Hello 🌼 I was thanking about facial hair. I'm AFAB. I don't plan on going on T (and I heard it can make you grow facial hair naturally?) but I still would want to grow a beard, or a mustache perhaps, in the future. Would hair transplant help me with that or would my hight estrogen/ lower testosterone make it not work out?

It's a common procedure for individuals who cannot grow facial hair naturally or who have patchy beards, and a beard transplant is a viable option for people who do not want testosterone, or were not happy with the amount of facial hair they grew on testosterone.
Transplanted hair behaves like natural hair. Once it's transplanted, it will fall out initially and then start to grow back over the next few months, eventually growing like regular facial hair, even if you're not on testosterone.

I believe it's open access and I would highly encourage you to read the whole thing!
Like natural facial hair, transplanted hair may require regular trimming or shaving, depending on your desired look.
Testosterone & facial hair:
However, as you've heard, testosterone can indeed stimulate facial hair growth, which is why individuals who take testosterone (T) may experience increased facial hair over time.
You mentioned you do not plan on taking T, which is perfectly valid, but if you think there's a chance that you may change your mind in the future then I would consider waiting for the facial hair transplant until you're at least a couple of years on T because it can take time for your facial hair to fill in.
If you choose to not take T and just go straight for the transplant, having higher levels of estrogen won't necessarily negate the effects of a hair transplant. The transplanted hair is genetically programmed to grow in its new location.
In theory, the lack of testosterone might mean that the hair grows less densely or slowly than it might for someone with higher testosterone levels, but I don't think that's necessarily the case here because the follicles being transplanted are from the head so they'd behave more like head hair than beard hair.
Finding a Trans-Friendly Provider (for a facial hair transplant):
If you're considering a facial hair transplant, the first step is to consult a doctor specializing in hair restoration. They can assess your hair quality, discuss your expectations, and determine whether you're a good candidate for the procedure.
Referrals: Consider asking for referrals from local LGBTQ+ groups, transgender support groups, or healthcare providers known to be trans-friendly.
Consultations: Schedule consultations with potential surgeons. Use this opportunity to ask about their experience with transgender patients and facial hair transplants specifically.
Online Research: Look for reviews or testimonials online. Some websites are dedicated to reviewing medical professionals, and others are focused on the LGBTQ+ community.
Professional Organizations: Check if the surgeon is a member of professional organizations like the World Professional Association for Transgender Health (WPATH), which could indicate they're up-to-date on best practices for transgender healthcare.
Keep in mind that any surgical procedure comes with risks and costs. Ensure you're aware of these, and don't hesitate to seek a second opinion if you're unsure.

Online communities, support groups, or LGBTQ+ centers can be great resources, but it isn't a very common procedure so you may have more look looking to cisgender men who have done similar things.
Facial hair transplants are often considered cosmetic procedures, which many insurance plans do not cover. However, coverage varies widely, so it's important to check with your insurance provider. It may be possible to appeal a denial for coverage by saying testosterone isn't indicated for you so this is a necessary treatment to resolve gender dysphoria etc.
If a procedure can be deemed medically necessary (for example, as part of gender-affirming treatment), insurance might cover it. A therapist might be able to help you make this case to your insurance company by writing a WPATH-style letter of support saying that you need it for Gender Dysphoria Reasons tm. It's also helpful if the office of the doctor doing the hair transplant writes a letter of medical necessity too.
Look for a surgeon who is board-certified in plastic surgery or dermatological surgery. This indicates they've met specific educational, training, and professional standards. It's also beneficial if the surgeon has experience with transgender patients, as they may be more attuned to your specific needs and goals, and may know how to get insurance to cover it.
It may seem obvious, but you have to make sure they have extensive experience with hair transplants, particularly facial hair transplants. Unfortunately some doctors are in it for the money and don't put patients first and/or think they're more capable than they are. To make sure they're up to the task, ask to see before and after photos of previous facial hair transplants they've performed, especially for patients with similar goals to yours.
And again, look for patient reviews or testimonials about their experiences with the surgeon. Pay particular attention to the experiences of trans individuals. I can't emphasize how important we are as a community-- we need to look out for each other and make sure that people don't see predatory providers!
