A long-awaited report out this week found that medical professionals in the UK who advocate for gender transition in children are misguided ideologues.
Written by British pediatrician Dr. Hilary Cass, The Cass Review, which is nearly 400 pages and took more than four years to compile, comes to the following conclusions:
Thousands of vulnerable young people were given life-altering treatments with “no good evidence on the long-term outcomes of interventions to manage gender-related distress.”
“It has been suggested that hormone treatment reduces the elevated risk of death by suicide in this population, but the evidence found did not support this conclusion.”
“Social justice” ideology is driving medical decision-making, and “the toxicity of the debate” has created an environment “where professionals are so afraid to openly discuss their views.”
Activists insist the science on this matter is settled, but Cass’s tone recalls a stern British nanny calmly explaining to unruly children how to get their room in order. She shows us that everything about this issue is unsettled, and unsettling. For instance, she notes that “social transition”—when very young children assume other gender identities—is an “active intervention” that may set youths on a path to medical transition. And it may even make gender dysphoria worse.
The review, commissioned by England’s National Health Service, comes after more than a decade of whistleblowing by clinicians at the country’s Gender Identity Development Services, or GIDS, which was established in 1989 (but mostly off the radar for its first 20 years, because few children and families sought its services).
These whistleblowers detailed how kids were fast-tracked to medication while a culture of fear grew around raising any concerns, even as demand for youth gender medicine exploded. Eventually, the NHS decommissioned GIDS and hired the neutral, no-nonsense Cass to detail what went wrong and what to do right moving forward.
Her report made the further damning conclusions:
Clinicians “are unable to reliably predict which children/young people will transition successfully and which might regret or detransition at a later date.”
A disproportionate number of patients were “birth registered females presenting in adolescence. . . . a different cohort from that looked at by earlier studies.”
Many parents feared their children had been medicalized by professionals who didn’t take other difficulties into account, “such as loss of a parent, traumatic illness, diagnosis of neurodiversity, and isolation or bullying in school.”
There is a lack of strong evidence to show that puberty blockers “may improve gender dysphoria or overall mental health.”
The majority of gender-dysphoric patients in early studies found that their symptoms desisted during puberty, with most coming out as gay or bisexual later.
Cass notes that “for most young people, a medical pathway will not be the best way to manage their gender-related distress.” She supports expanding the treatment to regional, holistic centers, essentially ending the specialist gender clinic model. That treatment should be based on unbiased psychological care, and robust and consistent evaluation tools must be developed so reliable evidence can finally be gathered.
This final report—and an interim one Cass issued in 2022—echoes what a number of Western nations, such as Finland and Sweden, have found when they reviewed their own youth gender services. It also underscores what we see in the United States: poor quality research, an unstudied population, and detransitioners traumatized by the treatment they received.
Today, red states are banning the medicalization of gender dysphoric youth, while some blue states have declared themselves medical sanctuaries for minors seeking transition. Medical associations—from the American Academy of Pediatrics to the American Psychological Association—continue to support the “affirmative” model criticized by Cass in her report.
In her review, Cass directly addresses the 9,000 young people who have moved through gender treatments via the NHS, stating bluntly: research “has let us all down, most importantly you.”
The U.S. needs to form a truly bipartisan commission that looks at the evidence regarding youth gender medicine. As things stand now, we will continue to be stuck in a perpetual culture war, with parents and distressed kids paying the price.

FIRST ON FOX: Rep. Cori Bush, D-Mo., has continued shoveling campaign cash to her husband, Cortney Merritts, but altered the description of the payments amid scrutiny. She has sent over $100,000 to him since she added him to the campaign's payroll nearly two years ago.
According to new filings reviewed by Fox News Digital, Bush's committee paid Merritts $12,500 for "wage expenses" between July and August. Merritts previously gathered money for security services, but the committee switched their characterization in April as they continued to bring headaches for the campaign.
The new cash follows the $30,000 Bush's campaign paid him before the third quarter. So far this year, Merritts has received $42,500 from the committee. He's collected $102,500 since he first emerged on its payroll in January 2022.
The "Squad" member has said she requires protection due to previous attempts on her life and has paid hundreds of thousands of dollars to security firms on top of the earlier payments to her husband.
