The psychosis warning is for multiple Cantos and/or side stories, but one of the Cantos needing that warning is Meursault's.

I was looking at the background art for volume 11 again bc like. The room is the eyes to the soul and all that. Haha definitely not back on my hyperfixed game.
Anyway, I'm overanalyzing mallek's background art benath the cut.

GOD I always forget how much of a slob he is. Mallek self shippers are so strong. From left to right:
The SEVERAL husk towers, now being either discarded or repurposed. So Mallek does buy / reroute new tech pretty frequently. Note how his couch is falling apart: he does not care for the finer things regarding comfort, only his hobby.

His limo! It's important to note that this limo background gets reused in Marvus's route, but Mallek's came first.
Note the camera above the TV and gamer controller (which is so hilarious: a joystick and one button. It's 100% a minecraft controller then). Could this imply that Mallek is a streamer?? He's always come across as rather private to me (side note: steaming in his coparatively tidy limo really hits my funny bone). Comprimise: Mallek is a streamer with one of those AI sprites instead of his face, ykwim?? Idk what they're called.
Mallek either canonically drinks alcohol, or the drinks just come with the limo.
He's drinking that same juice that Xefros does. For the life of me, I can't remember what it is/does, but iirc isn't it supposed to be good for psychic ability? Mallek is implied not to be psychic, however. Maybe it's just troll monster. Idr....
LMFAO i noted this when I deep dived into Marvus's limo, but I love the chair by the mini fridge having TWO seatbelts and what appears to be a booster seat?? Manlet Marvus move over, it's Manlet Malleks turn lmao.

I noted this detail a few years ago, but it's my favorite: Mallek keeps houseplants (possibly implied to be recently watered?).
Mallek keeps fruit in his hive, but it looks like he doesn't actually eat it. I wonder if it's wax fruit.
I'm sorry but looking at this image, I can not imagine Mallek actually cooks. He eats cereal and gets delivery and he's gotten grub sauce EVERYWHERE. There is no stove in his kitchen. Hell, no microwave either.

I think I woke up a couple of times and kept losing more of the dream, 1) I dreamt in some library/gala event nathan lane was there, me and sister found a packet of things related to wise guys, nathan lane wanted pictures of two things: 1) [???] (I forgot), 2) a purple tie with irises by van gogh (just looked it up, real painting). Dream me was taking the picture and was like ‘so do you want me to airdrop them or do you want. me to email them to you [trying So Hard to be casual]’ & nathan lane told me to email him and it was the email he used for medical stuff (???) and I kept having to ask him to repeat it because the email address was like. ‘w[???] the indomitable [???] for whom it is a miracle when he stands’ and in the end I asked him to type it out for me and I was so sure he was annoyed by that & dream me was like (internally) NOOOO I’M SORRY MR LANE I’M TRYING I REALLY AM… there was also rare footage of victor garber as willie doing a bit with some guy (real estate guy? government guy? some serious business guy) as some extra vaudeville in the show that got cut
I think I woke up here and was trying to remember the email address but went back to sleep
the second dream was I was at some charity auction at a theater with someone (idr who) and it was happening onstage and when I was trying to find my seat, which I had thought was with my companion, the number which was supposed to be really close to hers ended up really far away, and then a flock of old ladies enlisted me to swap my seat next to a different old lady who didn’t have her grandchildren to keep her company so I did, and when I sat next to her she didn’t respond at all, and then her grandson who was like. either a teenage or adult guy who just sat on me like I was part of the chair and he just started talking to his grandma while I was slapping his side to try to get him to stand up and in the end I just shoved him off. I left with the excuse that I was going to try and find out where people were getting bananas (I wanted to look at the auction but I didn’t know if I was actually allowed, and one of the old ladies was like ‘where are people getting these bananas). I think it semi-morphed into my undergrad because a big thing was there was always a bowl of fruit available. I found a bowl of shitty bananas and didn’t want to go back with those. Around the stage I found better ones, and then I decided not to go onstage to look at what was there because as long as I didn’t know then I wouldn’t want it (uuuhhhhh) (ok.) I took three bananas, went back to the seats, and gave one to the grandson and grandma.
this might’ve been where I woke up again and got even more fuzzy on nathan lane’s email
the third dream was willie from stardew (or a lookalike?) (rather than stardew it could’ve easily been influenced by chip’s nautical guys) had a brother that was near comatose and I was on the dock and going like. so. I don’t want to overstep. but are you playing something for him? like dramas of music, it really sucks to sit around with nothing if he has any presence of mind. and not-willie harrumphed a little (MAYBE CAPTAIN HADDOCK??) but said his brother can see the sky, the shifting clouds, and it was scientifically proven that something something something. and I told him, ok, that’s good. this one’s easy, I was worried about grandpa at the end of his life with everyone more focused on his physical care (understandable) to the point that they might forget to add something enjoyable to his unpleasant experiences.
anyway I’m awake for real. I can’t believe I had three celebrity dreams in a row, what the hell is my brain trying to process. can it be done already