Finally, it's crucial that you feel at least somewhat comfortable with the surgeon. They should treat you with respect, listen to your concerns, and provide thorough, honest answers to your questions. You don't need to be best buddies and they don't need a stellar personality either, but you need to feel like they're competent and that you are comfortable asking them questions about your care and could speak to them if you had complications.
While you wait for your consult, there is one alternative that you can try.
Minoxidil (Rogaine):
You can try minoxidil to increase your hair growth. A lot of people use minoxidil (rogaine) on their faces, and a few use it for body hair. It’s considered an “off-label” use- it’s officially approved for head hair and not other locations. Lots of medicines are used off-label, even prescriptions can be given off-label safely. Topical minoxidil is generally safe, as long as you use the dose recommended by the doctor/the bottle.
There's anecdotal evidence that minoxidil can stimulate new facial hair growth and may help thicken existing hairs. However, its effectiveness can vary, and there's limited scientific research on its use for facial hair specifically.
You can get it over-the-counter (aka without a prescription) online through Amazon or other sites, or at a pharmacy like CVS or Walgreens (it’s usually in the isle with men’s hair products like beard dye, and razors).
Minoxidil is generally safe but can cause side effects like dry skin, itching, and irritation. It's also a long-term commitment; you'll need to continue using it to maintain any new hair growth.
You shouldn’t use more than the recommended dose per day, no matter what body part it goes on (i.e. you shouldn’t cover your legs in it, unless you can stretch the normal dose that far). This is because minoxidil is a vasodilator, which lowers blood pressure. You also shouldn’t use oral minoxidil for hair growth because oral minoxidil is used primarily to lower blood pressure.

If you have some peach fuzz from Minox, dying your existing hair darker with facial hair dye might make it more obvious. This is also true for your body hair.

Remember, there's no one "right" way to be you. Whether through medical procedures, hormone therapy, or personal expression, what matters most is that you feel comfortable and authentic in your own skin. If you're sure that you don't want T, then maybe a facial hair transplant might be the right option for you!

I wanted to write a bit about sex segregated spaces, in regards to people who pass as the opposite sex. This is not actually about trans people, as much as it is about the safety, integrity and general rights of male-passing biological women. I am not the only gender non-conforming woman who gets tossed out of female only spaces, based on the false presumption that we’re men. I do not care about validating trans women, or even trans men, for that matter. I care about real life practicality, risks and safety for ALL women, not just those who look conveniently clearly female, which starts with accepting that some women, whether they've medically transitioned or not, pass as male. And none of them should have to feminise themselves to access female only spaces. Whether that be to ensure safety from males, or to just take a leak.
The fact that I choose to keep my beard has almost nothing to do with that I'm male-passing. It may be my strongest "male" feature, but it's hardly the only one. I still pass as male with a clean shaven face, which makes shaving my beloved beard rather pointless, in my opinion. I would realistically need to go through more than just facial hair removal to pass as my own sex again. I'd probably even need facial feminisation surgery, hair transplant, voice feminisation and full body hair removal, at the very least, to even get close to passing as female again. But even then, I'd probably STILL be read as a trans woman, i.e. male. And even IF I did all that... WHY should I have to mutilate myself (a second time) by buying into harmful patriarchal beauty standards, which would worsen my dysphoria and reduce my comfort in my own skin significantly, to be allowed the safety of male-free spaces?
Does that sound feminist to you? Because to me, it's incredibly misogynistic, and strongly counterproductive. To uphold patriarchal gender roles for the safety of women... is the most insanely anti-feminist double standard I can possibly ever think of.
To lay out my argument on this topic, I'm going to use my own experiences as examples a lot. Mostly because I cannot with any conscience speak for anyone else than myself, at least not in such detail and with such harsh judgement. But I'm sure a lot of my experiences are applicable to other masculine women as well.
First off, I still consider myself gender critical, but my allegiance to radical feminism has been waning lately. This is mostly due to that although I agree with the base premise of radfem, I tend to disagree with the proposed solutions to almost all of the issues, because to me they come across as unfounded beliefs (yes, BELIEFS) that "it would just work" without much of any evidence to back up such a claim.
And when it comes to trans people, I've noticed a lot of... shall we say, willful ignorance, going on among many radfems, which does affect opinions on gender abolishion as well as sex segregated spaces to appear rather... intellectually dishonest, to be frank. Although this is not intended as a call out by any means, I merely want for people of all sides of the radfem/gender critical/pro-trans fence to stay critical and keep questioning everything, even one's own beloved ideology. Which I don't see a lot of. Instead I see almost religious defending of radfem as the ultimate/perfect ideology... oh, guess where I've seen that before? I've come to believe that "hivemindedness" is probably part of every possible ideology out there. Even radfem.
