Here in Canada, what you're describing as "America: with NA and SA inside of it", we were raised raised to call The Americas, and anyone saying "I'm american" specifically means the USA. Nobody I know would even consider any other possibility. America is synonymous with the USA, here, potehto-potahto, and we're so used to being ignored and overshadowed by the US that most of us would rather eat our shoes than be called American.

To circle back to my original point, though- (before I, with context and in restrospect, kind of bitched the flip out about)- Canada is not The United States, and as someone who isn't particularly thrilled with our "Oh haha, Canada so cute" global persona that actively works to whitewash some serious crimes against humanity, it's very, very tiresome to speak out about a CANADIAN ISSUE just to see folks sweep in and talk about how "America is just like that". Like we're some cutesy wonderland where everyone is nice or something.
