"Doctors have begun trialling the world’s first mRNA lung cancer vaccine in patients, as experts hailed its “groundbreaking” potential to save thousands of lives.
Lung cancer is the world’s leading cause of cancer death, accounting for about 1.8m deaths every year. Survival rates in those with advanced forms of the disease, where tumours have spread, are particularly poor.
Now experts are testing a new jab that instructs the body to hunt down and kill cancer cells – then prevents them ever coming back. Known as BNT116 and made by BioNTech, the vaccine is designed to treat non-small cell lung cancer (NSCLC), the most common form of the disease.
The phase 1 clinical trial, the first human study of BNT116, has launched across 34 research sites in seven countries: the UK, US, Germany, Hungary, Poland, Spain and Turkey.
The UK has six sites, located in England and Wales, with the first UK patient to receive the vaccine having their initial dose on Tuesday [August 20, 2024].
Overall, about 130 patients – from early-stage before surgery or radiotherapy, to late-stage disease or recurrent cancer – will be enrolled to have the jab alongside immunotherapy. About 20 will be from the UK.
The jab uses messenger RNA (mRNA), similar to Covid-19 vaccines, and works by presenting the immune system with tumour markers from NSCLC to prime the body to fight cancer cells expressing these markers.
The aim is to strengthen a person’s immune response to cancer while leaving healthy cells untouched, unlike chemotherapy.
“We are now entering this very exciting new era of mRNA-based immunotherapy clinical trials to investigate the treatment of lung cancer,” said Prof Siow Ming Lee, a consultant medical oncologist at University College London hospitals NHS foundation trust (UCLH), which is leading the trial in the UK.
“It’s simple to deliver, and you can select specific antigens in the cancer cell, and then you target them. This technology is the next big phase of cancer treatment.”
Janusz Racz, 67, from London, was the first person to have the vaccine in the UK. He was diagnosed in May and soon after started chemotherapy and radiotherapy.
The scientist, who specialises in AI, said his profession inspired him to take part in the trial. “I am a scientist too, and I understand that the progress of science – especially in medicine – lies in people agreeing to be involved in such investigations,” he said...
“And also, I can be a part of the team that can provide proof of concept for this new methodology, and the faster it would be implemented across the world, more people will be saved.”
Racz received six consecutive injections five minutes apart over 30 minutes at the National Institute for Health Research UCLH Clinical Research Facility on Tuesday.
Each jab contained different RNA strands. He will get the vaccine every week for six consecutive weeks, and then every three weeks for 54 weeks.
Lee said: “We hope adding this additional treatment will stop the cancer coming back because a lot of time for lung cancer patients, even after surgery and radiation, it does come back.” ...
“We hope to go on to phase 2, phase 3, and then hope it becomes standard of care worldwide and saves lots of lung cancer patients.”
The Guardian revealed in May that thousands of patients in England were to be fast-tracked into groundbreaking trials of cancer vaccines in a revolutionary world-first NHS “matchmaking” scheme to save lives.
Under the scheme, patients who meet the eligibility criteria will gain access to clinical trials for the vaccines that experts say represent a new dawn in cancer treatment."

