Bulbous Inflatable Installations by Steve Messam Interact with Historic Architecture and Landscapes
U.K.-based artist Steve Messam is known for his artistic interventions in the landscape, reinterpreting historical monuments, buildings, or rural areas with bold, ephemeral installations. Often inflated, his works reimagine or disrupt perceptions of our surroundings and impact how people move around and through them. Bright colors and striking forms that jut from colonnades, facades, and river banks prompt viewers to consider their relationships to the built environment.

Your life in the next 4 months will be more focused in the area of self growth and love. What you give out to the world in the 1st month will comeback to you as an opportunity for growth in the 4th month.
During the 1st month, you will feel a surge of energy and want to start some creative projects, could be something simple as redecorating, writing short poems, planning a trip, learning a new skill, trying out a new recipe, etc. It will feel exciting. But there might be an unexpected event that can slow down your progress, a little hitch. This will be a sign for you to reevaluate how you are spending your energy and resources. Maybe you've been trying out so many different things, wanting to do many things at the same time, having grand plans for the future. This could make you feel burnout, so introducing divine intervention. You can always come back to your action-mode after taking a rest. You might feel restless about this hitch in the progress, thinking it's the sign of stagnancy and boredom.
But don't worry, you will pick up where you left off in the 2nd month. There's a feeling of compensating, you want to make up for the unproductive time by doing double, which can be counterproductive. I also see an opportunity for a connection, maybe a romantic one. This connection will take centre stage of your life in the upcoming 4 months. You might feel a little confused at first, of how to act, should you pursue this or not, are they interested. The desire to act is there but there's also hesitancy and reluctance. Maybe because this connection is not new, you might have known them before and are not sure of their intentions. They've come back and you don't feel too enthusiastic about it. Even though this connection will play an important role in your life, I see that in the next 4 months, you might not want to officially start this connection but focus more on yourself instead. There's a feeling of independent and going about life alone.
By the end of 3rd month, you will begin to look inward and address the lingering feeling of dread and dissatisfaction. Your emotional landscape will get a makeover, cleaning out unnecessary things that have been taking up your time and energy. But it can also be a literal change of living space. There might be some times when you feel a certain pain in the area of stomach and chest, pay attention to them, no matter how fleeting they feel.
After the spring cleaning of the 3rd month, the 4th month will bring new opportunities for learning and travel. But you will have to actively pursue them, they won't be placed in your hands readily. The tricky part is that you might not feel the desire to start something new at this period because your mind seems to be a little foggy and busy with noises from all around you. And a new insight about the connection from previous months will appear, this insights will help you plan your actions more realistically and more practical. The 4th month might not be an active month for you, the energy feels a lot more somber. This might be the preparation period for the upcoming changes in your life.

In these upcoming 4 months, there's an overall feeling of newness and unpredictability. This can create a sense of uneasiness.
In the 1st month, you might encounter some setbacks or difficulties in what you're doing. It's about something that you have set your eyes on for a long time, an ideal to achieve. I think you are aware that the things you want to do might go against a certain rule or against someone, you will know that there will be opposition, but you will want to proceed nevertheless. So the setbacks might seem like a challenge for you, you will feel it's unfair but it's not, it's a chance to reevaluate your commitment and focus, whether you have the right plan, is there some parts that's too impractical. These setbacks will slow you down, give you more time to think more thoroughly. You might want to fight back, to will things your way, exerting more force than necessary. These actions have an emotional drive behind them. In this period, it's best not to make major decisions when you're feeling too emotional. Also be mindful of emotional outbursts and misdirected angers.
In the next month, the situation will improve but won't be entirely smooth sailing. You will become more cautious in taking actions. In the 2nd month, there will be other responsibilities that demand your attention and divert you from your original plan. You might be swamped by mundane tasks that bore your mind but you will try to keep a positive outlook. It might not be easy, it might be hard to stay positive in some situations but you do realise that a lot of uncertainties and unnecessary mental burdens are created by your own mind. You will try to look forward to the future. Maybe doing something different to distract yourself, learn something new, talk with other people more, change the kind of show or content you usually watch, anything to keep you from the routine and boredom. This signals that you need to pay more attention to the part of you that wants to just have fun and enjoy life, being carefree.
At the start of the 3rd month, there might be something or someone from past wanting to make a comeback. It might be an old habit, a long forgotten project, something that you've done many times before, someone you've known for a long time or someone new but has similar traits to the people you've known from the past. But the comeback is just a possibility or a wishful thought, something that you absentmindedly reminiscent about or some fleeting words from someone that will remind you of the past. It won't be something that will come in direct contact with you in this period. Then there will be a surprise feeling of attraction for someone. This will be someone that you don't anticipate, an unlikely chance. In this 3rd month, you seem to be a lot more "floaty", the opposite of how you feel in the 1st month. You will allow your mind to dream more, to romantise things more. This attraction might not be long lived but it will lighten your heart and make you feel more confident in yourself.
This positive effect will be stronger in the 4th month. You will feel more bold, wanting to try out things that you've been afraid of before. This attitude will be much needed because I see a big change is looming from the distance future.

