Thinking the unthinkable
On SEPTEMBER 24th, I'll be speaking IN PERSON at the BOSTON PUBLIC LIBRARY!
Time and again, I find myself thinking about radium suppositories: specifically, I get to thinking about the day that the consensus shifted from "radium suppositories are great" to "stop putting radioisotopes up your ass."
The thing is, people really liked radium-based quack remedies. They drank radium-infused water, smeared radium cream on their faces and bodies, and yes, rammed radium suppositories up their assholes:

The fact that this made whatever ailed you sicker didn't deter the radium true believers: if you're getting sicker, then you must need more radium.
When I think about the debate over radium, I imagine that the people who understood that radium was really bad for you must have run up against critics who told them they were being unreasonable. "You can't tell people to stop using radium. Tell them to use suppositories with less radium. Tell them to use them less frequently. But you can't just tell people, 'stop putting radium up your asshole.' They won't take you seriously."
About 20 years ago, I started pitching various institutions that reviewed consumer tech policy on the idea that they should reject any product that had DRM. After all, DRM didn't just restrict how you used a gadget today, it provided a facility for nonconsensually, irreversibly field-updating that gadget to add new restrictions tomorrow. How could a reviewer in good conscience say, "Go ahead and buy this device if you need this feature," if they knew that at any time in the future, the gadget's maker could take that feature away and leave the buyer with no recourse?
Here's the warning I (half-seriously) suggested magazines run alongside such products:

No one took me up on my offer. Over and over again, magazine editors, managers of nonprofit review outlets, and indie gadget reviewers told me that it was unrealistic to publish a roundup of, say, this year's portable music players with the recommendation, "Just don't buy any of these. None of them are fit for purpose."
In other words: No one wanted to publish, "The correct amount of radium to stuff up your asshole is zero."
But the correct amount of rectal radium for you to administer is "none" and the correct car for you to buy today is none of the cars:

This isn't the first time the correct automotive recommendation was "don't buy any of these cars." Back before seatbelts came standard in cars, the correct car was "don't buy a car." Sometimes, the correct answer is "none of the above." Even if that makes you sound unserious, the alternative is that you counsel people to put radium up their asses in a bid to seem "reasonable."

Fearless of being branded as "unserious" and "unreasonable," they seriously, reasonably said the right amount is none, actually.
None. Which is why they're so mad at Khan and co. Which is why they're so bent on getting Kamala Harris to fire Khan – despite the fact that this would burn precious political capital in the senate. Stoma gave Käärijä a new life - english translation of an interview for Finnilco ry 8.5.2023
Translator’s note:
If you’ve been wondering about that scar on Finland’s favorite green boi’s belly …this is about that. (Because why be cha cha cha when you can be sad sad sad and read about how he was like a week away from death at one point in his life. Though I guess it kinda fits the post-ESC depression, really)
Okay, now some actual notes about the text. So, Finnilco is a Finnish organisation for patients with stomas and the like, so the interview talks a lot about medical stuff and is clearly geared towards people with similar health issues. It might not be as ”entertaining�� as all the other stuff you might’ve seen about him recently, but I recommend reading it anyways, as I feel it gives a lot of insight to who Käärijä is as a person. It hasn’t always been just crazy and party for him.
There is a lot of medical vocabulary in this, and I’ve done some intense googling and wikipedia-ing to figure out what the correct terms are, but I can only hope they are right. Trying to understand whether things are synonymous with each other or completely different things is kinda hard when you have zero knowledge about the subject. I deeply apologize for any mistakes that might occur.
I’ve also done some minor tweaks to the text (like cut down on repeating his last name in nearly every sentence) for the sake of easier reading, as the style of it is quite academic and ”dry”, but overall I’ve kept as close to the original as I could.

Jere Pöyhönen, known by his artist name Käärijä, is the finnish representative in the Eurovision 2023 Song Contest. The artist, known for his style and energetic live-performances, had his youth shadowed by serious health issues, to which he even almost lost his life.
By his own words Pöyhönen is still just a normal guy. Vantaa-born Pöyhönen was diagnosed with colitis ulcerosa, a type of imflammatory bowel disease, when he was young. As the disease got worse, he had to eventually have an emergency surgery, where he got a temporary stoma. Later the stoma was removed and replaced with a J-pouch (ileo-anal pouch), a reservoir pouch formed from the end of his small intestine. Currently in good health, he wants to be open about his disease so that he could offer peer support to others battling with the same issues.
- I am truly fine with this, I am not ashamed about it, on the contrary, I want to tell about this to everyone. I hope that by sharing my experience I could help someone else, Pöyhönen says with a smile.
Symptoms lead to an emergency operation
When Pöyhönen was at ninth grade, he was diagnosed with rectal inflammation. The inflammation was treated with suppository and oral medication, and it got better. When he was 18, the symptoms returned. For a year he was on an elimination diet that kept the inflammation under control, but eventually his condition got worse again, and in the end his entire colon got inflamed. Several treatments were attempted, but none worked. At the worst point Pöyhönen weighed only 49 kg, and his hemoglobin was swaying between 54-56.
- I was still somewhat right in the head, though I was feeling dizzy and kept bumping into bathroom doors. It was quite a rough time in every way, he reminisces. He defecated blood multiple times a day and was practically bedridden, his parents had to feed him. But nothing seemed to help. Pöyhönen remembers how his mother was crying by his bedside. Back then he had blood tests done regularly to control his condition, and after one time he got a call and was told that his hemoglobin was so low that he needed to be treated urgently. After the call his dad went to start the car and drove him straight to the hospital.
