the human cost rolls downhill

I hate having to write this. I hate that this is a thing that happened to be written about.
In early 2024 a private virtual clinic providing medical care for a vulnerable and underserved patient demographic allegedly replaced 80% of its human staff with machine learning software.
As far as I can find this hasn't been reported on in the media so far and many of the details are currently not public record. I can't confirm how many staff were laid off, how many quit, how many remain, and how many of those are medics vs how many are admin. I can't confirm exact dates or software applications. This uncertainty about key details is why I'm not naming the clinic. I don't want to accidentally do a libel.
I'm not a journalist and ancestors willing researching this post is as close as I'll ever have to get. It's been extremely depressing. The patient testimonials are abundant and harrowing.
What I have been able to confirm is that the clinic has publicly announced they are "embracing AI," and their FAQs state that their "algorithms" assess patients' medical history, create personalised treatment plans, and make recommendations for therapies, tests, and medications. This made me scream out loud in horror.
Exploring the clinic's family of sites I found that they're using Zoho to manage appointment scheduling. I don't know what if any other applications they're using Zoho for, or whether they're using other software alongside it. Zoho provides office, collaboration, and customer relationship management products; things like scheduling, videocalls, document sharing, mail sorting, etc.
The clinic's recent Glassdoor reviews are appalling, and make reference to increased automation, layoffs, and hasty ai implementation.
The patient community have been reporting abnormally high rates of inadequate and inappropriate care since late February/early March, including:
Wrong or incomplete prescriptions
Inability to contact the clinic
Inability to cancel recurring payments
Appointments being cancelled
Staff simply failing to attend appointments
Delayed prescriptions
Wrong or incomplete treatment summaries
Unannounced dosage or medication changes
The clinic's FAQ suggests that this is a temporary disruption while the new automation workflows are implemented, and service should stabilise in a few months as the new workflows come online. Frankly I consider this an unacceptable attitude towards human lives and health. Existing stable workflows should not be abandoned until new ones are fully operational and stable. Ensuring consistent and appropriate care should be the highest priority at all times.
The push to introduce general-use machine learning into specialised areas of medicine is a deadly one. There are a small number of experimental machine learning models that may eventually have limited use in highly specific medical contexts, to my knowledge none are currently commercially available. No commercially available current generation general use machine learning model is suitable or safe for medical use, and it's almost certain none ever will be.
Machine learning simply doesn't have the capacity to parse the nuances of individual health needs. It doesn't have the capacity to understand anything, let alone the complexities of medical care. It amplifies bias and it "hallucinates" and current research indicates there's no way to avoid either. All it will take for patients to die is for a ML model to hallucinate an improper diagnosis or treatment that's rubber stamped by an overworked doctor.
Yet despite the fact that it is not and will never be fit for purpose, general use machine learning has been pushed fait accompli into the medical lives of real patients, in service to profit. Whether the clinic itself or the software developers or both, someone is profiting from this while already underserved and vulnerable patients are further neglected and endangered.
This is inevitable by design. Maximising profit necessitates inserting the product into as many use cases as possible irrespective of appropriateness. If not this underserved patient group, another underserved patient group would have been pressed, unconsenting, into unsupervised experiments in ML medicine--and may still. The fewer options and resources people have, the easier they are to coerce. You can do whatever you want to those who have no alternative but to endure it.
For profit to flow upwards, cost must flow downwards. This isn't an abstract numerical principle it's a deadly material fact. Human beings, not abstractions, bear the cost of the AI bubble. The more marginalised and exploited the human beings, the more of the cost they bear. Overexploited nations bear the burden of mining, manufacture, and pollution for the physical infrastructure to exist, overexploited workers bear the burden of making machine learning function at all (all of which I will write more about another day), and now patients who don't have the option to refuse it bear the burden of its overuse. There have been others. There will be more. If the profit isn't flowing to you, the cost is--or it will soon.
It doesn't have to be like this. It's like this because humans made it this way, we could change it. Indeed, we must if we are to survive.

Cornell researchers have released a new, open-source platform called Cascade that can run artificial intelligence (AI) models in a way that slashes expenses and energy costs while dramatically improving performance.