CUBE 3

There is a surge of energy at the start of the 1st month. You feel like you can do anything, things that seemed challenging before will now be easy to tackle. If you're planning to start something, this time period will be a good start. Be careful not to be too forceful about executing your plan though, your actions and words might have a directness to them that can unintentionally hurt someone who is more sensitive or inciting heated debates or arguments that can make both parties regret later. Something you do might be usually considered difficult or even impossible for you, but you still have hope and see a path for you. A little of optimism will go a long way in helping you achieve your dreams. Also avoid eating things that you might regret later. This surge of energy might excite your body more than usual, making it working harder so avoid adding elements that might disturb the balance of your body.
In the 2nd month, you might be more emotional than usual and start to look back at the past, reminiscent bygone things. The energy of this month is very quiet and still. If you're someone who enjoy the company of other people or lively atmosphere, in this month, you might make people wondering about what's changed in you. You will want to seek more solitude, to quiet your mind. You might find out an information that was hidden from you in the past or lift off the veil of a confusion. Your mind will be clearer and also more heavier. Sometimes seeing the truth is not a pleasant feeling.
Into the 3rd month, at first, there won't be anything unusual, life continues on the same way as the past months. But then, by the end of the month, your mind will be active again, bustling with ideas, even wild ones, risky ones, ones that you aren't usually comfortable doing. You will suddenly feel like you're on a whole new path, so many unfamiliar things, but doing them will expand your horizon and the limit you put on yourself.
So this daring energy might not last long. After a while, into the 4th month, you will begin to doubt if those ideas you were so excited about are actually doable or practical. You look at the other responsibilities in your life and get an inkling that things might not be the same anymore, to be daring will be equal to be rebuilt. There will be things that you need to discard and change. That feeling can be overwhelming, the foresight of an transformation that will take a lot of work. But I see that there's supportive energy around you. I see a masculine figure, someone with lots of drive and energy, generous with their affection will support you, infuse you with confidence again.
Tumblr media
CUBE 4
Tumblr media
In the 1st month, an opportunity for something that you've been wanting to try will present itself to you. You won't be forced to do it but have a choice. This is something that you want or like to do, because instinctively you can feel that doing it will make you braver and grow more as a person. It's a little bit like a challenge, something that makes you anxious but still excited. It might require you to show yourself more, be more bold and be unapologetically yourself, which is not always a walk in the park. It feels like a performance. But overall the energy of the first two months is quite relaxing and fun. Even if your life is busy with other responsibilities in that period, you still try to make space for yourself and have some fun.
The 2nd month is quite uneventful. Whatever your life is at the moment of the 1st month, it will continue to be so in the next month. Just be mindful of the feeling of boredom that can lead to thoughts spiraling on and on. Maybe you will feel an itch, a nudging feeling that the inactivity is just a prelude of a big event lurking in the future. Which you're probably right. There will be a time when you need to be in deep introspection that will bring about lasting change in your way of thinking.
In the 3rd month, something about a place, could be your living place or your workplace will demand your attention. You might feel a little confused, maybe you wanted to change but you're still debating whether you should make such a drastic move in this time. Something will make you feel like you're not ready, there's so much to do and prepare, you haven't had enough resources yet, etc. You will be in worry mode by the end of this month. You want to look at the bright side and think positively but at the same time, the urgency of the issue will not help you thinking clear. That might be the external circumstance, on the internal note, you might have the feeling that your life right now is a little out of place, it's not where it's supposed to be and you feel like you're fretting for it to change.
In the 4th month, you will begin to calm down and work on the issue more practically. Addressing what's needed to be done, what can you do, what is your limit. I see some of your own duties will have to change, maybe it's about changing of job or your responsibilities in the house. I see an ending of the old. A different path is opened for you, to study or work oversea or work with many foreigners. You will have a chance to get to know more about different cultures and who knows, along the way, you might fall in love, could be a person, but could be the culture, the beauty of distant lands. In this month, you will act more unpredictable, at least to the onlooker's eyes. Your inner transformation has already happened for some time. The result is the unexpected decisions that you make. I don't see you being forceful or rebellious about it. You just have the freedom in your hands and are using it more to get closer to what you truly want and are called to.
Tumblr media
CUBE 5
Tumblr media
At the start of the 1st month, there will be a big chance for you to explore a new territory that you haven't thought of before. It's something very new but exciting. One thing will lead to another. You will feel like Alice spiraling down the rabbit hole. Maybe you will get inspired by another person. Someone who is in control of their feminine energy very well. The energy is mature and sensual. They might be artistic and have a killer sense of style and aesthetic. They will inspire you to explore new concepts about love, sensuality and beauty. The emphasis here is sensuality, being in touch with nature, you will train your senses, noticing more scents, more colours, textures will give different sensations. This group feels very artistically inclined. It's like you will be touched with the beauty around you. Feeling your life a lot more romantic.
For the first two months, the energy is relaxing and flowy, just enjoying and having fun, exploring new things. Into the 2nd month, you will want to delve deeper into the topics that interested you in the previous month. Learning, doing research, talking about it excitedly to people. It feels like a flower blooming. There's something changing here. You might need to change an electronic device. And you might want to change something else. You dream more and begin to have a different vision for yourself. The idea is kind of out of the blue but not that shocking. The feeling is very quiet but free. There's a lot of space without disturbances.