So, anyway.
One thing I come across time and time again is the belief (yes, I dare say "belief") that people never pass as the opposite sex, although it's mostly directed at trans people, this very much applies to people who are just gnc as well. Let's not forget that. And this belief seem to often lead to that transitioned/gnc people can just use the space intended for their biological sex, no problem. However, this is not the case. There is a problem. Very many trans people, and some gnc people, pass well enough to at least blend in enough to not raise much of an eyebrow among the opposite sex, and to most definitely stand out as an outsider among people of the same sex. OR they pass barely enough as either sex, and thus stand out as an outsider among both the same sex and the opposite sex, which can cause similar problems with single sex spaces.
There's also the thing that it generally is easier to pass as the opposite sex among complete strangers, compared to people who know you/your background. They tend to read you differently, depending on that.
At least in my experience, complete strangers assume I'm male and don't even as much as raise an eyebrow about how male I come across as. They accept their false assumption at face value. And why wouldn't they? 99,97% of people who "look like me" are biologically men. Then people who know I'm transitioned, but didn't know me pre-transition, tend to see me as a female who looks very convincingly male, whether that makes me a masculine woman, trans man, or any other (female) label in their eyes. They claim to be able to "see" my female nature, yet they somehow had no idea before I told them about my true sex. Then people who know about my history and saw it happen from the time before my transition (now only really my family) never quite succeeded to see me as anything other than a gnc woman. To quote my dad: "You look like a woman who's trying to look like a man." Although I'm sure my mom and sister don't have quite as harsh views about me, lol. They still seem to see me the way they always have, regardless of what name or pronouns they use for me.
This matters, because although people who know I'm transitioned and may even have witnessed my transition from the beginning, struggle to see me as a man (which I respect entirely and I'm VERY careful to not push wanting to be seen/read as anything in particular, but also, people do not want to be rude, especially irl) that does not go for people who have never even seen me before the moment I walk into... say, a public bathroom. To them I cannot possibly be anything other than a man, and it's almost impossible to change their view of me as male once their brains have registered me as such. I need to conjure up pretty fucking compelling evidence to shatter that view they have of me.
This is important, because it means I cannot feasibly use female only spaces, unless someone else (who is also female) vouches for me and explains my situation for me. This is, most likely due to people being more likely to believe an unlikely explanation when it's told by someone else, because maybe I could be lying; and only someone of the same sex as me can accompany/escort me into female only spaces, obviously. But even then, there's a ton of tension around my presentation. An air of distrust, basically. The question that hangs in the air: "Is that a trans woman?" even after they've been given a thorough explanation of my situation. It's uncomfortable for everyone involved. Imagine how it goes then if I'd just show up unannounced, and without someone to vouch for me. I just get booted on sight.
Yes, I can whine about this all day, but that is NOT my point.
My point is that I'm either directly, or implicitly, unwelcome in female only spaces, despite being biologically female, because of my transitioned appearance... despite I'm not even on testosterone anymore since 2 years ago. Sure, most gnc women (whether transitioned or not) don't seem to have turned out quite as passable as me, but clearly, it happens. So let's stop pretending that it doesn't.
So with that in mind, I don't always have access to a gender neutral space. Like for example when I travel with the ferry that goes between my island and the mainland of my country, there is only men's bathrooms and women's bathrooms. No third option. That's a 3 hour boat ride, and with my coffee drinking habit, I will need to pee at some point or another while aboard that ship, alright. And no, peeing in the ocean is not an option, as squatting over the railing would be incredibly dangerous, and most likely not even remotely allowed. Granted, I don't take the ferry often, it's just the most clear example I can think of. Because it's my only means of transportation to/from the mainland, except from flying, which is incredibly expensive, less reliable and obviously an environmental hazard. So when I do have to use that ferry, I'm kinda stuck with my choices.
So then, am I better off going with the men's or women's bathroom? I am much more likely to be left alone to do my business in the men's, so even though that is not the space I want to be in, nor do I think it's "right" for me to be there, sometimes it's even a bit scary, other times even impractical if there's only urinals and no stall, and it's absolutely not validating at all - it's the only bathroom that I can realistically use, without too much trouble. And I don't want trouble. But I also hate having to put my own safety on the backburner for the perceived safety of other women, who are not actually at any higher safety risk when left alone with me.
