So...I got the worst possible news on Murphy's 15th birthday on Sunday (3/3). It's...not diabetes.

TW: discussion of cancer.

Murphy has a tumor on his adrenal gland and two nodules on his pancreas. He's really too old to biopsy at this point without putting him at risk, but his symptoms weren't diabetes after all...he might have pancreatic cancer, and whether he does or not, he's basically in hospice care with me now that he's stable.

I knew my time with him was limited already since he's 15 now, but this hurts every time. The best I can do is keep him happy and loved until it's time, whenever that is.

He's at home, he's eating all his favorite foods, and he's so, so unbelievably loved, and I hope he knows how loved he's been for so long.

I made myself a little ribbon for him. I know he doesn't know what this represents, but I want everyone to see just how devoted I am to him.

However long we have, you are loved, Murphy, and I will continue to love you forever.

Thank you <3 It's now scheduled for June 7 and I am utterly terrified. I'll put some details below the cut for those who don't mind medical talk and are curious. Recovery will be 8-10 weeks, though I've been warned it could be 6 months or more before I'm feeling myself again so I don't really have a projection about what this means for my writing yet.

Unfortunately the surgeon can't tell the best way to remove the tumor in my small intestines without going in, so I'll be having an open abdomen exploratory surgery. There are four potential surgical results, from an easy and simple removal or minor resection if the tumor is on a stalk like the endoscopic surgeons think, all the way up to a Whipple (a major procedure usually used for pancreatic cancer where they remove some of your organs even if they're healthy and reroute your digestive tract; it has lifelong dietary changes and risk of things like diabetes and enzyme production failure.) I'll be in the hospital for a week and recovery is rough for a Whipple, less rough for the lesser procedures. No matter what I'll be recovering from the abdominal incision and at least a minor resection. I really really really hope it can be a lesser surgery.

I hate this. I'm terrified. The fact I won't know what they do to me until I wake up either with a nasal feeding tube or without one is agony. I'm sick and miserable in the meantime dealing with all sorts of symptoms; it's not a surgery I can opt out of with all my current health problems. We're taking blood samples every two weeks in the meantime to make sure I'm not bleeding out too quickly again and I'm setting up everything I can to run smoothly at home while I'm recovering. I'm out of work right now on medical leave until late July or August depending on which surgery I wind up with.

Cancer biologists discover a new mechanism for an old drug

Since the 1950s, a chemotherapy drug known as 5-fluorouracil has been used to treat many types of cancer, including blood cancers and cancers of the digestive tract.

Doctors have long believed that this drug works by damaging the building blocks of DNA. However, a new study from MIT has found that in cancers of the colon and other gastrointestinal cancers, it actually kills cells by interfering with RNA synthesis.

The findings could have a significant effect on how doctors treat many cancer patients. Usually, 5-fluorouracil is given in combination with chemotherapy drugs that damage DNA, but the new study found that for colon cancer, this combination does not achieve the synergistic effects that were hoped for. Instead, combining 5-FU with drugs that affect RNA synthesis could make it more effective in patients with GI cancers, the researchers say.

“Our work is the most definitive study to date showing that RNA incorporation of the drug, leading to an RNA damage response, is responsible for how the drug works in GI cancers,” says Michael Yaffe, a David H. Koch Professor of Science at MIT, the director of the MIT Center for Precision Cancer Medicine, and a member of MIT’s Koch Institute for Integrative Cancer Research. “Textbooks implicate the DNA effects of the drug as the mechanism in all cancer types, but our data shows that RNA damage is what’s really important for the types of tumors, like GI cancers, where the drug is used clinically.”

Yaffe, the senior author of the new study, hopes to plan clinical trials of 5-fluorouracil with drugs that would enhance its RNA-damaging effects and kill cancer cells more effectively.

Jung-Kuei Chen, a Koch Institute research scientist, and Karl Merrick, a former MIT postdoc, are the lead authors of the paper, which appears today in Cell Reports Medicine.

An unexpected mechanism

Clinicians use 5-fluorouracil (5-FU) as a first-line drug for colon, rectal, and pancreatic cancers. It’s usually given in combination with oxaliplatin or irinotecan, which damage DNA in cancer cells. The combination was thought to be effective because 5-FU can disrupt the synthesis of DNA nucleotides. Without those building blocks, cells with damaged DNA wouldn’t be able to efficiently repair the damage and would undergo cell death.

Yaffe’s lab, which studies cell signaling pathways, wanted to further explore the underlying mechanisms of how these drug combinations preferentially kill cancer cells.

