By Robert Pearl, M.D.
In the late 1970s and early ‘80s, a mysterious illness spread through America’s overlooked communities, mainly affecting intravenous drug users and homosexual men.
The disease, which caused a sudden and devastating collapse of the immune system, was unlike anything doctors had seen before. Patients arrived at hospitals with rare infections like Kaposi’s sarcoma and fungal pneumonia.
But despite the rising number of cases, public health officials remained silent for years. Few Americans saw it as a national emergency, especially since the disease seemed confined to society’s fringes, at least initially.
By the time the government and public fully grasped the threat in 1986—following Dr. C. Everett Koop’s “Surgeon General’s Report on AIDS”—tens of thousands of Americans had already died.
Looking back on this and other public health crises, it’s clear that medical science alone isn’t enough to save lives. To prevent similar tragedies, public health leaders and elected officials must first understand the role denial plays in people’s perception of medical threats. They must then develop effective strategies to overcome it.
The Psychological Basis For Denial Denial is a powerful, usually unconscious defense mechanism that shields individuals from uncomfortable or distressing realities. By repressing objective facts or experiences—especially those that provoke fear or anxiety—people can maintain a sense of stability in the face of overwhelming threats.
Historically, denial was vital to daily life. With little protection against illnesses like smallpox, tuberculosis or plague, people would have been immobilized by fear if not for the ability to repress reality. Denial, mixed with superstition, took the place of facts, allowing society to function despite the ever-present risks of death and disability.
Today, even with tremendous advances in medical knowledge and technology, denial continues to influence individual behavior with detrimental consequences.
For example, more than 46 million Americans use tobacco products, despite their links to cancer, heart disease and respiratory illness. Similarly, tens of millions of people refuse vaccinations, disregarding scientific consensus and exposing themselves—and their communities—to preventable diseases. Denial extends to cancer screenings, as well. Surveys show that 50% of women over 40 skip their annual mammograms, and 23% have never had one. Meanwhile, about 30% of adults between 50 and 75 are not up to date on colorectal cancer screenings, and 20% have never been screened.
These examples demonstrate how denial leads individuals to make choices that jeopardize their health, even when life-saving interventions are readily available.
A Pattern of Denial: How Inaction Fuels Public Health Crises When individual denial scales up to the collective level, it fuels widespread inaction and worsens public health crises. Throughout modern medical history, Americans have repeatedly underestimated or dismissed emerging health threats until the consequences became impossible to ignore.
Early warnings of the HIV/AIDS epidemic were largely ignored, as the stigma surrounding affected populations made it easier for the broader public to deny the severity of the crisis. Even within at-risk populations, the lengthy delay between infection and symptoms created a false sense of security, leading to risky behaviors. This collective denial allowed the virus to spread unchecked, resulting in millions of deaths worldwide and a public health challenge that persists in the United States today.
Even now, four decades after the virus was identified, only 36% of the 1.2 million Americans at high risk for HIV take PrEP (Pre-Exposure Prophylaxis), a medication that is 99% effective in preventing the disease.
Chronic diseases like hypertension and diabetes mirror this pattern of denial. The long gap between early signs and life-threatening complications—such as heart attack, stroke and kidney failure—leads people to underestimate the risks and neglect preventive care. This inaction increases morbidity, mortality and healthcare costs.
Whether the issue is an infectious disease or a chronic illness, denial causes harm. It allows medical problems to take root, it delays care and it leads to tens of thousands preventable deaths each year.
The Unseen Parallels: COVID-19 And Mpox Our nation’s responses to COVID-19 and mpox (formerly known as monkeypox) similarly illustrate how denial hampers effective management of public health emergencies.
By March 2020, as COVID-19 began to spread, millions of Americans dismissed it as just another winter virus, no worse than the flu. Even as deaths rose exponentially, elected officials and much of the public failed to recognize the growing threat. Critical containment measures—such as travel restrictions, widespread testing and social distancing—were delayed. This collective denial, fueled by misinformation and political ideology, allowed the virus to take root across the country.
