Donnie got a combination of tranquilizers/sedatives/muscle relaxers. The goal was for Donnie to fall asleep entirely and be too out of it to really be fully aware of what was going on, as well as be too sedated to fight them properly. And some of that was achieved! Just not to the degree that they were hoping for. Donnie is very much out of it right now and has way less control of his body than he usually would. (Under normal circumstances, there's no way Leo would have any chance of pinning Swanatello down like he is. The only reason why he can is because Swan is pretty heavily drugged r/n.)
However, keep in mind that they're operating under pretty limiting circumstances right now. It's not like they can go to a doctor to get this stuff prescribed to them-- they have to make do with what they have and are able to scrounge up, and they have to guesstimate the dosages themselves. These are some pretty heavy drugs, and they're using them on someone who they don't really understand the anatomy of right now-- they have no idea how Swanatello's body really works anymore, or how he's going to take this. They have limited supplies, limited time, and limited information. And if you have to choose between accidentally underdosing or accidentally overdosing your family member, you're probably going to shoot for the former.

Measure 110, or the Self-Fulfilling Prophecy
So if y'all aren't local to Oregon, you may not have heard that the Oregon state legislature just voted to -- essentially -- gut Measure 110, the ballot measure which decriminalized all drug possession and use in the state. It turned all drug use into a citation instead, and the citation and fine could be waived by completing a health screening. The entire point of Measure 110 was replacing jail with health care and services to help people instead, and while I could probably write a very long side post on the imperfections of that approach, it was at the very least a move in the right direction after decades of the pathetic failure and absolutely racist mess that is the "War on Drugs."
You may hear this pointed to in coming years as a reason why we have to just throw people into jail for using drugs, because Measure 110 failed. And like... it did fail, kinda. Sorta. It failed in that it did not manage to fix everything immediately, and it created some new issues while also exposing older issues more sharply.
It also saved the state $40 million in court costs prosecuting low-level drug offenses, kept thousands of people whose literal only crime was putting a substance into the body of a consenting adult (themselves) out of jail, put at least one addiction services center in every county in the state, invested $300 million in addiction services, and an awful lot more. See the end of this post for more reading.
But where it failed, it failed because it wasn't supported. Police and advocacy groups both asked for specific tickets for this new class of offenses which had the phone number to call to go through the health screening and the information about how going through that health screening would make the ticket go away printed on it prominently - lawmakers declined to fund this. Governor Kotek budgeted $50K to train officers on how to handle these new citations and how to direct people to the treatment and housing supports, but lawmakers thought that training officers on this new law at all was a waste of money. Money moved extremely slowly out to the supports that were supposed to come into play to help people obtain treatment or get access to harm-reduction strategies. People freaked the fuck out about clean-needle outreach, fentanyl testing strip distribution, Narcan training, and other harm-reduction strategies.
And at the end of the day, Measure 110 gets called a failure because it wasn't a silver bullet. Never mind that thousands of people are not sitting in jail right now for basically no fucking reason. Never mind that people have gotten treatment, harm has been reduced, overdoses have been prevented...
So, yeah. You'll probably start hearing this trotted out as proof that, well, we triiiied decriminalizing drugs, but look what happened in Portland! Well, what happened in Oregon is that we got set up to fail, and still didn't fail, just didn't totally succeed.
Measure 110 highlights, quoted directly from Prison Policy Initiative:
The Oregon Health Authority reported a 298% increase in people seeking screening for substance use disorders.
More than 370,000 naloxone doses have been distributed since 2022, and community organizations report more than 7,500 opioid overdose reversals since 2020.
Although overdose rates have increased around the country as more fentanyl has entered the drug supply, Oregon’s increase in overdoses has been similar to other states’ and actually less than neighboring Washington’s. A peer-reviewed study comparing overdose rates in Oregon with the rest of the country after the law went into effect found no link between Measure 110 and increased overdose rates.
There is no evidence that drug use rates in Oregon have increased. A cross-sectional survey of people who use drugs across eight counties in Oregon found that most had been using drugs for years; only 1.5% reported having started after Measure 110 went into effect.
