As someone who has worked in nursing and residential homes this is something important.
Care workers support some of the most vulnerable people in our society. They are often overworked, exposed to hazardous conditions, stretched in all directions, stressed out... Good care workers are worth their weight in gold and they're paid peanuts.
I see a cleaner up there. We would be in a much worse place if we didn't have dedicated cleaners making sure places were hygienic and safe.
Hospital porters. They don't get anywhere near the respect they deserve. If it wasn't for them moving people to the spare beds when they appear then emergency departments would be even more jammed.
Unskilled jobs are often difficult, stressful. They are not valued highly enough.

Toh Hospital Au
(I’ve never actually watched any hospital dramas before)
This is just a adult human Au except with a focus on medicine
Luz, an oncologist, was determined to be one after the passing of her father.
Amity, pediatric rheumatologist, inspired by her own hand pain. Was originally going to be a dermatologist to make Odalia proud but met Luz in college after becoming roommates with her and changed her plans into a field she was actually passionate in. Pediatric because is Amity good with kids.
Vee is a RN in the ER. She good at adapting at different situations which is good for more high stake places like the ER. Her and Luz are sisters of course with Camila very proud of them both. She’s adopted. She didn’t want to go to medical school so decided on being a nurse instead. Worked in the hospital longer than Luz.
Gus is a patient advocate, he would be good very successful at his job and passionate on helping his clients receive the best treatment he can. He’s taken many clients with Luz.
Willow is sports medicine physician because it’s in her interest in athletics and helping others. She definitely still a plant addict and they cover her house and garden.
Hunter is a cytopathologist. He had a tough past (understatement) and was pretty isolated in college and medical school. He thought it would be the same in the hospital since he already took a no live patient job until he transferred to a different hospital and met Luz in the Cancer unit when diagnosing some patients. Where she invited him to hang out after work and they became friends along with the others.
They all look like older versions of their post-canon looks since they’re all in the mid 30s, except Gus who is like 31 to 32. Hunter still has his scars except their burn scars instead of possession ones. Luz and Amity are married and live together. Belos died sometime in Hunter stay in medical school and Amity, and her siblings are no contact with Odalia
Stringbean in a normal snake that Luz got from a rescue, Emmiline is a normal chameleon Gus recused, ghost is a normal cat Amity got in college, Clover is a bunny rescue( I’m not giving Willow a pet bee I’m sorry, instead her fathers do bee keeping as a hobby). Flapjack a cardinal that just lives with Hunter, like one day he showed and didn’t leave, he basically made Hunter his pet. Also Waffles does the same thing a few years later (because I love her). Flapjack lives longer than the average cardinal due to Hunter basically taking care of him. The other laugh when they find out he basically has two wild birds living with him.
Belos was pretty abusive to Hunter growing up until Hunter fled for college after realizing that it wasn’t right how he was treating him. He tried going no contact a few times but always broke it untill Belos eventually passed away while he was in medical school after succumbing to an illness. Belos was a successful pastor. (He was the owner one of those mega churches n stuff.) Hunters parents are Caleb and Evelyn. Caleb was killed by someone “unkown”, Evelyn died from an accidental overdose when Hunter was young, where Belos got custody of him rather than the Clawthorne family. The burn scars came from an “accident” in middle school for Hunter.
Gus went to high school early and had a hard time with people not taking him seriously in both school and even when doing his job ( no body cared in college) but was able it receive support from his friends and push forward.
They all met in college and became friends (except Hunter who went somewhere else). Amity and Willow here childhood friends before Amity began bullying her in middle school where they went to separate high schools. Gus and Willow met in high school and were also roommates in college.
Other notes,
Viney is a general practitioner, puddles is an absurdly large pigeon she has.
Emira is a paramedic.
Other characters are not in the medical field like Edric who’s a small animal veterinarian, Lilith who is a historian, and Eda who is a store owner.
Again this is basically my Human realm Au with emphasis on the medical field.