Overview
Tequila and Red Eye successfully dismantled a rogue military organisation engaged in illicit human trafficking and arms dealing, which had also planned to launch a global bioterrorist attack in collaboration with the Pipovulaj Army. The plot involved spreading a zombie plague to control the population, transforming numerous innocent civilians into violent Man Eaters as a means to create a twisted form of super-soldier. Impressed by Tequila and Red Eye's exceptional performance as highly capable spies, the Intelligence Agency and the Regular Army jointly established a covert operations branch, S.P.A.R.R.O.W.S., through a mutual agreement.
The S.P.A.R.R.O.W.S. is responsible for gathering intelligence and managing information to prevent public panic and global hysteria. They provide their members with specialised training in high-risk covert operations that surpass the scope of regular Intelligence Agency agents, which are all conducted with utmost discretion and situational awareness. Some of these special covert operation missions involve precision targeting of high-priority threats and strategic disruption of complex criminal schemes.
Insignia
It features a cerulean square Iberian shield, rimmed with a spiky teal vine that’s outlined in bronze. Above the shield, the words "S.P.A.R.R.O.W.S." are inscribed in bluish-white, surmounting a stylized pair of bronze eyes with a yellowish-white star at their centre. S.P.A.R.R.O.W.S. stands for Special Pursuit Agents and Rapid Response Operations Worldwide Strikeforce.
Members of the S.P.A.R.R.O.W.S. are commonly known as "Sparrowers" or "Following Falconers", reflecting their affiliation with the unit and their close relationship with the P.F. Squad.
Despite being part of an elite covert operations branch, Sparrowers face a significant pay disparity: males earn a quarter of the average government agent's salary, while females earn about a third. Additionally, underperforming Sparrowers, both male and female, experience further financial hardship due to delayed salary payments, often waiting between one to two months to receive their overdue compensation.
The S.P.A.R.R.O.W.S. conduct their covert operations in collaboration with the Peregrine Falcons Squad who provide primary firepower and protection for their agents.
The handguns carried by Sparrowers are the Murder Model-1915 .38 Mk.1Am or Classic Murder .38 for short. It’s a double-action revolver that features a 6-round cylinder. Originally designed to enhance the Enfield No.2 .38 Caliber revolver in 1915, the Murder Model retained only the frame and grip from the original. All other components were replaced with newer parts in later years.

so. things are still up in the air with my mom. none of it good. i currently have a little extra time on my hand. but im desperately chasing dopamine. so. im going to probably just??? do whatever drafts / asks strike me from whenever vs trying to get to my oldest first.
im also not claiming that anything will happen. sometimes it’ll just be me posting ooc all day. i am so fucking depressed and anxious i can barely function. so.
all my bad qualities!! about to be worse.

im really not okay and it’s nothing anyone can do because life is a fucking dick and i have to function the best i can.
im also in a lupus / eds flare and i feel fucking awful even aside from what’s going on with my mom.
long story short, her cancer has seemingly metastasized into her blood? so it isn’t a tumor, but just, cancer in her bloodstream. and her mobility and pain and mental health has been officially going down. she’s currently in a specialised health / rehab facility for likely about a week. and we won’t know what comes next for the cancer journey until the 11th.
this also means, what was supposedly nearing the end of the worst, instead heralds the beginning of the worse. i don’t even know if she wants continued treatment or if she’d rather let cancer take its course.
i keep having breakdowns and sobbing today. so. im gonna go read my silly novels and write my silly replies. and pretend that for tonight, im okay.

(World building for my living weapon OCs) CW: living wepaon, dehumanisation, warfare, tortour, extreme conditioning.
Before the outbreak of the global conflict, the Facility was a small-scale organisation known for its expertise in psychological research, interrogation techniques, and psychological warfare. Initially hired by governments for extreme situations, the Facility’s role evolved dramatically as the war escalated.
From a niche contractor, it transformed into a dominant global entity with an expansive network of semi-autonomous complexes.
Evolution of the Facility
With the onset of the global conflict, the Facility transitioned from a third-party service provider to a central force in global affairs. Its operations expanded from dealing with prisoners of war and training operatives in psychological warfare, to capturing and conditioning children - often starting before their 10th birthdays. These children are then molded into specialised roles for the war effort, including front-line combatants, medics, infiltration specialists, engineers, and assassins.
Each Facility complex had its own methods, but the core processes of indoctrination, training, and conditioning remained consistent across the network.
Complex 27: A Closer Look
One of the key facilities in this network is Complex 27, a sprawling, isolated complex designed for discretion and security. Located in a remote rural area, Complex 27 is characterized by its stark, utilitarian design, reflecting the Facility’s emphasis on control and uniformity.
A Block: Dedicated to severe corrections, reconditioning, and decommissioning of assets. It is a place of fear and pain, where assets face their harshest punishments and psychological manipulation.