There has been no increase in 911 calls in Oregon cities after Measure 110.
Measure 110 saves Oregonians millions. Oregon is expected to save $37 million between 2023-2025 if Measure 110 continues. This is because it costs up to $35,217 to arrest, adjudicate, incarcerate, and supervise a person taken into custody for a drug misdemeanor — and upwards of $60,000 for a felony. In contrast, treatment costs an average of $9,000 per person. The money saved by Measure 110 goes directly to state funding for addiction and recovery services.
There is no evidence that Measure 110 was associated with a rise in crime. In fact, crime in Oregon was 14% lower in 2023 than it was in 2020.

It’s a cold Wednesday night in January that has Eddie turning the thermostat up and allowing the government supplied heat to fill the trailer. He glances up at the vents and gives them a quick middle finger, wondering if they bugged the place to observe him or make sure he isn’t spreading their secrets.
He doesn’t really care at this point if they’re watching though. They already held him at the hospital for long enough, poking and prodding as if he wasn’t even human. But he didn’t turn into a vampire or some shit because of those damn bats. No. The jagged, ugly scars littering his body served as a lovely reminder that he was ultimately human.
Eddie glances at a nearby mirror and cringes at his face, taking a look at the long scar running down his cheek, jaw, and neck. The Corroded Coffin guys all said it made him look metal, and he would throw in a, “Hell yeah,” before smoothly changing the subject to something that didn’t involve him for once.
He takes a finger and slowly trails it over the pale pink skin, wondering if there will ever be a day he won’t notice it.
“Eddie,” Steve calls out gently from the room down the hall.
Eddie jumps back and glances toward him, hand falling to his side and flexing uncomfortably as if he’s been caught doing something wrong.
“You okay?”
Eddie smiles and gestures toward the thermostat. “Damn thing wasn’t working for a minute there. You’d think with the amount of hush hush money they were able to pay all of us, they’d be able to give me and Wayne a better trailer.”
But Steve only crosses his arms and leans against the door frame, eyebrows raising gently. It’s not entirely accusatory, but it’s clear that he doesn’t believe a thing Eddie’s saying.
Although they’ve grown close while going through the same treatment and tests in Owen’s new secret facility, it still surprises Eddie how easily Steve can read people. More specifically, how easily he can read him of all people. “Just got lost in thought,” Eddie confesses while making his way back to his room as he sees Steve squint at the lights in the living area.
Steve steps out of the way as Eddie brushes by him and closes the door. He hope it’s enough honesty to end the conversation.
“What were you thinking about?” Steve asks, ignoring the signals Eddie is giving him.
Eddie sighs and runs a hand over his face and climbs back into his bed, quick to pull up the blanket around himself in an attempt to get some much needed warmth while simultaneously covering his scars from Steve. “Stuff.”
Steve rests his hands on his hips for a second and stares, mouth opening and closing for a moment before deciding against whatever he was going to say. Instead, he climbs into the bed with Eddie and joins him under the blanket, keeping enough distance so they’re not touching, but they can still feel each other’s body heat.
Eddie glances over at him, noticing the way the one lamp turned on in the room gives him a nice golden halo. He looks gorgeous and untouchable - exactly how Eddie used to think of him through high school and sometimes even now. The perfect golden boy. But despite the name Eddie gave to him years ago, he can’t ignore the flaws that Steve possesses, yet they somehow make him even more perfect to him. Or maybe just human.
Eddie shakes his head and glances away. He wishes Steve came over to smoke so Eddie could blame the drugs on the way his thoughts race when he’s next to him. Instead, he has to face up to his enormous crush on the perfect golden boy.
“Have any plans for Valentine’s Day?” Steve asks out of the blue.
Eddie snorts and glances at him, only to laugh harder when he sees the adorable look of confusion on his face.
Steve’s brows furrow but the edges of his lips quirk up. “What?”
Eddie pulls a strand of hair in front of his face to try to hide his wife smile. “Kind of random, don’t you think?”
Steve rolls his eyes. “I never said I was great at starting conversations. But I was just thinking about what holiday is next.”
“The worst one,” Eddie complains.