Now, into the next two months, things might pick up the pace again. You will need to be in your masculine energy more, making decisions, being assertive. After the first two months of being future oriented, wanting to take a different path, explore new things, in the 3rd month, you seem to revert back into your old self, acting like how you've been in the past. Maybe you think that the decisions of the previous months are too risky, you're not used to it. There's a part of you wanting to fight back the change, to keep the status quo. People might also chime in their opinions, they would remind you of the past, maybe some familiar situations had happened and they want to remind you of the lesson. Some people from the past might also come back, posing as the opposite force to the change. Maybe you try to move one from something, could be love, and now the past wants to come back. By the end of this month, you will have to make a choice, to be decisive. Something will have to end. It won't feel easy, you will feel like fate is playing with you. But this choice will help you grow into yourself more.
In the 4th month, you will feel strong enough to face your inner anguish. In this period, there's a need to settle down, to find a place for you to feel stable. Maybe you will face a decision to move and you will feel anxious about it. Be aware of upset stomachs. Nutrition is very important in this time period, you will need to nurture yourself, to give yourself more energy, and be mindful of how things we take in ourselves can influence our thoughts and feelings.
Tumblr media
CUBE 6
Tumblr media
The 1st month will start with some exchanges, of ideas and materials, or you will travel more, more movements, going to different places. It seems busy but not hectic. You will navigate situations with grace and flexibility. Whatever you do in this month will be towards a future that you've seen for yourself. They are not just mindless acts to fulfil some responsibilities but have a greater intention behind. Things might seem fast-paced, with many different areas requiring your focus. You will practise your verbal skills a lot, especially about persuasion, and tact is very important this month. You will need to convince people, charming them. Maybe these people will help you start something new, they will be your support.
In the 2nd month, things seem to slow down again, maybe in the 1st month, you will be preparing for something new, and in this month, you will focus on stabilise it, getting used to it. Something will require your commitment and trust. I see the energy of this month is pretty solitary. You work on your own instead of depending on other people like the previous month. Even if some difficulties arise, you will want to solve them by yourself. I feel very distracted in your reading. Maybe this will be the energy of your next 4 months, a lot of things will pull at your attention, so you need to be discerning and disciplined.
There are lots of activities and events in the 3rd month. You will start this month with an activity that you have done many times before, it has become a habit and you're good at it. It will be a valuable skill to help you in work. But I see that you won't be contented with it, you might feel a little bored with same old, same old things. It might be a pattern of behaviour or pattern of the people you're involved with. You feel like this pattern doesn't help you grow. It repeats itself over and over, even when you tried to do something new, it was still the underlying energy. So, in this month, you will begin to question more about your patterns in life and want to shake things up. Your life seems to be too quiet for you so you want to make some drastic changes. Thinking up some novel ideas, remembering your childhood dreams. Some people might wonder what has influenced you into this kind of action. You seem more rebellious, wanting to do things your way. Beware of the tendency to stir up conflicts or challenge other people's ideas and opinions just because you feel stifled or your freedom is threatened by them.
This feeling will continue on into the 4th month. Your ego and how you express yourself will be the focus of these four months. You will feel invincible, so sure of yourself that you might not see things in an objective lens and think things through carefully. Some well-meaning advice might be ignored by you. I see some friends, someone with more soft energy, will try to help you see things from a different perspective, but you seem to not yield. This will not create a serious conflict, but you might need to reexamine your stance more carefully, lest your ego might spoil some meaningful connections.

The Rise of DeFi: Revolutionizing the Financial Landscape
Tumblr media
Decentralized Finance (DeFi) has emerged as one of the most transformative sectors within the cryptocurrency industry. By leveraging blockchain technology, DeFi aims to recreate and improve upon traditional financial systems, offering a more inclusive, transparent, and efficient financial ecosystem. This article explores the fundamental aspects of DeFi, its key components, benefits, challenges, and notable projects, including a brief mention of Sexy Meme Coin.
What is DeFi?
DeFi stands for Decentralized Finance, a movement that utilizes blockchain technology to build an open and permissionless financial system. Unlike traditional financial systems that rely on centralized intermediaries like banks and brokerages, DeFi operates on decentralized networks, allowing users to interact directly with financial services. This decentralization is achieved through smart contracts, which are self-executing contracts with the terms of the agreement directly written into code.
Key Components of DeFi
Decentralized Exchanges (DEXs): DEXs allow users to trade cryptocurrencies directly with one another without the need for a central authority. Platforms like Uniswap, SushiSwap, and PancakeSwap have gained popularity for their ability to provide liquidity and facilitate peer-to-peer trading.
Lending and Borrowing Platforms: DeFi lending platforms like Aave, Compound, and MakerDAO enable users to lend their assets to earn interest or borrow assets by providing collateral. These platforms use smart contracts to automate the lending process, ensuring transparency and efficiency.