Facilities: Functional and austere, with desks, computers, and communication systems for encrypted coordination.
Interactions: Handlers maintain detailed records, plan development, and manage deployments according to established protocols.
Communication: B Block serves as the central communication hub with encrypted channels to ensure security and prevent unauthorized access.
C Block:
Purpose: Training grounds for younger assets. It focuses on education, conditioning, and isolation from the outside world until assets are ready for their roles.
Characteristics: A place of intense indoctrination, psychological conditioning, and early skill development.
D Block:
Purpose: The residential area where assets and living weapons are housed.

Lighting: The lighting in the quarters is bright, designed to keep the space functional and to prevent any shadowy corners where assets might hide or escape. It is on an automatic timer across all quarters, dimming at night and glaring in the day.
Overall Design: The design is uniform across all quarters, with no personalization allowed. This uniformity reinforces the Facility’s control over the assets, eliminating any form of individuality or personal expression.
Training and Conditioning:
Stages of Training: Assets undergo a rigorous, multi-staged process involving indoctrination, specialization, handler training, and advanced training as living weapons. Each stage is designed to strip away personal identity and mold assets into highly effective tools for the Facility’s objectives.

Focus: This stage involves erasing the asset’s prior identity, using isolation and psychological conditioning to create a blank slate. At the end of this stage, assets receive a number designation, which is tattooed on their neck.

hey sweeties!
a new update on the life of Soetkin:
so a couple of weeks after my last post(s) i started a treatment program of three group therapy days a week. it was at a local general hospital, so no specialised program i suppose. while my expectations weren't very high and i saw it more as a tool to not sink deeper into depression and anxiety while i waited out till it would be my turn on the waiting list for the program i actually thought would help me, i quit this program halfway though my planned time there. i felt a lot worse during therapy as it progressed than i did at home. i looked so forward to it being over because i was extremely anxious and felt like i was killing myself on the inside when i was there. home was my safe space, the total opposite of how therapy felt for me. and believe me i know that it's kind of expected that you'll feel worse for a bit during intense therapy since you start opening up and stop running away from what you feel and think. so i quite suddenly quit.
(since this post ended up becoming what resembles a whole novel, i'm going to cut in here so people who don't care don't have to scroll through it ;) anyway it's not all bad if you want to read on, it actualy quite hopeful, the start just isn't very much so.)
i had a plan to pick up some creative activities at facilities for disabled folks, which i have got a government recognition for. i found a place i could start at fairly quickly, i haven't gone very often yet, but all in all i felt such intense relief when i didn't have to go to group therapy there anymore.
i'm still overly anxious and get very (extremely) easily overwhelmed by everything in this world: sounds, proximity, weather. but i am not as deeply depressed anymore. i'm actually feeling more like my regular self in that regard.
i can start the pre-program i've been waiting for since november in two weeks (and the full program one month later) and i'm so very hopeful for the future. i'm not where i used to be yet, but i'll get there, i'll get beyond there. I've already learned more about myself these past 8-9 months, things i never really considered or took seriously. while it still feels strange to say i most probably also have adhd (i got my autism diagnosis as a kid). a lot of the stuff i also struggle with seems to always have something to do with dopamine. i also probably have DCD (i still find it out they didn't look into this when i was in residential treatment when i got my autism diagnosis, but they still mentioned me randomly walking into doorframes and stuff instead of through the doorframes). i'm learning to give myself more time regarding my DCD when doing stuff that's hard(er) for me. even stuff like taking the laundry out of the machine etc. or walking up or down stairs. i'm starting to stop caring about other people being faster. the recognition of that DCD element for myself is a huge thing towards self-acceptance. i now also realise my brand of neurodivergence comes with rejection sensitive dysphoria, which explains SO FUCKING MUCH OMG.
this bout of urgent mental health issues also came with more physical issues than i've ever experienced before. which felt odd, and mostly unexpected. i've had more visits to a physiotherapist than the rest of my life combined these past few months. i also struggled with eating, but not ED wise this time, that part of me is actually doing quite well this time, which is also unexpected but yay. i mostly didn't have the energy to lift my fork and everything tasted meh. but that part is also a lot better now, still happens sometimes, but it's far better now. (i also lost a lot of hair because of it, which is growing back now, so i have plucks of toddler hair in my face all the time, and let me tell you toddler hair is a lot more annoying than baby hairs, they're too short to go anywhere, and too long to not be an annoyance). my last post also came as i was only just healed from what could've been covid, or something similar. but the worst infection i've had since 2020 (and i have had it several times). I always got booster shots (since i worked at a hospital) and never was very inconvenienced, let alone for over a week. but my symptoms now started on new year's day (yay me) and lasted for about a month and they were unpleasant and painful. i guess i partially ought to thank the booster shots in the past for not getting that ill, but i also think i kinda blew my immune system over by not eating properly.
anyway, so here we are. my husband who's a teacher in upper secondary school has started his summer holidays, he's actually had a rather nice last month of the school year since his pupils' finals were evenly spread out for him so he didn't have to rush correcting them.
we're also in a very good place now. i'm so happy and grateful this guy is my husband, that i can call him mine, that we're us. he's also started therapy for his OCD, and while he was terrified he's doing so well, i'm really impressed (a bit scared too though that he might be taking on a bit too much at once in therapy). we're building a sturdy base for our future together (with hopefully a kid at some point sooner rather than later). He's feeling more sad about having to postpone our starting a family plans due to all of this, but we also both see this as a huge opportunity to become even better parents. and in a way we're quite lucky this all happened before i got pregnant and not during or after. not to say it can't or won't happen again, but then we'll be even more prepared to take on this challenge again than we were / are now.
anyway: i still have a long and scary road ahead of me, but i'm not at the start anymore, i've already been hiking for a bit and i'm hopeful about seeing the sun rise beautifully overhead once more.
i also really really really want to reblog stuff for this blog again, and more frequently. i am however still struggling with energy, spoons are often very depleted. i even have a very hard time editing my own pictures and it's not like i've taken many these past 10 months. i've also had people i know irl reach out to me because i'm so absent on all social media and that's very unlike me. and them asking me about it, gives me the feeling that i actually do might belong here and with those people and that people actually care and that i'm missed. and that's a bewildering but amazingly heart warming feeling, that's kind of new to me.