Steve turns toward him. “And why’s that?”
Eddie sighs and let’s himself go on a tangent. “It’s the one day of the year where people feel like they have to do all this shit for their partner, and the rest of the year, they think they can just get by doing the bare minimum. And people are left realizing what it would be like if their ‘other half’ actually put in an effort day to day. And then for all the single people, it’s a day where love is shoved in their face, and they have to feel bad and sometimes disgusted by all the public displays of affection going on around them and… I just hate it all. The stupid chocolates in the red heart boxes and the teddy bears and big heart shaped balloons and roses…”
“I didn’t realize you had such strong opinions about Valentine’s Day,” Steve says with a laugh.
“Well, now you do.”
They both sit in the silence for a few moments, Eddie thinking about all the other things he didn’t even touch on about Valentine’s Day that he hates, while Steve is probably taking in everything he just said.
Steve bumps his shoulder and asks, “So, I’m assuming that means you have no plans.”
Eddie laughs. “That’s what you got out of that?”
Steve shrugs and looks away with a smile.
Eddie glances at his clock and notices it’s technically Thursday now, and in these early hours, Steve will usually either silently fall asleep or he’ll lay awake in the silence until one of his thoughts has to make itself known.
Either way, Eddie knows he’ll be up for a few more hours, but he’s never regret the sleep he’s lost since they’ve made this silent arrangement.
The bed shifts, and Eddie follows Steve’s lead, laying down fully and staring at the ceiling, trying his best not to reach out for the hand laying beside his. He wonders if he should add something to the ceiling like some type of mural with stars and whatnot.
He tilts his head to the side, envisioning how it would look in the lamplight since he and Steve refuse to sleep in the dark. Or maybe it’s just Steve and Eddie’s picked up on the habit of leaving the lamp on.
“Do you think you’ll make plans?” Steve asks quietly.
Eddie turns to look at him, at a lost for a moment before realizing he’s still on the Valentine’s Day subject. He smiles sadly, “No.” Steve glances over at him and holds his gaze, expecting more. Eddie sighs and gestures at himself. “I mean, I’m not exactly what people want to bring home to their parents at the moment plus with the,” he gestures to his face and drops his hand quickly, averting his gaze back to the ceiling.
He hopes Steve will let it go and not connect the dots back to earlier.
A silence settles between them, but Steve’s gaze burns into the side of Eddie’s face. Then, he finally asks, “Is that what distracted you earlier?” When Eddie doesn’t answer he continues, “I saw you looking in the mirror, and I know you usually go out of your way to avoid them.”
Eddie wants to question how Steve noticed, but he doesn’t want to get his hopes up about something that was just passively rather than intentionally observed. “Yeah,” Eddie states simply.
The bed shifts as Steve turns on his side to fully face him. “And you really think you can’t get a date because of them?”
Eddie sighs and rubs both of his hands over his face. “Steve, who is ever going to love me like this?” He turns and continues, “Privately, yes, someone could maybe get past the scars. But in public? You really think someone is going to be proud to say, ‘Here’s my boyfriend,’ and show off me?”
“Yes,” Steve says instantly as if he has no doubt in the world.
Eddie turns away, trying not to get choked up about it. Because how can he explain to him that while it’s nice that Steve has that confidence in him, Eddie wants Steve to be the one to be proud of him. To want him like that.
“Do you think my scars make me unlovable?” Steve asks.
“No! Jesus, Steve,” Eddie rushes to say and turns to him. He reached out and lays a hand over his side, feeling the way the skin puckers under the thin t-shirt. “These are metal as hell. Hot even. They make you more lovable if anything.”
Steve grabs Eddie’s hand and slowly pulls it off his side to hold it up, the scar on it being presented out to Eddie. “And this doesn’t make you more lovable?”
“Steve…” Eddie protests quietly as Steve pulls his hand close to stare at it.
“The scars you got protecting us. You think those make you less lovable?” Steve asks, pulling his hand close enough that his lips ghost over the skin.
Eddie lets out a breath that sounds like Steve as Steve presses a soft kiss into the tough skin. He stares at Eddie with a worried look in his eyes as he whispers, “Too much?”
