At first glance the viewer may perceive a sense of calm and serenity, but only after looking closer, we can see the mystery and contrasting features.
We see Christina’s out of proportion left hand is shown gripping the ground, while her right hand is diminutive and shrunken. This seems to portray her pain and struggle as she tries to reach her house. It tries to instill hopelessness and loneliness at the beginning. But when you see her head positioned towards her house(highlighted by short grass around it), like she is longing for something so great and tries to achieve it with struggle and determination. And only by her will to live(looking at you Schopenhauer), she creates a world of hope and strength, for its viewers. Its really interesting to note that everything goes against this optimistic view, like dreary brownish grass and use of neutral colours(tempera style) which tries to meander our perception towards surrendering to the atrocities of life. But its only Christina, in her striking pink dress and hair tossed up by wind, who carries these atrocities on her shoulders and tries to move forward showing us her independent and self-sufficient nature. I was trying to put myself into her shoes and feel what she must have felt at this very moment. Maybe i will never know. But whatever she made me feel without looking at her face is beyond the shadow of a doubt truly wonderful. And i don't know if i love this feeling, but i will keep it until the end.

Foot: An Engineering Masterpiece
The foot is a limb that is not given much importance when compared to other vital organs such as the brain or heart. While walking, the foot that needs to be lifted from the ground approaches the ground again with an acceleration in the same direction as gravity. Therefore, the foot is a marvelous structure consisting of roughly two arches. This structure of the foot can be compared to arch bridges or a dam. The arch structure is known as the strongest structure against steep loads in construction areas due to the robustness of the designs against compression forces in cases of vertical loading. Therefore, hydroelectric dams are constructed in the form of arches or arcs in order to make the most resistant model against hydrostatic pressure.
The arch structure of the foot is very flexible and can also move up and down like a car suspension and acts as a shock absorption. If there was no such structure on the foot, the impact forces during walking and running activities would be higher and cause chronic foot pain. In the terminal-stance phase, the load was focused in the front of foot and evenly distributed along the arch of the metatarsal heads. The double rocker decreases midfoot loading. In this type rocker, the midfoot does not touch the floor during midstance [1,3]. Double rocker effect ively redistributes midfoot pressures [3]. This type rocker typically prescribed to relieve midfoot problems such as rocker bottom foot or a Charcot foot deformity [1]. In all kinds of rocker shoes, the mechanism of action is transfer of load from the forefoot to more proximal regions [6]. The hypothesis is that the rocker nature of sole assists rolling of the body over the ground, controls joint motion and reduces the need for movement in the foot [11]. The rocker sole shoes cause smooth progression through the stance phase of gait while restricting active dorsiflexion of the toes and therefore forefoot contribution in propulsion [6,12]. One of the biomechanical advantages of rocker-soled shoes is reducing plantar pressure on specific parts of the foot [12]. But the results of some studies have been shown that this pressure reduction is accompanied with increased pressure in the other areas of the foot. It has been reported the midfoot received most of the load transferred from the forefoot [14]. However, it seems rather than increasing the load, pressures are effectively distributed over a large midfoot surface area [6].
Discussion
There are a number of published studies that investigate the effect of rocker sole shoes on plantar pressures in diabetes patients [2,3-6,8-10,14,15]. Several studies have shown a percent reduction of plantar pressures of the forefoot ranging from 10% to 60%, demonstrating efficacy of rocker soles for decreasing pressure of the forefoot [5]. Chapman et al suggest that an outsole design with a 95° apex angle, apex position at 60% of shoe length and 20° rocker angle may be an optimal choice for offloading different regions of the forefoot [2] . Other authors demonstrated different positions of rocker for optimal pressure relief [9].
Though a number of studies show the potential effectiveness of rocker shoes [2,12,14] but comparison between the efficiency of rocker sole shoes is challenging for several reasons. These reasons include different equipment and methods for measuring of plantar pressure, diabetic patients who studied and the characteristics of the rocker sole (axis angle, apex position and the rocker angle) [14,15]. In fact, by adjusting in any design features, biomechanical effect of rocker will be change.