Thinking back to when I was like 11 (or 12 idr) when I had an actually fine and good set of oil pastels which had beautiful colors and blends very well, but I had like not even 10% art experience at the time so it was always misused and that will never fail to irritate me. Like If I had them now instead, I could have drawn things like fruits, desserts, other food (Cut me some slack, I’m hungry), or some cute animals or random inanimate objects and it would have been a great learning tool for traditional art.

tell me abt everything abt creole culture you possibly can but separate it under subheadings for adhd ease <3 i especially wanna know abt food/clothing/wedding traditions/religious traditions and festivals/superstitions!

overview
generally, creole culture here in st lucia (and dominica) is influenced by french and english culture, as well as west african culture (especially yoruba iirc) and the culture of the indiginous people of the caribbean (like the taino and kalinago). our culture is also influenced by the environment we live in, which is to say a tropical island, which in turn influences the resources we have available, like materials and foods, and what we can do with it. our culture is heavily influenced by slavery, of course, so a lot of our culture is things that can be made cheaply.
food
the main course has a few repeated motifs, such as using cured meats, like saltfish, smoked herring (my favourite) or salted pork. we also use ground provisions, which is a cultural term for starchy foods that often (but dont always) come from the ground (id describe it as a whole class of foods other than a potato that can do exactly what a potato does). my favourite ground provision is either breadfruit or sweet potato. here in st lucia our national dish is green fig (fig means banana in creole) and saltfish. we also use a lot of cheap cuts of meat, like pig trotters (which are used in a stew called souse which i love) and pig tail and chicken backs because historically these discards were the only thing we could afford to eat. another common motif is seafood, because a lot of our communities are fishing villages, and here in st lucia in anse la raye theres like a cookout called fish friday thats a huge tourist event. our food is very rich and flavourful, we have boullion here but i promise its not like what the french call boullion, its a lot more. More. like a bouillon is never a starter, its the meal.
as for bread, we have regular bread thats like, a different tougher texture from normal bread that we call creole bread, and we have a bread called bakes which you cook by either frying or roasting it (yeah the name is ironic lol) and a slight variation on that that uses yeast instead of baking powder called floats. i really want to experiment with using sourdough to make floats lmao. we also have rough dumplings we put into our boullion but theyre not like stuffed or anything its just lumps of dough boiled in soup. also cassava bread!! the process of making it is very slow because cassava is poisonous and you have to process it a lot to make it not poisonous but cassava bread is so good i love it.
for sweets and desserts, you often see a lot of coconut, from coconut cake, to coconut balls, to coconut tablet, to coconut in paime (which is a boiled corn pudding) (that last one is optional, some people use pumpkin and some people use grated green fig). you also see fudge, which is different from what americans call fudge, it has a snap to it but its crumbly and its caramelly and i love it. theres also tamarind balls which are little balls of tamarind mixed up with sugar, and guava cheese, which idk how to explain. its a sticky delicious mess. in latam they have it too but idr what they call it.
as for drinks, we have one called sorrel thats traditionally drunk at christmas, i think stateside you call it hibiscus tea, its bright red and has spices in it and i love it a lot, and we have a lot of juices made from fruits here like golden apple and green mango and lime and sour orange and grapefruit and my FAVOURITE drink which is cocoa tea which is made from cocoa solids being boiled in water and milk with spices and some people put dumplings in it. its traditionally drunk at breakfast and its really nice and warm and i would make it every morning if it wasnt so involved. this is a cocoa stick you grate it up and boil it with spices and milk to make cocoa tea.

in the crafts section i will talk about the coal pot, which is what these things are traditionally cooked in. i want to learn how to use a coal pot. its easy i just never did it.