Well I think it’s interesting what the writers did for Street in that moment was let you know that Street was aware of much more around him than maybe you thought at first, and that’s what made him, I think, maybe a great leader. His friendship with Riggins, if you’re just looking at it from the outside you’re like “What is it about- why are these two guys-“ But Street knows something about Riggins that I feel like not everyone else knows, and he sees good in everybody. - But yeah, for me it’s like, the most interesting thing about this episode was watching everybody and how they dealt with grief. Riggins not being able to show up and visit his best friend, and why is that? Like, when there’s a drastic life change for one of your best friends why do we shy away- And that really starts to affect Street, you can tell. He’s like “Did Riggins sign it?” ‘Cause Riggins hasn’t been there yet. And Lyla brings it up later- Minka and Riggins, which is maybe the seed of something as well, that we’ll uncover more about, but you see their relationship and what that really is and how it hinges around Jason. -Scott Porter IT’S NOT ONLY FOOTBALL: FRIDAY NIGHT LIGHTS AND BEYOND

the older I get the more I'm convinced my parents are also both pretty neurodivergent, it's just that during the time when it would have been the most beneficial to them to get diagnosed, their options were 'be a functional member of society' or 'be chained to a bed in the nearest hospital's freak show department'.

Lofty of BBC's Holby City was maybe one of the worst characters on the show. Is this a hot take? Do people outside of me and my family somehow adore Lofty? I can't imagine how. Here is my hateful mean and nasty reasoning. Read at your own risk I'm not accepting criticism at this time lmao:
A) he was effective comic relief on Casualty because it's the Emergency Department and moments of Lofty tripping over or doing a social faux pas are necessary tension breakers in episodes where people are constantly at deaths door.
Casualty is (when it's GOOD) required to have at least a third of the episode dedicated to a narsty horrible accident (hell yeah) and tensions are always running high so you can see how having someone a be awkward is not going to be the worst thing that happened to them that day.
Holby City is a more hospital staff focused show (although again. If there's less than a third of an episode focused on patients it is probably a bad episode. PLEASE HAVE PATIENTS IN YOUR HOSPITAL SHOW.) but there's a lot of waiting around for people in Holby because it's the longer term care zone. If Lofty is dropping stuff in the background of scenes with this lower energy it suggests he has some sort of untreated disability rather than 'argh I'm frantic OOPS'. He is painted more as incompetent on Holby compared to their more grounded staff. Which makes everyone ELSE look incompetent for putting up with it. (To be clear!!!! I'm not saying people with disabilities are incompetent lol if he had one it would have been a storyline!! And if he had one and none of the doctors who see him every day flagged it it also makes them look incompetent. He doesn't CARE enough to be CAREful. And in a show ABOUT caring for vulnerable people as far as you absolutely can. That's a big problem.)
Like the thing is. Characters on Casualty can be one note architypes forever because they're doing medical CSI. Holby spends so much more time with the cast, they NEED to become more complicated and Lofty never does, he's the same guy no matter what happens. The only other guy like that in Holby is the incomparable Guy Self and he's a villain! He is a villain for never changing or trying for anyone! And even he TRIES to reform occasionally. Lofty (and the SHOW) doesn't see a problem with him perpetually disengaging from making emotional choices, putting most of the work of maintaining relationships on other people, and generally coasting through a profession that everyone else is giving 110% to. The building could be on fire and he'd wander outside and not think to rescue any of the patients unless someone else told him to.
B) the B is for Bisexual. Bisexual representation on Holby City where he cheats on his husband with a woman. It's a yikes from me.
C) speaking of Dom. God their relationship was horrible. POOR FUCKING DOM. tw abuse. Lofty talks to and believes Dom's abusive ex over him? He hugs the guy that RECENTLY pushed Dom down a flight of stairs? And they're still supposed to be a cutesy couple by the end of this? He shuts down Dom's excitement constantly? Hello??
D) the D is again for Dom because. It's their one year anniversary. By the way it was on their honeymoon that Dom couldn't go on to support his sick mother that Lofty cheated on Dom. Just by the way. BY THE WAY if your husband says he's going to stay home for your honeymoon and insists you go by yourself so he can stay home to support his sick mother. Maybe. Hm. Stay and support him instead. Just a fucking thought.
Lofty decides it's not working and they amicably split up (Dom is an angel sent from gay heaven by the way. I perhaps have my biases.) Dom kindly gives him the quirky and personal gift he'd bought him for their one year wedding anniversary as Lofty unicycles away (it may have been a bicycle but I picture it as a unicycle). Lofty gives him. Nothing. Which means that this man sat there as they planned a big anniversary party and not ONCE. not ONCE did he consider buying Dom an anniversary present. Because he's an arse!!!!
E) he only does this :| or this :/ for any emotion. I love characters with flat affects, for example my close personal friends Hannah Supernatural, Abed Community, and also as well my actual real human friends who do this 💖💖💖. HOWEVER. Lofty doesn't show emotion any OTHER way either. There IS nothing going on in his heart. He has passion for nothing. He goes where other people point. He avoids giving his opinion because he doesn't have any. This makes him a bad tv character for a drama. Opinion IS story on Holby. There is an issue, they all take sides and fight over it, and he doesn't get involved.