So, onto the more general, political aspects of this issue:
Women in male only spaces may be less of an issue in regards to safety, at least for the majority of people (men) in that space, especially if the woman in question passes as male. No one gives a fuck, generally. But problem is then that she is at far greater risk than the majority of people (women) would be with a single male, in a female only space. As I think a group of women against one male is generally less risk towards the women, when compared to a group of men against a single female, which can be extremely dangerous for her. Although I've so far never been faced with any sort of violence in a male only space, let's not pretend that my presense in a male only space is somehow LESS dangerous for me, than how dangerous the presense of ONE male in a space with a whole group of women, would be for those women. Statistically and realistically, I'm at a far greater risk than they are, and no, I do not have any more choice in the matter than they do.
Thus, this kinda skewed idea of safety and choice, becomes a question of ethics, I think.
Furthermore, I'm a person of principles, and it wouldn't sit right with me that if males should never under any circumstances be allowed in female spaces, but females could be allowed in male spaces. I refuse to be a hypocrit on purpose! No, if males should never under any circumstances be allowed in female spaces, then females should also never under any circumstances be allowed in male spaces. OR, if females CAN under some special circumstances be allowed into male spaces, then males should be allowed the same in female spaces. Both of these solutions pose serious problems, which I keep seeing being brushed under the carpet a lot, and that annoys me.
But if we go with the first idea, of barring people from using opposite sex spaces altogether, then where the fuck do I pee? Should I utilise my "right" to use female spaces, despite making everyone uncomfortable and feeling threatened by my presense, as well as risking being kicked out and forced to use the equivalent male spaces anyway, which is exactly what that idea is meant to prevent - or should I completely avoid being in places which I know does not have a gender neutral bathroom, such as the ferry? Would that not be discrimination? Which is the most reasonable option here, what is the most practical, what's wrong and what's right? Do I even have a RIGHT to use female bathrooms, and if so, how do I prove it, considering my ID still says I'm male?
Trans men aren't gonna be nearly as willing to use female only spaces, and trans women definitely not eager to use male only spaces. But aside from that validation factor, I have the exact same struggle as trans people do on this particular point. Quite often they do toss and turn at which bloody bathroom to pick, not just out of validation, but because they genuinely struggle to figure out which one is the best option for them practically. Especially if they don't quite pass as either sex, and most and foremost just wanna do their business without unneccesary drama.
Also, to clarify: barring trans people from opposite sex spaces is NOT discrimination, as they never belonged there to begin with - but leaving them with no other option than to pee themselves, is. Which means that I think it's fucked up to barr them from those spaces BEFORE having solved the problem of "if they can't go there, then where?"
Perhaps I'm the only one around here who cares about males' integrity, safety and human rights. But even if so, I should not be the only one to care about gnc females' integrity, safety and rights. Male-passing females, whether transitioned or not, whether bearded or not, are still female, and if we don't want them in female only spaces, and not in male only spaces either; why? Because they "chose" to medically transition and/or dress in men's clothing?
Yeah, well, in most cases of transitioned females, they transitioned because of dysphoria, which no one chooses to have. It's a medical condition. Barring people from spaces they'd otherwise be welcomed into, due to the visual outcome of the treatment of their medical condition... is ableism. Barring a woman from a female only space she belongs in, solely because her unusual physical appearance freaks you out... is ableism. Also, simply being gnc and being viscerally uncomfortable with presenting femininely is also not a choice. And even if it was... shouldn't it be? That's why I cannot roll with that sorta solution. I dunno if it counts as a form of discrimination by definition, but it just smells a lot like it from where I sit. That it's no more right to toss me out of, or give me trouble, in a women's bathroom, than a masculine women who also passes as male but who has not medically transitioned.
That said, however, women's safety DOES matter a lot to me. Hence my reluctance to join their spaces, despite being a woman myself. I guess, what I'd want is complete sex segregation to work in my favour, but I can't promote a rule that would discriminate against me. I'm sorry, I just can't. I desire FUNCTIONAL sex segregated spaces, but realistically they cannot function. Truth is that the only womens spaces I've been allowed into since I began passing as male, are "trans inclusionary" ones that openly allow in trans women, ironically. I care about the safety of other women, and their right to have their own spaces... but not at the expense of my own rights, as a fellow woman. To say otherwise would be a crime against myself. I really wish this could be solved in some way that would work in practice, but honestly I don't think it can anytime soon. Not without some seriously tried and proven, practical and humane methods to check what sex people entering single sex spaces actually are.