Stablecoins: Stablecoins are cryptocurrencies pegged to stable assets like fiat currencies to reduce volatility. They are crucial for DeFi as they provide a stable medium of exchange and store of value. Popular stablecoins include Tether (USDT), USD Coin (USDC), and Dai (DAI).
Yield Farming and Liquidity Mining: Yield farming involves providing liquidity to DeFi protocols in exchange for rewards, often in the form of additional tokens. Liquidity mining is a similar concept where users earn rewards for providing liquidity to specific pools. These practices incentivize participation and enhance liquidity within the DeFi ecosystem.
Insurance Protocols: DeFi insurance protocols like Nexus Mutual and Cover Protocol offer coverage against risks such as smart contract failures and hacks. These platforms aim to provide users with security and peace of mind when engaging with DeFi services.
Benefits of DeFi
Financial Inclusion: DeFi opens up access to financial services for individuals who are unbanked or underbanked, particularly in regions with limited access to traditional banking infrastructure. Anyone with an internet connection can participate in DeFi, democratizing access to financial services.
Transparency and Trust: DeFi operates on public blockchains, providing transparency for all transactions. This transparency reduces the need for trust in intermediaries and allows users to verify and audit transactions independently.
Efficiency and Speed: DeFi eliminates the need for intermediaries, reducing costs and increasing the speed of transactions. Smart contracts automate processes that would typically require manual intervention, enhancing efficiency.
Innovation and Flexibility: The open-source nature of DeFi allows developers to innovate and build new financial products and services. This continuous innovation leads to the creation of diverse and flexible financial instruments.
Challenges Facing DeFi
Security Risks: DeFi platforms are susceptible to hacks, bugs, and vulnerabilities in smart contracts. High-profile incidents, such as the DAO hack and the recent exploits on various DeFi platforms, highlight the need for robust security measures.
Regulatory Uncertainty: The regulatory environment for DeFi is still evolving, with governments and regulators grappling with how to address the unique challenges posed by decentralized financial systems. This uncertainty can impact the growth and adoption of DeFi.
Scalability: DeFi platforms often face scalability issues, particularly on congested blockchain networks like Ethereum. High gas fees and slow transaction times can hinder the user experience and limit the scalability of DeFi applications.
Complexity and Usability: DeFi platforms can be complex and challenging for newcomers to navigate. Improving user interfaces and providing educational resources are crucial for broader adoption.
Notable DeFi Projects
Uniswap (UNI): Uniswap is a leading decentralized exchange that allows users to trade ERC-20 tokens directly from their wallets. Its automated market maker (AMM) model has revolutionized the way liquidity is provided and traded in the DeFi space.
Aave (AAVE): Aave is a decentralized lending and borrowing platform that offers unique features such as flash loans and rate switching. It has become one of the largest and most innovative DeFi protocols.
MakerDAO (MKR): MakerDAO is the protocol behind the Dai stablecoin, a decentralized stablecoin pegged to the US dollar. MakerDAO allows users to create Dai by collateralizing their assets, providing stability and liquidity to the DeFi ecosystem.
Compound (COMP): Compound is another leading DeFi lending platform that enables users to earn interest on their cryptocurrencies or borrow assets against collateral. Its governance token, COMP, allows users to participate in protocol governance.
Sexy Meme Coin (SXYM): While primarily known as a meme coin, Sexy Meme Coin has integrated DeFi features, including a decentralized marketplace for buying, selling, and trading memes as NFTs. This unique blend of humor and finance adds a distinct flavor to the DeFi landscape. Learn more about Sexy Meme Coin at Sexy Meme Coin.
The Future of DeFi
The future of DeFi looks promising, with continuous innovation and growing adoption. As blockchain technology advances and scalability solutions are implemented, DeFi has the potential to disrupt traditional financial systems further. Regulatory clarity and improved security measures will be crucial for the sustainable growth of the DeFi ecosystem.
DeFi is likely to continue attracting attention from both retail and institutional investors, driving further development and integration of decentralized financial services. The flexibility and inclusivity offered by DeFi make it a compelling alternative to traditional finance, paving the way for a more open and accessible financial future.
Conclusion
Decentralized Finance (DeFi) represents a significant shift in the financial landscape, leveraging blockchain technology to create a more inclusive, transparent, and efficient financial system. Despite the challenges, the benefits of DeFi and its continuous innovation make it a transformative force in the world of finance. Notable projects like Uniswap, Aave, and MakerDAO, along with unique contributions from meme coins like Sexy Meme Coin, demonstrate the diverse and dynamic nature of the DeFi ecosystem.
For those interested in exploring the playful and innovative side of DeFi, Sexy Meme Coin offers a unique and entertaining platform. Visit Sexy Meme Coin to learn more and join the community.

Biggest galaxy brain moment from visiting the Dune dunes is that it gave me a whole new perspective on why the terraforming of Arrakis is treated with such deep ambivalence by the text. Because the terraforming process that's described in great detail in the book? That's exactly what's happening to the Oregon dunes. And they're disappearing.
Tumblr media
At the beginning of the 20th century, the open sand you see in this picture stretched all the way to the Pacific Ocean, which is visible here as a faint blue-gray line about halfway up the photo. The sea washed new sand ashore, and the seasonal wind cycles blew it into a constantly-shifting landscape of dunes, tree islands, ghost forests and both permanent and ephemeral lakes and rivers.
