हेपेटाइटिस-ई का संक्रमण जच्चा-बच्चा की जान ले सकता है। दुनिया भर में प्रतिवर्ष 2 करोड़ से अधिक लोग हैपेटाइटिस-ई वायरस से संक्रमित होते हैं। यह वायरस लिवर (यकृत जिगर) पर आक्रमण करता है। पहली बार 1978 में कश्मीर की घाटियों में इस वायरस का पता लगा था। इस संक्रमण से लिवर फेल हो सकता है जिससे रोगी की मृत्यु हो सकती है। ऐसे मरीजों की चिकित्सा ऐसे केन्द्रों में करना आवश्यक हो जाता है जहां लिवर द्रांसप्लाट प्रत्यारोपण) की सुविधा उपलब्ध हो। लिवर फेलर के मामले में लिवर ट्रांस प्लाट ही रोगी को बचाने का एकमात्र तरीका होता है।
हेपेटाइटिस-ई का वायरस मानव मल के द्वारा फैलता है। इसलिए साफ पेयजल, भोज्य पदार्थों (सब्जी-भाजी) की स्वच्छता एवं हाथों की स्वच्छता का पूरा ध्यान रखा जाना चाहिए। हेपेटाइटिस-ई वायरस से संक्रमित होने के बाद इसके लक्षण कुछ सप्ताह या महीने में उभर सकते हैं। कभी कभी इसके कोई विशेष लक्षण सामने नहीं आते और रोगी अपने आप 6 से 8 सप्ताह में स्वस्थ हो जाता है। कुछ मरीजों में बुखार, चक्कर आना, उल्टियां होना, आंखों में पीलापन, भूख न लगना, थकान महसूस होना, पेट में दर्द होना शामिल है। हेपेटाइटिस-ई एक आरएनए वायरस है। इसके चार जीनोटाइप पाए जाते हैं। एशिया में जीनोटाइप-1 के ही मामले ज्यादातर सामने आते हैं। यह गर्भावस्था के दौरान ज्यादा परेशान कर सकता है। गर्भावस्था की तीसरी तिमाही में संक्रमण गर्भवती तथा गर्भस्थ शिशु को मृत्यु का कारण बन सकता है।
गर्भावस्था के दौरान भ्रूण के विकास के लिए रोग प्रतिरोधक क्षमता एवं हार्मोन में कुछ परिवर्तन होते हैं। इसके चलते गर्भवती के संक्रमित होने का खतरा बढ़ जाता है। गर्भावस्था की तीसरी तिमाही में हेपेटाइटिस-ई का संक्रमण होने पर स्थिति गंभीर हो सकती है।
हेपेटाइटिस ई का संक्रमण गर्भवती मां से बच्चों में होने की संभावना तकरीबन 50 फीसदी होती है। बच्चे में संक्रमण होने से नवजात मृत्यु या स्टिलबर्थ भी हो सकता हैं।
संक्रमित मरीजों में से 15 से 25 फीसदी रोगों की मृत्यु हो जाती है। 15 से 60 फीसदी मरीजों में एक्यूट लिवर फेल्यर की स्थिति बन सकती है। ऐसी स्थिति में कैडेवेरिक लिवर (शव का जिगर) प्रत्यारोपित किया

Before understanding the Legal Framework for organ transplantation in India, let us know the meaning of organ donation and transplantation. The process of transplanting an organ from one body to another is called an organ transplant, and the person who donates the organ is called a donor, while the one who receives the organ from a donor is called the recipient. Organ donation happens from a healthy living individual.
The Transplantation of Human Organs Act was passed in 1994. It covered the removal of organs, the transplantation process, storage regulations for therapeutic purposes, and also the prevention of commercial dealings in human organs.
The Act was initiated at the request of Maharashtra, Goa, and Himachal Pradesh. These states adopted the Act by default, and in due course, other states adopted this Act as well. In recent years, there have been a few amendments passed by the parliament. However, all states and union territories now follow this legal framework by default.
Legal framework for organ transplantation and donations in India
It is legal to transplant living human organs, tissues, and cadavers (after cardiac/brain death).
Brain death is identified as a form of death. The process and criteria for brain death certification are defined in law.
Living donors are bifurcated as either relative or non-related donors. 1. Relative donors are spouses, children, parents, grandparents, and siblings who require permission from the doctor in charge of the transplant centre to donate their organs. 2. Non-relative donors require permission from an Authorization committee established by the state to donate the organs.
Swap Translation: When a relative living donor is medically incompatible with the recipient, the pair can do a swap transplant with another unmatched donor pair.
Authorization for organ donation after brain death: 1. Usually, it is given before the death of a person himself/herself. 2. The doctor shall ask the patient or relative of every person admitted to the ICU regarding any prior authorization has been made. If not, the relative should know the option to authorize such donations.