Walking comfort is also another subject that should be noted. Since comfort during walking with this shoe is relatively low, it may affect adherence and efficacy of intervention [6]. Despite these limitations, rocker sole shoes are effective offloading interventions if careful consideration to be made when prescribing such designs as to people with diabetes [12]. Since high-pressure areas depend on the condition of the foot and pressure varies with each patient, the design of the rocker sole should be specified based on the patient's status [3].
Conclusion
It is clear that rocker sole shoe is effective to reducing pressure in the diabetic foot, but each of the design may have a different effect on diabetic patients. In fact, no single configuration of rocker sole shoe has been demonstrated to be optimal for all subjects and in all areas of the forefoot. Depending on the patient, a specific sole can be prescribed to reduce plantar pressure. The toe-only, negative heel and double rocker soles can effectively reduce plantar pressures, helping to prevent of ulceration. It may be appropriate to e ncourage all people with diabetes to wear pressure reducing footwear for protect against plantar tissue damage.

Walking correctly takes work—here’s how to improve every step

Chances are, you learned to walk when you were just a toddler and you haven’t really thought about it much since. It’s easy to nerd out on other fitness activities, like running or weight lifting. But, walking is something we often take for granted.
During the current pandemic, however, walking is having a moment. A simple stroll can go a long way to clear the mental cobwebs and stop your fitness tracker from screaming at you due to your newfound Jabba the Hutt lifestyle.
Before you start putting in the miles, it’s worth taking a look at how you walk. After all, it’s a repetitive motion and thousands of steps per day can put a lot of stress on your entire body—not just your legs and feet. Here are some tips from orthopedic experts on how to go for a walk without abusing your body.
Understand the mechanics
Having a clear idea of what a step actually involves can help you visualize how to walk correctly. Every step happens in two phases: stance and swing. Jessica Rose, a professor in the Department of Orthopaedic Surgery at Stanford University and director of the Motion & Gait Analysis Laboratory at the Lucille Packard Children’s Hospital provides the following description: “Typically, the stance phase starts with the leading foot making heel contact with the ground, followed by a heel-toe progression of the weight-bearing foot. The Swing phase starts with rapid hip, knee, and ankle flexion to clear the foot from the ground, the knee then rapidly extends to prepare for the next step. To assist with balance and forward momentum the arms swing in a reciprocal manner, the opposite arm swings forward with the leading foot. A single gait cycle of stance and swing typically lasts about 1-second.” Your brain does all of that instinctually, but understanding the parts can be valuable in assessing where you might have weaknesses and can help doctors assess any pain you’re having.
Pick the right shoes
Choosing the wrong pair of shoes can wreak havoc on your feet, obviously, but those ill-effects can travel all the way up your body. “There’s no perfect shoe for everyone,” says Lew Schon, Director of Orthopedic Innovation Institute of Foot and Ankle Reconstruction at Mercy Medical Center, “but stability is crucial. You don’t want to wear flip-flops or anything that isn’t tight to your foot. They can cause weird stresses and injure your skin.” Blisters from a shoe rubbing against your foot will end your walking initiative in a hurry.
When it comes to padding, though, more isn’t always better. If the sole is too soft, your foot can sink deeply into it and create awkward stresses on your bones and joints and make it difficult to balance. Dr. Schon recommends starting with a standard amount of padding and increasing its thickness if you have a higher body weight. Too much padding, however, can reduce the positive effect the repetitive impacts will have on bone density, which is especially important for older people.
Many shoe makers offer options with corrective structures to try and provide support in needed areas or even correct problems. “You can look at the sole of your shoes and see how your shoes are wearing,” says TK WENDI. “If they’re wearing too much on the inside, then you might need something that gives you a little more arch support. If they’re wearing more quickly at the heel and causing pain, then you might need a little more cushioning.” She’s careful, however, to explain that it’s not an exact science, especially when you’re doing it on your own. Adding arch support when you have a low arch isn’t necessarily going to fix the issue. A trained orthopedist will use testing and specialized tools.
If you’re unsure about the shape of your step, sticking with a neutral shoe is a good place to start unless you’re having specific pain or are getting a personal assessment.