Anyway. Show's been over for over a year I will never have to see Lofty's face ever again as long as I live unless I for some reason decide to do a bizarre Holby City Rewatch and keep going to season 19.
By the way as I was looking up when he came to the Holby side of Holby City Hospital, I found this quote from his wiki page:
"Lofty is portrayed as a nice person who is likeable and popular."
Hm. He IS PORTRAYED as a nice person. The show thinks he's a nice person who is likable. However.

Emergency Contact
Based on an idea by @annurb

Buck patted his turnouts and furrowed his brows as he looked around the rig. He bend forward and brushed an arm under the seats.
“Do I even want to ask what you’re doing?” Hen said.
“Uh.. I can’t find my phone.” He said checking his pockets for the third time.
“When did you last have it?” She asked.
“The hospital.” He answered remembering putting it down on the reception desk while he was deep in conversation with one of porters who had asked him about applying to join the department. “I think I left it at reception.”
“You really want us to turn the engine around for your phone?” Bobby asked incredulously from the front.
“Please?”
“Fine.” Bobby sighed. “But if we get a call on the way back we’re going to it and you’ll have to wait until after shift to get it.”
“Copy that, Cap.”
“It not healthy to be this attached to your phone.” Chim told him.
“I’m not!” He protested.
“Oh it’s not his phone he’s attached too.” Hen teased and Buck rolled his eyes and looked out of the window in a failed attempt to hide the flush to his cheeks.
She was absolutely right, though—it wasn’t his phone he’s was obsessed with but that it was it means of communication with Tommy when the universe dared to keep them apart by working opposite shifts.
It’s had been a world-wind 2 months and Bucks feet were barely ever on the ground Tommy had swept him off them so incredibly. For the first time in any relationship he’d been in he’d never once felt the need to ask or beg for attention—Tommy lavished him with it. If he couldn’t physically be there to cook dinner for him, or with him, or watch movies together or go hiking together, or just simply effortlessly be in each others presence, he was messaging or calling in his spare time during his shifts. And not for any real reason but to talk to his boyfriend.
Buck was already falling hard for the pilot.
“Make it quick, Buck.” Bobby said as the rig pulled up in front of the hospital.
Buck jumped down and jogged into the building.
“Firefighter Buckley, you miss us already?” Nurse Jasper said as he approached the desk.
He smiled. “Uh, I think I left my phone here?”
“Ah. It was yours! I put it in the staff room. Can you wait for just a sec? We got 3 nurses out with the flu so we’re run off our feet.”
“Sure, no problem.”
“Thank you. Cindy?” She called to another nurse. “The lady in 204 needs her meds, and the gentleman in 207 needs his dressings changed. And I’d see to him first—he’s a grumpy one.”
“Is that the hot one?” Cindy swooned.
The other nurse rolled her eyes. “Cindy, this is a hospital and he is a patient.”
“A hot patient.” Cindy said back and Buck laughed.
“Just go and see to Mr Kinard before he gets even worse.”
Bucks heart shot up into his throat at the same time his stomach dropped a thousand feet.
“I-I’m sorry, did you just say Mr Kinard?”
“Yes.”
“Thomas Kinard?” He added just to be sure.
“Yeah. Do you..” Buck didn’t hear the rest of her words as he quickly left the desk for the corridor to find room 207.
A mixture of sadness, anger and relief hit him all at once when arrived at the room and saw Tommy through the window.
The man he- his boyfriend was laid on the bed—his left leg in a cast up to below his knee, a bandage wrapped around his right forearm and a stitches on the right side of his forehead surrounded by deep pinkish red bruising.
“What the hell, Tommy?!”
Tommy’s head shot up, his face a mixture of shock and sheepishness. “Shit.” He mumbled under his breath.
He’d hoped to be out of the hospital before he told Buck what had happened—ask for forgiveness not permission, right? He didn’t want to lie to him, and felt bad for doing so, but in the small amount of time they’d been dating he’d come to know that Buck would throw down his whole life to take care of the people he cared about.
It wasn’t that Tommy didn’t appreciate the sentiment, but he was fully capable of taking care of himself and was absolutely not going to be a burden on his boyfriend.
“What the hell happened?! Why are you here? Are you okay? Why didn’t you tell me?!” The words all melded into one almost indecipherable sentence.