so we actually have a national outfit! here is a little infographic about it

one of the most important features of our national dress is the fabric, which i think you may be familiar with jace! its called madras, and its named after the former name of chennai in south india.

heres an example of the kind of fabric we'd consider madras in the creole sense! this material is visual short hand for creole here in st lucia
music and dance
we have a lot of folk music which features violins, chakchak (like a maraca but its like one long thing) and drums, in lieu of explanation ill just share a short video on the music and the quadrille dance.

craft
okay so there are a lot of hand crafts that we do here, for starters we have traditional creole brooms made from palm leaves, we have bowls made from calabash (which is our national fruit but is not edible this is all we use it for) we have coal pots, which are made of clay that we use to cook, we have these reversible doll things. ive made the small version of the brooms before its fun.

festivals
well first of all we have jounen kwéyòl! its a celebration of creole heritage that we have on the last sunday of october, we usually celebrate with foods like a creole breakfast but we use the whole month to celebrate creole culture every year. we also have carnival, which is a big parade thing we have dancing in the street in big fancey costumes. here in st lucia we have la woz and la maguerite, or the flower festivals, which are based on these like, elite societies we used to have, where communities pick someone to be each member of the court, the king and queen and prince and princess and on and on all the way down to like doctors and nurses and go on parade in either red/pink for la rose or purple/blue for la maguerite. we ALSO have masquerade, which is a masked dancer thing we do around christmas. there are a bunch of different roles in masquerade, here is a picture of the regular masqueraders

theres also roles like papa djab (father devil) and the ti djabs (little devils) and pay bannann (dry plantain, hes covered in dry banana/plantain leaves) and the acrobat (an acrobat) and marie anset (a pregnant woman) and chouval bwa (a man on a horse) uncle sam on stilts because we hate america so hes also there. masquerade is so fun.

Have you ever tried making something from one of those short cooking videos? How did it turn out? No, but my friend once tried making that "muffin in a cup" thing and it was absolutely horrid. Very eggy. When was the last time you had to wait in line for longer than a few minutes? Good question. Going into places is so rare nowadays, so it's hard to encounter this issue. Well wait, maybe at a fast food place? Idr. Have you ever written a review for a product you bought online? No, but I should do that once I actually use my own money to buy from small businesses in particular... if I had something good to say, anyway. I'd feel bad trash-talking a product. My PHP therapist felt confident enough in my progress to step me down to three sessions a week versus every weekday. Last awkward situation you were in? Thankfully it was a one-on-one session, but the same aforementioned therapist was asking me questions that really stumped me, so I kept going very quiet as I thought and felt awkward and dumb. 📍✈️Traveling to Bali: Part 3 - Seminyak, Bali (Day 6 & 7)

Hey there! 🤗 This will be the last part of my Bali experience last November 2018.
Days 6 to 7 was spent in Seminyak, Bali, Indonesia. We decided to shorten our stay here last November 2018 since Seminyak is near the beach and there were Tsunami and Earthquake news that we heard during that time.
Two whole days in Seminyak were mostly spent on eating, eating and eating! 😅 This travel vlog will probably will look more like a food vlog so, enjoy!
DAY 6 November 26, 2018

Here’s the menu of Sisterfields in Seminyak. Mainly, they dishes for breakfast.

1st breakfast at Seminyak, I ordered Sisterfields Big Breakfast and a Strawberry Milkshake.
Sisterfields Big Breakfast costs IDR 130,000 (PHP 481). As per their description, it has eggs your way, bacon, mushroom, roasted tomato, potato rosti, hollandaise, sourdough and the Strawberry Milkshake costs IDR 40,000 (PHP 148).
It is a little pricey, however, serving was big as if it is for 2 people. I was just trying to finish it all since I was full before I could finish my meal.

A little toilet break and their restrooms were actually pretty. This was shot inside their toilet.
After our breakfast, we decided to visit Love Anchor in Canggu, Bali.
LOVE ANCHOR

We found out about Love Anchor through Laureen Uy’s Bali vlog and learned that its a place where you can shop.
Going to Love Anchor, we took a cab from Seminyak to Canggu. If I remember it right, it was around IDR 100, 000 - 150,000 (PHP 370 - 555) for the taxi fare. The cab we rode was good for 6 persons. It took us about an hour or more for us to arrive at Love Anchor since it was a little traffic during that time.