According to her filings, her campaign began paying a new security firm in the third quarter. The committee sent $5,000 to a St. Louis-based company called All American Protective Consultants, which was established in March, business records show. The firm is registered under a company called 720 Firm, which launched late last year.
Bush's campaign did not respond to Fox News Digital's request for comment on its payments to her husband and why they had switched their description from security to wage expenses. They also did not respond to a question regarding All American Protective Consultants.
After Bush and Merritts married in February, her office announced they had been together more than a year before her campaign began paying him in early 2022. Her campaign regularly sent Merritts bimonthly $2,500 checks while disbursing hundreds of thousands to other protection firms, primarily PEACE Security.

Merritts, meanwhile, did not have a private security license as of late February, Fox News Digital reported. He also did not appear in a Washington, D.C., database of licensed security specialists, and Bush's campaign did not respond to several prior emails on the matter. The payments have subsequently triggered at least two FEC complaints from watchdog groups.
The campaign also paid Nathaniel Davis, who has claimed he's 109 trillion years old and can summon tornadoes, tens of thousands of dollars for security services.
Even before adding Merritts to her payroll, Bush faced criticism for using private security. In July 2021, Fox News Digital first reported on Bush's security payments while pushing to defund police, prompting CBS News inquiries about the cash and whether hiring a security detail while pushing to strip money from law enforcement was hypocritical.

“It was mainly in the 18th and 19th centuries that churches were fitted out with fixed pews. Prior to this, naves tended to be open spaces and medieval churches were commonly used not only as places of worship but as a focal point for the neighbourhood, with evidence of markets, dances and other civil functions being held in them.
JG Davies wrote in his 1968 book The Secular Use of Church Buildings: “There can be no question that in the Middle Ages the church was an all-purpose building. It is difficult to think of any secular activity that has no connection with it.””
Churches should boost their efforts to double up as community hubs by hosting more playgroups, yoga classes, concerts, exhibitions and cafés, according to more than 70 per cent of Church of England priests in a Times survey.
A poll of almost 1,200 active Church of England clergy found that most respondents want to see churches do more to transform themselves into places where people gather for non-religious events, activities and amenities.
A Times analysis found that 250 churches per year are applying for permission to rip out pews, replacing them with foldable chairs that can be moved aside to create space for community groups to use.
For churches with dwindling congregations, it is seen as a way to raise money while inviting people to a place where they encounter a Christian space and may perhaps be inspired to return as worshippers.
Many churches have faced criticism from heritage groups that do not want to see pews removed or modern amenities fitted into ancient church buildings. Cathedrals have been criticised for hosting events such as fashion shows, crazy golf courses and helter skelters.
The survey asked priests: “Some churches and cathedrals have been granted permission to alter or offer their buildings for use outside of service times by community groups such as playgroups and yoga classes, for events such as exhibitions and concerts, and for amenities such as cafés and post offices. Do you think churches should be doing more or less of this?”
In response, 71 per cent of priests said they should do more while 21.9 per cent said the current level was “about right”. Only 3.7 per cent said they should do less.

Emily Gee, the Church of England’s director for cathedral and church buildings said: “Our churches and cathedrals have long been, and remain, at the centre of our communities. Church buildings are often flexible enough to provide a wide range of activities that support the wellbeing, enjoyment and uplift of the people and places they serve, but with worship and prayer at their heart.
“We are pleased there is such support for creatively offering community use alongside worship and with the significance of our wonderful buildings in mind.”
Meanwhile 60 per cent of priests say that they have confidence in the church’s often criticised safeguarding policies
The Church of England has faced criticism over its handling of safeguarding, while archbishops’ efforts to form an independent safeguarding watchdog collapsed this summer.
The Times survey asked priests how confident they were “that the Church of England’s safeguarding system and policies are adequate”.
It found that 60.5 per cent of respondents were confident, with 52.2 per cent “fairly” and 8.3 per cent “very” confident. This compared with 36.9 per cent who lacked confidence, with 26.9 per cent replying “not very” and 10 per cent saying they were not confident “at all”.
Richard Scorer, head of abuse law at Slater and Gordon solicitors, said: “Parish priests will probably answer this question from their local parish perspective, rather than speaking of the Church of England as a whole, and therefore there may be an optimism bias. My own experience … is that there is both good and poor practice in the church.”