By the time the severity of the pandemic was undeniable, hospitals and health systems were overwhelmed. The opportunity to prevent widespread devastation had passed. More than 1 million American lives were lost, and the economic and social consequences continue today.
Mpox presents the most recent example of this troubling pattern. On August 14, the World Health Organization declared mpox a global health emergency after identifying rapid spread of the Clade 1b variant across several African nations. This strain is significantly more lethal than previous variants, having already caused over 500 deaths in the Democratic Republic of Congo, primarily among women and children under 15. Unlike earlier outbreaks associated mainly with same-sex transmission, Clade 1b spreads through both heterosexual contact and close family interactions, increasing its reach and putting everyone at risk.
Despite these alarming developments, awareness and concern about mpox remains low in the United States. International aid has been limited, and vaccination efforts have fallen far behind the growing threat. As a result, by the time the WHO issued its emergency declaration, only 65,000 vaccine doses had been distributed across Africa, where more than 10 million people are at risk. Already, cases have appeared in Sweden and Thailand, and the U.S. may soon follow.
Even with the added danger of the new variant and the proven efficacy of the JYNNEOS vaccine, only one in four high-risk individuals in the United States has been vaccinated against mpox. Our slow and delayed response to Covid-19, mpox, HIV/AIDS and nearly-all chronic diseases demonstrate how widespread denial is, the lives it continues to claim and the urgent need to address this hidden defense mechanism. The best way to overcome denial—both individually and collectively—is to bring the risks into clear focus. Simply warning people about the dangers isn’t enough. Strong leadership is crucial in breaking through this subconscious barrier.
Lessons To Learn, Actions To Take Dr. C. Everett Koop’s public health campaign on AIDS in the 1980s demonstrated how clear, consistent messaging can shift public perception and drive action. Similarly, former Surgeon General Luther L. Terry’s landmark 1964 report on smoking educated the public about the dangers of tobacco. His report spurred subsequent efforts, including higher taxes on tobacco products, restrictions on smoking in public places and health campaigns using vivid imagery of blackened lungs—leading to a significant decline in smoking rates.
Unfortunately, government agencies often fall short, hampered by bureaucratic delays and overly cautious communications.
Officials tend to wait until all details are certain, avoid acknowledging uncertainties, and seek consensus among committee members before recommending actions. Instead of being transparent, they focus on delivering the least risky advice for their agencies. People, in turn, distrust and fail to heed the recommendations.
Early in the COVID-19 pandemic, and more recently with mpox, officials hesitated to admit how little they knew about the emerging crises. Their reluctance further eroded public trust in government agencies. In reality, people are more capable of handling the truth than they’re often given credit for. When they have access to all the facts, they usually make the right decisions for themselves and their families. Ironically, if public health officials focused on educating people about the risks and benefits of different options—rather than issuing directives—more people would listen and more lives would be saved.
With viral threats increasing and chronic diseases on the rise, now is the time for public health leaders and elected officials to change tactics. Americans want and deserve the facts: what scientists know, what remains unclear and the best estimates of actual risk.

Maya Yang at The Guardian:
Alcohol use is the third leading preventable cause of cancer in the US after tobacco and obesity and should carry a label warning consumers about their cancer risks, according to a new advisory by the US surgeon general. Released on Friday, the advisory revealed that alcohol use contributes to nearly 100,000 cancer cases and about 20,000 cancer deaths each year. It also found that alcohol-related cancer deaths shorten the lives of those who die by an average of 15 years.
The US surgeon general, Vivek Murthy, also called for the guidelines on alcohol consumption limits to be reassessed so consumers can weigh the cancer risk when deciding whether or how much to drink. Alcohol in the US already has warnings on birth defects and impairments when operating machinery. “Alcohol consumption is the third leading preventable cause of cancer in the United States, after tobacco and obesity, increasing risk for at least seven types of cancer,” Murthy’s office said in a statement with the new report. Any such change to warning labels has to be made by the US Congress. According to the advisory, the largest burden of alcohol-related cancer in the US is for breast cancer in women, with an estimated 44,180 cases in 2019, marking 16.4% of the approximately 270,000 total breast cancer cases for women.