There were cafes and restaurants in Love Anchor but we didn’t try them since we were still very full from our breakfast.
At the end of Love Anchor, was a bazaar. They were selling bags, clothes, accessories, and more. It was a little more expensive compared in Ubud. So if ever you would want to shop, I suggest to shop in Ubud Art Market instead.
Afterwards, we walked around the area and saw Mad Pops.

If I remember it correctly, I ordered Mint Chocolate Chip and Rainbow Coconut. 🍨 It costs IDR 45,000 (PHP 166.50) for two flavor.
After spending some hours around Canggu, we decided to go back to Seminyak and have our late lunch at around 4 in the afternoon.

Decided to eat at Batik for its interiors. We saw a couple of amazing reviews about its food and decors that made us decide to eat here.

The staff are very nice at Batik. They even suggested us to go up, look at their 2nd floor, and we could even take some photos there.

I want to have a taste of all Bali food and tried their Nasi Campur. One Nasi Campur costs IDR 90,000 (PHP 333). A meal includes a small sample Indonesian dishes. As I remember, it has chicken satay some chips, vegetables, and a dish that tastes like Caldereta.
Not in the photo, but I also ordered a refreshing Strawberry Pleasure that costs IDR 45,000 (PHP 166.50). The juice includes strawberry, orange, and mint.
After having our late lunch, we decided to go back to Casa Kayu Aya to have a rest and freshen up for Motel Mexicola.
MOTEL MEXICOLA

Motel Mexicola was a 10 to 15-minute walk from our hotel. It was a must to have a reservation here to have seats instantly and to avoid waiting in line.

It is an open place however, there were fans and air conditions around the place.

My opinion about Motel Mexicola is that there were no blogs or vlogs that mentioned that it is not a good place for asthmatics like me. 😅 The smell of smoke was all over the place (even in the toilets!), and it was overly crowded like the people they let in was not controlled.
The alcoholic drinks and food was so-so, and quite overly priced for its quality. There’s not even a hint of alcohol for their alcoholic drinks. We spent a total of IDR 2,047,980 (PHP 7,577.526) including tax and service charge for our 2-hr stay in Motel Mexicola. The following items were our orders:
9 cocktails - IDR130,000/ea (PHP 481/ea)
2 Calamares - IDR 85,000/ea (PHP 314.50/ea)
2 Guacamole - IDR 50,000/ea (PHP 185/ea)
2 Que Conchinita - IDR 85,000/ea (PHP 314.50/ea)
2 Que Queso - IDR 65,000/ea (PHP 240.50/ea)
Service was also quite poor since the same waiter keeps asking us to order more drinks. We know it’s business however, I think it was rude to ask every time the waiter passes by our table and sees we almost finish our drinks to ask if we would want to order. Also, the approach of asking if we would want more was not kindly done.
Personally, I think I won’t come back to Motel Mexicola again during nighttime.
DAY 7 November 27, 2018
REVOLVER ESPRESSO

Breakfast on our last day in Bali was spent in The Revolver. I wasn’t able to take a lot of photos of the place since we were hungry and it was a little dark in our area. You can have sneak peek on the place on my vlog at the end of the post.

For my morning drink, instead of the regular coffee or hot chococolate, I got to try their Coolers since it was a sunny day. I ordered Poison Ivy that has organic roselle, dried hibiscus, lime, bitters, and soda in it. One drink costs IDR 45,000 (PHP 166.50)

Breakfast meal for me was The Mother Clucker on the left side most of the photo above. A order consists of a southern fried chicken burger, smoked cheddar, pickles, lettuce slaw, jalapeno & buttermilk dressing, and a black bun. One order consists of IDR 95, 000 (PHP 351.50).
I wasn’t able to get all of my other 4 friends’ order since it was a bit difficult to take photos. However, I got to take a photo of one of my friends’ meal that morning which was Underbelly. The Underbelly consisted of glazed thick cut bacon, fried eggs, chorizo sausage, house baked beans, rosti, mushroom duxelle, sourdough that costs IDR 120, 000 (PHP 444).
We ordered an extra dish to be shared by all which was Ammo. Ammo consisted of Tiramisu hotcakes, white chocolate mascarone, coffee ice cream, coffee jelly, with chocolate powder for IDR 95, 000 (PHP 351.500).
Overall, The Revolver Espresso was a good experience to dine in. Staff were very accommodating and food was very delicious. I highly suggest to try their Poison Ivy drink and Ammo pancakes, it will be worth it!
W BALI: WOOBAR

Woobar is located inside the W Bali hotel. We had our reservation for the daybed, however, we were transferred first inside since it was raining that time.

