Chronic Pain Is Such A Trip
A little background - I was diagnosed with scoliosis in the 5th grade, but was not treated for it until I was a sophomore in high school. This saw me going to the nearest Shriner's hospital, where I was fitted for a wretched plaster-cast back brace that was fitted from under the armpits to over my hips at sweet 16. It was horrible. And, from pressing insistently on my right hip, so fully killed the nerves there that I can't feel it to this day.
I moved through my teen years and into my adult life trying to manage this condition as best as I could on my own. According to the Shriner's doctor when I was a teen, any attempt at corrective surgery would be considered not medically necessary and purely cosmetic. So I should be able to live a fully happy life, yeah? No issues.
Except that the curvature progressed into the severe category, until I eventually got updated xrays in my late 20s that revealed the full extent of my situation, which was an S curve with a twist, a result of degenerative disc disease, spina bifida, and scoliosis. As someone who had been managing on my own with a strict pain medication management plan with my doctor, it was recommended that I pursue surgery. It took years, but eventually, I was able to have the surgery and Harrington Rods were installed in my back.
The surgery was traumatic. The healing process was traumatic. But, eventually, I recovered. That was four years ago.
The pain has returned in a whole new way. I've been going to the gym, eating better, managing with ibuprofen, and trying to do everything that my physical therapist has instructed me to do over the years. But some days, it's so bad that I physically cannot get myself out of bed. I tried urgent care and went to see a doctor, who referred me to a pain clinic. Guess what they did?
They photocopied some book covers, told me I was basically imagining the pain, and said I should get on antidepressants before shoving me, sobbing hysterically, out of the door. It was traumatizing and I refused to go to another doctor again.
That was two years ago. When it was bad enough again that I had to go home from work a coupla weeks ago, I finally scheduled an appointment with a new doctor. With updated xrays. Guess what they informed me?
There's degenerative tearing in my neck.
Holy shit, I'm not crazy. I was over the moon. Even if there was nothing to be done about the pain, at least it was good to know that I wasn't imagining the whole ordeal. I had them forward the report to my physical therapist, so that he could go over the documents with me.
He reported to me that nothing was wrong. According to the report, I was perfectly fine. In fact, his degenerative tearing was probably worse than mine, according to him.
To say that hearing this was devastating is... an understatement. He looked at me like I was imagining the whole thing. And so now I'm more confused and angry than ever. I don't know what to do with this information, especially considering I'm in between doctor's appointments while we try and figure out what is wrong with me, which I hurt constantly, and how we can fix it. (Cherry on top was phys therapist pointing out all my shitty posture and the like as I walked away. It was really hard not to cry)
I'm so tired of being in pain all the time.

Like a hummingbird’s wings, the rapid fire heartbeat filled the room, making my handsome older husband break out into a huge smile as he slipped his fingers into my hand.
“She’s saying hello,” I hummed softly, bringing my soulmate’s hand up to my mouth to press a sweet kiss into the backside over his tattoo.
“Hello, Baby Violet Marie,” Peter murmured.
“Let’s count her toes, alright?” Dr. Isles suggested before pointing to the digits.
“One… two… three… four… five… six… seven… eight… nine… ten!” counted the kids, overjoyed with be able to interact with the appointment.
“All ten toes, how about her fingers next?” the woman suggested.
“One… two… three… four… five… six… seven�� eight… nine… ten!” the kids counted.
“All ten fingers,” she smiled, making the notation in my file.
“Mommy, I think Baby Violet Marie is waving to us!” Katie squealed, bouncing up and down on the balls of her feet.
Dr. Isles frowned all of a sudden, the wand coming to a stop over a certain part of Baby Violet Marie as she stared intently at the screen.
“Well, there’s no easy way for me to tell you this,” she announced in a grim voice, Peter grabbing at my hand in fear. “But I’m almost positive that Baby Violet Marie will be born with spina bifida.”
“What is spina bifida?” Katie asked, chewing at the end of her braid.
“Spina bifida is where the spine doesn’t form correctly in utero,” she explained, moving the wand around a bit more. “There- see? The spine has formed in an unnatural way and will be almost certainly be positioned on the exterior of the body.”

I couldn’t stop crying as Peter took me into his arms, letting out a soft whistle from between his teeth as he called the kids to follow closely behind him.
“Bitty, can you go and pay the copay while I put mommy into the car?” my husband asked, Elizabeth reaching into his back pocket and coming up with his wallet before going to stand in line with Baby Tommy holding onto her hand while Peter took the rest of the family out to the car to get settled in. “I’ll be back in just a moment, sweetheart.”
He shut the door before jogging back into the clinic, where I saw him joining Elizabeth and Baby Tommy at the check in desk, where he chatted with the receptionist as he paid the copay and collected the necessary paperwork to file for taxes.
“Mommy?” Katie meeped, drawing my attention back to her. “Will Baby Violet Marie need an IEP in school?”“I don’t know, mo stór,” I shrugged, my hand nestled over my popped out tummy. “It’s honestly too soon to know for certain, but if she does end up needing an IEP, we will already be well versed.”
“If she does need an IEP, I can advocate for her!” she declared as Peter, Elizabeth and Baby Tommy got into the car and buckled up for the ride back home. “I’m her big sister!”
“Are you going to be alright, sweetheart?” he asked me, placing a hand onto my knee as he carefully maneuvered the family station wagon. “What can I do to make you smile?”
“I don’t know,” I answered honestly. “My love, I know that Baby Violet Marie will be a very much loved babies, but what will we need to do to ensure her comfort?”
“We can look into it when we land, alright?” he told me with a hum. “We have two hours before I need to leave for the audiologist for my appointment, alright? What do you want to do in the meantime?”