He added that recent reports should “eliminate any complacency” and that “independent oversight is so important”.
The Rt Rev Joanne Grenfell, lead bishop for safeguarding, said: “Work is under way to review and improve policies … We welcome the survey and will continue to build on this feedback to ensure clergy and others are more confident with the policies.
“We must never forget the legacy of poor safeguarding in the Church of England and the enormous impact that this has had.”
The Church of England must also pay attention to views of Anglicans overseas, the survey suggested. Priests believe that the opinions of the 80 million Anglicans in foreign churches should be taken into account when changing Church of England policies and doctrine at home.
There has been division within the Anglican Communion, a fellowship of 42 Anglican churches, between those in places such as the United States and Scotland that have started conducting same-sex weddings and those in countries that strongly oppose them, including Uganda and Nigeria.
Some traditionalist churches have threatened to depose the Archbishop of Canterbury as spiritual leader over his backing for blessings for those in same-sex relationships.
In debates at the General Synod, some have argued that the Church of England must not change its doctrine to endorse gay relationships if the majority of the Anglican Communion opposes it, while others have said this should not delay reforms.
The Times survey found that more than half of respondents want the church to start conducting same-sex weddings, but it appears priests are aware of the reaction this might provoke from traditionalist Anglicans abroad.
The Times poll asked how important it was to “consider how other churches across the global Anglican Communion might react” to changes in Church of England policy or doctrine.
In response, 76.7 per cent said it was important, with 43.5 per cent saying it was “fairly” and 33.2 per cent “very” important.
Only 22.6 per cent said it was not important, with 18.8 per cent saying “not very” and 3.8 per cent “not at all” important.
The General Synod has backed moves to boost the number of Anglican Communion representatives on the body that will choose the next Archbishop of Canterbury, from one out of 16 members to five out of 17.
The Rt Rev Jo Bailey Wells, the bishop for ministry in the Anglican Communion, said the body was like a family and said it was “heartwarming to see that relational awareness revealed in the survey from Church of England clergy”.

Entry 6: 31/10/24 - Smeg it; the Paradigm Daleks
Yeah, fuck it why not. I was gonna do this entry on the costuming (or rather the lack of armouring) when it came to female women jedi in The Clone Wars,* but I’ve lost my motivation for that topic. Perhaps it has something to do with Niamh shooting it down as an actual problem. I dunno, it’s a complicated discussion rife with ignorance on both sides; I know more about Star Wars, she knows more about feminism (one of us is doing significantly better in life than the other - guess which).
So yeah, not feeling the Star Wars equal rights campaign, but what I am feeling is daleks. I don’t even really wanna do an entry today, but it’s been a while since my last one and I gotta keep it up for the sake of my touch-typing practice, and I can, and probably do, talk about daleks in my sleep (I’m not actually ace, as it turns out, I just say that to assuage the embarrassment of driving off all my one night stands with my incessant, nocturnal ranking of dalek variants [the winner is the dalek emperor for those wondering, followed shortly by Rusty, the Girly Dalek - otherwise known as the dalek from Dalek** - Dalek Sec and then Dalek Caan]).
And prompted by a reddit post I saw asking why the paradigm daleks weren’t received too well, I feel like… answering it.
Ah, the paradigm daleks. Moffat’s new paradigm… daleks. Conceptually cool and, in person, intimidating, towering over even the lanky Mr Matt Smith. What happened? They should have been a slam dunk.
Well, it just so happens that their reveal was orchestrated in such a way that it undermined almost literally every effect the alterations were meant to emphasise.
For a start, they were massive. They were dominating. They were scary.
In person.
On camera… not so much.
For one, we never actually saw them next to anything other than other paradigm daleks, at least until the season finale. The Russel T Davies time war daleks present didn’t get to share so much as some gossip, let alone a shot, before being summarily EXTERMINATED, so there went the possibility of that comparison. And what certainly didn’t lend some much needed fear factor to their unwieldy bulk was the set in which they made their debut in. I mean, what happened, for god’s sake? This was meant to be a dalek ship, straight from the end of series four. What was this bleak, white industrial setting? The fucking dalek rec room? I’m fairly sure to make room for some of the sci-fi shit they had to move out a ping pong table.