That is the reality that people have to face. And personally I'd rather focus on women's rights than trans rights, but as a woman who's medically transitioning, I'd shoot my own foot no matter which one I'd choose. That's quite a dilemma.
So where my opinion stands on this right now, is basically this: I think female only spaces should only be for biological women, but I'm reluctantly okay-ish with males who pass as female utilising female only spaces, and vice versa for females in male only spaces. However, this does not feel ideal at all. It's a compromise. Ideally, I want such spaces to be entirely sex segregated, and for even people who pass as the opposite sex (like myself) to be allowed into spaces of their biological sex. My appeal here is both realistic practicality with the reality that some people really do pass as the opposite sex, as well as the safety, rights and integrity of male-passing women.

Patton Agnes Wilson
Race/Ethnicity- Black
Height- 5’7”
General Appearance- Patton’s skin is a deep oak color. He has wide dark brown eyes and black very kinky curly hair. He cut it real short when he first came out as trans, but he generally keeps it longer now. He’s had dreads, long twists, and an afro. He’s been on testosterone for years now and has had top surgery and pretty much always passes as male. But he still cannot grow facial hair. It’s a thing. He’s got a round (but not very curvy) body and is fat.
Clothes- Patton will wear just about anything, but often pastels, light colors, and whites. Either aesthetically pleasing, or clashing horribly there is no inbetween. Prefers shorts over pants. Very much an overalls person. Will wear skirts but generally not dresses. Often wears more casual clothes. He’s also known for stealing Roman’s worn-out oversize t-shirts. Also wears the occasional cute short sleeve button-up with fun patterns or designs.
Roman Hussain-Barlo
Race/Ethnicity- Arab (Yemeni)
Height- 6’1”
General Appearance- Roman has golden bronze skin and deep russet eyes. He’s the tallest of the four. He has black hair and generally keeps it shorter on the sides with a bit of length in the front, but it’s really not that dramatic of a difference. It is a mix between wavy and curly. His body is soft and he is slightly overweight. Narrow shoulders, more rectangular in shape than broad. He also grows a beard after college, but keeps it pretty cropped due to fear of racist views of Muslim people and their beards. Has a notable scar on his forearm from his brother attacking him with a whisk.
Clothes- Roman generally wears dark jeans and t-shirts. At work his t-shirts are also usually dark, as he is a theater director. While he pretty much wears the same things at home and work, his work clothes are definitely nicer. Paints his nails a lot. He also wears a necklace with each of his families birthstones on it. Is also not afraid to wear skirts, dresses, heels, etc. It’s not really his style, but he’s done it more than once to make A Statement against the ideas of toxic masculinity.
Virgil Jude Torres
Race/Ethnicity- Ashkenazi Jew, but if you asked him he’d probably just say he’s Jewish and leave it to you to assume he’s white European descent.
Height- 5’5”
General Appearance- Virgil has light sandstone skin that tans easily. He has deep set chestnut colored eyes. He keeps his hair a similar shorter length as Roman. His hair is a deep brown almost black and pretty curly (between Roman and Patton). He’s gangly and has limbs that seem to just go all over the place. Seems smaller than he is because he’s always hunching in on himself. He has self-harm scars on his arms, legs, and chest. He also has two long vertical scars on both wrists from a past suicide attempt. He has a scar on his stomach where his liver transplant was. He also has little scars all over his body because they’ve accumulated during his time at the cult. There’s a large burn scar on his back and on his feet almost all the way up to his knees, also via cult.

General Appearance- Deceit has fair skin that has patches of depigmentation due to a condition called vitiligo. These patches are focused around his face, hands, elbows, and feet. His vitiligo is fairly symmetrical, with unsymmetrical patches forming due to trauma. He has a large, noticeable, and very severe burn scar on half of his face and extends to his ear and neck. The depigmentation covers this in its entirety. His eyes are an aqua green color. His hair is a light golden brown color that falls to his shoulders. He has a slim body that has a certain softness to it. He is very tall and has good posture, but his habit of hanging off to the sides makes this often times unnoticeable. Deceit also has scars all across his body that you should not ask him about.

