FDA Indications/Dosages:
For the prophylaxis and treatment of pulmonary embolism, the treatment of atrial fibrillation with embolism, the prophylaxis and treatment of venous thrombosis and its extension, and as an adjunct in the treatment of coronary occlusion: Dosage must be individualized. This is a potent anticoagulant medication.
Initial: 10-15 mg/day given in divided doses for 2-3 days, adjusted according to prothrombin time response. A single loading dose of 20-60 mg may be used as an alternative.
Maintenance: 2-10 mg/day given in a single or divided dose.
Monitor: INR
Pharmacology/Pharmacokinetics: Warfarin antagonizes vitamin K resulting in a decreased hepatic synthesis of the vitamin K-dependent clotting factors (II, VII, IX, X). Peak plasma levels are reached in 1-9 hours. Half-life is 2.5 days. Protein binding equals 97%. Metabolism occurs in the liver and excretion in the urine.
Drug Interactions: ENHANCED anticoagulant EFFECTS occur with alcohol, allopurinol, amiodarone, adrenocortical steroids, methyl tetrazole thiol, cephalosporins, chloral hydrate, cimetidine, clofibrate, dextrothyroxine, disulfiram, erythromycin, glucagon, metronidazole, phenylbutazone, quinidine and quinine, salicylates, sulfamethoxazole/trimethoprim, sulfinpyrazone, and thyroid medication. Variable effects are seen with thioamides. May increase the effects of hydantoins. Decreased anticoagulant effects occur with barbiturates, carbamazepine, cholestyramine, ethchlorvynol, glutethimide, griseofulvin, rifampin, and vitamin K.
Contraindications/Precautions: Use is contraindicated in pregnancy (Pregnancy Category X), in patients with hemorrhagic tendencies or blood dyscrasias, recent or contemplated surgeries, bleeding tendencies associated with active ulceration, overt bleeding of any major organ or organ system, eclampsia or preeclampsia, inadequate mental state, spinal puncture, lumbar block anesthesia, and in patients with malignant hypertension. WARFARIN INCREASES THE RISK OF FATAL BLEEDING. PERFORM REGULAR MONITORING OF INR. Use with caution in nursing mothers, in patients with hepatic or renal impairment, infections, sprue, indwelling catheters, moderate to severe hypertension, congestive heart failure, polycythemia vera, vasculitis, severe diabetes, severe allergic or anaphylactic disorders, and in patients where a situation or physical condition adds the risk of hemorrhage.
Adverse Effects: Unusual and unexplained bleeding, alopecia, dermatitis, fever, nausea, diarrhea, abdominal cramping, and urticaria.

Lạc nội mạc tử cung.
Liều lượng – Cách dùng
1 viên/ngày, bắt đầu vào bất cứ ngày nào của chu kỳ kinh, liên tục các viên, bất kể chảy máu âm đạo hay không; khi hết một vỉ, bắt đầu ngay vỉ tiếp theo không có thời gian nghỉ gián đoạn. Chống chỉ định:
Quá mẫn với thành phần thuốc. Thuyên tắc huyết khối tĩnh mạch tiến triển. Hiện có hoặc tiền sử bệnh động mạch, bệnh tim mạch, bệnh gan nặng với chức năng gan chưa trở lại bình thường, u gan. Đái tháo đường biến chứng mạch máu. Đã biết hoặc nghi ngờ ung thư phụ thuộc hormone sinh dục. Chảy máu âm đạo chưa được chẩn đoán.
Tương tác thuốc:
Tăng thanh thải hormone sinh dục (giảm hiệu quả của dienogest): Phenytoin, barbiturate, primidone, carbamazepine, rifampicin; oxcarbazepine, topiramate, felbamate, griseofulvin. Ảnh hưởng độ thanh thải của hormone sinh dục: Chất ức chế protease HIV/HCV, chất ức chế sao chép ngược không phải nucleoside. Giảm thanh thải hormone sinh dục (có thể tăng nồng độ dienogest huyết tương): Thuốc ức chế trung bình-mạnh CYP3A4 như thuốc kháng nấm nhóm azole, verapamil, macrolide, diltiazem, nước ép bưởi.
Tác dụng phụ:
Thường gặp: tăng cân; tâm trạng trầm buồn, rối loạn giấc ngủ, căng thẳng, giảm hưng phấn tình dục, thay đổi tâm trạng; đau đầu, đau nửa đầu; buồn nôn, đau bụng, đầy hơi, đầy bụng, nôn mửa; trứng cá; rụng tóc; đau lưng; khó chịu ở vú, u nang buồng trứng, bốc hỏa, chảy máu tử cung/âm đạo bao gồm ra ít máu; suy nhược, kích thích.
Chú ý đề phòng:
Loại trừ tình trạng mang thai và dừng các biện pháp tránh thai bằng hormone trước khi bắt đầu điều trị. Phụ nữ có tiền sử thai ngoài tử cung, suy giảm chức năng vòi trứng: cân nhắc lợi ích/nguy cơ. Nếu tình trạng/yếu tố nguy cơ (rối loạn tuần hoàn, khối u, thay đổi trong chảy máu kinh nguyệt, thay đổi mật độ xương, bệnh lý khác) là hiện có hoặc xấu hơn: phân tích lợi ích/rủi ro trước khi bắt đầu hoặc tiếp tục Visanne. Cân nhắc sử dụng ở bệnh nhân không dung nạp galactose, thiếu hụt Lapp lactase, kém hấp thu glucose-galactose. Không chỉ định cho trẻ em chưa có kinh nguyệt và bệnh nhân cao tuổi. Không khuyến cáo điều trị trong thời kỳ cho con bú.
Thông tin thành phần Dienogest
Tác dụng :
Dienogest là một dẫn xuất hóa học của nortestosterone. Nó có thể liên kết với các thụ thể progesterone của tử cung, mặc dù nó có ái lực nhỏ đối với phân tử progesterone trực tiếp (ái lực của các phân tử dienogest và progesterone là khoảng 10%). Do dienogest ràng buộc này có tác dụng progestogen mạnh mẽ, thể hiện bản thân trong một số hiệu ứng điều trị:

Chống chỉ định :