At the hospital, an emergency surgery awaited. While waiting for his turn he wondered about what would happen to him in the surgery, afraid that he’d need a stoma. At the same time he thought that the most important thing was to stay alive.
- Do whatever you have to, as long as I’ll get better, he remembers thinking before going in. A stoma had been suggested to him already before, but until then he hadn’t been able to accept it and had tried to manage by other means. Afterwards he has been thinking that the fear was due to the issue being so unknown. He didn’t know much about stomas and had never seen anyone with one.
- It was a tough spot. I wondered how the stoma would affect my life and me as a person. How would I look like, what would happen to my sexuality. Those kind of things scared me the most about it, he recalls.
Life as a young person with a stoma
The first thing he did after waking from the surgery was feeling his stomach and the collection bag.
- It was a weird feeling, confusing. But at the same time I felt just immense calmness. The root of the problem, the inflamed colon had been removed entirely. Confusion soon turned into acceptance.
- I wasn’t sad about it for that long, on the contrary. When it sank in that I was still alive, the stoma felt like a pretty small thing, considering everything. It was a happy thing that I had it.
Despite feeling thankful about the stoma, it was still a shock at the beginning.
- When I was taken to get a shower for the first time and I saw it, I nearly fainted, he laughs.
The emergency surgery was lifesaving for Pöyhönen. if it hadn’t been done, the inflammation could’ve spread from his bowel to the rest of the body within weeks, or even days. So Pöyhönen came really close to death, but thanks to the stoma he got to continue living.
– Getting the stoma gave me a second chance. A chance that not everyone gets to have. But if they get it, they should take the offer with a smile.
Pöyhönen tells that he got used to living with the stoma quite quickly. But it required him to adjust his own attitude – he had to accept the situation as it was. Luckily he was able to enjoy life even with the stoma
- When I had it, I did all the same stuff as other people. I did sports, went to restaurants, I truly lived a really ordinary life.
Of course he faced also some difficult times. At the time 18 years old Pöyhönen was in a relationship, and he tells that at first things relating to sexuality felt difficult.
- It was indeed nerve-racking. Overall, you are only starting to try out stuff at that age, and then there is the stoma on top of it all.
But one thing was clear for him already at the time:
- If the other party in the relationship doesn’t accept your situation or the stoma, then that person isn’t worthy of you.
From stoma to J-pouch
Pöyhönen lived with the stoma for five months until it was replaced with a j-pouch. In the beginning the pouch got clogged, but he didn’t tell about it right away. He was fed up with spending his time in a hospital and wanted to live a normal life. When he finally told about the clogging, the issue was fixed and the difficulties eased.
- I’ve done all the normal stuff. I’ve travelled around the world, done and eaten the same things as everyone else.
Pöyhönen has had the j-pouch for almost eight years now. He hasn’t had any serious complications, but occasionally there’s been some milder issues.
- Sometimes there’s been minor inflammation or bleeding. Once I went to have an endoscopy after there was more blood and I got frightened. Old fears about how things were in the past rose to the surface, Pöyhönen tells.
Overall he is feeling positive about everything.
– At the moment I’m really contented with my situation, and I wouldn’t change anything. I wouldn’t even want that colon back, as this all has become a part of my identity, he says.
Music as a part of life
Music has always meant a lot for Jere Pöyhönen. Yet it wasn’t always obvious that it would turn into a career.
While spending his time in hospitals, listening to music comforted him and gave him hope. Laying in a hospital bed with an IV drip next to him, Pöyhönen also wrote his own songs. If other patients wondered about his doings, he simply answered that he was making music.
During his time in hospital he realized that life might be short. He decided that if he’d be alive and healthy again after the emergency surgery, he’d go and try doing music for real and with everything he had. Of course, at the time he had no idea how far that decision would eventually bring him.
- My values became clearer there in the hospital. I realized what are the things I love and what is truly important for me. One of those things is music, and doing that was what I set my mind on to.
Pöyhönen tells that he especially enjoys doing live shows, because then he gets to entertain people. He feels he is at his best while performing.
Daily life of an artist
Nowadays health issues don’t cause much trouble in Pöyhönen’s everyday life as an artist, but he still needs to take good care of his body. At gigs he must pay extra attention to what he drinks and eats, when and how much. He is sweating a lot while performing on stage, and to balance that he drinks salt/mineral water. The excitement also affects his bodily functions, and during stressful moments he’ll need to use the bathroom more often. But he tells that he doesn’t really get nervous about doing gigs anymore.
However, the approaching Eurovision song contest is a kind of gig he has never experienced before. The event is big and the place as well as the proceedings are all new to him. Despite all that, Pöyhönen seems trustful.
- I don’t know how it is going to be like in there, but I don’t think I’ll have any problems. His confidence relies both on his general attitude and that over the years he has learned to know the way his body functions quite precisely. He knows when his energy levels are getting low and when he needs to drink or eat.
Family’s support has been important
When Pöyhönen was sick, the support from his family was what helped him to keep going. Thanks to his family he has always felt valued and loved.
– The contribution from my parents has been enormous. I will never be able to repay their efforts, other than by being alive.
Pöyhönen tells how his parents gave him their full support while he was sick.
- When I first got diagnosed with the rectal inflammation, they wanted to figure out what could cause it right away. They delved deep into the matter, made phone calls and searched for information from the internet.
His parents drove him to his tests and put their time and money into finding out what was going on. The financial support made it possible that he could have all the different tests done despite them costing a lot.