Cascade is designed for settings like smart traffic intersections, medical diagnostics, equipment servicing using augmented reality, digital agriculture, smart power grids and automatic product inspection during manufacturing – situations where AI models must react within a fraction of a second. It is already in use by College of Veterinary Medicine researchers monitoring cows for risk of mastitis.
With the rise of AI, many companies are eager to leverage new capabilities but worried about the associated computing costs and the risks of sharing private data with AI companies or sending sensitive information into the cloud – far-off servers accessed through the internet.
Also, today’s AI models are slow, limiting their use in settings where data must be transferred back and forth or the model is controlling an automated system.
A team led by Ken Birman, professor of computer science in the Cornell Ann S. Bowers College of Computing and Information Science, combined several innovations to address these concerns.
Birman partnered with Weijia Song, a senior research associate, to develop an edge computing system they named Cascade. Edge computing is an approach that places the computation and data storage closer to the sources of data, protecting sensitive information. Song’s “zero copy” edge computing design minimizes data movement.
The AI models don’t have to wait to fetch data when reacting to an event, which enables faster responses, the researchers said.
“Cascade enables users to put machine learning and data fusion really close to the edge of the internet, so artificially intelligent actions can occur instantly,” Birman said. “This contrasts with standard cloud computing approaches, where the frequent movement of data from machine to machine forces those same AIs to wait, resulting in long delays perceptible to the user.”
Cascade is giving impressive results, with most programs running two to 10 times faster than cloud-based applications, and some computer vision tasks speeding up by factors of 20 or more. Larger AI models see the most benefit.
Moreover, the approach is easy to use: “Cascade often requires no changes at all to the AI software,” Birman said.
Alicia Yang, a doctoral student in the field of computer science, was one of several student researchers in the effort. She developed Navigator, a memory manager and task scheduler for AI workflows that further boosts performance.
“Navigator really pays off when a number of applications need to share expensive hardware,” Yang said. “Compared to cloud-based approaches, Navigator accomplishes the same work in less time and uses the hardware far more efficiently.”
In CVM, Parminder Basran, associate research professor of medical oncology in the Department of Clinical Sciences, and Matthias Wieland, Ph.D. ’21, assistant professor in the Department of Population Medicine and Diagnostic Sciences, are using Cascade to monitor dairy cows for signs of increased mastitis – a common infection in the mammary gland that reduces milk production.
By imaging the udders of thousands of cows during each milking session and comparing the new photos to those from past milkings, an AI model running on Cascade identifies dry skin, open lesions, rough teat ends and other changes that may signal disease. If early symptoms are detected, cows could be subjected to a medicinal rinse at the milking station to potentially head off a full-blown infection.
Thiago Garrett, a visiting researcher from the University of Oslo, used Cascade to build a prototype “smart traffic intersection.”
His solution tracks crowded settings packed with people, cars, bicycles and other objects, anticipates possible collisions and warns of risks – within milliseconds after images are captured. When he ran the same AI model on a cloud computing infrastructure, it took seconds to sense possible accidents, far too late to sound a warning.
With the new open-source release, Birman’s group hopes other researchers will explore possible uses for Cascade, making AI applications more widely accessible.
“Our goal is to see it used,” Birman said. “Our Cornell effort is supported by the government and many companies. This open-source release will allow the public to benefit from what we created.”

Bastien is a human who has a day job fixing rich people's cars which pays for the side job of fixing androids who can't afford to turn to official systems for help. He is highly intelligent, resilient and private person with a dry sense of humor. His clinic is in a run down part of the city away from prying eyes and entirely disconnected from the upper class world he occupies with his boyfriend, Matteo. He is an only child and is estranged from his family.
He has always been deeply and maybe naively moved to help people, wherever they come from, which is why he initially studied to become a surgeon. Circumstances out of their control caused these ambitions to be taken away from him - an altercation ended with a bullet in his shoulder, forever damaging nerves in his dominant arm. The wound put him out of commission physically and mentally for a while and he never officially graduated or got the prestigious position he was set up for.
Matteo having had a crush on them since their days in university had his rich family secure a job for Bastien fixing cars for the wealthy friends of the family. This at least he could do even with the lack of fine motorics in his arm after the injury. They have been in a relationship and living together in Matteo's apartment since.
Around this time he first happened to meet Wolfgang, half dead in a ditch, and got an ugly cut on his face for his attempt to help them. That meeting inspired him to eventually open the clinic and study up what he could find on android biology. He finds an assistant in Élise, who's more confident in the software side of things. Years later, when they meet for the second time, they strike a deal so that Wolfgang occasionally procures parts and resources for the clinic and in turn its doors are open to them at all times.
Bastien is someone stuck in a life that should be good enough but isn't and he is stuck lying to himself because he feels a tremendous debt to Matteo. He feels he owes him loyalty even if he doesn't feel anything for him and he feels 'kept' rather than loved or understood by Matteo. He's resigned to a sense of unhappiness with his life. This has left him emotionally stunted and guarded, and unable to make any decision that would set him free. He doesn't feel like he deserves to have what he wants or what would make them truly happy.
The clinic and namely Wolfgang's repeat visits is the fire that he keeps returning to, despite the stress and danger from potential legal ramifications. He doesn't know the details, but on some level he is aware of what Wolfgang does and it doesn't really bother him. Their professional relationship grows into a friendship grows into complicated feelings and deep desire. Even at this point Bastien can't help being a little selfish and cruel - the right thing would be to tell Wolfgang 'no' once and for all and end it - but he keeps letting them further in.