Narrow shoulders, more rectangular in shape than broad, while still seeming physically strong. Has a notable scar on his upper leg from where he stabbed himself with a pencil as a child.
Clothes- Remus generally wears jeans and long sleeve t-shirts. To work he often wears semi-athletic clothing to work, as he has a job in choreography. Paints his nails a lot with his sisters and niblings. He wears a necklace with a cross on it. Is also not afraid to wear skirts, dresses, heels, etc., though he tends to wear them to special occasion or when a nicer dress-code is required.

LGBT - How To Prepare For Your H.R.T. Meeting

SO YOUR SEEKING HORMONE REPLACEMENT THERAPY:
    WHY?
    Why do you want to seek hormone replacement therapy? What is the underlying cause to have you seek transformation? Is it because you’re sexually disadvantaged? A victim of abuse?      WHAT NEXT?
     Prior to the clinical appointment with a physician, it is totally natural to feel nervous, anxious, excited, ambivalent, scared or all of there emotions all at once. When I was waiting, I felt all these emotions...I felt bipolar when it comes to the altering range of emotions.
     One moment I am nervous (Will I be given the injection hormone? Or pills?), then anxious (time, place, money and open mindedness). Excited (I can’t believe I actually am doing this!), ambivalent (I pray this isn’t a phase...hate to ask my PCP for breast reduction!), scared (What if they deny me, or I take on too many female characteristics? Will my parents actually kill me if I beginning looking like a woman in two to six years? What will my friends think of me?).
   One feeling I would add and that is: doubt
   For 33 years I danced with doubt, and the last four months have been our grand ballroom dance. Even before I hear what they think, I already begun doubting myself, mostly because I realized that being on estrogen meant my transition was real, physical changes will happen and people will notice. No more hiding, no more Illuminati-like secrets...they will see and my brain will change as my testosterone levels are replaced with estrogen.
   Even though I am scared, nothing will kept me from my appointment...I have been working my way to this point...it will be the greatest scientific experiment I will ever conduct...a second puberty occurring on my own body...I will document it to the tee. This might be a capstone in biological transformation that I’ve written and drawled so many times. I always placed myself in my characters shoes, but never could say what it was truly like...so now I will know!
   The night after the call I kept thinking of all the things I was going to ask the doctor. Things the doctor might ask of me. HRT is like a transplant...if you are not mentally prepared, you will reject it! I ran simulation after simulation of different ways the appointment might go and my mind goes blank and the representative of me is left mumbling uh and um and yeah. I wanted to prevent this from happening, so I wrote down a few simulations. Sometimes it is better to have a list of questions prior to going into your appointment so you are collected and ready. It is also a great way to judge the doctor to see if they are the right one...remember...they are the one rebirthing you to your second birth!
PREPARING FOR DAY ONE:
   Here are three areas to focus on during the first appointment...
1) To assess the providers knowledge and comfort with the different forms of treatment being oral, epidermal or endodermis.
2) To go over side effects and what to expect from starting on estrogen. This is a good way to see if the doctor knows the risks and emotional effects at which stage.
3) And to begin building a relationship with a healthcare provider that hopefully will be based on trust and openness.
   It is important to be able to assess the provider’s knowledge and comfort because you want someone that can properly screen for any health issues that may be a problem in the future (I.e. heart trouble, cancer, blood clots and stroke).
   Since I am Gender Neutral (closely related to Gender Fluid), it would be in my best interest to communicate with the provider that there are some things I am looking to physically change (I.e., breasts, fat distribution around the hips and butt, feminine features in the face and hair growth decrease on chest with halting male pattern baldness on head), but others that I wish to remain the same (I.e., penis, male facial features and muscular support that I have).
   Lastly, having a provider that is up-to-date with the different types of dosing and methods to administer hormones is always helpful because then you are given options, options are extremely important when it comes to hormone therapy because a certain method may work well for one person, but not so well for another. (as seen in Operation B.O. H.R.T.)
SAMPLE QUESTIONS FOR PROVIDER:
   So, when learning about the provider, questions that may help you assess their knowledge and comfort are as follows:
What methods do you give as options when prescribing hormones to your patients?