Stories from peers bring hope
Though his family and friends have been there for him, they haven’t been able to offer him peer support. When he was sick, Pöyhönen did sometimes feel very alone with his issue.
- I didn’t know anyone else in a similar situation as myself. I didn’t get to talk face to face with peers, he says. He did search for peer stories from internet, but people online were usually anonymous, and though he gained information through it, he was longing for human connection and faces to relate to. Lucky for him, an acquaintance of his was in the hospital at the same time as him due to a similar issue. They became friends and messaged daily through Facebook, asking each other about the number of times they went to bathroom and the like.
Pöyhönen says that those kind of discussions with a peer were a big help. It was important to hear that someone else was experiencing similar things as him. An ice hockey player Teemu Ramstedt gave him another face to relate to.
– It gave me lot of faith to see someone else with the same stuff going on as me. That an athlete, a hockey player, had been dealing with the same issues, he tells.
Attitude and dreams helping to go forward
While being sick Pöyhönen gained strength from daydreaming and steering his thoughts towards future.
- I kept thinking that at some point I’ll have good moments with my family and friends again. That one day I’ll be healthy and able to feel happy about everything. In the end it was quite simple things that helped, and also humor helped to get through it all.
Pöyhönen tells that he has been a joker since he was young, and when he was unwell, he also used humor to deal with the difficult things. But there was also something else hidden underneath the jokes.
- Maybe all the joking was also a survival tactic. A way to escape from it all. In the beginning I didn’t want to accept the reality, even though I tried to convince myself that I had done so.
Pöyhönen tells that the songs he used to listen to while in hospital were difficult to listen to after he got out of there. Also some familiar places would bring up old memories in an unpleasant way.
But in the end, time heals, and years later those same songs are back on his regular playlist. Now they just uplift his mood and push him forward.
It’s worth it to open up
When Pöyhönen was sick, he didn’t always tell about his symptoms to his parents or the hospital staff right away. One reason that he mentions was shame. At first he himself didn’t want to believe it to be real when he first saw blood down in the toilet.
- But when it happened again, I realized that this might not be something that would just go away. That it might be something more serious that should be taken care of, he recalls.
And to his younger self, or someone else in a similar situation he would give the advice that you should be open about your problems. He also encourages to try and find some peer support, as he himself was left without it for the most part. Though the bit he did get was a big relieving factor..
– When a person suffers from an illness, the most important thing really is that you are mentally in a good condition. If you are feeling down, the healing process will be really hard, he points out.
At the moment he dreams about that he and the people closest to him would stay as healthy as possible. He wishes that he would get to do things he enjoys in his life and to spend time with the people that are important to him.
To the readers of Finnilco he sends the following message:
– Go forward with humility, but don’t be too meek either. Love yourself, your body and mind. Be well, and if problems arise, react to them right away. Enjoy life and do things that make you happy.

Drug Orders and Doses

Cool, so I think the first thing to know is how medication is ordered.
Generally speaking, it will be ordered in 5 parts, known as the "5 Rights" of medication administration:
#1 What patient is getting the medication
#2 What medication is to be given
#3 How much medication is to be given
#4 What time it is to be given (or how often)
#5 What route it is to be given
So an order might be "Give John Smith (5/13/1995) lorazepam 0.5mg IV once prior to MRI"
In this example, John Smith is the patient and 5/13/1995 is his birthday to differentiate him from all the other John Smiths. "Lorazepam" is the drug's generic name, "0.5mg" is the amount of the drug. "IV" is the route, and "once prior to MRI" is the time.
Drugs have generic and brand names. For example, acetaminophen is a generic name. Many companies make acetaminophen, and each has their own brand name for the drug. Probably the most well-known brand name for acetaminophen is Tylenol, but there are others, like Calpol and Panadol. For most people, it doesn't matter which brand of a particular drug is used, just that the active ingredient (the generic name) is the same. For some people it matters because the non-active ingredients may be different between brands, and they may be allergic to a non-active ingredient that is in one brand, but not another.
In a hospital setting, we're going to use the generic name, because the brand of the drug that is cheapest to the hospital pharmacy varies contract to contract, and there are a lot of drug shortages these days. That's why if you're in the hospital you might get an oval green pill one day and a round white one the next day. They're the same drug, just different brands.
The dose is given in milligrams, usually abbreviated "mg". Milligrams are a measure of weight. Cubic centimeter (cc), on the other hand is a measure of volume. At some point we switched from volume based to weight based measures because we had a lot of different concentrations and using volumes for everything made mistakes really common. If you're using weights, it doesn't matter if the concentration you have is 1mg/mL or 10mg/mL for a given drug, you can do the math and come up with a volume that is right instead of just hoping you picked the one the doctor was thinking about when they wrote the order.
There are many routes a drug can take into the body. There is oral (a pill or liquid), IV (injection in a vein), IM (injection in a muscle), SQ (injection into fat), rectal/PR (a suppository, gel, or liquid inserted into the rectum), SL (under the tongue), TD (a paste or patch that sends medication through the skin) and many more.
Times can be once, once every x hours, once every x hours as needed (PRN), once under a particular circumstance, daily, or pretty much any other interval you can think of. "Stat" is a term meaning "right now".