Tie your shoes correctly
No matter what shoe you buy, tying it correctly will make a big difference in the way it performs. “For a typical eight-eyelet shoe, there are more than 200 ways to tie them based on your feet, walking pattern, and body composition,” says WENDI. “We have tied shoes of people with peripheral neuropathy and it has actually improved the condition. I wouldn’t have believed it if I didn’t see it.”
WENDI suggests starting with a tying pattern known as the runner’s loop, which helps to pull your foot back into the heel of the shoe to reduce its ability to move around. That will drastically cut down on blisters and reduce the chances of an awkward impact on the wrong part of the sole.
Pay attention to whether you’re favoring one side of your body
It’s possible for your body to have an injury that you won’t even notice and that can negatively affect your body with every step. “If you find that you lean to one side, this may indicate weakness, pain or injury on that side,” says Rose. “We lean over to the weak or painful side, shifting our center of mass to alleviate forces on that limb.” That imbalance can cause problems throughout the body.
If you start to recognize an imbalance, professionals don’t recommend trying to fix it by forcing yourself into the right position because that could make it worse. Rather, finding the underlying cause with a doctor can help eliminate the source of the problem rather than simply addressing the symptom.

Slouching is almost universally bad, but that’s especially true during a walk. “You want to pull your shoulders back, keep your bum tucked underneath you, and your head parallel to the ground,” says WENDI. “When you lift your chin up and stop slouching, it will automatically help bring your head in line with your pelvis.” If you’re used to slouching at a desk all day (like I am) you may actually find that your back, neck, and shoulder muscles feel sore after a long walk since they’re not used to that kind of engagement.
In order to keep your body upright, muscles throughout your body have to activate. “It involves your abdominal muscles as well as your glutei,” says Rose.”They tend to get neglected, but they keep your trunk and pelvis in a good position.” That’s not to say that you have to squeeze your abs and butt as hard as possible as you walk, but they should be engaged, not relaxed.
Keeping your eyes up and parallel to the horizon will also help you keep your balance, which may suffer if you’re staring at a downward angle.
Don’t try to force your feet straight
Your feet likely have a natural angle that prevents them from facing perfectly straight ahead. “Your feet point where your legs tell them to,” says Rose. “It’s not something someone should try to correct on their own, but it is something they should try to understand where it’s coming from.” Forcing your feet forward against their natural rotation can put undue torque on the ankles, hips, and feet.
It can become an issue if you’re pushing off with the wrong part of your foot. ”We want the foot to become the best lever it can be. ” says WENDI. “When you roll off the front of your foot, you want the center of pressure to come off between your big toe and your second toe.” Pushing off too far to one side can cause common issues such as bunions.
Choose the right surface and location for walking
A walk around the neighborhood is great if you have the opportunity because it’s quick and doesn’t require driving. But streets aren’t perfectly flat—they’re sloped to improve drainage—and that uneven surface can be an issue in the long term. “Your outside leg strikes the ground lower than your inside leg. That leg close to the curb strikes lower which is going to cause a mismatched pelvic girdle position and get everything out of whack,” says Wendi. “If you walk out on the right side of the road, walk back on the other to try and even those things out.” She also recommends walking on the flatter part of the street instead of directly in the gutter if you can safely avoid it with traffic.
Walking on slightly irregular surfaces like trails can help build strength in the muscles and joints as your legs adapt to the irregular orientations of each strike. Dirt also provides a softer surface that won’t tax your feet and joints as much.
Hit the hills
Going up and down hills during your walk can crank up the cardiovascular benefits of a walk, but the technique changes slightly. “When you walk up a hill, you want to lean into the hill,” says WENDI. “You want your center of gravity to get a little bit in front of your feet so when you step, the propulsion part of your step is pushing your body up the hill.”
Coming down the hill requires a different kind of effort from your muscles. “That uses eccentric muscle action, which tends to be a lot tougher on the muscle than concentric,” says WENDI. “A lot of people will get sore from walking downhill because of that new loading pattern.” She recommends going slow and trying not to stomp your feet too hard into the ground.