The big boss man straight, white, cis supreme dalek is millimetres away taking out a light fixture (or something, I dunno what it is - it’s a big aluminium box hanging from the ceiling, it’s like a set Red Dwarf would try and flog as a spaceship), making them feel awkward and clumsy and cramped, which is funny because that was exactly the description the operators inside them gave when asked how it was to pilot the buggers. Yeah, turns out all that extra heft made them even more uncomfortable.
So they were heavier, fatter, hunchbacked, harder to control and this was all in service of an effect that was not only countermanded, but overpowered, by the environment they emerged into. Hey, I know the feeling; the world just ain’t built for tall people - I have to duck in my local pub or otherwise get brained by a beam.
And whilst all these issues were detractors, certainly, nothing quite dammed them like the colours. On reflection, it seems like nothing new - different colours denoting different stations in the echelon - but something about these ones… just stank. The boldness was admirable, but misguided, and nothing highlighted this blunder quite like the procession of their unveiling ceremony.
One by one, they came sliding out, like some weird car show for peculiarly shaped dodgems. And whilst I stand by the big boss man straight, white, cis dalek looking dope and acting as a serviceable alternative to the three-bulbed supreme dalek of yester-series, the choice to have an ambassador of each colour variant lined up in such a fashion as would be appropriate for the in-box toy line, turned them from a menacing council of evil, the latest in ethnic-clensing technology, into the mighty morphin power daleks. They look like their role on Skaro was to run the dalek branch of cbeebies.
(LOOK CHILDREN! CAN YOU SEE THE BIRD? YOU CAN?! THEN IT WILL BE EXTERMINATED!!! AND REMEMBER CHILDREN OUR LESSON FOR THE DAY: THE ONLY GOOD TIME LORD IS AN EXTERMINATED ONE.)
It’s a shame the show never got to expand on its intention to have different dedicated roles for each dalek colour, as it wasn’t initially evident that the main dalek foot solider was the red ones and that the red one present in this batch was just the show model, along with the blue strategist, the orange scientist, and the yellow eternal dalek, whatever that means (I’m almost certain even the people that came up with the yellow eternal dalek don’t know what the fuck he does). Maybe if they’d been able to emphasise the differing roles of the daleks, things could have been different. But even so, Moffat didn’t end up doing all that much with the daleks in general, so ultimately it doesn’t matter.
I’m not sure I’d really have gotten into the red drone daleks being the default foot solider. Gone was the rough, rugged, frankenstein-bolted exterior of the time war weathered bronze models, here were the daleks at gay pride. Something about the smoothness and the brightness of colour made them feel too toy-like (though the shiny chrome ones shown off in later episodes look dope as fuck - I totally think they worked better as an officer class).
But yeah, that’s about the long and short of it. It’s a comical blip in Doctor Who history, and a valiant attempt to shake things up, but I think returning to the bronze model was the right call, even if we’re definitely due an update. The tricky thing with dalek designs I feel is that they’re effective in their simplicity, so trying to make too dramatic a change could be playing with fire.
One underrated aspect I find with the daleks, particularly the classic ones, is that they age remarkably well. Even in their first ever episode back in the Hartnell years, for all the flak their basic looks garners, despite all the mocking lampoons about their ability to create stiff peaks with one hand and unclog a sink with the other, they maintain a certain kind of elegance. You look at a silurian back in the day and you laugh at the shoddy costume; a dalek back in the day just looks like a dalek, and in my mind that is the pinnacle of iconic and effective design. Good on you, Terry.

I can only assume this is a case of the designers being too happy with how they looked in their base states and being unwilling to alter them to account for different environments - the sort of mentality that keeps women characters in high-heels at all times. What’s especially curious is that there’s a good mix of conservative outfits and slutty outfits, neither of which are particularly suited for a battlefield. They’re meant to be generals. And don’t get me started on Ahsoka’s first outfit.

Last night, I had drinks with a friend I hadn’t seen in a long time, determined not to talk about The Issue. But a few minutes before I arrived, I found out that The New York Times had decided not to publish a part of a story about the World Professional Association for Transgender Health—an advocacy group that creates “standards of care” for trans medicine, which American medical groups avow to adhere to (they don’t) and claim are evidence-based (they aren’t).
That part of the story would have discussed recently unsealed WPATH documents, subpoenaed by the state of Alabama, as part of a lawsuit, Boe v. Marshall. Alabama parents, medical providers, and a Birmingham pastor named Paul Eknes-Tucker sued the state because of its ban on “gender-affirming care” for minors—and the criminalization of those who practice it.