Here's a list of common medications and their dosages:
CODE DRUGS:
Epinephrine 1mg IV for cardiac arrest every 3-5 minutes, 0.3mg for anaphylaxis
Amiodarone 150-300mg IV over 10 minutes for cardiac arrest
Lidocaine 75mg for cardiac arrest initially, if that doesn't work then 37.5 10 mins later
Adenosine 6mg given very quickly for PSVT, if that doesn't work, give 12mg
Atropine 1mg every 3-5 minutes for low heart rate until heart rate is normal
OTHER DRUGS:
Albuterol 2.5mg in nebulizer for brochospasm/asthma attack
Metoprolol 5mg IV every 5 minutes up to 15mg for severe high blood pressure
Furosemide 20-80mg IV for fluid on lungs
D50 25g IV for low blood sugar
Diphenhydramine 12.5-50mg IV for allergic reaction
Morphine 2-10mg IV or IM for pain
Fentanyl 50-200mcg for sedation
Mannitol 20-150g for increased pressure inside the skull
Nitroglycerin 0.3-0.6mg every 5 minutes up to 3 times for chest pain (angina)

A Tale of Woe, Ep. 61: Substitute
Mary: "Myst, tell me about Claire. If I'm going to be her, I need to know all about her. Tell me about her life. Where did she come from, and why oh why is she in this dreadful place?"
Myst: "Well, I can only tell ya what she's told me. But that's a lot! There's not much for me and Claire to do in here except talk to each other.
"Claire was born in the year 2000, to loving parents. She grew up not far from here. Claire's parents died in a car accident in 2009, and she was raised the rest of her childhood by her older sister Liz. She went to college and got a degree in business administration. She was working at some big company downtown, in the Human Resources department.."
Mary: “Human.. resources?”
Myst: “Yeah, I’ll explain what that is some other time. Anyway, that was just before she came here..”
Mary: "What happened?"
Myst: "The way she tells it, there was a mistake at work on some paperwork about her mental health and someone decided it was better safe than sorry to send her to the hospital. The poor thing has been here ever since, for over a year now. Nobody cares that she's completely sane!"
Mary: "And what is she like? Is she sweet, is she kind? What kind of.. great great great granddaughter do I have? Would I like her? And what about her relationships? Surely such a pretty girl, as I see when I look in the mirror, must have admirers. And why do they keep her here for so long?"
Myst: "She's the nicest person I've met in this place! She's my best friend, one of the best I've ever had in my life, here or anywhere. I would die for her, Mary, and I think she'd do the same for me.
I don't know much about her social life on the outside, but I'm gonna warn you, a certain doctor here is definitely an 'admirer' of hers, and I'm telling you now to avoid him if possible! He's the main reason she's still here, he's keeping her around like a bird in a cage. I know because he keeps me here, too. I’ve been here even longer than Claire. He enjoys keeping the things he finds pretty, locked up for his amusement. This whole hospital is his menagerie.. keep away from him as long as you can!"
Mary: "Oh Myst, this hospital is a wonder and a bewilderment to me. It is the same place as it was in 1894, the same horrible conditions, the same humilations, but all of these new things, these strange beeping devices, blinking lights! It's positively infernal. Tell me what kind of therapies and treatments am I to expect in this hospital now? If this dream does not end soon, I am certain to find out"

Baby Bakugou's first heat
Bakugou has his first heat. He didn't even know he was an omega. Thankfully Aizawa and Hizashi are there with slick absorbent undies, soft blankets, and medicine to make him feel better.
AKA: Bakugou has to have his temp taken rectally and has to have suppositories. No Scat, No sex.
“Hey bud, I need to get your temp. Just roll on your side and lift your leg. I know, I know you wanna sleep. You can go right back to it once I get this over with. Make sure not to push it out.” Hizashi whispered as he finished wiping the thermometer down with sterilizing pads. He rubbed gently at the boy's left leg as he sat on the edge of the bed.
Bakugou frowned and rolled onto his right side and pulled his knee. He knew the drill by now. Every two hours for the last ten they had done this. The first seven hours Aizawa, daddy, had done. Now for the night shift Hizashi, papa, was caring for him.
Why was the explosive blonde getting his temperature taken every two hours and calling his teachers these names? Because Katsuki Bakugou was having his first heat. The thought to be beta was having a heat. A bad heat. A heat that made him curl up and scream in pain in the kitchen. Kirishima and Denki had guarded him up away from some… rude alphas in their class. Specifically a certain alpha who popped a knot the second they smelled him.
Mina had gotten Aizawa who had called Recovery Girl and now here they are. Giving him pain killers because he couldn’t even move from the cramps and pain.
Why the names? Because the second Aizawa picked up the omega, Bakugou thought he smelled so safe, so much like a calm place, like peace, that it slipped.
‘ Daddy you smell good’
‘What did you call me?’
‘Daddy… um… I’m sorry you just smell-’
‘It’s okay. You’re in pain and you’re just a boy. It’s okay.’
Now he’s here. Laying in his english teacher’s bed with a slick pad covering his bum wrapped up in a nice soft blanket.
The older man picked up the soft blanket that was covering the boy's bottom and scrunched it up in his hand until he could pull down the multiple layer padded underwear. He prepared himself for the scent. An Omega’s first heat is always… messy. Bakugou had very thin slick that smelled of gas and burnt bread, it made the older man feel like his eyelashes were gonna melt. He guessed that in his adulthood Bakugou would smell similar to a fireplace or fresh baked bread. Something less so… rotten.
Surely enough the scent bombarded the air and a rather nasty squelch noise sounded out. The underwear were quite full of the boy’s slick. Hizashi frowned, a wet one of these can cause a pretty bad rash. He knows from his own experience.