My friend and I ordered one from their promos which ‘Pick The Best.’ It was a promo staring from 11am until 10pm. A ordered included one pizza or burger, one hot or cold tapas selection, and unlimited softdrinks (which could be interchange with other soda options/brands). One promo costs IDR 265, 000 (PHP 980.50).
We chose to order Sweet & Spicy Chicken Wings (sweet soy glaze, sesame seeds, coriander, lime), and a Pepperoni Pizza. Our drinks were either 7-Up or Pepsi Blue.

We waited for time to be 4 in the afternoon since the staff mentioned a promo of a two drinks for the price of one on their signature cocktails from Monday - Thursday from 4PM - 6PM. Each order costs IDR 165, 000  (PHP 610). The following were the different cocktails to choose from:
Passion Fruit Mojito  - Old school favourite: The Mojito. Try our fruity twist With passion fruit. Together with mint and fresh lime wedges and finished with a little bit honey and Touch of soda. Enjoy.         
Pineapple Vanilla Rum Sour - Refreshing fruit cocktail with perfect blend or fresh pineapple, rum, sweet sour, vanilla syrup.         
Mango Tango - Tropical fruit cocktail with sour creamy taste flavoured of mango, coconut and lime.         
Bubble Gum Sour  - This drink will bring you back to your childhood memories. We made our own bubble gum vodka and combined this with bubble gum syrup. Sweet bubble gum flavours with some sourness of lemon.         
Smoky Margarita  - A twist on classic margarita with hint of smoke.        
Coco Old Fashioned  - The classic old fashioned cocktail, smooth flavour of whiskey combined with coconut and cherry syrup. 
All of 5 out of 6, ordered the promo, which was a total of 10 drinks. I ordered the Passion Fruit Mojito and Mango Tango, which were both flavorful. I was also able to taste Smoky Margarita which was not in my interest because it tastes like as if I drank a glassful of cigarette. It was also the least favorite within us. Other drinks such as Pineapple Vanilla Rum Sour and Coco Old Fashioned were also good as well.
My other friend ordered the Pink Raspberry Coconut Slushy that was a frozen pink slush puppy cocktail that is refreshing ice cold and fruity flavours of raspberry combined with coconut and light rum. One drink costs 145000  (PHP536.50).

Rain stopped and finally we were able to transfer to the daybeds! Perfect weather for a sunset view by the beach.

We had additional orders of French Fries Duo that has 2 flavors which were Truffle - garlic aioli and Sea salt - Ketchup. One ordered costs IDR 115, 000 (PHP 425.50). Classic Margarita  Pizza was also ordered that has fresh tomatoes, mozzarella, parmesan, and oregano rich tomato sauce. An ord.ered costs IDR 155, 000 (PHP 573.50)

Our W Bali: Woobar stay had a total bill of IDR 1,820,205.405 (PHP 6734.76) including tax and service charge. We ordered the following items for its price:

Promo of Chicken Wings + Pizza + Unlimited Soda
1 Pizza
1 2-flavored fries
Food wise, every food we ordered were delicious and very appetizing. In terms of their cocktails, it was actually way better than in Motel Mexicola. Woobar’s cocktails has more alcohol taste compared to Motel Mexicola’s. The only cocktail we didn’t like was the Smoky Margarita.
Service at Woobar was excellent! ❤️ All of the staff were very friendly and accommodating to our needs. Upon giving us the menu, first thing they said was we didn't need to hurry up and just to take our time in ordering our food/drinks. A staff actually passed by our daybed to ask if we need anything and if we were enjoying our stay. He even had a little chat to know us a little like the country we came from, and if we were still students or working girls.
If I’ll give a rating for my whole experience at Woobar from 1 to 10, 10 being the highest, I’ll rate it at 100! 😂 My friends and I even want to go back to Seminyak just to experience and stay at their hotel.
Woobar was the place where we felt that every money you spent, were so worth it for its quality and quantity. ❤️ ❤️ ❤️ To the Woobar staff last November 27, 2018 around 2 - 6pm, thank you for your excellent service! ❤️ ❤️ ❤️

