generally, I don't think Wilson would cheat on House except for one situation:

after Wilson gets his diagnosis (in an established-Hilson universe), I imagine he'd have a crisis that would involve him cheating on House. Maybe it's because he needs a distraction, maybe it's because he knows House won't take the news well and part of him wants to sabotage their relationship so House is more okay with him dying (which wouldn't actually happen, but Wilson is freaking out and not thinking clearly)

House finds out about the cheating before he finds out about the diagnosis. And the entire time after that, every fight and argument, Wilson knows that if he told House why he did it, everything would change. But he doesn't want to guilt-trip House into not being mad at him for fucking up