The OEIS complex – clinical & radiological evaluation
Although the first description of a case of OEIS complex was reported in 1709, it was only in 1978 that Carey et al first used the term OEIS complex [1,2]. The anomaly is thought to be due to a defect in early blastogenesis or of mesodermal migration during the primitive streak period [7,8]. These defects lead to improper closure of ventral abdominal wall due to failure of convergence of four ecto-mesodermal folds (a cranial, a caudal, and two lateral) of an embryo with associated defects in the development of cloaca and urorectal septum during 4th gestational week. The cloacal membrane is composed of ectodermal and endodermal layers and it covers the region from the umbilicus to the tail portion of the embryo at the 4-mm stage. In subsequent stages, the primitive streak mesoderm invades the membrane to form the lower abdominal wall. Lack of mesoderm in the infra-umbilical abdominal wall results in omphalocele. During the 8- to the 16-mm stage, the urorectal septum meets the cloacal membrane and divides the cloacal chamber into an anterior urogenital and posterior alimentary systems. Cloacal exstrophy results when the membrane ruptures before the formation of the urogenital septum. The coexistence of cloacal exstrophy and spinal dysraphism may be explained by a single insult in the embryonic tail in early pregnancy [6]. Caudal dysgenesis interferes with somite formation resulting in defective vertebrae [9].
Because of its rarity and sporadic occurrences, some environmental and genetic factors may be responsible for its causation [6]. One study reported maternal exposure to diphenyl hydantoin and valproic acid and cigarette smoking as possible risk factors [2,6,9]. Higher incidence of OEIS in monozygotic twins than in dizygotic twins suggesting a possible genetic contribution to the occurrence of these defects was reported [10].
The OEIS complex involves multiple body systems. The classical presentation consists of omphalocele superiorly with an open plate mucosa inferiorly consisting of two posterior walls of hemibladder on either side with a central strip of ithe ntestinal mucosa [11]. Similar findings were noted in our case with the passage of meconium and stool from the protruding bowel loop in the upper part of the central bowel strip. Ambiguous genitalia is common with bilateral cryptochidism in males. In the present case, one testis was found in the hemiscrotum, but the other remained high in the inguinal canal. Renal anomalies such as agenesis, rudimentary kidney, ectopic kidney pelviureteric junction obstruction etc have been reported [12]. The congenital vertebral malformations associated with cloacal exstrophy vary from congenital scoliosis, kyphosis, abnormal lumbosacral segmentation, sacral agenesis, interpedicular lumbar widening to hemivertebra, spina bifida occulta, and myelomeningocele [13,14]. In the present case, there was terminal myelocystocele with tethered cord and syrinx formation. The baby had a partial neurological deficit in both lower limbs below the level of the knee. Talipes in association with OEIS complex is related to a tethered cord. Central nervous system anomalies are not commonly encountered in OEIS complex.

Basal cell carcinoma resection in an Ecuadorian patient with Gorlin-Goltz syndrome by Andrea Villarreal-Juris in Journal of Clinical Case Reports Medical Images and Health Sciences 
ABSTRACT
Gorlin-Goltz syndrome is an inherited dominant autosomal disorder characterized by a predisposition to numerous cancers. The clinical-pathological findings of this syndrome are very diverse. The objective of this report is to present an Ecuadorian patient with Gorlin-Goltz syndrome who underwent surgical resection of basal cell carcinoma compatible lesions.
Conclusion: Gorlin-Goltz syndrome encompasses a variety of clinical signs and symptoms, including important oral manifestations and skin lesions that must be recognized to achieve an early specialty referral, thus reducing the risk of malignancy through a multidisciplinary treatment.