Among women, breast cancer makes up approximately 60% of alcohol-related cancer deaths. Meanwhile, liver cancer, at approximately 33%, and colorectal cancer, at approximately 21%, make up the majority of alcohol-related cancer deaths in men. Overall, consuming alcohol increases the risk of developing at least seven types of cancer. In addition to colorectal and breast cancers, alcohol consumption increases the risk of mouth, throat, esophagus, voice box and liver cancers. The advisory also found that about 83% of the estimated 20,000 alcohol-related cancer deaths in the US annually occur among people who drink at levels above the federally recommended limits of two drinks daily for men and one drink daily for women. It also stated that the remaining 17% of the estimated 20,000 annual alcohol-related cancer deaths occur at levels within the recommended limits.
Surgeon General Vivek Murthy recommends that alcohol carry a warning label for various cancer risks.

as always, we're complaining under the cut. tw for medical PTSD, mentions of medical procedures, personal/graphic descriptions of my fucked up little body, extremely privileged whining, it's worth it for the cat at the bottom tho
I am exhausted by the number of appointments I have. I've become the kind of person who has multiple appointments every single week. This week and next week are three appointment weeks, and those appointments always lead to scheduling MORE test/procedures/office visits.
I had an appointment with my urogynecologist. Given my history of neurogenic bladder and severe stage IV endometriosis, she's in agreement with me that it might be time to at least consider a bowel diversion, if not a total colonoscopy. This would be done in addition to a bladder diversion because self-catheterization has proven unsustainable and, frankly, dangerous for me to try to continue. I'll likely have a foley placed tomorrow morning that will stay in until I can get in to the urological surgeon, who has not called to schedule yet. I have the colorectal surgery consult next month and the neurourology consult a month after that. Just thinking about it gives me a headache.
I also have an EEG in a few days to see if the blackout episodes I've been having are epilepsy, and if it is, I'm not sure what we'll do since I'm already on a good amount of anticonvulsants. I have a feeling I'm going to leave that test with a migraine and no answers, which was exactly how the EMG/NCS I had done in my hand last Friday went.
Other things I've started include using a CPAP machine. I thought this shit was supposed to help you sleep better, but instead I'm waking up many many times in a two hour window, the pressure being pushed into my lungs is dislocating my ribs, I'm getting bloated from wearing it, and the pressure is coming out of my eyes and ears too due to a deformity in my inner ears called patulous eustachian tubes, so now my constant migraine is back in full force. I'm nothing but bloated and irritable as hell, but if I don't use it for at least four hours a day for at least 24 days a month, medicaid won't pay for it and I will have to pay out of pocket to buy the machine. My mother price checked it, it costs ~$1300 to buy the machine I have. I might just give up and give it to my brother, who also needs a CPAP but who doesn't also have EDS and is, therefore, not prone to ribs dislocating.
I met with my 4th electrophysiologist and that was a frustratingly and dangerously nonproductive appointment. He did not speak or understand spoken English well enough to be allowed to practice medicine in an English-speaking country imo. I don't give a shit about an accent, I don't think people need to "go back to where they came from" or that they need to speak English exclusively, but man it is so important in the field of medicine to be able to actually understand what your patient is saying to you. He had no idea what the condition I have even was, and he assumed I made all of these diagnoses for attention but would "humor me" and wrote IN HIS NOTES that I "insisted" on a holter monitor and tilt table test. I got these orders from Duke university, the closest university to me that has a genetic electrophysiology department and a dysautonomia clinic, both of which I was being seen at until Duke stopped taking my insurance. I was trying to tell this absolute worm brain that I was telling him what the top specialists GLOBALLY told me to have done, but I was just being young and attention-seeking I guess, I'm so dangerously angry about it, it makes my chest hurt to try and articulate just how badly this EP fucked it up for me.