The researchers began by testing 5-FU in combination with oxaliplatin or irinotecan in colon cancer cells grown in the lab. To their surprise, they found that not only were the drugs not synergistic, in many cases they were less effective at killing cancer cells than what one would expect by simply adding together the effects of 5-FU or the DNA-damaging drug given alone.

“One would have expected that these combinations to cause synergistic cancer cell death because you are targeting two different aspects of a shared process: breaking DNA, and making nucleotides,” Yaffe says. “Karl looked at a dozen colon cancer cell lines, and not only were the drugs not synergistic, in most cases they were antagonistic. One drug seemed to be undoing what the other drug was doing.”

Yaffe’s lab then teamed up with Adam Palmer, an assistant professor of pharmacology at the University of North Carolina School of Medicine, who specializes in analyzing data from clinical trials. Palmer’s research group examined data from colon cancer patients who had been on one or more of these drugs and showed that the drugs did not show synergistic effects on survival in most patients.

“This confirmed that when you give these combinations to people, it’s not generally true that the drugs are actually working together in a beneficial way within an individual patient,” Yaffe says. “Instead, it appears that one drug in the combination works well for some patients while another drug in the combination works well in other patients. We just cannot yet predict which drug by itself is best for which patient, so everyone gets the combination.”

These results led the researchers to wonder just how 5-FU was working, if not by disrupting DNA repair. Studies in yeast and mammalian cells had shown that the drug also gets incorporated into RNA nucleotides, but there has been dispute over how much this RNA damage contributes to the drug’s toxic effects on cancer cells.

Inside cells, 5-FU is broken down into two different metabolites. One of these gets incorporated into DNA nucleotides, and other into RNA nucleotides. In studies of colon cancer cells, the researchers found that the metabolite that interferes with RNA was much more effective at killing colon cancer cells than the one that disrupts DNA.

That RNA damage appears to primarily affect ribosomal RNA, a molecule that forms part of the ribosome — a cell organelle responsible for assembling new proteins. If cells can’t form new ribosomes, they can’t produce enough proteins to function. Additionally, the lack of undamaged ribosomal RNA causes cells to destroy a large set of proteins that normally bind up the RNA to make new functional ribosomes.

The researchers are now exploring how this ribosomal RNA damage leads cells to under programmed cell death, or apoptosis. They hypothesize that sensing of the damaged RNAs within cell structures called lysosomes somehow triggers an apoptotic signal.

“My lab is very interested in trying to understand the signaling events during disruption of ribosome biogenesis, particularly in GI cancers and even some ovarian cancers, that cause the cells to die. Somehow, they must be monitoring the quality control of new ribosome synthesis, which somehow is connected to the death pathway machinery,” Yaffe says.

New combinations

The findings suggest that drugs that stimulate ribosome production could work together with 5-FU to make a highly synergistic combination. In their study, the researchers showed that a molecule that inhibits KDM2A, a suppressor of ribosome production, helped to boost the rate of cell death in colon cancer cells treated with 5-FU.

The findings also suggest a possible explanation for why combining 5-FU with a DNA-damaging drug often makes both drugs less effective. Some DNA damaging drugs send a signal to the cell to stop making new ribosomes, which would negate 5-FU’s effect on RNA. A better approach may be to give each drug a few days apart, which would give patients the potential benefits of each drug, without having them cancel each other out.

“Importantly, our data doesn’t say that these combination therapies are wrong. We know they’re effective clinically. It just says that if you adjust how you give these drugs, you could potentially make those therapies even better, with relatively minor changes in the timing of when the drugs are given,” Yaffe says.

He is now hoping to work with collaborators at other institutions to run a phase 2 or 3 clinical trial in which patients receive the drugs on an altered schedule.

“A trial is clearly needed to look for efficacy, but it should be straightforward to initiate because these are already clinically accepted drugs that form the standard of care for GI cancers. All we’re doing is changing the timing with which we give them,” he says.

The researchers also hope that their work could lead to the identification of biomarkers that predict which patients’ tumors will be more susceptible to drug combinations that include 5-FU. One such biomarker could be RNA polymerase I, which is active when cells are producing a lot of ribosomal RNA.

Your cousin had two and a half years of treatment and hospital visits and having to be careful with her activities, and you couldn’t stand it. You didn’t want to be careful, you wanted to enjoy your life. But life always has to throw a curve-ball, doesn’t it?

It was another six months before you received the news that you were cancer-free. It was like you’d been punched in the gut, but the relief came over you not long after. Then the water works came, and as you left the hospital, the realization of what you’d been told finally sunk in. The leukemia is gone, now the only thing you have to worry about is whether or not it’ll come back.

As you got outside, you took out your cell phone and dialed your mother’s number. She’d usually come with you to your appointments, but she had to work a double shift at her job and couldn’t make it.