After the events of the rooftop of St Bart’s (Reichenbach), the post-mortem was done, during the transit from the rooftop he had appeared deceased, this view changed rather radically when signs of brain activity were picked up using an electroencephalogram, the team had been all but too aware of his reputation and wanted to prove mainly to themselves that it wasn't some form of trickery. In an attempt to discover more, several brain scans were done yet all that appeared on the screen was a black spot instead of a clear visual of the bullet. The bullet was made of vibranium which for some reason would not show clearly on their scans, not that they know what the bullet was made of, it was something that James himself only knew, he had intended to make his death permanent. Any attempt at surgery was immediately out of the question, any attempt to remove the bullet would likely have catastrophic consequences. Under the orders of high-ranking government officials as well as in the knowledge of Mycroft Holmes, he was given to a research company that specialised in neuroscience, it was an attempt to save his brain in the idea that it would be useful later on. Not wanting to risk his seeming survival becoming public, his death certificate was signed and any evidence that suggested otherwise was either changed or destroyed.
The company gave him the best medical treatment they could in order to heal his body and mind. What no one realised was what was happening in Moriarty’s mind. The vibranium bullet in his mind had been reacting with the neurotransmitters in Broca’s area, this was where the bullet had been lodged. It had ultimately caused a chemical reaction between the neurological receptor and the metal connector, this created a connection between two receptors that previously weren't touching and that in the rest of the human population, would never be connected. As the reaction grew and the connection strengthened with time it unlocked a new frequency undertone. The first time he managed to form a clear demand he noticed that they were unable to resist his commands. Using them for his own gains he made them figure out how his new power worked. It was discovered that the frequency his voice emits had a unique way of resonating in the minds of others, almost like the effect a siren would have on prey, with their assistance they would learn the vibranium lodged in his brain was the source of these new abilities.
Recovering as fast as he could to get out of the horrid place he also tested this new ability. He would learn that his voice would have the same effect as it does in person if transmitted through a radio or any other audio-emitting device. The lasting effect on those exposed to his voice varies on how much mental resistance they have, for those who are easily manipulated it could take years for the effects to wear off but for those with a strong resolve, the effect could wear off within a few hours, James learned that for the most effective method of use, further exposure to his voice was necessary. Yet as powerful as this new ability was he wasn't invincible. The strain the brain injury put on him weakened his physical muscles, whilst he could still move lifting anything 40 lbs and more would be a struggle. This would give him a need to surround himself with others as bodyguards of sorts which also fed his growing paranoia, he developed a fear of those under his control rising up against him in this weakened state. Having already survived one bullet to the head anymore would be fatal as well as any severe head trauma. His power also had limits, it only worked if his voice was audible or if the sound could be transmitted, it also would not work on the deaf unless they had an implant.
With all of these things in mind, he broke out of the facility, ensuring all those who had worked there made it seem as if he had died at the facility, he also made them cover the whole thing up so even the highest officials believed it. Moriarty started to take more interest in those with powers as a meta-human himself, he found those easy to manipulate first and turned them into bodyguards before adding more. His goal was to form a new organisation, once he was set he turned recruitment to those who could boost the powers of others, offering services as he did before he gained his powers he would now aim more at those who would keep him safe as well spread fear of him, failure to comply with maintaining his secrecy would be met with the harshest of punishments.

