularly important for many aspects of brain function. Other nutrients are involved in the inner workings of neuronal membranes, responsible for ensuring that electrochemical signals, which make up our thoughts, transmit efficiently and reliably. Finally, antioxidants, important throughout the body, are especially important in the brain due to its high energy production rates and concurrent high capacity for free radical leakage. Keeping this in mind, it is readily apparent that nutrition provides the building blocks for our brain's structure and function, and therefore cannot be ignored.
Exercise is a clearly established component for promoting brain health as well. No longer can we think that the brain is completely separate from the brawn.