1a) Current options for estrogen, antiandrogens include: injectable, sublingual or oral, and transdermal.
1b) 17-beta estradiol (estradiol) is most commonly delivered to transgender women via a transdermal patch, oral or sublingual tablet, or injection of a conjugated ester (estradiol valerate or estradiol cypionate). No outcome studies have been conducted on injectable estradiol valerate or cypionate, presumably due to their uncommon modern use outside of transgender care settings; due to this limited use manufacturers have little incentive to produce this medicine, and shortages have been reported. Other delivery routes for estradiol such as transdermal gel or spray are formulated for the treatment of menopausal vasomotor symptoms and while convenient and effective in some transgender women, in others these routes may not be able to achieve blood levels in the physiologic female range. Compounded topical creams and gels also exist from specialty pharmacies; if these are to be used it is recommended that the prescriber consult with the compounding pharmacist to understand the specific details and dosing of the individual preparation. Compounded estradiol valerate or cypionate for injection also exists, and may be an alternative in times of shortage or more cost effective for those who must pay cash for their prescriptions.
Conjugated equine estrogens (Premarin®) have been used in the past but are not recommended for a number of reasons, including inability to accurately measure blood levels and some suggestion of increased thrombogenicity and cardiovascular risk. Equine estrogens are obtained from the urine of pregnant, catheterized horses; no evidence exists to suggest that these estrogens are superior to bioidentical human estradiol.
Ethinyl estradiol is a synthetic estrogen used in contraceptive preparations and is associated with an increased thrombotic risk. In the context of contraception, ethinyl estradiol has more consistent and reliable cycle control and as such is better tolerated, balancing out the potentially increased risk of VTE. In the setting of gender affirmation there is no need for cycle or bleeding regulation, and thus the use of ethinyl estradiol and its inherent risks are not warranted.
General Side effects of estrogens may include migraines, mood swings, hot flashes, and weight gain.
Spironolactone is the most commonly used androgen blocker in the U.S. Spironolactone is a potassium sparing diuretic, which in higher doses also has direct anti-androgen receptor activity as well as a suppressive effect on testosterone synthesis. Doses of 200mg daily in non-transgender women being treated for hair loss have been described as safe, though doses of up to 400mg/day have been reported without negative effect. Hyperkalemia is the most serious risk but is very uncommon when precaution is taken to avoid use in individuals with renal insufficiency, and use with caution and frequent monitoring in those on ACE inhibitor or ARB type medications. Due to its diuretic effect, patients may experience self-limited polyuria, polydipsia, or orthostasis.
5-alpha reductase inhibitors include finasteride and dutasteride. Finasteride blocks 5-alpha reductase type 2 and 3 mediated conversion of testosterone to the potent androgen dihydrotestosterone. Finasteride 1mg daily is FDA-approved for male pattern baldness, while the 5mg dose is approved for management of prostatic hypertrophy. Dutasteride 0.5mg more effectively blocks the type 1 isozyme, which is present in the pilosebaceous unit and therefore may have more dramatic feminizing effects. Since these medications block neither the production nor action of testosterone, their antiandrogen effect is less than that encountered with full blockade. 5-alpha reductase inhibitors may be a good choice for those unable to tolerate, or with contraindications to the use of spironolactone. 5-alpha reductase inhibitors may also be an option for use as a single agent in patients seeking partial feminization, or for those who continue to exhibit virilized features or hair loss after complete androgen blockade or orchiectomy.

First of all- the “im not much of an artist” if you art, you’re an artist. That’s it, that’s all it is. And it would be super cool if you did draw art, but know that there is absolutely no pressure.
As for the characters and their appearances, be prepared for me to ramble…

Patton Agnes Wilson
Race/Ethnicity- Black
Height- 5’8”
General Appearance- Patton’s skin is dark, somewhere around a deep oak color. He has wide dark brown eyes and black very kinky curly hair. He cut it real short when he first came out as trans, but he generally keeps it longer now. He’s had dreads, long twists, and an afro. He’s been on testosterone for years now and has had top surgery and pretty much always passes as male. But he still cannot grow facial hair. It’s a thing. He’s got a round (but not very curvy) body and is fat.
Clothes- Patton will wear just about anything, but often pastels, light colors, and whites. Either aesthetically pleasing, or clashing horribly there is no inbetween. Prefers shorts over pants. Very much an overalls person. Will wear skirts but generally not dresses. Often wears more casual clothes. He’s also known for stealing his boyfriends clothes the most, specifically Roman’s worn out t-shirts. Also wears the occasional cute short sleeve button-up with fun patterns or designs.
