You can’t believe your luck. 
You’re not sure what exactly it was about this house in the dead of night that had you so transfixed, but your intuition has paid off in spades. 
The area’s been abandoned, to your knowledge leaving you the sole inhabitant meandering around. 
Or maybe waddling would be a more apt description.
Fear and uncertainty of the outside hurry you along into the house. Most everyone- the survivors- has splintered off into groups. There’s no evidence of anyone still living here (admittedly it’s not like you’ve taken the time to check every room, but there are signs when a house is inhabited), but you luck out that the cabinets haven’t been picked over. 
It’s been entirely too long since your last meal, and it takes a good amount of restraint to not devour the can of ravioli too quickly. 
As much as you’re tempted, you know there’s a fine line between what will and won’t have you immediately throwing up in the sink- grazing seems to keep the worst of the upset down.
There’s no hospitals to jaunt off to if you end up dehydrated. Excessive vomiting is not ideal post end of days.
If you were in your right mind- not frightened, isolated, starving, cold- and not focusing on how the unheated chef boyardee might as well be a five star michelin meal for all you can think right now, you might have been paying more attention.
The sound of a safety clicking off behind you freezes your blood far more than the cold. That sound is deliberate. Whoever’s behind you- gun pointed at you- wants you to know they got the jump on you.
“Hands where I can see them,” the order is gruffly barked at you.
You feel stupid. Of course all of this was too convenient for you to simply be catching a break. It wasn’t exactly well lit and designed to draw you in- but you’re an animal caught in a trap regardless.
The fork clatters against the counter next to the can as you go to comply.
“Turn around. Slowly.”
You’re not much of a threat in your current condition. That much is obvious.
Time stopped having any sort of tangible meaning a while ago. You should know how many weeks you are, but the days run together fending for yourself and you just know that you’re close. There’s no hiding the swell of your belly.
The man at the doorway looks as gruff as he sounds. Your mind spins like a tire in mud to process everything in front of you in the poor moonlight. Military, that much is obvious. You’re not actually sure if that’s a good thing. Handsome from what you can see, though historically your type has been men who don’t have a weapon leveled at you.
The taciturn expression on his face falters when he spots your bump, but you’ve learned by now to not expect any sort of special treatment.
“I’m sorry,” you apologize immediately. “I-I didn’t know anyone was here. I’ll leave, I swear.” 
He looks at you another moment before a look of resignation washes over him.
“Turn back around. Keep your hands up.” Oh God. Your mind immediately goes to the worst- That this man, for whatever reason, has decided that your infraction has signed your death warrant. That he can’t quite bring himself to fire on a pregnant woman staring him in the eyes, so the last thing you’re ever going to see is some tacky wallpaper and ugly cabinets.
You yelp when one of his hands finds the pistol on your hip. Holy shit you didn’t even hear him cross the room.
“Easy, love,” he soothes as he starts to frisk you for more weapons. “Not gonna hurt you. You have anything else on you?”
“A knife in my back pocket.” It doesn’t even occur to you to lie; putting yourself in his good graces is your only option and you can’t do that by lying.
His hands slip under your jacket, the hem oversized and hanging even with your arms up, making a wrong guess at the first pocket he checks before grabbing the knife out of the second one.
“Anyone going to come sniffing around looking for you?” A fair question, but one that sticks like a knife between your ribs.
The “No,” that escapes you is softer than you meant it to be, voice warbling as you try not to cry.
Hormones would have had you on the verge of tears at any given point, and that would have been before the end of the world and before your group abandoned you. You’re well entitled to your tears, you think, but try to stuff them back down anyway.
“You’re out here alone,” he grouses, sounding like he doesn’t believe you. The like this? is implied.
Your arms are still up, and they’re getting tired. Everything tires you out these days.
Like he can read your mind, he releases you with a “you can set your arms down now, love.”
“Thank you,” you’re in full fawn mode, turning to face him. While he’s clearly decided against killing you, you’ve been scared and alone for the past few days and you really don’t want to be separated from the only person who will give you the time of day right now. 
“Is there anyone else here? Other soldiers?” Your fate is sealed and lies in the soldier’s hands regardless of his answer.
Nothing with change, no matter what he says, but you think you’re less intimidated if it’s just the two of you. 
The world’s gone to hell in a handbasket, and yet you’ll never forget watching 28 days later when the line I promised them women was dropped.
“Got separated from my team.”
He turns away from you, gesturing to follow him out of the kitchen and towards the living room.
He’s limping.
You haven’t seen him move until now. You’re more an expert on busted hardware than busted body parts, you can’t tell if it’s a fresh injury that’s still healing, or an old one that’s set in place.
“They left you.” They left me, too.
“They didn’t leave me for dead, they think I am dead. Gonna take a bit more than that to get the job done, though.” 