INTRODUCTION
Gorlin-Goltz (G-G) syndrome or nevoid basal cell carcinoma syndrome (NBCCS) is a dominant autosomal genetic disorder with high penetrance. Fifty percent of patients who suffer from it have a mutation in the long arm of chromosome 9q22.3 in the area of the PTCH gene (protein patched homolog) (1–3), a homologue of the Drosophila patched gene (PTC), which encodes a transmembrane receptor protein (4). This protein binds to a soluble factor of the hedgehog family (Hh), thus activating the Smo (smoothened) receptor, which unblocks the transcription of several growth factors. Therefore, the PTCH gene is an oncosuppressor that is part of the Sonic Hedgehog Homolog (Shh) signaling pathway and is crucial in embryonic development, cell division control, and tumor growth (5).
Its approximate prevalence is 1 in 57000 to 1 in 256000 and the ratio of males to females is 1:1 (6). Binkley and Johnson reported this syndrome for the first time in 1951 (7), then, in 1960, Gorlin and Goltz described the association between multiple basal cell carcinoma, odontogenic keratocysts (OKC) and bifid ribs, which account for the characteristic triad (8). In 1977, Rayner et al. added additional features, including calcification of the falx cerebri and palmar/plantar fossae (9).
According to the criteria of Kimonis et al., the diagnosis of G-G syndrome requires the coexistence of at least two major criteria or one major and two minor (10).
The characteristic that is usually diagnosed first is OKC, because it can be detected during the first decade of life and appears in almost 80% of G-G syndromes.
Other manifestations include palmar and plantar ulcers that appear as shallow pits, caused by partial or complete absence of the corneal layer, which can also appear along the sides of the hands and fingers and even on the tongue; spina bifida (10,11); medulloblastoma (can be an epiphenomenon of G-G, especially in children who are ≤5years-old) (12); cardiac tumors, including fibromas and ventricular histiocytomas, usually congenital (13); hypertelorism, congenital cataract, nystagmus, coloboma and strabismus (14); and ameloblastoma (extremely rare) (15,16).
Early diagnosis of G-G syndrome and its subsequent treatment are very important due to neoplasm susceptibility (2).
The case of a patient with a previous diagnosis of Gorlin-Goltz syndrome who presented multiple lesions compatible with basal cell carcinoma is presented below.
CASE REPORT
A 35-year-old male, living in Quito, Ecuador, without allergies; no history of tobacco or alcohol intake. His mother and his brother presented Gorlin-Goltz syndrome (G-G).
The patient presents hereditary and congenital G-G syndrome (multifocal basal cell carcinoma and maxillary keratocysts), whose manifestations began at the age of 22. He underwent surgery for bilateral keratocysts at the age of 25, and multiple biopsies of the upper left eyelid were taken since 2012. In August 2015, with a positive tumoral activity biopsy report, a wide resection involving 60% of the left upper eyelid plus flap reconstruction was performed.

Physical examination revealed multiple surgical scars on the left upper eyelid with tumoral activity on the eyelid margin, as well as on the outer third of the ipsilateral lower eyelid and on the right side.

In October, a wide resection of the left upper and lower eyelid was planned, plus reconstruction and transoperative study, which are performed without complications.

Subsequently, the patient attended scheduled control, reporting the presence of a left superciliary nodular lesion. Physical examination revealed multiple lesions located in the left superciliary region, left helix, concha, and antihelix, in the inner corner of the left lower eyelid, in the left parieto-temporal and occipito-temporal regions, and other small bilateral genian lesions. There were no alterations in the flap. Left external campimetry was limited. It was decided to perform a facial bone and skull simple and contrasted tomography (CT) and laboratory tests.
Then, resection of the previously mentioned lesions and resection of the lower eyelid with transoperative study, shield-type incision and external canthoplasty of the lower left eyelid was planned. A plastic surgeon was included in the surgical team.

Altogether, 9 skin lesions located in the left superciliary, left frontal, interparietooccipital, posterior occipital, auricular, and left retroauricular regions were resected, which were positive for basal cell carcinoma.
In the subsequent control, the patient's campimetry showed improvement.

DISCUSSION
Reports about Gorlin-Goltz syndrome are scarce in the literature (17). The rarity and phenotypic variability of this syndrome causes a delay in its diagnosis. Syndromicassociated keratocystic odontogenic tumors are often treated in the same way as nonsyndromic cases (18) and associated systemic signs can easily be missed due to lack of understanding of the syndrome. In addition, their characteristics vary globally, so doctors and even dentists must identify them in a timely manner, considering those that are more prevalent in their population or similar populations (19,20).
The pathogenesis of basal cell carcinoma (BCC) is thought to involve increased sensitivity to ultraviolet light and to involve ineffective mechanisms that repair UVinduced DNA damage. In any case, this theory is not accepted by all authors since these lesions can also appear in areas that have not been exposed to sunlight. Especially in children, patients with G-G syndrome who undergo radiotherapy for other cancers have shown to be at increased risk of radiation-induced BCC (21).
In 50% of patients with G-G syndrome, jaw keratocysts appear, characterized by a thin surrounding layer of epithelial cells, which tend to reappear locally after excision in 6 to 60% of cases, therefore, the indication for surgery should be carefully considered, also due to the possibility of intensive clinical and instrumental monitoring (22).
In recent years, new drugs have been developed to inhibit certain components of the sonic hedgehog signaling pathway. In 2013, the FDA approved vismodegib, the first small molecule to target this pathway (11). Although these agents seem promising options for patients with G-G syndrome, their efficacy is limited by adverse effects and the development of resistance (23). Logically, a more aggressive approach is necessary if basal cell carcinoma is suspected; subsequently, depending on the lesion site and the surgery type, a reconstruction can be performed, as in the case presented in this article (24,25).
CONCLUSION
Gorlin-Goltz syndrome encompasses a variety of clinical signs and symptoms, including important oral manifestations and skin lesions that must be recognized to achieve early referral to a specialty, thus reducing the risk of malignancy through multidisciplinary treatment.