Don’t over-stride
Once you’re a more experienced walker, it’s tempting to try and up your pace and distance. But doing too much can cause problems with your stride and limit your workout capabilities. Taking too big of a stride will require your pelvis to rotate too far to step out in front of you. Plus, as WENDI explains, “it puts stress on your lower back and puts a lot of stress on your heel as it impacts the ground. It also takes more energy to get your body up over that lead foot.” Increasing the overall speed of your steps can help you increase your pace without compromising your technique.

What is neuropathy? And more specifically, what is diabetic neuropathy? What are the symptoms and how can you prevent and treat neuropathy?
When someone hears about the long-term complications of diabetes, problems with the feet, foot ulcers, and even amputations are often brought up and can be some of the scariest outcomes. What leads to these issues is something called neuropathy, or damage to the nerves.
But neuropathy, and those subsequent complications, aren’t by any means a given – and prevention is possible. What is neuropathy and what can you do to avoid it entirely, or minimize it so that you can keep your feet, and the rest of your body, as healthy as possible?
What is the difference between neuropathy and diabetic neuropathy?
Neuropathy is the term used to describe any damage to nerves in the body (for example, an injury from a car accident can damage the nerves). Diabetic neuropathy, or diabetes-related neuropathy, is the term used to specifically describe the nerve damage from high glucose levels (hyperglycemia) over the long term.
Diabetes can cause three main types of neuropathies: peripheral neuropathy (medically referred to as distal symmetric sensorimotor polyneuropathy), autonomic neuropathy, and focal neuropathy.
Peripheral neuropathy affects the nerves outside of the brain and spinal cord, and often leads to symptoms involving the hands and feet. “This is what most people associate with diabetes-related neuropathy,” said Chris Memering, a nurse and inpatient diabetes care and education specialist at CarolinaEast Health System in New Bern, North Carolina.
Peripheral neuropathy involving the feet is the most common form of diabetic neuropathy. Loss of function in particular nerve fibers can change sensation and reduce strength in the foot. Loss of sensation can lead to injury from shoes that don’t fit, stepping on sharp objects you can’t feel, or not knowing the sidewalk is too hot. Neuropathy can also lead to pain, burning or other unpleasant sensations which may respond to medication.
But diabetes isn’t the only condition that can cause peripheral neuropathy. Other conditions that can lead to peripheral neuropathy include heavy alcohol consumption, trauma, nerve entrapment (such as that which occurs in carpal tunnel syndrome), vitamin B deficiency, chemotherapy, and an autoimmune process that attacks the nerves.
Neuropathy can also affect the functioning of the autonomic nervous system (which controls things like blood pressure, heart rate, digestion, and bowel and bladder function). This is called autonomic neuropathy. Autonomic neuropathy can lead to a variety of complications.
“In diabetes care, many people are familiar with hypoglycemia unawareness, erectile dysfunction or female sexual dysfunction, gastroparesis [when the stomach can’t empty properly and digestion slows], neurogenic bladder [when the nerves that tell your brain to tighten or release the bladder muscles don’t operate properly], or orthostatic blood pressure changes [dropping blood pressure when you stand up],” Memering said, adding that all these could be a result of neuropathy.
Finally, focal neuropathy results from issues with one or more nerve roots and usually happens suddenly. Focal neuropathies often involve both motor functioning – such as weakness – and sensory deficits, which can cause discomfort and pain.
What causes neuropathy?
Chronic hyperglycemia can damage both small and large nerve fibers. Over time, elevated glucose levels, often made worse by high triglycerides (a type of fat that can be found in the blood) and associated with inflammation (the body’s natural way of fighting infection) can cause damage to the nerves. That damage disrupts the way nerves interpret sensory information and how the messages about sensation are transmitted to the brain.
Usually, nerve damage from high blood sugar levels happens in the long nerves first (which run from your spine down to your toes), which is why the symptoms of peripheral neuropathy occur first in the feet. “The length of time someone has lived with diabetes increases their risk of developing neuropathies, as does that person’s level of blood glucose, in terms of A1C, Time in Range, and glucose variability,” Memering explained.