He tried to be as gentle as possible as he lifted his left buttock so he could access the small puffy opening that was overly wet and had a steady stream of slick running out of it. Hizashi’s chest tightened. ‘ First heat… poor thing must’ve been in agony! A nice treat might help.’ He thought before shaking himself out of it.
“We’ll change your undies when I turn in for the night so you can sleep through the night nice and dry. Now, here we go. Breathe, buddy.” Yamada said as he gently slid the small tip of the thermometer in the heating omegas wet hole. He let go of his bottom and let the boy relax as he reached for the pack of wet wipes on the bedside.
“Papa… want something better inside me.” Bakugou whimpered and tried to get the small instrument deeper by flexing the muscles inside his hole. Hizashi smirked. First heats were usually much earlier than nearly seventeen years old as Bakugou. Before boys started doing things . And by the looks of it Bakugou had already started doing ‘ things’ . So the poor thing had even worse urges.
“Buddy I know. I know you do. But you're too sick for us to let you have anything else. We have to get you better and get you all leveled out. Once you’re out of heat we’ll make sure you’re prepared for your next one.” Hizashi explained and gently wiped away the slick from where it was getting slick and sticky on the boy's bottom and small testicles, trying to be as gentle as he could. He was trying his best to be comforting and careful. It had been a while since he dealt with a first heat.
“Okay papa.” Bakugou said in a tense voice. He was panting a bit again. He stopped panting once the other teacher had given him some ‘skin time’, meaning Bakugou had been chest to chest with him for a few hours. Skin contact is important to omegas, alphas in rut as well but Bakugou had calmed.
The thermometer beeped and Hizashi eased it out and looked at it. 100.6. Better. When Recovery Girl first checked him he was 106.8 but Bakugou ran hot. So it was similar to being 103 for someone without an explosive quirk. Still horribly hot though.
Hizashi pulled the underwear back up and folded the blanket back down. He patted Bakugou's hip and stood up, wiping down the thermometer and laying it and the wipes back on the nightstand.
“Do you want anything? A snack or a clean blanket? You need to stay nice and comfy! Staying relaxed and calm will help you keep away that fever and keep your blood pressure down. If you need something make sure to tell me okay?” Hizashi said in a voice usually reserved for when he was alone with Aizawa or talking to the cats, soft and caring. Bakugou let out a whine and moved one hand to rub over his stomach around his belly button.
“My tummy feels bad. Like I need to potty but I can’t. I tried papa.” Bakugou says and groans, sticking out his bottom lip.
“Can I feel it? I’ll be super gentle. Just roll to your back.” Hizashi said. Thought of what it could be flashed through his head. Impacted slick glands, a tear in the inner wall, a slick blockage, heat constipation. The older man hoped it was the last.
Bakugou rolled so he was belly up. Hizashi moved so the blanket was pulled down, revealing the boy’s torso. Hizashi looked first. The skin was stretched tight around his stomach but it wasn’t red or bruised. It didn’t look like it was pushing up around his hips. Not slick gland impaction, not an inner wall tear.
Hizashi then had two possible possibilities. A short rub around the boy’s abdomen would tell him if it was one of those. Hizashi rubbed his hands together to warm them and reached to lay his hands down. He hauled at a jerk of the young body below him.
“Be careful. Hurts. Tender.” The hero in training said quietly and looked up shyly at his teacher. Hizashi gave him a small understanding smile.
“Okay well you're bloated for sure. I’m not sure if we have heat safe medicine to help if it’s just gas. I need to press a little bit okay. It might hurt but I’ll be so fast. It’ll all be okay.” Hizashi offered and kept his hands floating a few inches above the bloated belly.
“You gotta be fast, it hurts when I even try to lay on my tummy.” Bakugou whined and pouted. Hizashi nodded and first just laid his hands down. If it was slick blockage there would be pressure down low and then none. If it was constipation the pressure would go low and there would be pressure all the way down.
He pressed lightly and ran his hands down following the way of the student’s gut. Pressure, pressure, pressure…
“Papa, that hurts.” Bakugou whined and sniffled. Hizashi quietly pulled his hands away. Sure of himself.
“I’m sorry honeydew. I know that must’ve been very uncomfortable.” Hizashi said in an apologetic way. Bakugou pouted and stared at the man with a guilt-causing face.
“Sweetie, when was the last time you went potty?” Hizashi asked as he pulled the blanket up to the boy’s chest.
“Papa…” Bakugou whined and blushed, turning his head to the side in embarrassment.
“It’s okay just tell me. No need to be embarrassed. We can make your tummy stop hurting. ” Hizashi said nicely and moved to hold the younger boy’s hand, squeezing softly. Bakugou sighed and hunched his shoulders.
“I dunno… last week? Thursday?” Bakugou said and kept his eyes trained down away from the man.
“Oh buddy… that’s not gonna help your fever. I'm gonna go see if I need to run to the 24/7. You just lay back on your back for now. Poor little thing.” Hizashi said. He wasn’t panicking. He was just worried because Bakugou said Thursday. It’s Wednesday night. Nearly a week without having a bowel movement and packing in food on his preheat binge. Poor thing must feel so heavy, so weighed down.