You have no reason not to believe him. Despite having just met him, the man is like a living manifestation of everything masculinity is supposed to be- down to the surly attitude despite him herding you further into the house. It doesn’t take much to figure out that he’s tough as nails and sure why not flirt in death’s face that her last attempt wasn’t good enough?
You sit on the couch he points to, as he settles into the leather chair across from you.
“Christ what’d I’d do for a fucking smoke right now,” he mumbles, pawing at his chest absent mindedly on reflex.
You mean to sit stiff as a board, but your body is tired and the couch is surprisingly comfortable.
The soldier, however, sits like he owns the house. “And now for the question of what to do with you.”

Gladiator Luffy, trapped in the ring, forced to fight everyday just to survive. He can't leave the tower, there is no exit, and he has tried. They are all cursed to be there, and the only way out is to win and take the empty throne, to be a king.
Those who win get food, money, and medicine that they are not allowed to share with others.
We all know Luffy, he is not going to follow the rules, he shares what he can, especially with children that are even younger than him and can barely pick up a sword, kids that would have no choice but to fight and die just for some bread.
He had help from the last lower arena winner (Shanks) and he is going to pay it back, which the only way he can is to help others.
Luffy is determined to climb this tower and be the victor no matter what, and break this prison they are all trapped in. That is his goal.
Since he doesn't follow the rules, Luffy is often in bad shape, getting the hardest fights for little reward so he has to fight way more then any other person, his name always on the scoreboard in matches, he has the most wins but also plenty of loses. Though, if anyone bothered to look that he would fight the same opponent over and over until he won.
Law is a doctor (ofc) and unlike Luffy who was born in the tower to parents he doesn't know, Law chose to be there, as a young child he traded his life so his family could live. He knows that this tower is managed by gods for their own entertainment. His dream is to kill them. Thats the only way he believes they will be free. There is no way any of them in this little hamster wheel will be able to take the empty throne and escape. Rumors that one Gold Roger did so are bullshit as far as Law tells himself. He tries to be a realist but still hopes.
He does battle in matches but few and far between, prefering to trade and barter with people for survival. Which is following the rules, trade is encouraged. Law helps kids or anyone that is sick, running a 'free' clinic, where the price ranges from food to the odd book, or wven a scrap of broken armor that he can fashion into splints or bandages. He manages and always looks for a way to reach the god he made a deal with (doffy).
Luffy, after a long fight, stumbles home, which is just a small tent in the lower floors city. He doesn't need much more, not like there are houses in the tower. Just makeshift shacks or tents made by those trapped.
He doesn't stick to the same street to go home every day, since new shacks pop up all the time, everything is different from day to day. He has to circle around closer to the outer tower wall where the oldest construction is. Some of them even have wood walls, while others have repurposed cells to be functional shops.
Law notices him stumble along. He thinks nothing of it, gladiators come through bloody all the time, and they dont always trust doctors to treat them.
His clinic runs all night, so if the gladiator changes his mind, he can seek treatment.
Then Luffy trips, and isn't moving. Law knows it's dangerous to help him, since this isn't a trade, but his desire to help people wins out.
He carries Luffy into the clinic, looking him over. He is not hurt that bad. It's mostly dehydration and exhaustion. Law setting up an IV and closing the clinic curtains, so if on the off chance nobody saw him help Luffy, he can get away without punishment in the form of a mandatory fight.
Punishment fights are usually out of his strength level, he is skilled yes, but exhausted quick and those matches can go all day. Still he risks it.
Luffy gasps awake hours later in a clinic bed, looking around confused. He usually ends up on the street with his winnings gone after collapsing post hard fight, but not only is all his stuff in one place, he is also all bandaged up, the scent of medicine sticking to his skin.
Law passed out at the table where he usually sleeps after a long night of treating patients. Luffy was not the only one who needed tending after their last series of matches.
Luffy poking him awake with a wide grin. Law groans, mumbling in his sleep, too tired to get up just from that. Luffy shrugs, decides to leave a note along the lines of
'I'm Monkey D. Luffy! And im gonna be king of the tower!!! Thanks for helping me out!!!'
Leaves a bag of gold coins, more in one place than Law has seen in his life in the tower. Way more than he charges for treatment.
Then, he runs off to check out what new matches are placed on the floor he has gotten to.
Law assumes he won't see Luffy again, but the next night, he shows up. Just as bloody as the first day, with a wide grin and asking Law to patch him up, again dropping a bag of coin. Law tries to refuse, tell him uts to much but Luffy waves him off, he dont need the coin anyway, as long as Traffy keeps taking care of him!
It is not long until they settle into a routine, moving from just patching Luffy up after fights, to Luffy just stopping by to have lunch or dinner between bouts.