Eddie shakes his head, too stunned to get the words out.
Steve intertwines their hands and pulls Eddie’s arm toward him. “These scars,” he says kissing the next one on his forearm, “Are beautiful on you.” He moves on to the scar on his elbow stretching to his bicep, lips trailing against the sensitive unmarked skin and leaving a trail of goosebumps behind. “How could anyone hate these?” He asks leaving three soft, lingering kisses before shifting on the bed to hover above Eddie, still holding his hand but now against his stomach so he can press a kiss against his shoulder. “These scars show everyone what you were willing to sacrifice for us.”
As Steve moves to the scar on his neck, Eddie’s head drops back, giving him more access as he groans out, “Steve.”
“These scars,” Steve says, kissing up his neck over and past the scars, “Are exactly,” he murmurs as he kisses past his jaw and peppers kisses up his cheek, “Why I love you,” Steve finishes by pressing a kiss to the corner of his mouth near where the scar that Eddie traced earlier ends.
Eddie glances up at Steve as he hovers over him, trying to make sense of everything he’s saying until it finally clicks. “You love me?” Eddie asks.
Steve nods and squeezes his hand before letting it go so he can lightly caress his cheek. “I have since you decided to be a hero and sacrifice yourself. Which was exactly what I told you not to do by the way.”
“I’ve never been great at following rules,” Eddie breathes out and reaches a hand up to run through Steve hair. “Steve?”
“Mhm?”
“I love you, too,” Eddie confesses.
Steve smiles and asks, “Yeah?”
“Yeah,” Eddie says. He pauses before adding, “You know, one time when I was little, I captured a squirrel and it may have attacked me and left a scar on my lip.”
Steve laughs. “Is that so?”
Eddie smiles and nods.
Steve’s eyes dart down to Eddie’s lips and he moves his thumb to swipe over his top lip. “You know, I think I see it.”
Eddie debates telling him that it was actually his bottom lip, but instead he just breathes out, “Steve.”
“Yeah?” Steve asks with a teasing smile.
“Steve.”
Steve keeps smiling as he hums, “Hmm?” When Eddie huffs, Steve fakes surprise with a gasp, “Oh. You want me to find the picture for you!”
Eddie groans, “Steve!”
“Uh huh?”
Eddie huffs and cups his face. “You are infuriating.”
“Is that s-”
Eddie interrupts him by taking matters into his own hands and leaning up to kiss him. He feels Steve smile against his lip before finally kissing him back.
Steve pulls away and breathlessly asks, “So, do you think you’ll have plans for Valentine’s Day now?”
Eddie’s head thumps back on the pillow. “Oh my god.” Steve laughs. “Oh my god!” Eddie says and shoves Steve off of him only to roll over so he hovers above him. “You were trying to ask me out this whole time?”
“No, I just wanted to know your opinion of Valentine’s Day.”
Eddie gives him a light punch to the arm and smiles wide as he stares down at Steve, lying beneath him in the golden lamp light. His perfect golden boy.
“I still hate it by the way. Even if I have plans now,” Eddie comments seriously.
“Don’t worry, I’ll put in the effort year round for you and make sure to keep you away from the public that day,” Steve says running a hand through Eddie’s curls before tracing it down the scar on his cheek in a way he thought no one would be able to do - lovingly.
Eddie leans down and gives Steve a quick peck. “I’ve also got some scars on my hips I might want you to check out.”
Steve laughs loudly and pulls him into a kiss that truly makes Eddie breathless, all while tracing his hand over the scar on his cheek. And for the first time, Eddie learns to love the scars adorning his body.

You and Wanda are currently in Bari, Italy, on a mission to find locate and arrest a dangerous drug dealer that is rumored to be heading towards London for an important deal.
Your goal is to find his base of operations and destroy his current supply. The suspect in question, Cecil Adams, otherwise known as 'The Count', has been injecting his victims with his test products.
Almost all of them ended up dead within minutes.
"Oh come on."
"It's for the good of the people" Tony said and you rolled your eyes.
"So me and Wanda have to cancel our honeymoon just to find some wack ass Peter Piper? Why can't the police just deal with it?" 