“Bakugou, I’m going to explain something and then we’re either going to do it here or you will go to the doctor in the morning. Do you understand?” Aizawa said as he walked in the room behind the blonde man. He said it in his day to day solid no argument voice. It was a serious conversation. Bakugou didn’t like it. A few hours ago the man had been so sweet and soft and gentle with him, only speaking in a quiet rumbling tone that made the omega feel relaxed. Now… is daddy mad? Did he do something wrong? With tears in his eyes the boy
“Daddy? Is it gonna hurt more? I don’t wanna hurt anymore, I wanna be okay. I didn’t mean to not go, I’m sorry.” Bakugou was already crying. Sobbing out the last few words. Aizawa shook his head and sat on the edge of the bed and pulled the boy up so he could wrap his arms under the boy’s and hold him close. Aizawa cradled the back of the boy’s head against his chest.
“You didn’t do anything wrong. I’m just worried. I’m sorry I spoke in that tone. You’re not feeling good and I should know not to be so gruff. I’m sorry but we need to talk serious okay?” Shouta explained in the once again gentle rumbling tone. Bakugou listened and sniffled. He sat up on his own and nodded.
“Okay. I just don’t want it to be- to hurt, daddy.” Bakugou sniffled as he wiped his eyes and nose. Aizawa shushed him and rubbed Katsuki’s knee through the blanket as Hizashi quickly opened the bedside and grabbed a few tissues to clean up the omega’s face. Aizawa took a big breath as the hero in training copied him. They let it out and Aizawa explained.
“It won’t hurt if you hold still and breathe. We’ve got to get you to go potty. You should have had a heat purge the day before your heat started but since it’s your first and you're a late bloomer, it didn’t happen. Now we can either try suppositories which go in your bottom and melt and help you relax.” Aizawa said trying to make the medications sound not terrible or scary.
“Bleh.” Bakugou said and pulled a grossed out face.
“Or we can give you an enema which is where we use a small tube and let water and laxatives loosen everything up and then you expel it, you let it all out.” Aizawa explained trying again to make the process seem as minimal as possible. However when he finished, the boy’s face went pale and he shook his head.
“I don’t want that.” He said quickly and put his hands down to back away from the dark haired man.
“You need to pick one okay. Papa’s got the suppositories if you want to see. I’ll give you two. You can just see them, look at them then decide.” Aizawa said reassuringly. Bakugou swallowed and looked at the blonde man who was standing with his hip resting against the attached bathroom door frame.
Bakugou nodded and Hizashi made quick word of getting the medication out from the kit under the sink. He handed them to his husband and rolled his chair to be beside the bed. Aizawa pulled back one of the foil covers and picked up the small jelly-like bullet.
“See they aren’t big, buddy. They’re just little things. You’re slicking really well too so you’ll barely feel it.” Hizashi says. He hopes that the boy picks the suppositories, they’re much less messy and much less stressful.
“We do have to push them in though. You can hold onto papa and I’ll be quick. You just need to breathe and relax and it’ll be all over. Do you want those or the other option?” Aizawa asked and Bakugou made an unsatisfied face. He looked down at himself and chewed his bottom lip for a few moments.
“Um… can Papa push them in?” The boy questioned in a timid voice and kept his eyes down.
“Well I suppose I can. Can I know why?” Aizawa said and looked at Hizashi who shrugged and nodded.
“Daddy gots bigger fingers.” Bakugou said and ran his fingertip down the top of one of Aizawa's. Hizashi covered his mouth with his hand to muffle his laugh. Aizawa smiled and nodded.
“Okay well let me go grab a pair of gloves and wash my hands.” Hizashi said and went to the bathroom.
“Just like with the thermometer. Good job, you’re doing so well. What a smart smart boy. I’ll hold your knee so you can relax. Once papa puts them in you have to hold them in for about fifteen minutes. After that I’ll go sit with you on the potty.” Aizawa said as he positioned Katsuki to lay on his side with one leg straight and one leg pulled up. Aizawa was sitting on the bed. He used one hand to hold the back of the boy’s knee as he vocalized praises while Hizashi got prepared.
The blonde man opened two foil packets and laid the medication on top of the sterile folded back foil. He slipped on a pair of blue latex gloves on his freshly scrubbed hands.
“Okay daddy.” Bakugou said and let the older man take the weight of his leg, he let the muscles relax and go slack.
Hizashi pulled down the absorbent underwear and shoved a small towel under his bum and lifted one buttock to expose his hole. He used his other hand to run one finger to tough the puffy pink skin lightly.
“Okay relax, relax bub, you feel my finger?” He asked and used the boy’s slick to get his finger coated and slippery. Bakugou sighed. The finger touching and rubbing at and around his hole felt so so good. Overwhelmingly good.
“Yeah” Bakugou sighed out and Aizawa moved some hair out of the boy's face.
“Good. Here’s the medicine. You feel it?” Hizashi asked once again as he took one of the suppositories and touched the pointed end against the center of his hole.
“It’s little.” Bakugou said. He sounded almost disappointed. Aizawa looked down and saw a pout. Another reason late bloomer’s heats. They already know what it feels like to be full and satisfied. So during heat when the need, the urge, intensified twenty fold… they’re impossible to satisfy when unprepared.
“Yep it’s little. Now I’m going to push it in. Ready?” Hizashi said and smiled. It was cute, he couldn’t deny it. He pushed it lightly against the hot entrance, melting it slightly but the boy tensed up.
“I’m nervous.” Bakugou said and stiffened up all the way and scrunched his eyes up.
“It’s okay, just hold daddy’s hand.” Aizawa said in the third person and took one fist into his own much larger hands and rubbed his thumb up and down the small blonde’s knuckles.
The youngest breathed in and out and forced his muscles to relax on his backside. His fist was still tight in Aizawa’s hand but his hole was soft.