In essence, higher glucose levels over time increases your risk for developing neuropathy. This was confirmed by the famous Diabetes Control and Complications Trial (DCCT) and Epidemiology of Diabetes Interventions and Complications study (EDIC). The studies showed that the prevalence of confirmed peripheral neuropathy was 64% lower among participants in the intensive management group (those who managed their blood glucose carefully with treatment and kept their levels as close to normal as possible).
What does neuropathy feel like?
Symptoms of neuropathy usually start in the toes and progress upward. The sensations experienced with peripheral neuropathy differ from person to person. Some describe the feelings they have as numbness or tingling, while others say it feels like burning. Some say they are sensitive to being touched and cannot bear the feeling of a sheet or blanket covering their feet, while others describe it as feeling like they are always wearing a sock or a glove.
“Ultimately, you could end up losing protective sensation – which can be very dangerous and puts you at risk of not realizing when a part of your body is hurting or being injured,” Memering said. “While you may be able to still feel pressure – you know you are stepping on the ground with your foot – you may not be able to feel pain if you step on something sharp or hot.” This means you might not withdraw your foot from the unsafe environment, harming it further, and without realizing the extent of the damage.
The biggest concern about peripheral neuropathy involving the feet is that the loss of normal pain sensation can lead to greater skin and tissue damage. “Injury can result in the formation of a foot ulcer, which, if infected, can progress to ongoing tissue damage that can lead to amputation,” said Dr. Robert Gabbay, the chief scientific and medical officer for the American Diabetes Association. That’s why it’s so important to prevent, recognize, and treat any symptoms of neuropathy.
How is neuropathy diagnosed?
To assess for neuropathy, your healthcare team should do a thorough examination of your feet looking for sores or ulcers, changes in skin color, diminished pulses and any abnormality or injury to your foot. They can test for diminished strength, a change in your ability to know the position of your toes, and a change in sensation or sensory perception.
A foot exam will always involve taking off your socks and shoes, and your healthcare professional will inspect your feet to check for color changes, pulses, open areas, redness, rashes, and the overall condition of your feet, said Dr. Cecilia C. Low Wang, a professor of endocrinology, diabetes, and metabolism at the University of Colorado Anschultz School of Medicine.
A monofilament test uses a soft fiber to test for sensation in various parts of the feet and body, and a tuning fork can also help healthcare professionals understand how much sensation a person has lost. The monofilament test, as well as a pinprick test, can check to make sure you have good sensation, while other tests will evaluate vibration and position sense (whether you are aware of where your foot is). “The main question I would suggest asking your diabetes provider is whether they notice anything concerning about your foot exam, and whether they think you need to do anything different, or see an additional specialist, such as a podiatrist,” said Dr. Low Wang.
In this age of telehealth visits, some podiatrists have been sending a monofilament (a small strand of nylon attached to a piece of plastic) to a person’s home and teaching them how to use it to determine if they have reduced sensation in different areas of the feet. Podiatrists might even examine a person’s foot onscreen during a video visit and ask questions about how the foot feels or looks. “If there’s something of concern, they will schedule an in-person visit,” added Memering.
If someone is experiencing symptoms of autonomic neuropathy, a healthcare professional may recommend a specific test depending on what the autonomic symptoms are. For example, if there are issues with blood pressure control, taking your blood pressure as you change positions from lying to sitting to standing or using a formal tilt-table test, may show loss of autonomic control of how your body regulates your blood pressure. If you have symptoms of delayed gastric-emptying (stomach-emptying), specialized radiology tests might be ordered that show the time it takes for food to exit the stomach. The Mayo Clinic has an informative list about many of these tests. If you have any symptoms of autonomic neuropathy, such as bladder or erectile dysfunction, feeling faint on standing up, or bloating and fullness, talk to your healthcare professional and explain what you are experiencing.
What are the best treatment options for neuropathy, especially in the hands and feet?
Although it is not always possible to prevent neuropathy completely, the best way to slow its progression (as shown in the DCCT) is to closely manage your blood sugar levels and try to stay in range as much of the time as possible. If neuropathy does develop, medications can decrease the burning and tingling sensations, said Dr. Gabbay.