Do you believe in dream symbolism? If so, do you ever look up what certain things in your dreams may mean? Some, yes, others, no. A lot just seem way too random to have stories behind them, but just as well, some dreams are obviously stemmed from our experiences, fears, desires, etc. Who was the last person who exploded on you? What was the reason? I have no idea, but probably Mom. Is there a certain name that keeps popping up in your life, like a name that many people you know have? No. What is something you have a "love/hate relationship" with? What do you hate about it, and what do you love about it? The first thing that comes to mind is technology. I grew up way too dependent on in despite my mom being a good mother that really tried to limit my time of it. It just never worked. Almost everything I do involves a computer. Through the Internet though, I’ve made spectacular friends, discovered great things (coughcoughmark), it helps me through depression and stuff… but I nevertheless hate how attached I am to it. Sometimes I feel like I’m so disconnected from the “real” world, “real” experiences, but then again, I also believe tech is a part of the “real” world. Idk how to really explain it; I have such mixed feelings. Do you like things that are cliché or not? What kinds of things that are regarded as "cliché" do you like? Sure, especially romantic cliches. Sickened By Billing Abuses, Readers And Tweeters Stand Up For Patients’ Rights

Private Equity Predators?
First of all, let me thank you for writing about such an important issue (“Investors’ Deep-Pocket Push To Defend Surprise Medical Bills,” Sept. 11). I am going to tell you about my personal pain regarding surprising medical billing.
They can call themselves Doctor Patient Unity group or any other fancy name, but the reality is that they are all predators. I say this because I am the victim of these doctors groups. I work as a linguist with the U.S. Army and have top-secret clearance. I could lose my clearance if any of my bills go to a collections agency.
I had appendix surgery on Nov. 6, 2018, at the Davis Medical Center in Layton, Utah. The surgeon had told me that my surgery would take only 10 to 15 minutes. After two hours of surgery, I was released from the hospital and sent home. My insurer, CIGNA, paid all the Davis hospital bills and I paid my portion of the bills. Then in March 2019, I received two separate doctor’s bills totaling about $48,000. My insurance was not paying the doctor’s bills because they were out-of-network. The doctor’s office kept sending me letters to pay $48,000 from my pocket; if I did not pay, I was told, my bills would be sent to collections.
Finally, I received a letter from the collections agency saying that if I did not pay $26,770, it would send the information to the credit agencies. Can the Doctor Patient Unity group tell me what I should do? Should I lose my clearance and my job because I cannot afford to pay $26,770? Should I borrow money from the bank and then pay high interest to the bank for the rest of my life?

This is a very misleading article. Many independent physicians speaking out want IDR to solve the problem of outliers and take patients #OutOfTheMiddle. Insurance-backed lobbyists are using this issue as a #TrojanHorse to enrich corporate shareholders at expense of physicians.

On Twitter, Dr. Amy Mecozzi Cho of Minneapolis diagnosed holes in the article. For example, she told KHN, “the contracted rates for insurance are misleading since our bad debt for patients with high-deductible insurance is greater than 60% of their deductible, but insurance companies won’t bill them because they know this. And so our effective rates for commercial insurance are actually much lower than contracted. The medical loss ratio and the CBO estimates are not capturing these costs to patients and physicians.”
Religious Malpractice
My sister, a hospital chaplain (of Roman Catholic faith), informs me that it’s considered chaplain malpractice to try to force a patient to cope with suffering in the exact way others think they should (“Firing Doctor, Christian Hospital Sets Off National Challenge To Aid-In-Dying Laws,” Aug. 30). It would follow, then, that a hospital’s requiring staff to “help” patients “appreciate the Christian understanding of redemptive suffering” is a paternalistic dismissal of patient integrity and a form of malpractice. The medical relationship is between the patient and the doctor, not the patient and the Vatican.

Squeezing The Most Out Of Student Aid
I saw Jenny Gold’s excellent article in the Los Angeles Times about a disabled student’s need for assistance as she starts school at Stanford (“Spotlight: A Young Woman, A Wheelchair And The Fight To Take Her Place At Stanford,” Sept. 4). I wanted you to be aware that the Department of Rehabilitation in California pays for additional expenses for students to train them for future employment. This includes laptops, supplies, transportation expenses, necessary expenses. They might also pay for expenses for care above and beyond the approval of the state Department of Health Care Services.
All students face the issue of how to support themselves while in college. Stanford’s lovely gift of education for all who are accepted is often not used due to the high expenses of that area. Many students are unable to afford college, even with a full scholarship.
Those who have paid their way with student loans and are now employed in high service areas, such as medicine, psychiatry, social services and teaching, are strapped with lifetime payments. Our best and our brightest who serve our communities are in debt to the point where their income barely pays their lifetime of student loans. Sylvia Colt-Lacayo’s situation may have more expenses, but the debt of $2,000 a month is not unique for a full-time university student.