As European colonization of the Pacific Northwest grew, the new settlers and the logging and commercial fishing industries they brought with them wanted permanent towns and roads that weren't constantly being swallowed by the moving sand. Starting in the 1930s, European beachgrass and other non-native species were introduced to try to hold the dunes in place.
The invasive species did hold the dunes in place--too well. The deep roots of the beachgrass shaped the sand blowing in off the beach into a permanent dune parallel to the ocean, called the foredune.
Tumblr media
As the foredune got taller, it blocked both wind and the movement of sand, which allowed the land behind it to become grassland...
Tumblr media
then forest.
Tumblr media
Walking through this area, you might never know there was a dune under your feet. You can be close enough to hear the ocean, but there is almost NO wind--the main force that shapes the dunes.
There are (slow, difficult) remediation efforts underway to control the European beachgrass and restore at least some of the area to the natural dune cycle that created the miles and miles of open sand. But the ecological feedback loop created by introducing the beachgrass is stubborn, and without any further intervention, the dunes could be completely covered with forest in as little as a few decades. (I've heard estimates from 50 to 150 years, both of which are a blink of an eye in geological timescales.) The Oregon dunes are at least 100,000 years old, and within the span of just a few human lifetimes the ecosystem could be irrevocably changed.
The dune stabilization project is what Frank Herbert came to Florence to research for a never-written magazine article. Herbert began writing Dune in the mid-1950s, but by the mid-60s when the book was published, his own politics had shifted as he was influenced by the growing environmental movement and by Native activism happening around him in the Pacific Northwest. Like the story of the Oregon dunes, the terraforming of Arrakis is initially promoted as triumph of science and human rationality over nature that will make people's lives easier. But it ends up destroying the native ecosystem and the way of life of the planet's indigenous people, as becomes clear in Dune Messiah when Paul actually implements the terraforming project. (In the book, Dr. Kynes, the main architect of the terraforming project, dies in a spice blow--literally swallowed whole by the planet he tried to control.) It's one of the many political/ideological tensions in the story that's presented but not resolved, and I'm super curious to see how this element of the story is handled in Villeneuve's Dune Messiah.
All photos above taken by me at the Oregon Dunes National Recreation Area in September 2024.

Cigna’s nopeinator
Tumblr media
I'm touring my new, nationally bestselling novel The Bezzle! Catch me THURSDAY (May 2) in WINNIPEG, then Calgary (May 3), Vancouver (May 4), Tartu, Estonia, and beyond!
Tumblr media
Cigna – like all private health insurers – has two contradictory imperatives:
To keep its customers healthy; and
To make as much money for its shareholders as is possible.
Now, there's a hypothetical way to resolve these contradictions, a story much beloved by advocates of America's wasteful, cruel, inefficient private health industry: "If health is a "market," then a health insurer that fails to keep its customers healthy will lose those customers and thus make less for its shareholders." In this thought-experiment, Cigna will "find an equilibrium" between spending money to keep its customers healthy, thus retaining their business, and also "seeking efficiencies" to create a standard of care that's cost-effective.
But health care isn't a market. Most of us get our health-care through our employers, who offer small handful of options that nevertheless manage to be so complex in their particulars that they're impossible to directly compare, and somehow all end up not covering the things we need them for. Oh, and you can only change insurers once or twice per year, and doing so incurs savage switching costs, like losing access to your family doctor and specialists providers.
Cigna – like other health insurers – is "too big to care." It doesn't have to worry about losing your business, so it grows progressively less interested in even pretending to keep you healthy.
The most important way for an insurer to protect its profits at the expense of your health is to deny care that your doctor believes you need. Cigna has transformed itself into a care-denying assembly line.
Dr Debby Day is a Cigna whistleblower. Dr Day was a Cigna medical director, charged with reviewing denied cases, a job she held for 20 years. In 2022, she was forced out by Cigna. Writing for Propublica and The Capitol Forum, Patrick Rucker and David Armstrong tell her story, revealing the true "equilibrium" that Cigna has found:
https://www.propublica.org/article/cigna-medical-director-doctor-patient-preapproval-denials-insurance
Dr Day took her job seriously. Early in her career, she discovered a pattern of claims from doctors for an expensive therapy called intravenous immunoglobulin in cases where this made no medical sense. Dr Day reviewed the scientific literature on IVIG and developed a Cigna-wide policy for its use that saved the company millions of dollars.
This is how it's supposed to work: insurers (whether private or public) should permit all the medically necessary interventions and deny interventions that aren't supported by evidence, and they should determine the difference through internal reviewers who are treated as independent experts.
But as the competitive landscape for US healthcare dwindled – and as Cigna bought out more parts of its supply chain and merged with more of its major rivals – the company became uniquely focused on denying claims, irrespective of their medical merit.
In Dr Day's story, the turning point came when Cinga outsourced pre-approvals to registered nurses in the Philippines. Legally, a nurse can approve a claim, but only an MD can deny a claim. So Dr Day and her colleagues would have to sign off when a nurse deemed a procedure, therapy or drug to be medically unnecessary.