I realized in the middle of a visit with my PCP on the 30th that I am not working towards a goal. Most people go to the doctor with the goal of Get Better Enough To Work, or Get Better Enough To Take Care Of The Kids. I don't have that. I'll never be able to hold down a job and I can't and don't want to have children.
I feel like the shittiest friend on Earth too because a good friend of mine only lives four hours away, but as much as we'd both like to visit, I have to schedule everything in my life around what's starting to look like a year packed with surgeries. I can't just pack up and go visit him, I come with medical equipment now. Between meds, splints, incontinence supplies, and the CPAP, I have to basically haul around a small urgent care center everywhere I go. I hate that I have to be planned around.
For a few wins, I do not have carpal tunnel, and when I had my A1C checked at my last PCP appointment, it was 4.9! Every doctor who finds that out informs me even their own A1C isn't that good because they like some specific sweet treat too much. My secret? No one has said they liked something that doesn't have dairy in it, and I've had a dairy allergy since birth.
anyway. I turned 26 just over a month ago and my beloved medical advisor turned 1 year old the same day. she's my birthday buddy :) it's weird having an Adult Cat in the house now, she's not the teeny tiny kitten that sneezed in my eye and gave me pinkeye anymore.
thanks 4 reading, besties. until my next frustratingly whiny and Packed Full of Info update <3
pictured: my big adult girl and medical supervisor 🥰

I am lucky.
It's hard to see in the middle of everything that's been happening to me and around me, but I am lucky.
Cancer:
I was lucky that someone finally ordered the CT scan that resulted in finding a 10cm tumor in my guts. I'd been trying since October of 21 only to be told 'menopause' and 'covid after effects' and basically anything for my GP to pocket the copay and get my out of his office. GP is no longer in practice - which is lucky for someone else.
I was lucky the morning I woke up in unbelievable pain that I could get to Cedars Sinai. The tumor was closing off my left ureter. I was in a bed within hours.
I was lucky to have changed my insurance to a Silver PPO from a Silver HMO - otherwise I'd be dead.
I am lucky to have had support on the work front - including six months of my rent paid from March to August, when I was a chemo'ed mess.
I was lucky to have young, aggressive docs and a knowledgeable, experienced, and highly skilled surgeon (if he does say so himself, and he does - often).
I'm lucky to be in remission, because colorectal cancer is a killer. It's estimated that about 53,000 people will die from it in 2023, about evenly split between men and women. It is the second leading cause of cancer deaths in men, and third in women. I know that it is likely to recur, and if it does it is likely to be fatal. I am lucky to have each day, as hard as some of them are.
Mother:
I am lucky to have seen her when she was still somewhat herself, back in Ferbruary of 2020, literally weeks before COVID hit. We discussed her selling the house and going into assisted living. I said I'd be back in a few weeks, having to go hold down the fort while my boss did some trade shows.
If she had been in assisted living, she likely would have died in that year, when at the peak of COVID, the local paper had eight pages of obituaries, and a six-week turnaround for cremation, nine weeks for burial/ennichement.
I kept her at home, buying her groceries, getting home repairs and upgrades done, keeping up the maintenance.
In March 2022, the week I was hospitalized, she developed a severe and almost fatal UTI. She was two weeks in the hospital, three in rehab, before being transferred to assisted living and ultimately to memory care.
I wanted to give her safety, comfort, and dignity for however long she had left. I've done that. It's a great place, she had friends, companionship, and care all the way.
If she dies on my birthday, then well, it's the last one she gets to torpedo for me. I've done what I feel is my duty, and if I also feel like it wasn't enough, I never was enough. Mom had her own trauma from two traumatized people. I can let go of it, but maybe not forgive the string-pulling and button-pushing. There's things you have control over, and things that you don't. I'm guessing about some of her motivations, but others were billboards 20-by-30 in blinking LED.