“Okay okay I’m ready just do it!” Bakugou said in a rush and took practiced breaths in and out. Preparing himself for pain but when there was none but rather just an easing sensation around his hole he waited… and waited. Then Hizashi was speaking again.
“Good job, one more. Here it comes.” The man said and without stopping for Bakugou to question and slid the second one in. He pushed his finger in as deep as last time, all the way until his knuckles hit the soft skin of his bottom. When he pulled his finger out the boy sighed and relaxed as Hizashi let out of his buttock.
“Good boy… Just lay here for a while. I’ll help you to the potty when time’s up. Okay?” Aizawa explained.
Daddy and papa take such good care of me…

i do find it mostly just funny to see people get all prescriptivistically pedantic about usage of different words for toxins in everyday conversation
but there's also some linguistic curiosity of my own, as to how this classification system works
like. if "venomous" means you get the effect by being bitten (like by a snake) does that mean all toxins that take effect through the bloodstream are venom
does execution by lethal injection count as venom? Does it matter whether intravenous, intramuscular or subcutaneous? (those distinctions are actually pretty important when dealing with medical injections because they do influence the effectiveness of the med, so maybe they also get different names when the injection is for killing instead of healing? i wanna know)
if we have different words for "it kills you if injected inside you through your skin" (venom) and "it kills you if you ingest it through your mouth" (poison) I want a whole vocabulary for all administration routes
i want words for toxic
inhalations
suppositories, both rectal and vaginal
topical cream/lotion/ointment/gel (possibly different words for each)
nasal spray
ear drops and eye drops
all the different types of injections, separately
maybe also further categorize the oral route, because you'd be impressed how much difference it makes whether you ingest something in a capsule, tablet, coated delayed-release tablet, liquid suspension or sublingual melt
if you're gonna police people's words about putting deadly stuff in your body, i want your entire legal system in writing. Not because I'm gonna follow the laws. just because I am autistic and this is the sort of thing i study for fun

[ I have been thinking a lot about Joker's Smilex intake and ways she could do them.
The most common way I refer to is that her Estrogen is laced with Smilex so she can keep both her dosages on level. And the second most commonly referred to method is through injection.
But I was thinking: what other fun options are there? And what would certain AU Jokers do?
Joker uses liquid Smilex while gaseous forms are often used to terrorize Gotham. So what if Joker uses a modified inhaler like those used for asthma patients with Smilex cartridges.
The other one I like for a few specific AUs are cigarettes. Smilex laced cigarettes that when recently inhaled let Joker blow green smoke from her mouth. Smilex is already messing with her body anyway, might as well add on bad habits for the fun of it! Per extension and going back to liquid: drinking it. Just straight from a flask, maybe spike it with alcohol.
And finally, and most importantly, one that requires the help of others!

this isn’t meant to be serious analysis or anything, i’m just having a bit of fun so bear with me.
so this post is the third or fourth one i’ve seen wrt jack being a parallel to dracula/“vampire coded”, and i really like that interpretation.
and seeing that post reminded me of a connection i made a couple of days ago, when i saw this post by @thegoatsongs, which pointed out the sweet and bitter scent that both jonathan and lucy report in relation to vampiric attacks. and when i saw that post i was like “hah, just like jack seward”, because a couple of months ago when i checked out some information about chloral, i found out that its smell is often described somewhat similarly.
chloral hydrate is described as having "a characteristic pear-like odor" (x), "a fruity or pear like smell on the breath is classic" (x), and wikipedia puts it as "aromatic, slightly acrid" (x), while others say it is "pungent, but not acrid" (x). one person in a forum describes the smell as "like swimming pool chlorine, slightly bleachy and has an odor of rotten fruit" (x), (which, while rotten fruit doesn't smell nice, it certainly smells sweet).*
we don’t know exactly how often jack uses chloral. when he mentions it, it’s when he’s specifically not using it, but he still says that he’s careful not to let it grow into a habit, which would suggest that he’s using it at least sometimes.
as for administration, wikipedia lists oral syrup or rectal suppository. i'm not usually one to jump to conclusions, but in this case i will do just that and say that jack is probably drinking the chloral (although suppositories were apparently used in the 19th century, so who's to say).
all of this to say that jack could possibly sometimes have quite a sweet/bitter breath. just like a vampire.
now, there’s a lot of what-ifs in this suggestion, not to mention that since jack would take chloral before bedtime, any hint of that sweet/bitter scent might be gone by morning(?). or not, i couldn't find any information on how long the scent hangs around.
i don’t know! perhaps there is something here, perhaps there isn't.

A trans-identified male residing in Canada who claims to be HIV positive and inserts progesterone rectally has been allegedly “breastfeeding” his child with the support of established medical clinics. Former men’s rights activist Murray Pearson, 52, who uses the name Margaret (Margie) Fancypants on social media, has been criticized after he shared an image of himself at a lactation clinic holding a young infant.
The photo was first posted three months ago on Reddit community titled r/TransLater, a board dedicated to males who transition later in life. In a post titled, “Milk, baby, milk! MTF 52,” wherein Pearson appears ecstatic that one “benefit of being transfemme” includes “that you can be pregnant and get drunk” with no undue problems.
“I have a baby almost 9 months old… I cannot wait to connect through feeding. And yes, I will stop drinking before it negatively affects anything they drink!” said Pearson
“This is a wish I have had for decades. My egg cracked a year ago on December 12 and I realized I could nurse my baby already on the way. That lit a fire under me, and I have gone from having lean pectoral muscle in March to full B-cups now and growing fast,” he added.