This is a complex determination to make, even under ideal circumstances, but Cigna's Filipino outsource partners were far from ideal. Dr Day found that nurses were "sloppy" – they'd confuse a mother with her newborn baby and deny care on that grounds, or confuse an injured hip with an injured neck and deny permission for an ultrasound. Dr Day reviewed a claim for a test that was denied because STI tests weren't "medically necessary" – but the patient's doctor had applied for a test to diagnose a toenail fungus, not an STI.
Even if the nurses' evaluations had been careful, Dr Day wanted to conduct her own, thorough investigation before overriding another doctor's judgment about the care that doctor's patient warranted. When a nurse recommended denying care "for a cancer patient or a sick baby," Dr Day would research medical guidelines, read studies and review the patient's record before signing off on the recommendation.
This was how the claims denial process is said to work, but it's not how it was supposed to work. Dr Day was markedly slower than her peers, who would "click and close" claims by pasting the nurses' own rationale for denying the claim into the relevant form, acting as a rubber-stamp rather than a skilled reviewer.
Dr Day knew she was slower than her peers. Cigna made sure of that, producing a "productivity dashboard" that scored doctors based on "handle time," which Cigna describes as the average time its doctors spend on different kinds of claims. But Dr Day and other Cigna sources say that this was a maximum, not an average – a way of disciplining doctors.
These were not long times. If a doctor asked Cigna not to discharge their patient from hospital care and a nurse denied that claim, the doctor reviewing that claim was supposed to spend not more than 4.5 minutes on their review. Other timelines were even more aggressive: many denials of prescription drugs were meant to be resolved in fewer than two minutes.
Cigna told Propublica and The Capitol Forum that its productivity scores weren't based on a simple calculation about whether its MD reviewers were hitting these brutal processing time targets, describing the scores as a proprietary mix of factors that reflected a nuanced view of care. But when Propublica and The Capitol Forum created a crude algorithm to generate scores by comparing a doctor's performance relative to the company's targets, they found the results fit very neatly into the actual scores that Cigna assigned to its docs:
The newsrooms’ formula accurately reproduced the scores of 87% of the Cigna doctors listed; the scores of all but one of the rest fell within 1 to 2 percentage points of the number generated by this formula. When asked about this formula, Cigna said it may be inaccurate but didn’t elaborate.
As Dr Day slipped lower on the productivity chart, her bosses pressured her bring her score up (Day recorded her phone calls and saved her emails, and the reporters verified them). Among other things, Dr Day's boss made it clear that her annual bonus and stock options were contingent on her making quota.
Cigna denies all of this. They smeared Dr Day as a "disgruntled former employee" (as though that has any bearing on the truthfulness of her account), and declined to explain the discrepancies between Dr Day's accusations and Cigna's bland denials.
This isn't new for Cigna. Last year, Propublica and Capitol Forum revealed the existence of an algorithmic claims denial system that allowed its doctors to bulk-deny claims in as little as 1.2 seconds:
https://www.propublica.org/article/cigna-pxdx-medical-health-insurance-rejection-claims
Cigna insisted that this was a mischaracterization, saying the system existed to speed up the approval of claims, despite the first-hand accounts of Cigna's own doctors and the doctors whose care recommendations were blocked by the system. One Cigna doctor used this system to "review" and deny 60,000 claims in one month.
Beyond serving as an indictment of the US for-profit health industry, and of Cigna's business practices, this is also a cautionary tale about the idea that critical AI applications can be resolved with "humans in the loop."
AI pitchmen claim that even unreliable AI can be fixed by adding a "human in the loop" that reviews the AI's judgments:
https://pluralistic.net/2024/04/23/maximal-plausibility/#reverse-centaurs
In this world, the AI is an assistant to the human. For example, a radiologist might have an AI double-check their assessments of chest X-rays, and revisit those X-rays where the AI's assessment didn't match their own. This robot-assisted-human configuration is called a "centaur."
In reality, "human in the loop" is almost always a reverse-centaur. If the hospital buys an AI, fires half its radiologists and orders the remainder to review the AI's superhuman assessments of chest X-rays, that's not an AI assisted radiologist, that's a radiologist-assisted AI. Accuracy goes down, but so do costs. That's the bet that AI investors are making.
Many AI applications turn out not to even be "AI" – they're just low-waged workers in an overseas call-center pretending to be an algorithm (some Indian techies joke that AI stands for "absent Indians"). That was the case with Amazon's Grab and Go stores where, supposedly, AI-enabled cameras counted up all the things you put in your shopping basket and automatically billed you for them. In reality, the cameras were connected to Indian call-centers where low-waged workers made those assessments:
https://pluralistic.net/2024/01/29/pay-no-attention/#to-the-little-man-behind-the-curtain
This Potemkin AI represents an intermediate step between outsourcing and AI. Over the past three decades, the growth of cheap telecommunications and logistics systems let corporations outsource customer service to low-waged offshore workers. The corporations used the excuse that these subcontractors were far from the firm and its customers to deny them any agency, giving them rigid scripts and procedures to follow.