I'm lucky not to be more of a mess, lucky to have had good therapy and good friends.
Most of all, I am lucky to have had the friends around me. Even moreso as I come out of treatment and surgery and into remission, survivorship, and recovery. I am lucky that they understand that it's not over when the chemo stops. I love them dearly and am endlessly grateful to them.

Pristyn Care: Common Types of General Surgeries and How They Are Performed
General surgery is a critical specialty in the medical field, addressing a vast array of conditions that may require surgical intervention. From managing acute emergencies like appendicitis to performing complex procedures such as colorectal surgery, general surgeons play a pivotal role in improving and saving lives. Their expertise extends to several systems of the body, and advancements in technology have significantly enhanced surgical outcomes. At Pristyn Care, patients benefit from a modern, patient-centric approach that includes cutting-edge surgical techniques and dedicated post-operative care. This blog Pristyn Healthcare will explain the different types of general surgeries, their applications, and how they are performed.
Types of General Surgeries and How They Are Performed
1. Appendectomy: Removing the Appendix
An appendectomy is one of the most common emergency surgeries performed worldwide. It is the removal of the appendix, a small pouch-like organ attached to the large intestine. The appendix can become inflamed and infected, leading to appendicitis, which can cause severe pain and potentially life-threatening complications if left untreated.
How it's performed:
Traditional appendectomy: A small incision is made in the lower right abdomen to remove the appendix.
Laparoscopic appendectomy: Using three or four small incisions, a laparoscope (a small camera) is inserted to guide the surgeon in removing the appendix with minimal disruption to surrounding tissues.
Both methods are effective, but laparoscopic surgery typically offers benefits such as smaller scars, less pain, and quicker recovery times.
2. Gallbladder Removal (Cholecystectomy)
The gallbladder is responsible for storing bile, which helps in the digestion of fats. Gallstones or inflammation of the gallbladder can cause significant pain, nausea, and even infection. When conservative treatments fail, surgery may be necessary to remove the gallbladder.
How it's performed:
Open cholecystectomy: A larger incision is made in the abdomen to remove the gallbladder, typically used for more complex cases.
Laparoscopic cholecystectomy: This is the preferred method in most cases today. Surgeons make small incisions and use a camera to guide the removal of the gallbladder. It is minimally invasive, offers reduced pain, and results in a faster recovery compared to traditional surgery.
Patients who undergo gallbladder removal are typically able to live without the organ, though dietary changes may be required to adapt to the absence of bile storage.
3. Hernia Repair
A hernia occurs when an organ or tissue pushes through a weakened area in the surrounding muscle or connective tissue. This condition commonly occurs in the abdomen but can happen in other areas, such as the groin. Hernias can cause pain and, if left untreated, lead to more serious complications like strangulation, where the blood supply to the organ is cut off.
How it's performed:
Open hernia repair: The surgeon makes an incision near the hernia site, pushes the protruding tissue back into place, and reinforces the area with stitches or mesh.
Laparoscopic hernia repair: Smaller incisions are made, and the surgeon uses a camera to guide the procedure. Mesh may still be used to reinforce the area. Laparoscopic hernia repair has the advantage of faster recovery times and smaller scars.
Hernia repair surgery is highly effective and offers significant relief from pain and discomfort.
4. Bowel Resection
Bowel resection is a procedure in which a part of the intestine (either the small or large bowel) is surgically removed due to conditions like cancer, diverticulitis, Crohn's disease, or blockage. The remaining healthy parts of the bowel are then rejoined to maintain the continuity of the digestive system.
How it's performed:
Open bowel resection: A large incision is made in the abdomen to access and remove the affected portion of the bowel.
Laparoscopic bowel resection: Surgeons may opt for smaller incisions and use a camera to remove the diseased section. This technique is less invasive, reduces scarring, and has the potential for faster recovery.