New Scientist: Racial and ethnic disparities undermine dementia care in the US
In the US, people of colour receive worse treatment than white people at every stage of dementia care, from diagnosis to end-of-life treatment, according to an analysis of dozens of studies.
Dementia-causing conditions, such as Alzheimer’s disease, are one of the leading causes of death in the US, and older Black people in the US are twice as likely to be impacted as whites. As people in the US live longer, with the risk of dementia roughly doubling every five years they age, more people of colour than ever will be living with dementia and cognitive impairment.
“Access to good-quality care for people living with dementia is essential to achieve equity,” says Ladson Hinton at the University of California, Davis.
Hinton and his colleagues reviewed 71 studies published over the past two decades to gain a clearer picture of how racial and ethnic disparities impact dementia care. The team found key differences in diagnosis, medication use and end-of-life treatment.
Every study that analysed diagnosis showed lagging diagnosis times for Black and Hispanic people with dementia. A recent comprehensive study revealed that the waiting time was 11 per cent longer for Black people and 40 per cent longer for Hispanics compared with white people.
“If there is a delay in diagnosis, that means that there’s going to be a delay in treatment,” says Joyce Balls-Berry at Washington University in St. Louis, Missouri. “That impacts one’s ability to have a good quality of life.”
Studies also found lower prescription rates of anti-dementia medication for Black and Hispanic people. Black people in the US were also less likely to receive care at long-term facilities or hospices specialising in dementia treatment. Perhaps as a result of delayed treatment, many studies reported increased US hospital admission rates and longer stays for Black people with dementia.
“The higher rates of hospital admission and longer length of stay are really not desirable,” says Hinton. The analysis confirms the concerning ethnic and racial inequities in dementia care, but many questions are still unanswered, he adds.
The review found limited data on Asian Americans, Pacific Islanders and Native Americans with dementia, so it remains unclear how these groups are impacted. Hinton says future research in the field must include people of all racial and ethnic backgrounds.
The root cause of these inequities also remains unproven. Balls-Berry believes access to specialists, proximity to high-quality medical centres, lack of insurance, language barriers and cultural differences could all be contributors, but more research is needed to confirm these connections.
Ladson and Balls-Berry both hope this data spurs changes in clinical practice and policy to close the gaps in dementia care. “Some of these systems were not created for patients of colour,” says Balls-Berry.