"They've tried, but every officer they sent has come back in a casket. You know we wouldn't have asked you if we didn't need to, but the rest of the team is already assigned elsewhere." Steve informed, crossing his arms and expressing a look of sympathy.
You groaned as you buried your face in your hands in annoyance. Wanda grabbed your hand, stroking the back of it her thumb as an attempt to help you calm down.
"A week in Italy wouldn't really be so bad, just think of it as a last minute change of venue." She whispered and you brought your head up to look at her and her bright green eyes.
"I... alright, fine." You couldn't bring yourself to argue with her, turning back to look back at Tony and Steve.
"You owe us two weeks off after this." Both men nodded in agreement.
Four days into your stay, you and Wanda had found his headquarters, an abandoned wear-house at the edge of the city.
You and your wife are rigging the place to blow. Members of S.H.E.L.D waiting just outside of the property as backup.
Wanda had used her magic to knock out the guards surrounding the area, putting handcuffs onto each of them, and with a snap of her fingers, she'd sent them straight into the secured van.
"You done, baby?" You ask Wanda through your earpiece.
"Everything's all set over here"
"Alright then, just make sure you're safe ok?"
You back away from the building with the remote trigger in your hand, your backpack filled with product in the other, you'd managed to sneak in prior and retrieve some per S.H.E.L.D's instructions.
"I will be detka, are you?"
"I'm safe" You assure her as you take small steps backward and away from the building.
Suddenly you feel a cool piece of metal brace itself against your throat, strong muscular arms wrapping themselves around you.
You sigh, reaching for the man's wrists and throwing him on the ground, the knife in his hand flying into the distance.
You hear him groan before you flip him on his back, grabbing a pair of handcuffs from your bag and putting his hands behind his back, locking him in place.
"For someone who's created such a hassle, you really need better fighting skills." You mutter, Wanda picking everything through the mic.
You cuff him and grab him by his hair, holding his head up. Pulling a chloroform-laced cloth from your pocket, you knock him unconscious.
"Luybov? What happened?"
"We've caught our culprit." You chuckle as you picked the man up, throwing him over your shoulder and heading towards the front of the warehouse.
You set Adams down, his body limp against the cemented ground.
"Ready?"
"Ready as I'll ever be."
"5... 4... 3... 2... 1..."
Boom.
The warehouse erupts into a riot of flames, painting the area in orange and red.
You notice a puff of smoke head in your direction but it's unlike anything you've seen before, it was... pink?
You tried your best to avoid it but you couldn't, the cloud reaching you and the moment you inhaled it, you cough heavily.
Wanda uses her powers to contain the fire and puts it out, red wisps of her magic encasing the remaining smoke, clearing it into thin air.
Once you can finally breathe, you turn to pick Adams up and hand him into S.H.E.I.L.D's custody. A group of agents taking him into the back of the van along with rest of his henchmen.
You hand one agent your bag of evidence, receiving a nod in thanks before they get inside and drive off.
"You alright?" You feel Wanda's hand grab your shoulder from behind you, her voice is laced with so much love and a hint of worry that makes you fall even deeper for this woman.
You nod, turning to face her and wrapping your arms around her neck, pulling her in for a desperately lustful kiss.
The moment your lips make contact you can't help the moan you let out at the sensation. When you pull apart, she noticed how of how your pupils have dilated drastically.
"I think it's time we go back to the hotel, don't you agree?" Your fingers playing with the pendant of her necklace, Wanda smiles back at you and nods, giving you another peck that makes your heart skip a beat.

She brushes her hand past your ear making your breath hitch, disappoint running though you when you realize she was only trying to grab your earpiece, taking her's out right after and putting them in her pocket.
Hand in hand, you both head to car that Tony had lent you for your time here. Wanda opens the door for you, a blush painting your cheeks at her chivalry.
She then heads into her spot in the drivers seat, starting up the car and putting the hotels address in the built-in GPS system.
Once on your way, you feel Wanda's hand on your thigh, giving it a light squeeze. You were starting to lose your composure, though the act was innocent, the heat in your pants was becoming unbearable.