This was a very usefully dysfunctional system. As a customer with a complaint, you would call the customer service line, wait for a long time on hold, spend an interminable time working through a proscribed claims-handling process with a rep who was prohibited from diverging from that process. That process nearly always ended with you being told that nothing could be done.
At that point, a large number of customers would have given up on getting a refund, exchange or credit. The money paid out to the few customers who were stubborn or angry enough to karen their way to a supervisor and get something out of the company amounted to pennies, relative to the sums the company reaped by ripping off the rest.
The Amazon Grab and Go workers were humans in robot suits, but these customer service reps were robots in human suits. The software told them what to say, and they said it, and all they were allowed to say was what appeared on their screens. They were reverse centaurs, serving as the human faces of the intransigent robots programmed by monopolists that were too big to care.
AI is the final stage of this progression: robots without the human suits. The AI turns its "human in the loop" into a "moral crumple zone," which Madeleine Clare Elish describes as "a component that bears the brunt of the moral and legal responsibilities when the overall system malfunctions":
https://estsjournal.org/index.php/ests/article/view/260
The Filipino nurses in the Cigna system are an avoidable expense. As Cigna's own dabbling in algorithmic claim-denial shows, they can be jettisoned in favor of a system that uses productivity dashboards and other bossware to push doctors to robosign hundreds or thousands of denials per day, on the pretense that these denials were "reviewed" by a licensed physician.

Edvard Munch, Kiss in the Field , 1943. Woodcut, 15 13/16 × 19 5/16 in. (40.2 × 49 cm) Sheet: 24 1/4 × 26 1/8 in. (61.6 × 66.3 cm). Munch Museum, Oslo © Munch Museum, Oslo
One year before his death, at the age of eighty, Munch printed this woodcut from a plywood block. The subject of a couple in an intimate embrace, set against a landscape, is his final variation of the motif of the kiss that he explored throughout his life. Here, his approach is virtuosic as well as experimental. He not only accepted that the grain of the wood would show in the print but relied on its flowing and swirling lines to help shape his composition. Munch developed the scene with a bare minimum of artistic intervention. By adding only the outlines of the kissing couple and indicating a low sky over the mountains, he fashioned an image that seems to oscillate between appearance and disappearance, taking shape and being not quite there.

Landscape helps capture the forms in which nations and movements literally and figuratively 'construct' or 'produce' nature, engineering its appearance and infusing it with significations—rendering landscape a 'cultural practice' rather than a given fact. Here landscape is both an object of investigation and a site of intervention; the very medium within which power and resistance are represented and conducted. Put differently, landscape is far from a neutral backdrop but is rather activated, serving as the medium of violence. Dispossession, deforestation, planting, land-grabbing, and acquisitions, privatization, re-modeling, clearance, or the destruction of infrastructures of life, including food sources, buildings, or supplies, all mobilize the landscape in their domination.
Representations of Middle Eastern and North African landscapes nearly invariably include desolate scenes of endless empty and parched deserts, decorated perhaps with an isolated string of camels, or a beach with large mounds of golden sand, a minaret, or an oil tower in the background. The temporality and general impression of these landscapes is slow, hazy, and dizzying, as if they are waiting for 'activation' by someone or something outside of it. Whether reproduced in academic scholarship, literature, film, tourist advertisements, or news media, these imagined colonial representations of the region's landscape place the environment centrally within them, projecting an understanding of the Middle East and North Africa as marginal, on the edge of ecological viability or as a degraded landscape facing imminent disaster due to human inaction. With this, an environmental imaginary enabled storytelling that pushed forward imperial interests in the name of 'development' and, later, of environmental 'sustainability' and 'protection.' In the case of the constructed 'Middle East,' as Diana K. Davis explains,
"Deforestation narratives have been particularly strong in the Levant region since the nineteenth century, where some of the most emotional accounts of forest destruction have hinged on the presumed widespread destruction of the Lebanese cedar forests illustrated in the cover image by Louis-François Cassas. Similar narratives of overgrazing and desertification were used during the British Mandate in Palestine to justify forestry policies as well as laws aimed at controlling nomads, such as the 1942 Bedouin control ordinance, in the name of curbing overgrazing. Such environmental imaginaries, once constructed, can be extremely tenacious and have surprisingly widespread effects."
In Palestine, the construction of an 'Israeli landscape' to redeem the purported damage done to the land by its indigenous population commenced with the first Zionist settlers in the nineteenth century and intensified with the establishment of the State of Israel in 1948. Reflected in former Prime Minister David Ben Gurion's 1951 public address to the newly formed Israeli Knesset (Parliament):
"We must wrap all the mountains of the country and their slopes in trees, all the hills and stony lands that will not succeed in agriculture, the dunes of the coastal valley, the dry lands of the Negev to the east and south of Baer Sheva, that is to say all of the land of Edom and the Arava until Eilat. We must also plant for security reasons, along all the borders, along all the roads, routes, and paths, around public and military buildings and facilities [ . . . ] We will not be faithful to one of the two central goals of the state—making the wilderness bloom—if we make do with only the needs of the hour [ . . . ] We are a state at the beginning of repairing the corruption of generations, corruption which was done to the nation and corruption which was done to the land."
