Bactrim isn’t here but I HATE it, it gives me anxiety every time I have to give it. I worked at a burn/wound center and we would occasionally get SJS/TENs, and more often than ANYTHING it would be triggered by bactrim. Sulfa/Tri my beloathed.

Medblr, I think we ought to have an antibiotics bracket. Just for fun. Everyone can submit their favorite antibiotic regimens and write propaganda for them (or anti-propaganda for the ones you hate) before we crown the one true antibiotic (regimen). Doesn’t have to be a single medication, it can be a combo. Here’s some of what I’m thinking of including:

The dialogue must feel a lot less ‘other’ in Japanese. Even using two levels of formality in English makes it feel foreign in the sense that it indicates either ‘historical’ or ‘fantasy’ since no one actually uses thou anymore but it would just sound normal in any language that does regularly use multiple levels of formality. The book already struggles sometimes with making the elves and goblins feel like elves and goblins instead of humans from different ethnicities that happen to have very expressive ears, and a lot of that otherness is achieved in the English version with things like thou form, we as a formal first person singular, and different words (it looks like the Japanese version uses “your majesty” but in the English version it’s pretty important to the fantasy world flavor that they use “serenity” instead of “majesty”, etc. Plus there are cultural implications and a hint at some kind of history behind the idea that an emperor is supposed to be “serene” as opposed to “majestic”)

I’m super curious so I’m gonna keep asking questions lol, feel free to ignore me but: all the different prefixes that would translate to さん - there’s Min (Miss), Merrem (Mrs.), and Mer (Mr./no separate one for unmarried men which makes sense with the cultural idea that women change hands and men don’t) plus Os and Dach that stack on based on how important the person is. How did they do the prefixes like Osmer and Dach’osmin? Something like オスミン and use it as a suffix in place of さん and explain it somewhere? Or do some of the important court people get 様?

@hatshepsut9 I hope you don't mind me answering your question in a new post! You asked if in the Japanese translation of the Goblin Emperor, if it used typical language conventions for formality registers or if it used an invented formal like in the original English.

I haven't read past chapter 1 in Japanese yet as I got extremely distracted reading other books this month, but here's what the formality registers in that chapter are:

Maia and Setheris begin the chapter using informal (する・だ) language with each other. When Csevet comes to Edonomee bearing news, he uses keigo/respectful, deferential language (いらっしゃいます), and Maia responds using regular polite language (です). Setheris uses informal language throughout the chapter regardless of who he is talking to, which comes across as quite rude when Maia is announced as the new emperor.

When Maia makes the switch from "I" to "We" for his first person pronoun in English, the Japanese TL switches from 僕 to 余, which is a rare and extremely formal first person pronoun used by very high ranking people like kings. He also drops down a register and begins to speak in informal language with Csevet even as Csevet continues to use deferential language. This befits Maia's station as the high-ranking social superior and Csevet's as the social inferior.

When Maia and Setheris have their private conversation at the end of the chapter, they both use informal language like they did before Csevet arrived.

In short, the use of formality registers is typical of regular Japanese, excepting Maia's use of a pronoun not often found outside of fiction these days, although it makes sense that he uses it here. Once I've gotten farther in the Japanese translation, I hope to post more about it as I read!

Quick warmup got a lil carried away.

Been a while since I drew my big guy Baruti. With spyro coming out I was excited to see Lofty Castle as that level inspired the color scheme for Fairy as well as well as provided Baruti his name since it is the name of one of the dragons trapped in that level.

Baruti has had a bit of a journey character wise but currently he’s the king of the dreamons. He was not really chosen by anyone or anything in particular to be king. If you recall earlier posts dreams are just concepts kind of come to life in this weird world and so Baruti really is just the embodiment of the idea of a King, a good King. Everyone else kinda went along with it.

As King Baruti is very decadent, he loves luxuries but he also shares his spoils with his subjects. His key objective is keeping MerreM safe and sound and making sure its running ok which tends to be a very loose idea because a dream world kinda just does what it wants. So he ends up being a figure that dreamons go to for wisdom and advice. 