"Oh my poor baby," she coos, "I can read your thoughts; they're quite loud."
"Are you desperate for me?"
You feel your eyes shut for a moment at her accent slipping into her words, your thighs clenching at the rasp in her voice.

If there's street medic collectives, mutual aid groups or groups like Food not Bombs in your area, you can ask people in them who might know where to find harm reduction services in your area!
Drug Users Unions:
Drug users unions are activist groups made for people who use drugs, by people who use drugs! Drug users unions do advocacy work to end criminalization, as well as providing vital community support. Many drug users unions are also inclusive of sex workers and work to decriminalize sex work as well. You can search for "drug users union" in your state.

Let me share you some examples of people outside of a spiritual realm using the law of consciousness. Reading about placebo opened my eyes to realize whether I believe it or not, use it or not, it is always operating.
1. During wartime, particularly in World War II, when medical supplies were limited, the use of a saline solution as a placebo became prevalent. One notable figure associated with this practice is Henry Beecher, a medic during the war. When morphine, a powerful painkiller, was scarce, Beecher resorted to injecting injured soldiers with a saline solution (a mixture of salt and water) as a substitute.The fascinating observation was that many soldiers responded positively to the saline placebo, reporting a reduction in pain. Beecher’s experience led him to further investigate what is now known as the placebo effect. He discovered that even inert substances like saline could elicit a therapeutic response in individuals, highlighting the power of belief and the mind’s influence on healing. Using saline as a placebo during wartime was a practical solution to address the scarcity of medical resources. It allowed healthcare providers to provide some form of treatment while conserving limited supplies for critical cases. The phenomenon observed in these wartime placebo administrations contributed to our understanding of the placebo effect and its role in medical practices.
2. And then there was another placebo test done with surgeries demonstrated the power of the placebo effect in the context of surgical interventions for knee pain.
The study, often referred to as the “fake leg surgery” study, focused on patients with osteoarthritis in the knee. Participants were randomly assigned to either receive real arthroscopic surgery or undergo a sham procedure where no actual surgical intervention took place. The sham surgery involved making small incisions and mimicking the actions and sounds associated with the actual procedure.The surprising finding was that both groups, those who underwent real surgery and those who had the sham surgery, reported similar improvements in their knee pain and functionality. This suggested that the positive outcomes experienced by the participants were not necessarily due to the physical intervention but rather to psychological factors such as the placebo effect.
3. The most fascinating one was this one: The study aimed to explore the role of mindset in reversing some aspects of aging.
In this experiment, Langer and her team created a simulated environment reminiscent of the 1950s to immerse a group of elderly participants. The participants were instructed to act as though they were 20 years younger and encouraged to engage in activities that required physical and mental activity. It aimed to create an atmosphere where the participants felt as if they were stepping back in time.The results of the experiment were described as astonishing. Participants reportedly experienced improvements in various areas, including physical health, cognition, and overall well-being. The study suggested that by changing one’s mindset and engaging in an environment that challenges typical aging stereotypes, individuals may experience positive effects on various aspects of their lives.
4. The Man Who Overdosed on Placebo" is a story about a 26-year-old man, often referred to as "Mr. A," who was part of a clinical trial for an antidepressant drug. In a desperate state of mind, he attempted suicide by ingesting 29 capsules of what he believed to be the experimental drug. This act was triggered by his depression, which had worsened after a breakup with his girlfriend.
However, unbeknownst to him, the pills he had taken were not the actual antidepressant, but rather placebos - essentially inert substances, often sugar pills, used in clinical trials as a control group. Despite this, Mr. A's vitals showed alarming signs similar to those of a drug overdose, reflecting the power of belief over the physical body, a phenomenon known as the "nocebo effect."
The nocebo effect is essentially the evil twin of the placebo effect. While the placebo effect can lead to improvements in health due to positive expectations, the nocebo effect can cause negative symptoms or even exacerbate existing ones due to negative expectations. In this case, Mr. A exhibited symptoms of an overdose solely because he believed he had taken an overdose.