“Y/N?” Concern came over your mom’s voice after she answered. Only your family was aware of your leukemia, you didn’t want the media to make a big deal out of something incredibly painful and personal. “Are you okay?” your mom asks. “Is something wrong? I knew I shouldn’t have taken that extra shift, I’ll come down to the hospital...”

“No,” you laugh, your voice thick with tears. “I’ll come by your work, I have some big news.”

Your father made your favorite dish, your sister made your favorite dessert and your mother got off work early with a heap of goodies that she knew were your favorite. Of course, you were cautious to eat all of the food they got and made, still worried it’d cause some kind of health problems due to the side effects of your medication. You underwent so many changes compared to the early days of your diagnosis. Some of your favorite foods began to taste differently thanks to medication, some of the meals you once loved you couldn’t eat thanks to your treatment.

Of course you can’t complain. If you didn’t get the treatment, God only knows what state you’d be in. You’re just grateful you made it to where you are now.

So as you’re sitting at the kitchen table, smiling at a joke your sister told, picking at the food on your plate. Your father, after hearing the news, came to the hospital as fast as he could, over the moon that his child was now in the clear.

“You okay, N/N?” your father asks. “You haven’t touched your food. I made your favorite.”

“I know,” you sigh, putting your fork down. “I appreciate it, it’s just... I got a text before dinner.”

“What kind of text?” your sister presses.

“From one of my costars.” You lean back in your seat. “Chris... the one who plays Cap, he sent it. Everyone’s been invited to Comic-Con to promote Infinity War, and I’m expected to be there.”

“Are you going?” your mother adds. “You’ve just gotten news of your remission, I don’t want you traveling if you’re not up for it. Especially if it’s going to put your health at risk.”

“I haven’t seen them all in so long. They’ve all been busy with their own thing, and I’ve been in and out of the hospital. I’d like to go, especially since I’ve put so much blood, sweat and tears into that movie.”

“When is it?” your brother asks.

“A couple of weeks.” You grimace. “I’ll be sure to set up an appointment with my doctor to see what she says. I won’t go if she thinks I’m in no condition, but I don’t see why I can’t if I’m otherwise fine.”

But that does nothing to ease your nerves.

You managed to squeeze in some time with your doctor and explained the situation. She gave you the okay to go, but made you promise not to push yourself too hard. She gave you some precautions and you promised to adhere to them. And as Comic-Con neared, you began to worry. You looked different compared to when you finished filming Infinity War. You’d been using chemo to help fight the leukemia, among other treatments the doctors felt were necessary for your overall health. As a result of the chemo, you lost your hair. You also became fairly pale, among other physical changes. You can only imagine how everyone will react once they see you.

The day of Comic-Con came and you felt anxious. This is the first time in months you’ll see your costars, and it feels like your stomach is doing somersaults. Your hands are clammy and your heart is beating a mile a minute. You decided beforehand that you weren’t going to meet up with your costars yet, you wanted to work up the courage first. So when you were given directions to where the group panel would be, you kept your distance from the others, not wanting to draw their attention to you. Part of you longed to be with them though, to involve yourself in their conversations and laugh at their jokes; to rekindle that familiarity you missed in those months you spent at the hospital. But you didn’t. You couldn’t, not yet.

The panel started after a good fifteen minutes of waiting, the host warming up the audience before calling everyone to come out. You could hear the cheers, and a small smile came across your face. Only to disappear when you heard your name.

“...lastly, the talented Y/N Y/L/N!” the host called, their voice loud and jovial over the sound of the cheering audience.

Swallowing thickly, you begin your journey from where you stood to the stage, your heart stuttering against your ribs. It feels like an eternity before you reach the stage, and despite your anxiety and hesitation, you plaster a smile on your face and wave to the crowd as you walk out, hoping they don’t notice just how overwhelmed you are. You give hugs to everyone on stage with you, but you can see the confusion subtly written on their faces at the sight of you, though their excitement at seeing you was definitely a sight for sore eyes.

You take your seat next to Chris (Evans), and you’re more than hyper-aware of all the eyes on you.

“Y/N, I have to say,” the host begins, “we haven’t seen much of you lately. The cast of Infinity War has been busy with promotional work, but you’ve been gone for most of it. What happened?”

You give a smile, a nervous chuckle escaping your lips. “Well, I’ve been busy with some personal stuff lately,” you say. “I guess... I was diagnosed with cancer shortly after filming Infinity War, so I decided to take a break from acting until I knew I was cancer-free.”

You heard gasps and murmurs, and you resisted the urge to shift in your seat.

“You have cancer?” Robert looks at you, his brows furrowed in concern as he studies you, a deep frown on his face.