I’m 68, a widower, disabled after two open-heart surgeries, with no family left. The last person to visit me at home was the yearly home health care nurse, back in February.
I am just like the people in your article: old, worn-out and forgotten. After a lifetime of work, I get by on a check that’s half of what a minimum-wage worker flipping burgers might make at $15 an hour. I can barely afford to eat the burgers now. No one, no insurance company, no politician is trying to help seniors out of poverty.
My biggest fear is dying and my little dogs being left alone for weeks or months to die before anyone finds me. Having moved to a rural area after my wife died, people don’t warm up fast to outsiders. It’s awful not to speak to anyone for years — yep, years — outside of cashiers. Just letting you know there are a lot of us out here.

Double Checking Fact-Check Facts
As the chairman of Physicians for Fair Coverage (PFC), I have joined doctors around the country in working to protect patients from surprise medical bills. I am writing now to set the record straight on the implications of various federal policies under debate in Congress. Not only did KHN’s recent article (“Doctors Argue Plans To Remedy Surprise Medical Bills Will ‘Shred’ The Safety Net,” Aug. 7) overlook research from the American Journal of Managed Care, the Centers for Disease Control and Prevention, and the Congressional Budget Office, it created a myopic interpretation of our argument in order to label it as “false,” which resulted in an inaccurate conclusion with respect to a complex issue that deserves a thorough, data-driven and factual examination.
According to the CBO, a benchmarking approach would cut payments to in-network physicians by as much as 20%. This translates into tens of billions of dollars shifted away from in-network physicians who are not sending surprise bills over the next decade. Emergency physicians treat all patients regardless of their insurance status. As a result, 70% of their patients are uninsured, seniors or poor families and children. Therefore, there is no practical difference between reducing commercial insurance payments and reducing Medicaid or Medicare rates. KHN’s own previous case study found that contributing factors to hospital closures include “high uninsured rates and a payer mix dominated by Medicare and Medicaid.” So, a benchmarking policy would create even larger disparities in quality and access to care over time for vulnerable populations.
Others agree that commercial payments play a critical role in supporting the care of America’s most vulnerable patients, including: American Academy of Orthopaedic Surgeons, American College of Emergency Physicians, American College of Radiology, America’s Essential Hospitals, American Hospital Association, American Medical Association, Association of American Medical Colleges, Catholic Health Association of the United States, Children’s Hospital Association and Federation of American Hospitals.
Instead of speaking with health care experts to better understand how reimbursement dynamics and patient access are inextricably linked in today’s economy, this article relied exclusively on the opinions of two biased research fellows who advocate for the very policy that our ad opposes and whose statements are misleading at best. The assessment also disregards a quote from the California Medical Association describing a lack of available anesthesiologists under California’s benchmarking approach.
PFC’s mission is to protect patients from surprise medical bills. That is why, this year alone, we helped drive constructive compromises that produced new laws in Texas, Colorado, Nevada and Washington. Similarly, at the federal level, we support legislation that uses a proven independent dispute resolution model to protect patients without disrupting responsible, in-network practices. To further protect patients, it’s important that Congress does not create an even larger public health issue with respect to the safety net while fixing the real issue of surprise bills.

Infected With Advocacy
Your story about Medicaid expansion fails to attempt any serious description of the reasons for opposition to this expansion (“How Political Maneuvering Derailed A Red State’s Path To Medicaid Expansion,” Sept. 6). The reader is left with the clear impression that opponents are ill-informed, parsimonious, uncaring of others (especially the poor) or all of the above. There is no suggestion that such opposing might be in good faith. You really can do better than this. You insult the intelligence of your readers when you fail to deliver the whole story in a fair and evenhanded manner so they can decide for themselves. There is enough “us vs. them” in current media. Don’t drag it into health care. If you want to take positions on important health care issues, please create an editorial page; don’t infect your “news” with those positions. And no, I’m not a Republican. I’m an independent tired of constant advocacy in the media disguised as news. Thank you.