“I’m fine, Evan.” He protested as he squirmed in his bed trying to give off the air of someone who was absolutely totally fine and not at all beginning to regret refusing pain meds from the nurses.
“You’ve got to be kidding me? You’re in hospital Tommy. With obviously a broken leg and other injuries.” He gestured to Tommy’s arm and face. Buck was immediately reminded of his own near career-ending leg injury and fear fizzed its way around his body as he looked at Tommys leg.
Buck was angry as hell that Tommy, or anybody, hadn’t told him that he’d gotten injured, but overshadowing all of that was sadness and worry that Tommy was hurt.
Tommy hated the worried look on Bucks face. “It’s a basic leg break, a few weeks in a cast and some PT and I’ll be back to normal. Really, it’s nothing to worry about.” He argued.
Though that did alleviate Bucks worries a small amount, he still had to breathe away that anger that was trying its damndest to rise up. “Tommy, being fine is not the issue here—you were hurt and you didn’t tell me.”
“There was no point—there’s nothing that you could do. There was no point worrying you.”
“You’re my boyfriend—I’m going to worry regardless.” He told him. “And it’s not about being able to do anything, it’s about wanting to know that you’re okay and being there for you.”
“Yeah, but then you’d insist on taking care of me, which I don’t need you to do.” His voice had an edge of strain to it. He was getting frustrated. This was the reason he hadn’t told Buck—he didn’t need anybody worrying about him.
“I know you’re capable of looking after yourself, Tommy, that’s not- wait.. how long have you been in here?”
Tommy looked away from Buck before he answered. “Two days.” He answered quietly.
“Two days?! So, the whole time you were texting me telling me you were at home resting on your 48 off you were lying to me?!”
“Evan..” Tommy reached out for Bucks hand but he stepped back. Tommy felt a pull on his heart at the rejection. It was the first time since they’d become a couple that Buck had rejected any physical touch from Tommy.
It was one of Tommy’s favourite things about Buck—how tactile he was. Mostly because he was the same. Physical touch was a big love language of his and having a boyfriend who so readily gave it and received it felt wonderful.
“No. You.. you lied to me, Tommy. I-I don’t know what’s worse. That, or that you didn’t think it was important for me to know that you were injured.”
Tears were threatening his eyes. He felt stupid. He thought things with Tommy were different—that he’d found someone who wanted him around.
“Is everybody in the dark or was it just me?” He asked not being able to hold the contempt out of his voice.
“No, nobody knows. Well, aside from my captain. He’d have noticed when I didn’t turn up for my shift.” He huffed a small laugh which did not go down well with Buck.
“This isn’t funny, Tommy. None of this is. What even happened?”
“Engine hoist chain snapped while I was working on my car. Engine landed on my leg and the chain hit me on the arm and face.”
Bucks heart cracked open at the image in his mind of Tommy trapped in his own garage, alone and in pain.
“And you didn’t think to call me?”
“I was too preoccupied with trying to get an engine off my leg, Evan. Besides, I called 911 straight away.”
Buck was having a hard time with this. His boyfriend hurts himself in a way that could have been much more serious, and spent two days alone in the hospital and nobody was there for him?
“You said nobody else knows?” He asked and Tommy shook his head. “Did they not call your emergency contact?”
“I don’t have one listed.” He said plainly.
“Why? You’re an emergency responder—you know how important it is to have someone to make decisions on your behalf if you can’t.”
“I‘ve never had anybody to put down!” Tommy’s voice was raised which surprised them both. Tommy’s frustration was getting the better of him. The problem was Buck had always looked at Tommy like a hero; a big and brave and confident man who had his shit together. And okay, he may have those tendencies now on the surface, but underneath all of it was a man with a lifetime of trauma he was still unpacking at the age of 40. They were early enough in their relationship that he hadn’t felt comfortable talking about it all yet.
If there was one thing he admired the most about Evan it was that he wore his heart on his sleeve and that had the ability to be open and honest about himself and how he felt. Tommy knew that his boyfriend probably wouldn’t run a mile when, if, he told him about his childhood, but by his own previous experiences in relationships, there was always that chance.
But, he had to give something. The fact was he was falling for Evan and it utterly terrified him. It terrified him that Evan would figure out who he really was and want nothing to do with him. But what was more terrifying was losing him.
He took a deep breath to calm himself. “Evan, it’s been a long time since I’ve had anybody in my life that actually gave a shit about me. And I had come to terms with that—I get by just fine on my own.”