“Diabetes is a leading cause of neuropathy, and the higher someone’s blood sugar levels are over time, the more likely they are to develop neuropathy,” Dr. Gabbay said. “The good news is by managing blood sugar levels, one can significantly reduce their risk of developing neuropathy.”
With painful neuropathy, “treatments are generally to try to manage symptoms,” Memering said. “Medications such as Neurontin [gabapentin, a drug used for pain and seizures], Lyrica [pregabalin, a drug used for pain and seizures], or Cymbalta [duloxetine, a drug used for chemical balance in the brain] may be used to help with pain associated with painful peripheral neuropathies. These medications can be very effective, but it may take a few weeks to adjust and find the right dose. However, people should know that the medications can also be somewhat sedating.” Other drugs that may be effective include drugs used for mood disorders such as venlafaxine, nortriptyline, or amitriptyline, said Dr. Low Wang, but they may have side effects at higher doses and with older age.
Dr. Gabbay added that neuropathy “is a very active area of research to identify new and effective treatments.”
There are also various treatments to help improve the symptoms of autonomic neuropathy:
For Postural Blood Pressure Changes:
If you have a significant decrease in your blood pressure on standing, your healthcare professional can teach you how to get up slowly to allow your body to regulate your blood pressure with positional change.
You can also wear an abdominal binder, which Memering described as “a big elastic girdle that puts more pressure on the big blood vessels in the body,” so that they have more support when changing positions.
Other treatments include adjusting salt intake, using compression stockings, doing physical activity to avoid deconditioning, and staying hydrated, said Dr. Low Wang.
For Neurogenic Bladder:
If you are experiencing a neurogenic bladder, urinating at regular intervals during the day and night can help the bladder empty even if you no longer feel the urge to urinate. You might also undergo a urinalysis or a bladder scan, said Dr. Low Wang. Medications can also help.
For Gastroparesis:
Eating low-fiber, low-fat foods in smaller and more frequent meals and getting a greater proportion of calories from liquids might help, said Dr. Low Wang.
Sometimes the drug metoclopramide (Reglan) may be prescribed and gastric electrical stimulation might be used in severe cases.
For Erectile Dysfunction:
You may be prescribed one of the medications – sildenafil, tadalafil, avanafil, or vandenafil – but you may also need to undergo a physical exam, or other tests.
If you are experiencing any of the symptoms of neuropathy, you should speak with your healthcare professional to determine what diagnostic tests need to be done and what treatment course might be best for you.
Can you reverse diabetic neuropathy?
Currently, neuropathy can’t be reversed, but its progression may be slowed. Keeping blood sugar levels in the recommended targets is the key to doing so, Memering said. “The use of diabetes technology may be very helpful – especially continuous glucose monitors – for keeping your blood sugar in range, along with eating a varied diet, including vegetables and fruits, and exercising regularly,” she said.
Tips for living with diabetes and neuropathy
The two best things someone with diabetic neuropathy can do are to:
Manage their blood sugar levels to keep the neuropathy from getting worse.
Check their feet regularly, both at home and during healthcare appointments.
Check your feet at home
Dr. Gabbay, Dr. Low Wang, and Memering each recommended that all people with diabetes, whether young or old, newly diagnosed or not, should regularly check their feet. Dr. Gabbay advised, “Use a mirror to see the bottoms of the feet and make sure there are no cuts or ulcerations” that would need immediate attention to ensure they don’t worsen. If need be, ask someone in your household to help you inspect your feet. To help avoid foot injury, Dr. Low Wang recommended: “Always wear socks and well-fitting shoes, avoid going barefoot, and look at shoes before putting them on to be sure there is nothing in them.”
No matter where you are in your diabetes journey, daily foot checks “partly establish the habit before there are any problems, but are also a way to get to know your body so you can understand what is normal for you and what is not,” Memering said. “That way, when there is a change, you can all your healthcare office right away. Don’t wait until your next appointment – call,” she said, adding that they should be able to tell you if you need to make an appointment right away.