Roman Hussain-Barlo
Race/Ethnicity- Arab (Yemeni)
Height- 6’1”
General Appearance- Roman has golden bronze skin and deep chocolate eyes. He’s the tallest of the four. He has black hair and generally keeps it shorter on the sides with a bit of length in the front, but it’s really not that dramatic of a difference. It is a mix between wavy and curly. His body is soft and he is slightly overweight. Narrow shoulders, more rectangular in shape than broad. He also grows a beard after college, but keeps it pretty cropped due to fear of racist views of Muslim people and their beards. Has a notable scar on his forearm from his brother attacking him with a whisk.
Clothes- Roman generally wears dark jeans and t-shirts. To work his t-shirts are also usually dark, as he is a theater director. While he pretty much wears the same things at home and work, his work clothes are definitely nicer. Paints his nails a lot. He also wears a necklace with each of his families birthstones on it. Is also not afraid to wear skirts, dresses, heels, etc. It’s not really his style, but he’s done it more than once to make A Statement against the ideas of toxic masculinity.
Virgil Jude Torres
Race/Ethnicity- Ashkenazi Jew, but if you asked him he’d probably just say he’s Jewish and leave it to you to assume he’s white.
Height- 5’7”
General Appearance- Virgil has light sandstone skin that tans easily. He has deep set chestnut colored eyes. He keeps his hair a similar shorter length as Roman. His hair is a deep brown almost black and pretty curly (between Roman and Patton). He’s gangly and has limbs that seem to go places. Seems smaller than he is because he’s always hunching in on himself. He has self-harm scars on his arms, legs, and chest. He also has two long vertical scars on both wrists from a past suicide attempt. He has a scar on his stomach where his liver transplant was. He also has little scars all over his body because they’ve accumulated during his time at the cult. there’s a large burn scar on his back and on his feet almost all the way up to his knees.
Clothes- Virgil covers as much of his body up, mostly due to being self conscious about his scars. Wears black jeans or other dark pants. Band shirts with his notable hoodie, or long sleeves if he goes without it. He’s comfortable wearing more revealing clothes at home. At work, he goes for dark button ups instead of t-shirts.
Logan Kasem Dāwsukpon
Race/Ethnicity- half Thai and part Roma (a nomadic people that have been continuously persecuted, and still are. Better known -inaccurately- as the racial slur “gypsy”)
Height- 5’10”
General Appearance- Logan has light copper skin that tans more in the sun. His eyes are almond shaped and a honey brown color. His hair is a leather black and very straight. He keeps his hair shorter on the sides and longer on top. Generally starts off by buzzing the sides, but it grows out quick and he kind of just leaves it. Logan has a very average androgynous build. He’s the fittest of the four, but still not really muscular. Has good posture and seems to fill up space, so people always assume he’s taller than he is. Pretty noticeable jawline. Talks with his hands, but otherwise generally shoves them in his pockets or fiddles with them.
Clothes- Pretty much like is the Sander Sides videos, bit more color variance. Still stays to neutral tones, dark colors, or black. Wears a tie. Patton has gifted him more than one extravagantly patterned one, and he does wear them, just always with a black shirt or its “too distracting.” Hates all shoes but tennis shoes with a burning passion.
Remy Alan Zurko
Race/Ethnicity- half white and Roma
Height- 6’0”
General Appearance- Remy is very pale and its a fifty-fifty chance if he’ll burn or tan. He has hazel eyes. He has medium brown auburn hair with a bit of a wave to it. Has a dramatic undercut. Generally looks very youthful and androgynous. Tall and has a fairly athletic build.
Clothes- Wears bright button ups that fit the definition of “business casual” perfectly. Generally wears brown, beige, or white pants and shorts. Is not afraid to show off a bit of skin, either with short-shorts or undoing a few of his shirts button. Wears sunglasses in any remotely bright setting due to photophobia (light sensitivity). Has like ten pairs that all look the same. Likes to wear sparkly nail polish, because he can see the shine in the sunlight and he thinks it looks cool.
I know this was a lot, so if you do draw them, feel free to take some creative liberty, this is just what I’ve been imagining in my head as I write them.
