Rewatch Missing Scene: S01E02 Metal Saw - On the jet on the way back to LA, from Sarah's POV
~~
The private jet was nice, especially after having been held hostage and all. Sarah watched from the back of the plane as Jack spoke with his team. She'd seen Jack administer first aid before and been on the receiving end of it herself when they were partners, but she'd never seen him as tender as he was with Riley and Mac.
Riley protested that she hadn't been involved in the physical aspects of the mission, so she just sat back and smirked as Jack got handsy with Mac, trying to push his shirt and jacket aside to check for injuries. Mac smacked at Jack's hands but eventually gave in and let Jack probe his ribs and what looked like a recently healed gunshot wound in his chest.
After handing out Tylenol and water bottles, Jack dropped into the seat beside her and waved the first aid kit. "Let me clean up those scrapes, yeah," he said, and it wasn't a question.
Sarah leaned forward and gave him a soft smile, careful of her split lip. "I'm okay, Jack."
He shrugged and held up an alcohol wipe. "There's no reason to sit here and let infection fester."
She couldn't argue with that, so she gave him a 'get on with it' gesture and sat back to suffer through the smarting of the antiseptic.
Jack carefully cleaned the scrapes on her cheek and forehead, leaning in to blow on each one to ease the sting. He wiped the alcohol over her lip last, and Sarah pulled away before he could get within kissing range for his little blow maneuver. A hurt look flashed across his face, and Sarah opened her mouth to explain that she had a fiancé, but what came out was, "You're really good at this."
"The first aid or the mission or something else?"
"All of the above." She nodded toward Mac and Riley, who were playing a game of War while discussing statistical probabilities of card shuffling. "How long have you been partnered with them?"
Jack chuckled. "Well, Riley's only been on the team a couple of weeks. We pulled her out of supermax to help with a case and decided to keep her on at the Phoenix."
There was a much longer story there, but Sarah let it go for the moment. "And Mac?"
"Mac is a different story entirely. I was his Overwatch in Afghanistan in 2011. Kid is crazy smart, like genius-level smart. He went to MIT before joining up." Jack's eyes lit up like a proud papa. She half-expected him to pull up pictures on his phone of Mac's first mission. "He was the slowest bomb nerd in the whole damn place, but he was also the best. We've been partners ever since."
"I've never seen you like this before."
His eyebrows shot up in bemusement. "Like what?"
"Paternal."
Jack spluttered. "What are you talkin' about?"
"You and the kids." Her eyes flicked to them, and they were still engrossed in their game. "It's cute."
"Cute? What are you talking about? Cute, pfft. I should check you for a concussion. Are you seein' double?"
She laughed and pushed away the hands that were reaching for her head. "I'm fine, and I'm glad that you've found them. And I'm glad that you came for me. I can't thank you enough."
He held her gaze for a long moment before settling back in his seat. "No thanks necessary. I'll always come for you when you need me."
And she knew that he meant it. Jack Dalton was nothing if not a man of his word.

Occam’s Razor
CC: Patient presents with cough, rash, dyspnea, syncope, and abdominal pain.
Dx: Colchicine poisoning
Syncope
Temporary loss of consciousness caused by a fall in blood pressure.
aka fainting, passing out
Hypothyroidism
Reduction in the production of the two hormones produced by the thyroid, T3 (triiodothyronine) and T4 (thyroxine).
The two hormones regulate the body’s temperature, metabolism, and heart rate.
T3 is the active hormone, T4 is the precursor, both circulate through the bloodstream.
T3 functions include the 6 B’s:
Brain maturation
Bone growth
Beta-adrenergic effects
Basal metabolic rate
Blood sugar (glycogenolysis, gluconeogenesis)
Break down lipids (lipolysis)
Most commonly affects women above the age of 50.
Symptoms include:
Goiter: enlargement of the thyroid gland, it’s the characteristic neck mass shown in most tv shows, not to be confused with enlargement of salivary glands which show growth more along the jawline.
Metabolic: Cold intolerance, hypohidrosis (reduction in sweat production), weight gain, hyponatremia (reduced sodium in the bloodstream)
Skin/hair: Dry, cool skin (due to reduced blood flow), coarse, brittle hair, diffuse alopecia, brittle nails, puffy facies and generalized nonpitting edema (aka myxedema)
Ocular: Periorbital edema
Gastrointestinal: Constipation, reduced appetite
Musculoskeletal: Hypothyroid myopathy (proximal weakness), carpal tunnel syndrome
Reproductive: Abnormal uterine bleeding, reduced libido, infertility
Neuropsychiatric: Hypoactivity, lethargy, fatigue, weakness, central nervous system (CNS) depression, reduced reflexes
Cardiovascular: Bradycardia, dyspnea on exertion
Common underlying causes:
Iodine deficiency - because the hormones are basically composed of iodine with some extra spice.
Malfunctioning pituitary gland - because the pituitary gland signals the thyroid to produce hormones (through TSH-Thyroid stimulating hormone), classified as Secondary Hypothyroidism because the problem does not directly derive from the thyroid.
Trauma to the thyroid gland.
There are a lot of other etiologies/causes that I can’t go over one by one just by necessity because the list would add another 10 pages. But my favorite is definitely cretinism (congenital hypothyroidism) just because it was actually mentioned in Paternity (s01e02) as part of a crossword puzzle.
Colchicine poisoning
Pharmacy error in this case, a switch-up with the cough medicine.
Used to treat gout: a form of inflammatory arthritis, characterized by recurrent attacks of red, tender, hot and swollen joints. Due to persistently elevated levels of uric acid in the blood which form crystals within joints and kidneys.
Overdose begins with a gastrointestinal phase 10-24 hours after ingestion, followed by multiple organ dysfunction 24 hours to 7 days after ingestion.
Symptoms include:
Burning in the mouth and throat, fever, vomiting, and abdominal pain.
Hypovolemic shock due to extreme blood/fluid loss through the gastrointestinal tract.
Kidney failure follows
Alopecia: loss of hair, aka baldness
Bone marrow depression: decrease in production of cells responsible for providing immunity, carrying oxygen, and/or those responsible for normal blood clotting.
Agranulocytosis: lowered white blood cell count
Aplastic anemia: failure of production of blood cells
