This is a great question.

Tbh, the answer at the moment is 'he will one day, but I don't know if it will happen in the duration of the story.'

Originally it was going to be something I tried to sort out around the same time as Efnisien having the surgery to deal with Crielle's incompetence, but I realised that in terms of specifics (recovery time etc. and Efnisien being really unable to walk comfortably, etc.) it would kind of hamper the 'Efnisien is supposed to wake up from this surgery feeling better than he normally would from Crielle's surgeries' storyline that felt very crucial to him having some realisations about Hillview.

I also think it's the kind of surgery that Temsen wouldn't feel comfortable doing, because it's different to say, removing some stitches, or a microchip, and I feel like he'd outsource it to an external hospital, or alternatively, hire a surgery and a full team of people and a surgeon who specialises in these kinds of prosthetics.

So initially it was going to be something I dealt with fairly early, and then upon further research I was like 'I...think this needs to wait.' The intention was always there to remove them, so it will happen one day, but it might be too disruptive to the plot to have it happen within the timeline. The major events of Underline the Black actually happen fairly close together, due to the nature of Crielle in general, lol. Efnisien wants them removed, and Gary and Temsen will work hard to make that happen, so eventually what's more likely is that this will come up in the story, and then Gary will talk about arranging it, and Efnisien's agoraphobia and difficulties with like... medical trauma, will mean it needs to get put off for a little while.

So yeah! I hope that helps! I don't plan for them staying forever, but I also don't think I can get them removed during the course of the story with Efnisien's current psychological state. The surgeries he had re: lare glands and neck were medical emergencies, but the testicle implants aren't, so that has to be a choice, and I'm not sure Efnisien can make the choice for himself right now with the other complicating factors.

But he will one day make that choice, and he will be fully supported in it, when he does!

gray's anatomy s3 trans episode. impressively bad. i know they always have to do the morality play thing and make one character arbitrarily decide they disagree with the other's decision, but jeez, it's hard in this one. idk if i'll ever be able to wrap my head around the 'trans widow' thing, much less sympathize with it. i mean, when your partner is making a major and permanent change in their life, if you're not okay with it you don't stay with them and rally against it and make both of you miserable, you cut your losses and move on. i know it happens but it is so frustrating to see it on a tv show played as if the cis wife here is totally in the right for making her trans wife fucking miserable at her own expense. like you don't own her, she doesn't owe you her misery in order for you to keep having your male partner. also the medical information in this one is straight up wrong lol yeah if you have breast cancer feminizing HRT can be too dangerous to be recommended but you don't have to detransition entirely you can still get various kinds of SRS and it'd obviously make this woman happier than detransitioning. stopping HRT doesn't make your breasts 'deflate' although it can slightly change the tissue composition over time. and estrogen therapy increases your risk of breast cancer but it doesn't GIVE you cancer automatically, i don't like how they framed that considering how common the HRT=immediate turbo cancer myth is. they are really just very flippant with the patient in this episode frankly if this was me i'd kill myself in front of sloane.

dunno how bad this is for the time - it's obviously making a mockery of the concept of SRS and the misgendering is constant but it at least doesn't frame the trans woman in the episode as inherently hideous or worthy of mockery imo. but all in all, spectacularly hard to watch. maybe not quite as painful as the insanely hypocritical skinny girl gastric bypass episode in that the dumbass message the show is trying to push is at least outright.