Why Prescribed Weight Loss Is Ill-Advised
What the author of the article dismissing the opinion of the dietitian who claimed stigma and yo-yo dieting cause more harm than obesity itself failed to mention is that, for many people, how weight might contribute to the development of physical illnesses is immaterial (Obesity Stigma And Yo-Yo Dieting, Not BMI, Are Behind Chronic Health Conditions, Dietitian Claims, Sept. 17). The prescription to lose weight, in and of itself, is damaging. Most dieters regain 100% or more of weight lost within five years. Additionally, people who are told to lose weight and subsequently diet are at significantly increased risk for body dissatisfaction, bingeing, disordered eating and eating disorders.
Health is not just physical health. Descartes invented mind-body dualism in a thought experiment in the 1600s. It is an idea that permeates modern culture and medicine to this day. However, we are not separate from our minds. Our mental health is not separate from our “true” health.
Obesity may or may not cause illness. If the process of losing weight creates more illness in the form of shame and eating disorders as well as health care avoidance, then continuing to blindly recommend it is irresponsible. The recommendation to diet should be made with an informed consent process. “Being overweight may put you at risk of developing X, Y and Z. I am recommending caloric restriction to remedy that. Caloric restriction is known to be ineffective in the long term, and puts you at risk of developing an eating disorder or of weighing more than you do now. The alternative to caloric restriction is to thoughtfully examine obstacles to your access of whole, nutritious foods, whether they be financial, psychological or otherwise.
“Compassionately adopting a diet that over time puts more colors on your plate and replaces processed foods with whole foods may in itself lead both to weight loss and decreased risk of chronic medical conditions. Additionally, finding a way to increase your physical activity, such that the activity is associated with joy and self-care, will also be protective against chronic conditions and may contribute to weight loss. However, if at any point weight loss becomes the goal rather than the logical outcome of sustainable changes, then we are back at the dieting step, with all its consequent risks.”
It doesn’t matter whether obesity causes illness. Deliberately losing weight is an ineffective approach to addressing what may or may not be a problem, and additionally causes harm. Let us guide patients toward what they have control over: food choices and increased activity, and let go of the number, not because it doesn’t influence health, but because focusing on it is ineffective and counterproductive.
If a formerly obese person is now skinny but miserable, hungry, obsessive, depressed and food-preoccupied because that’s what we evolved to do in the context of weight loss, why is that better? Any solution has to respect mental and physical health outcomes, or it is not truly a health-based recommendation.

Summer Camp Rehab — Or Torture?
The article “At This Summer Camp, Struggling With A Disability Is The Point” (Aug. 13) portrays what you describe as “necessary” but what many of us adults with disabilities remember as rehab torture — traumatic memories of painful so-called therapy with questionable or no demonstrable results. Even if the program had results, many of us would have chosen not to experience the pain. It’s old-style rehab of pretending it isn’t torturous and “we know what’s best for you.” This sort of rehab was vehemently rejected by disability activists years ago. It’s too bad adults who experienced this kind of therapy weren’t consulted. I believe few of them would describe this in positive terms. It’s not a new concept: If you work hard, with pain, you can become independent! So if you can manage to dress yourself in four hours and get ready for bed in three hours, you’re independent and met the goals set for you, even though you’re so tired you can’t enjoy the remaining few hours of your day. Most adults with disabilities would set a goal of having a quality of life over being independent. This was a very biased story and should have been more balanced from very different perspectives, especially from those with disabilities.

Prescribing A ‘Deep-Dive’ Into Hospital Excesses
I think KHN Editor-in-Chief Elisabeth Rosenthal’s piece on hospital excesses is right on the mark (“Analysis: How Your Beloved Hospital Helps To Drive Up Health Care Costs,” Sept. 5). I was affiliated with a New York City hospital as a voluntary internist and retired four years ago. I wonder if it would be possible to do a “deep dive” analysis of a representative hospital’s charges and expenditures (several New York hospitals come to mind as candidates) by an investigative reporting group. I suspect there is a lot of money being spent that does not enhance patient quality of care (i.e., excessive numbers of administrators getting egregious salaries). I think you’ve discussed the salient reasons these contributing cost factors have not been widely discussed or debated ― but they must be.
I remember a news story about a patient who bought his artificial hip implant in the U.S. wholesale and, to save money, took it with him to Belgium to have it inserted. The hospital looked like a factory, and he was reluctant to go in — but he did, and as it turned out everything went well and he saved a lot of money!