We got a sneak peak of some WPATH internal communiques a few weeks ago, when the advocacy group Do No Harm shared emails between Johns Hopkins and the federal Health and Human Services’ Agency for Healthcare Research and Quality. AHRQ had been asked to conduct a systematic evidence review on “gender-affirming care,” and knew that WPATH had hired Hopkins researchers to conduct such a review. Maybe they could share?
The emails show that Hopkins did conduct a systematic review, and that—like all the other SRs—it found diddly squat in terms of evidence supporting the efficacy of hormones and surgeries. But WPATH prevented Johns Hopkins from publishing these reviews because they didn’t come to WPATH’s preferred conclusions. WPATH hid this very important information from the entire world, then published standards of care saying an evidence review was impossible. And a government agency knew this!
We are talking about kids and the most invasive possible interventions here. We are talking about venerable academic institutions and government agencies and censorship and secrets.
Turns out, there’s a whole heckuva lot more of these damning emails. The New York Times had access to them but chose not to cover them. A source told me this is because no one from Johns Hopkins would comment on the record. The documents will be available via the LGBT Courage Coalition tomorrow (I will add a link and start a thread when it’s up), but I had a chance to preview them. If you have not yet had what GIDS whistleblower Anna Hutchinson called her “holy fuck!” moment, now’s the time.
An executive summary of the docs tells us:
WPATH leadership went to great lengths to suppress systematic reviews (SR) commissioned from Johns Hopkins because the reviews’ conclusions did not support the WPATH plans to recommend wide access to hormones and surgeries for all those who desired them. The evidence suppression was achieved via a 2-prong strategy. First, WPATH forced JHU to withdraw the manuscripts that were already submitted for publication as they did not meet the desired conclusions. Next, WPATH instituted a new policy whereby WPATH would have to approve all future publications by JHU.
More on the policy:
WPATH developed an approval checklist, which required that the authors must have the “intention to use the Data for the benefit of advancing transgender health in a positive manner,” the content approval must involve SOC8 chapter leads, and the review must include “at least one member of the transgender community in the design, drafting of the article, and the final approval of the article.”
Two levels of WPATH approval were required before JHU could submit a publication: first, the proposal which includes the conclusion had to be approved; and then, the actual manuscript draft, with WPATH retaining the rights to alter content.
Only the reviews passing both levels of approval could be submitted by JHU for publication—and they had to carry a specific disclaimer that “the authors are solely responsible for the content of the manuscript, and the manuscript does not necessarily reflect the view of WPATH in the publication.”
Can you believe the John Hopkins folks agreed to this? This is not science. WPATH is not credible. And this is why we in America are the outliers: we’re not basing guidelines on systematic reviews, or reality. We’re basing them on an activist group’s political agenda, and even the HHS knows there’s no good evidence. In fact, AHRQ was asked to review guidelines for treating gender dysphoric youth back in 2020, because, the request said:
There is a lack of current evidence-based guidance for care of children and adolescents who identify as transgender, particularly regarding the benefits and harms of pubertal suppression, medical affirmation with hormone therapy, and surgical affirmation. While these are some existing guidelines and standards of care,2, 5-6 most are derived from expert opinion or have not been updated recently so a comprehensive evidence review is currently not available.
What did AHRQ decide, after communicating with the Hopkins researchers?
The EPC Program will not develop a new systematic review because we found protocols for two systematic reviews that addresses portions of the nomination, and an insufficient number of primary studies exist to address the remainder of the nomination.
Basically, they said someone was already doing it, and there wasn’t enough evidence to sort through. But the someone already doing it had already agreed to put science aside and only discuss benefits, not harms.
How could a federal agency abdicate its responsibility? Wouldn’t you know it, the Assistant Secretary for Health for Health and Human Services (HHS) is Dr. Rachel Levine. And as the NYT did report, and as I summarized briefly in The Free Press this morning, Levine pressured WPATH to remove age restrictions from guidelines—not based on science, again, but on advocating for Levine’s own trans community.
It’s officially a scandal now, and it goes all the way to the White House. Holy fuck.
Needless to say, I was not able to ignore The Issue at drinks.