Pearson continues on to claim to have the assistance of “medical expertise,” including “five physicians in three clinics in two world class hospitals,” with one of the clinics named as the Goldfarb Clinic in Montreal.
“Two endocrinologists, Newman and Goldfarb, created a protocol to induce lactation in adoptive mothers,” Pearson explained in the comments. “It works for trans women as well, it works best when breast growth is mature but I am taking domperidone while my primary breast growth is underway. By an astonishing coincidence, I live right next to their breastfeeding institute!”
According to statements Pearson has made on social media, he began identifying as transgender in December 2022, and the following year, began taking progesterone. Approximately eight months ago, Pearson announced that he had found his “true self” as a result of performing in a drag show for a friend’s birthday celebration.
Earlier this month, Pearson described the first moment he believed he was a woman after trying on used women’s clothing at a thrift store. “I realized that the beautiful curvy woman in the mirror was the real me and I could be her every day… after more than two decades of stealth resistance. 87 days later I started spiro, 14 days after that estradiol, and now I am looooooving my boobs. Having curves without fakery is AMAZING.”
In a post made to the TransLater community last week, Pearson described how to insert the female hormone progesterone rectally, as a suppository, rather than swallowing it as a tablet.
“My breasts get a wonderful plumpness and pleasing jiggliness when I have had progesterone the night before…. You’ll need some lubricant (personal lube such as K-Y jelly or similar, or silicone personal lube which may be overkill, or some sort of non-irritating oil; I use my own mixture of cocoa butter and shea butter) to allow free clearance for the capsule… Some people simply pop the capsule in their mouth to use saliva, but I like a more effective lube,” he commented.
Pearson has also shared images of himself in a blue and pink wig attempting to eat ice cream containing the hormone in a seductive manner.
Disturbingly, Pearson has also revealed that he is HIV positive and is aware that the deadly virus can be transmitted through breastfeeding.
“I am HIV+, continuously controlled for 18.5 years now,” said Pearson in a Reddit post six months ago. “The viral suppression into undetectability [sic] makes sexual transmission impossible. But transmission through milk IS possible if viral load becomes detectable so I will test viral load monthly (opposed to semiannually) to keep a VERY close eye on that.”
In a shocking display of further disregard for the child’s health, Pearson speculated as to whether his experiment would qualify him as a candidate for the participation in academic research.
“Fortunately, Dr Lenore Goldfarb, creator of the protocol, has her clinic at the same hospital we birthed our baby in. I may even end up in the medical literature.” While again discussing his HIV status in relation to ‘breastfeeding’ a child, Pearson alleges that his case is being documented by infectious diseases researcher Dr. Marina Klein, who is affiliated with McGill University.

Reduxx reached out to Dr. Klein to request information on her involvement in Pearson’s experimental lactation, and she confirmed that she had been monitoring Pearson for his HIV infection at the Chronic Viral Illness Service of the McGill University Health Centre. However, she stated she had not been involved in his transition nor had she been consulted on his induction of lactation.
“It’s important to emphasize that we do not recommend breast feeding for people with HIV as this is the only way to be certain that no HIV transmission will occur after a baby is born,” Dr. Klein said in her response. “However, guidelines have evolved over time with the recognition that the risk of transmission is very low when HIV infection is undetectable with effective therapy … If, after informed discussion, a person expresses a wish to breastfeed they may chose to do so provided they are willing to follow a close protocol of viral monitoring and have their baby followed closely with pediatric specialists who would generally recommend that they receive preventive medication.”
Dr. Klein further states that Pearson had expressed a desire to breastfeed and then had been referred to an endocrinologist.
Prior to identifying as transgender, Pearson was involved in politically advocating for the US-based, anti-feminist men’s rights non-profit A Voice for Men (AVFM).
The organization was founded in 2009 by Paul Elam, who has said that there would no longer be “any place to hide on the internet” for “lying bitches,” and members associated with the group have previously published personal information about women who opposed their activities.
Pearson has linked to the organization’s website on his YouTube channel biography, where he has uploaded videos of himself advocating positions held by AVFM, such as accusing women of lying about sexual violence.
In one video shared in July 2013, titled “Rape, Posters, Traffic Lights and Consent,” Pearson claims that he was previously drugged and raped by a woman on the University of Alberta campus.
He also encourages discussion of posters produced by Men’s Rights Edmonton which featured the headline, ‘Don’t Be That Girl’, created with the intention of parodying an anti-rape campaign designed by a women’s rape crisis shelter, Sexual Assault Voices of Edmonton (SAVE), which used the slogan, ‘Don’t Be That Guy.’
Pearson has also been active in a Facebook group for “trans lesbians,” a term which labels men who call themselves transgender as same-sex attracted women.
Criticism of Pearson’s claims to be “lactating” and “breastfeeding” have focused on concerns that he is “motivated… to experience sexual arousal from lactation,” as one X user, Lulu Solomon, said.
“Because it is not motivated by what is best for the baby it’s automatically risky because the baby becomes a prop in the fantasy,” she stated, alongside a photo of Pearson at the Goldfarb clinic.
This is not the first controversy that has involved trans-identified males breastfeeding babies, with a number of recent examples triggering concern over the past year.
Last year, a lactation professional in Australia along with a women’s rights campaigner were warned that they had broken the law after criticizing a trans-identified male who had boasted of breastfeeding his infant. Shortly after, a trans-identified male in the UK dismissed critics of the practice as “transphobes” after he posted images of himself with a baby latched to his nipple.