Law finds himself smiling more, as he can help more people with Luffy's help and the fact that he knows everybody, so gladiators that would get infections or lose limbs because they didnt trust doctors or anyone in the tower, start coming to him because Luffy vouched for Law.
He is exhausted, often trying to go beyond what his body allows him, thats when Luffy picks him up and takes him to go get some rest, cuddling until Law gives in and falls asleep, taking the night off.
Though Luffy always leaves in the morning for his next fight.
Law hates waking up alone in bed, he tries to catch Luffy but the man can move quietly when he wants to.
Law also hates that as the days go by and Luffy climbs the tower, he comes back home with more and more injuries with each subsequent fight. Some that are almost fatal if Luffy was anyone else. Law starts to worry that one day Luffy wont come back.
Him sitting in the clinic, counting the hours and dreading that he will hear Luffy's name in a list of casualties. He has lost alot already in this stupid tower. And he cant take another death of someone he loves. Law is not sure when he started to accept that he loved Luffy.
Yet Luffy always comes home, always with a smile. No matter how hurt he is.
And one day, he is going to kill God xd.

It's Gastroparesis Awareness Month
Hi! I have gastroparesis and I'm an insufferable know-it-all so let's talk about it!
Gastroparesis, or a paralyzed stomach, is a condition that causes delayed gastric emptying.
This can cause a range of symptoms and complications:
nausea
vomiting
early satiety/fullness
upper gastric pain
heartburn
malabsorption
dehydration
malnutrition
Gastroparesis can be treated by a gastroenterologist, but often needs to be managed by a motility specialist due to a lot of misconceptions about the condition. Providers, especially in the emergency department, will commonly misdiagnose gastroparesis as cannabis-hyperemesis syndome, cyclic vomiting syndrome, gastritis, food poisoning, etc.
There are several commonly known causes of gastroparesis like vagus nerve damage from diabetes, injury to the stomach, and stomach surgery like hernia repair or bariatric surgery. There are also idiopathic cases with no known cause. Other causes of gastroparesis are:
Connective tissue disorders like HSD and EDS (commonly hEDS and cEDS)
Post-viral (like COVID, viral gastritis, mononucleosis/Epstein-Barr)
Restrictive eating disorders
Autoimmune diseases like Systemic sclerosis (scleroderma), Lupus, Hashimoto's
Central nervous system disorders
Gastroparesis also has common comorbidities with conditions like:
POTS and other forms of dysautonomia (POTS, EDS, and gastroparesis are a common triad of diagnoses)
MCAS
SMAS (which can also present with similar symptoms to GP)
Intestinal dysmotility and esophageal dysmotility disorders (known as global dysmotility)
PCOS with insulin resistance
Endometriosis
SIBO/SIFO
Chronic intestinal pseudo-obstruction
Migraines
Certain medications like Ozempic and other drugs in that class act on the digestive system to delay gastric emptying, which has caused people to be diagnosed with gastroparesis. Some people report that their cases have not gone away since stopping the medication, others report feeling better after stopping. Other drugs like opiates and narcotics can cause delayed gastric and intestinal motility as well, but these are commonly known side effects of those painkiller classes.
Gastroparesis is classed based on severity and graded based on how you respond to treatment.
Severity of delay ranges from mild to very severe, and this is based on your actual stomach retention calculated at 4 hours into a gastric emptying study.
The grading scale ranges from one to three, one being mild and three being gastric failure.
There is no consistent single treatment that is proven to work for gastroparesis, and there is no cure. Treatments can consist of:
Diet changes (3 Step Gastroparesis Diet, liquid diet, oral sole source nutrition)
Prokinetic (motility stimulating) drugs
Anti-nausea medications
Proton-pump inhibitors
Gastric stimulator/gastric pacemaker
Pyloric botox and dilation
G-POEM/pyloroplasty
Post-pyloric tube feeding
Gastric venting/draining
Parenteral nutrition
IV fluids
Other surgical interventions like gastrectomy or rarely, transplant
Gastroparesis is a terrible disease and I hope that if any of these symptoms resonate with you that you can get checked out. I was misdiagnosed for a long time before getting a proper gastroparesis diagnosis, and all it took was a gastric emptying study. This is ESPECIALLY true if you're having post-COVID gastrointestinal problems that are not improving. I almost died from starvation ketoacidosis because of how serious my GP got in a short period of time post-COVID (I had GP before COVID), and now I'm tube reliant for all my nutrition and hydration.

Shock
SHOCK! Everyone loves writing about it, so we're gonna talk about it.
What is shock? Shock is a condition in which the blood flow in your body is no longer able to sustain the function of critical organs. We call this hypoperfusion. Blood carries a lot of stuff, but most importantly, it carries oxygen. When your tissues don't have oxygen, they're hypoxic. If they're hypoxic for long enough, they die.