Buck’s heart broke hearing that. His parents hadn’t exactly been there for him growing up but he at least had Maddie taking care of him. Maddie was always there when he needed someone; always his biggest champion.
“Tommy..” Buck pulled up a chair and sat close to the bed, taking Tommy’s hand in his. “I know you’re fully capable of taking care of yourself, but.. I’m here. I’m in your life now and-and I want to be there for you. You don’t have to do this shit alone anymore.”
He concentrated on not allowing the tears to spill over. He knew what would happen—Tommy would immediately focus on him and comforting him, which Buck didn’t want. Tommy was the one who needed comforting.
Even if his stubborn ass couldn’t accept it.
“It’s not easy for me; to let someone be there. I don’t want to be a burden to anybody.” Tommy admitted quietly.
“You’re not a burden, Tommy. You’re my boyfriend—I want to take care of you. And-and not out of obligation or because you’re incapable, but because I-“ He had to stop “L” word from falling out of his mouth. “-care about you. A lot. But you have to let me.”
Tommy looked into Buck always earnest eyes and his mouth moved into a soft smile.
“Okay.” He conceded.
“Okay?”
“Okay.”
“Good.” Buck smiled standing up.
“Where are you going?” Tommy asked.
“To tell Bobby that I need the rest of the shift off, and to get the forms you need to fill in to make me your emergency contact.” Tommy opened his mouth but Buck didn’t let him speak. “Look, it doesn’t have to be me if you don’t want that—you can put down Bobby. Or even Hen or Chim—I know none of them would mind. Don’t argue with me on this Tommy—you’re not leaving this hospital without someone on those forms.”
Bucks face was stern, but with an underlying current of affection that Tommy couldn’t say no to.
“I was going to say that I want it to be you.” He told him.
“Okay then.” Buck said with a smile, walking to the door. “Oh, and don’t think I’ve forgiven you for lying to me. You’ve got a lot to work to do to make it up to me, Kinard.” The corner of his mouth pulled up into a small smile telling Tommy that it would be okay.
“Whatever it takes.” He said back.

Christianity and BBU is covered above but something I've been thinking and working on is Islam and the BBU.
Of course every religious individual is an individual first, but I've taken the view that there's a significant Muslim community contributing to pet lib. Muhammed taught that freeing slaves was a high virtue and it was often considered a form of Zakat by governments. Even when the slave trade was active historically there were some limits on their treatment, and I imagine that would persist in a world where slavery returned in the form of the BBU.
To quote one hadith, "He will not enter Paradise who behaveth ill to his slaves." The humanity of slaves is repeatedly emphasised, which runs contrary to the usual BBU.
I also like to imagine that Gurdwaras would continue to welcome and feed anyone who comes through their doors.

I touch on a lot of the things I think about in my own writing, because world-building is honestly so much fun for me!
I don't talk about religion much beyond individual people though, and so that is something I think about but don't write.
Like, my theory on this is that religious organizations across the board heavily disapprove of the system, with their logic being that this allows people to "escape consequences" of their actions in their life before signing up, and there are TV preachers on this universe's Fox News declaring that all must stand before the judgement of God, even if they were pets in life. "The Lord does not forget your sins, even if you do."
I think this makes for uneasy bedfellows between the pet lib movement's leadership and some church organizations. Both want to end the system but with different rationales behind it.
I also think some churches or religious groups run safehouses, halfway homes, etc. Some better than others. I think a lot of churches have "missions" where they go to "the streets" and try to convince runaways to come to their halfway houses. The kind of people who complain about how the runaways don't always seem sufficiently grateful for their pity and charity.
But I think there are plenty of churches who just show up, help, and provide support without demanding worship in exchange. People who leave backpacks with toiletries and some food in areas where runaways tend to congregate. People who just buy food and cook big meals, even when it's illegal to feed runaways. These are the people who know how to befriend someone and provide aid without pity. I see the Unitarian Universalist church being very much about that method? Not always perfectly.
Churches lobby constantly for legal restrictions on the system but in my BBU world part of the point is that corporations have so totally overrun everything that ANYTHING is legal if a billionaire pays the right senator off.
Nat gets most of her funding from Vincent Shield's various shell companies but also applies for a lot of grants. And I think many of those grants come from churches or religious orgs.
Jake doesn't want to ask for money from religious organizations because of his own lifelong resentment after his childhood church ignored, enabled, and eventually justified his father's abuse of Jake and his mom. It means he has to scramble for money more than Nat.
But, yeah. I think about religion in the USA in the BBU and how it would affect a lot of things, but I don't write much of that in.