This 'Israeli landscape' was largely cultivated through the multifaceted and by now well-documented eco-colonial practices of the quasi-governmental Israeli organization, Keren Kayemet L'Yisrael, the Jewish National Fund (JNF), which has since made striking efforts to position Israel as an environmental pioneer. Established in 1901, the JNF may very well be the first transnational environmental nationalist NGO, seeking to 'make the desert bloom' by planting forests, natural reserves, and recreational parks over the ruins of Palestinian villages, holy places, and historical sites. Distinguishing itself from other transnational Zionist organizations, such as the World Zionist Organization and the Jewish Agency, the JNF has since its inception portrayed itself as an environment-oriented nationalist organization, supporting the 'redemption' and 'reclamation' of the land through colonial policies presented in the language of preservation, maintenance, protection, and development of vital ecosystems and ecologically sound environments. Indeed, its public-facing promotional materials boast proudly that "Israel is the only country in the world that entered the twenty-first century with a net gain in the number of trees"—without context, of course, of the ways in which trees and the 'greened' landscape in the country are mobilized as weapons of erasure as part of a colonial imaginary that naturalizes non-Palestinian presence.

As (y/n) and her friends ventured through the Texas countryside, the thrill of backroading filling the air, they eventually pulled into a quiet gas station. The sun was setting, casting a warm and golden hue across the landscape.
As they parked, (y/n) couldn't help but notice a scene that caught her attention. A young man, Thomas Hewitt, sat in the driver's seat of a car, his head down, his demeanor downcast. A group of local teens stood near the car, laughing and taunting him.
A sense of injustice flared within (y/n), and without a second thought, she made her way over to where the teens were harassing Thomas. With a firm tone and an unwavering gaze, she interrupted their cruel taunts. "Hey, cut it out. Leave him alone."
The teens exchanged surprised glances, clearly taken aback by her assertiveness. They begrudgingly backed off and dispersed, their laughter fading into the background. (y/n) turned her attention to Thomas, her expression one of genuine concern.
"Are you okay?" she asked, her voice kind and understanding.
Thomas lifted his gaze to meet hers, his expression a mix of surprise and gratitude. Despite his inability to speak due to his mute condition, he appreciated her intervention more than words could express. He nodded, his eyes avoiding direct contact as a blush tinged his cheeks.
(y/n) smiled at him, unperturbed by his appearance. His disfigurement and the skin disease he struggled with didn't faze her; she saw beyond the surface and treated him with the respect and kindness he deserved.
"Don't let their comments get to you," she said gently, her tone reassuring. "You're stronger than that. And honestly, I think you look handsome."
Thomas's blush deepened, his heart hammering in his chest. He couldn't believe that someone would find him attractive despite his condition. He tried to respond, making noises and using hand gestures to communicate his gratitude, but he found it challenging.
(y/n) smiled warmly, understanding his struggle. "It's okay, no need to say anything. Just know that you're not alone, and there are people out here who see you for who you are."
Thomas's eyes met hers again, a mixture of emotions swirling within them. Gratitude, relief, and a newfound sense of hope blossomed in his heart. He might not be able to speak, but his appreciation for her actions and kind words transcended language.
As (y/n) rejoined her friends, Thomas watched her with a mix of admiration and amazement. Her act of kindness had touched him deeply, and he felt a connection he had never experienced before. And as he drove away from the gas station, he couldn't help but smile, grateful for the unexpected encounter that had brightened his day.
As the car pulled away from the gas station, the sound of the engine and the fading scenery slowly became the backdrop to Thomas's thoughts. He replayed the brief encounter with (y/n) in his mind, a mixture of emotions swirling within him.
A little while later, the door to the gas station opened again, and Thomas's brother entered the car, bringing with him a small bag of snacks. He settled into the driver's seat, casting a curious glance at Thomas.
"So, who was that girl you were talking to?" he asked, his tone casual but his eyes sparkling with curiosity.
Thomas turned to his brother, a faint smile tugging at the corners of his lips. He gestured and signed with his hands, "She defended me. She told those guys to stop. She said I looked handsome."
His brother chuckled softly, raising an eyebrow playfully. "Oh, really? Is that so?"
Thomas nodded, his blush deepening as he remembered (y/n)'s words. He had never expected someone to be so kind to him, let alone call him handsome. It was a foreign and exhilarating feeling.
His brother's grin widened, and he gave Thomas a playful nudge with his elbow. "Seems like someone's got a crush."
Thomas playfully swatted at his brother's arm, a small smile breaking through his usually reserved demeanor. He couldn't help but feel a sense of lightness, a departure from the usual challenges he faced due to his condition.
While his brother drove, Thomas continued to think about (y/n), the mysterious and kind stranger who had brightened his day. He replayed her words and the way she had defended him, feeling a warmth spread through his chest.
As the car continued down the road, Thomas couldn't shake the feeling that this encounter might have been more than just a passing moment. He had felt a genuine connection with (y/n), and he couldn't help but wonder if their paths might cross again in the future. And for the first time in a long while, he allowed himself to entertain the possibility of friendship and even something more.