What does shock look like? The patient might have hypotension, syncope (fainting), chest pain, abdominal pain, cool fingers/toes, shortness of breath, and/or altered mental status (I know you guys love that last one, lol).
What are the stages of shock? Trigger Insult -> Compensated Shock -> Uncompensated Shock -> Irreversible Shock (organ failure) -> Death
Not all of those stages are obvious in every patient, but that is the general progression. So, what does that all mean? It means that first, something is going to happen (trigger insult). This could be trauma, sepsis, anaphylaxis, heart attack, etc. Something has gone wrong with the normal blood circulation (we'll talk about what in a moment).
Next, the body will compensate to maintain perfusion and keep blood pressure normal. This can cause an increase in heart rate, constriction of blood vessels, rapid breathing, stress, and cool extremities.
When the body can no longer compensate, shit goes wrong. Mechanisms are failing, blood pressure is falling, and the organs are in dysfunction. You may see confusion, syncope, weak pulses, and metabolic acidosis (that's actually deadly but not as fun to write about, lmao).
Finally, when shit has gone very, very wrong, you get organ failure. If there were no interventions in the previous stages, the organs will begin to shut down. Heart rate, blood pressure, and respiratory rate will freefall. The patient will no longer respond to interventions. The nervous system's electrical activity will fade, and the patient will go into a comatose state. Then they die.
The Five Types of Shock
Distributive: this is from the dilation of vessels, which drops the blood pressure to unacceptable levels. This can be do to anaphylaxis, sepsis (bacterial infection), or adrenal crisis. One thing to note with this type is that the skin at the extremities will be warm, not cold.
Cardiogenic: the heart is not working correctly. This can be do to heart disease, heart attack, arrhythmia, etc. This leads to low cardiac output but constricted peripheral vessels. This may present with pulmonary edema.
Hypovolemic: this is from low blood volume. This can be due to hemorrhaging, severe vomiting (or, on the other end, pissing out your butt), or severe dehydration. The patient will have cold extremities and may be pale. Usually, this type of shock is easy to spot because there's either blood everywhere or some other type of trauma causing internal bleeding.
Neurogenic: this is due to nervous system failure from injury to the brain or spinal cord. Note that this type of shock does not have a fast heart rate. Since the autonomic nervous system is non-functioning, there is no response mounted to the shock.
Obstructive: this is a blockage of circulation to the heart. There are two types, pulmonary and structural. Pulmonary is due to increased pulmonary circulation resistance. This can be due to a tension pneumothorax or a collapsed lung. Structural is due to resistance in the heart, such as with cardiac tamponade (fluid in the pericardial space). This case of shock requires you to realized that there is clinically a right and left heart, as the blockage of either side will result in vastly different symptoms and require different treatment.
How do you treat shock? For treatment, it really depends on the type of shock. For all types (except that caused by pulmonary edema), you should give the patient a saline IV to increase blood pressure and volume. Then, it really depends on the case as to what you do next. For distributive shock, you want to give the patient vasoconstrictors (like norepinephrine or vasopressin). For cardiogenic shock, give them inotropes (to increase cardiac output). For hypovolemic shock, fix whatever was torn up and give them IV fluids. For neurogenic shock, you can give fluids and vasoconstrictors. For obstructive, fix whatever is wrong, like breaking a clot or draining the pleural cavity.
Okay, I hope this is a good guide. I think hypovolemic and distributive shock would probably be the most fun to write about, but I mean you could probably make any of these pretty interesting (except obstructive, maybe). Hey y'all, weird question time again! I'm pretty sure I know the answer to this one, but asking anyway: If you are having severe muscle cramps/knots/tension all over your body and go to a doctor/urgent care/hospital for them, assuming the doctor believes it's something that needs treatment, the only things they can give you are anti-inflammatories, pain relief, muscle relaxers, or like an IV to rehydrate you if it turns out to be caused by dehydration, right? I know those are three very broad categories of treatment, I'm not asking for specifics within those categories, just if there are options outside of them (I am allergic to NSAIDs, already on corticosteroids for unrelated reasons, probably shouldn't have muscle relaxers or stronger pain meds for "my blood pressure looks for any excuse to crash" reasons, and am staying on top of hydration and electrolytes, but this round of muscle cramps is bad, even for me)

Hurricane Helene unleashed torrential rains and unprecedented flooding, significantly disrupting operations at Baxter International Inc., a leading supplier of critical medical products. Baxter’s plant, which plays a pivotal role in the production and distribution of IV saline solutions, suffered considerable damage, leading to an immediate halt in manufacturing processes. IV saline, a vital component in medical treatments ranging from surgery to dehydration, is used in nearly every hospital and clinic, making the shortage a significant concern for healthcare providers nationwide. North Valley Hospital said, “It will take time to bring the plant back to full production, 00and like every other hospital, our supplies are running low.