Bowel resections are typically followed by careful monitoring to ensure proper digestion and bowel function.
5. Breast Surgery (Mastectomy or Lumpectomy)
Breast surgeries are primarily performed to treat conditions like breast cancer or benign breast tumors. Depending on the severity of the condition, different surgical approaches are used.
How it's performed:
Mastectomy: This involves the removal of one or both breasts, typically when breast cancer is present. The extent of the surgery depends on the stage of cancer and whether the cancer has spread.
Lumpectomy: Also called breast-conserving surgery, this procedure involves the removal of just the tumor and some surrounding tissue, leaving the breast intact. This is typically recommended for smaller, localized tumors.
Both procedures may be followed by reconstructive surgery to restore the shape of the breast, depending on the patient's preferences and overall health.
6. Thyroid Surgery
Thyroid surgery is done to treat disorders related to the thyroid gland, which can include thyroid cancer, goiter (an enlarged thyroid), or hyperthyroidism. The surgery may involve the removal of part or all of the thyroid gland, depending on the condition.
How it's performed:
A small incision is typically made in the neck to access the thyroid. If the entire gland needs to be removed, it’s called a total thyroidectomy. Partial removal is referred to as a lobectomy. This surgery may also involve the removal of nearby lymph nodes if cancer is present.
Post-surgery, patients may need hormone replacement therapy to make up for the lack of thyroid hormone production.
Conclusion:
General surgeries are essential procedures used to treat a wide array of medical conditions, from simple hernias to complex cancers. With advancements in minimally invasive techniques, surgeries today are more efficient, safer, and require shorter recovery times than ever before. Pristyn Care, a leading provider in healthcare, ensures that patients receive the best possible care, leveraging the latest technology and expert surgical teams. Best Doctor For Fissure In Mumbai
Anal fissures are small tears or cracks in the skin around the anus, often causing significant pain and discomfort. While these tears can be a common issue, they should not be ignored. If left untreated, an anal fissure can lead to a range of complications that can significantly affect a person’s quality of life. If you're experiencing symptoms of a fissure, seeking timely treatment from a qualified healthcare provider is crucial. At Kapadia Hospitals, one of the leading fissure hospitals in Mumbai, we provide expert care to ensure a quick recovery and prevent long-term problems.
What Happens If a Fissure Is Left Untreated?
Chronic Pain and Discomfort One of the primary consequences of leaving an anal fissure untreated is persistent pain. Anal fissures typically cause sharp, burning pain, especially during or after bowel movements. Over time, this pain can become chronic, leading to a reduced quality of life and a constant feeling of discomfort.
Development of Chronic Fissures If an anal fissure does not heal properly, it may evolve into a chronic fissure. Chronic fissures are more difficult to treat and can lead to recurring pain. These fissures are often deeper and more prone to infection, making the healing process longer and more complicated.
Fistulas and Abscesses An untreated fissure can also lead to the formation of anal fistulas or abscesses. A fistula is an abnormal tunnel that forms between the anal canal and the skin around the anus, often due to an untreated infection. An abscess is a collection of pus that may develop due to the bacterial infection caused by a fissure. Both of these complications require surgical intervention and can significantly disrupt your daily life.
Bleeding and Infections As fissures continue to irritate and tear the anal skin, they can cause persistent bleeding. While bleeding may be minimal at first, chronic fissures can lead to significant blood loss, which may increase the risk of infections. Over time, this increases the chances of developing more serious conditions that require more intensive treatment.
Impact on Bowel Movements If a fissure is left untreated, it can cause the muscles around the anus to tighten in response to pain, leading to constipation or difficulty passing stool. This can create a vicious cycle where the strained bowel movements worsen the fissure, causing further pain and discomfort.
Psychological Effects Chronic pain, bleeding, and discomfort caused by untreated fissures can also have a psychological impact. People with persistent anal fissures may experience anxiety, stress, or embarrassment, making it harder to seek medical help. In some cases, this can affect a person’s social life and work productivity.