Baruti has a nobility and something intimidating about him that commands respect  but hes never really been cross or aggressive with anyone. In fact he really tends to take the matters of any subject seriously. When not aiding his dreamon subjects he tends to just relax and recline and enjoy good food and drink, company and dance or if he prefers quiet he retreats to his room to do some knitting.

Common Injectable Dosage Forms:

Powder for Injection: 500 mg and 1 gram in vials and ADD-Vantage vials *ADD-Vantage is a trademark of Abbott Labs.

Dosage Ranges:

For the treatment of complicated appendicitis and peritonitis caused by viridans group streptococci, E. coli, K. pneumoniae, P. aeruginosa, B. fragilis, B. thetaiotaomicron, and Peptostreptococcus species: The adult dose is one gram given by IV administration every 8 hours. May be given by IV infusion over 15-30 minutes or by IV bolus over 3-5 minutes. The pediatric dose is 20 mg/kg every 8 hours.

For the treatment of bacterial meningitis (in patients over 3 months of age) caused by S. pneumoniae, H. influenzae, and N. meningitidis: The adult dose is same as above. The pediatric dose is 40 mg/kg every 8 hours.

Dosage in adults with renal impairment:

CrC 26-50 mL/minute: 1 gram every 12 hours.

CrC 10-25 mL/minute: 500 mg every 12 hours.

CrC <10 mL/minute: 500 mg every 24 hours

Administration and Stability: Bolus Injection: Reconstitute the vials with Sterile Water for Injection (10 mL for the 500 mg vial, 20 mL for the 1-gram vial). Shake to dissolve and let stand until clear. Stable for 2 hours at room temperature and for 4 hours under refrigeration. Infusion: Compatible with most infusion solutions. When prepared with Sodium Chloride Injection 0.9%, it is stable for 4 hours at room temperature and for 24 hours under refrigeration. When prepared with Dextrose Injection 5%, it is stable for 1 hour at room temperature and for 4 hours under refrigeration. Do not mix with or add to solutions containing other drugs. pH 7.3-8.3

Pharmacology/Pharmacokinetics: Meropenem is a carbapenem antibiotic with bactericidal activity against may Gram-positive and Gram-negative aerobic and anaerobic bacteria. Its mode of action is through inhibition of cell wall synthesis and is mediated through meropenem binding to penicillin binding proteins. Meropenem has significant stability against hydrolysis by beta-lactamases of most categories, both penicillinases and cephalosporinases produced by Gram-positive and Gram-negative bacteria, with the exception of metallo-beta-lactamases. In vitro tests show meropenem to act synergistically with aminoglycoside antibiotics against some isolates of P. aeruginosa. Meropenem is eliminated primarily by the kidneys with a mean plasma half-life in healthy adults of approximately 1 hour.

Drug and Lab Interactions: Coadministration with probenecid decreases the renal clearance of meropenem. May reduce serum levels of valproic acid to subtherapeutic levels.

Contraindications/Precautions: Contraindicated in patients hypersensitive to other beta-lactam antibiotics. Use with caution in patients with renal impairment (see Dosage Ranges). Pseudomembranous colitis has been reported with nearly all antibacterial agents, including meropenem, and may range from mild to life-threatening. This diagnosis should be considered in patients who present with diarrhea during or after therapy. Use has been associated with a risk of seizures therefore use with caution in patients with brain lesions or a history of seizures. It is unknown whether meropenem is excreted in breast milk. Pregnancy Category B.

Monitoring Parameters: Signs and symptoms of anaphylaxis

Adverse Effects: The most common adverse reactions seen in clinical studies include diarrhea (4.8%), infused vein complication (2.5%), nausea (3.6%), headache (2.3%), rash (1.9%), sepsis (1.6%), constipation (1.4%), apnea (1.3%), shock (1.2%), and pruritus (1.2%).

Common Clinical Applications: Meropenem is usually reserved for use in complicated intra-abdominal infections and meningitis infections.
