Let's use Jack as my wonderful little guinea pig, as I always do in Beanstalked situations

If two of the same person existed, it's quite a big problem because of balance reasons and so, to rectify this, there is a special magic that occurs where the Lore equivalent will wind up merging with the Earthly equal.

So in a realistic situation, if Jack went to Earth, his body would wind up entering his Earthly one, like a ghost possessing someone. But instead of possession, it's both minds fusing memories into a singular Jack. Similar to how you carbonate a fruit juice or mix sparkling water with a drink.

You have the fruit juice and you can still taste and recognize it but now it's all fizzy so you also taste the fizz.

And if modern Jack went to Lore, he would enter Lore Jack.

Whoever is the resident of the planet determines the body and the visitor is who fuses with them. And whenever it's time to go, the visitor leaves and they manifest back into a solid form aka their body on their planet.

I imagine a magical contraption like a magic mirror with portal abilities activated would be the only way for such a thing to take place without going terribly wrong.

NOW if we are talking COMPLETELY for fun and modern Jack and Lore Jack wind up actually MEETING meeting, no fusion involved, they would probably gush about their respective Nanas and bond over their planetary equivalent of each other's events.