5. Sam Londe, is one of the best but sad classic example of the nocebo effect, as detailed in Dr. Joe Dispenza's book "You Are the Placebo."
Sam Londe was diagnosed with esophageal cancer, a condition known for its grim prognosis. His doctors informed him that he didn't have much time left to live. Accepting this diagnosis, Londe quickly became bedridden and his health deteriorated rapidly, following the trajectory his doctors had predicted.However, upon his death, an autopsy revealed a surprising fact: there was not enough cancer in his body to have caused his death. Testing conducted by the Food and Drug Administration on pasteurized commercially purchased milk has found genetic evidence of the H5N1 bird flu virus, the agency confirmed Tuesday. But the testing, done by polymerase chain reaction, or PCR, cannot distinguish between live virus or fragments of viruses that could have been killed by the pasteurization process.
The agency said it has been trying to see if it could grow virus from milk found to contain evidence of H5N1, which is the gold standard test to see if there is viable virus in a product. The lengthy statement the agency released does not explicitly say FDA laboratories were unable to find live virus in the milk samples, but it does state that its belief that commercial, pasteurized milk is safe to consume has not been altered by these findings.
“To date, we have seen nothing that would change our assessment that the commercial milk supply is safe,” the statement said.
The document was long on assurances but short on details of what has been undertaken or found. It does not specify how many commercial samples were taken or in how many markets, nor does it indicate what percentage of the samples were PCR-positive for H5N1. The statement did not indicate if the testing suggested the amounts of viral genetic material in the milk were low or high.
Furthermore, the statement did not reveal if the milk products were purchased in parts of the country where outbreaks have occurred, or in areas where cows haven’t been seen to have been infected.

Got hit by the Adderall shortage lads.
I raised my voice to the pharmacy technician and her boss said, "You have been yelling at her for five minutes now."
I wasn't yelling, and that just made me way angrier. Oh, is it frustrating to have to spend five minutes dealing with an unhelpful person? Is five minutes a lot of precious time that you won't get back? Wow, I'm so sorry, I'll get out of your hair and go spend half an hour calling other pharmacies individually to see if they have any supply, and when it turns out they don't I'll just wait a few days and see if the shipment you're about to get has Adderall in it. And if it doesn't I'll spend another hour or so calling around to figure out what I'm supposed to do.
Gosh, it sure must be frustrating for you to spend five minutes on this, I can't imagine.
You know what pisses me off? I get that it's not your fault that the FDA and DEA have their heads buried up their own asses.
You know what is your company's fault? The fact that you have 12 locations plausibly within driving distance of me and the procedure to see whether an alternate location can fill the prescription is for me to look up each one individually, call them, wait on hold, and then ask each individual pharmacist. There's no way to track the supply, or to find out if it's committed to other patients, and that's nothing to do with the shortage.
After talking to several pharmacists the answer I got several times was, "Yeah they can't tell us what's going to get delivered ahead of time, a lot of times they say they have stock and then when we get the shipment it just randomly doesn't have some of what we ordered."
Like, I'm not just pissed off at the shortage, I'm pissed at this really bizarre inability to track whether anybody has it or doesn't have it. Isn't this a super dangerous drug that the FDA and DEA have to monitor closely? But the warehouse you order from just... doesn't track how much they have or where it goes?
I can't emphasize this enough, I've been told by multiple pharmacies that not only do they not know when they are getting more Adderall, they don't know when they'll know if they are getting more Adderall. They can't give you a no, they can only say, "We're getting a shipment of drugs on Thursday and it might have Adderall in it or it might not, we don't know until then."
And since it's a controlled substance, your doctor has to specify which pharmacy you are getting the drug from, if you find another pharmacy that has it you have to first call your doctor and then have them send a new prescription to that particular pharmacy.
There was a lot of this during COVID also, getting tests and things. Not just a shortage of supplies, a completely decrepit infrastructure which left each individual out in the cold without information about where supplies might be or how you would find them.
Like, why is the procedure for patients, "Just call every pharmacy in the city to see if they have some"? That's not caused by supply shortages.

Cigna – like all private health insurers – has two contradictory imperatives:
To keep its customers healthy; and
To make as much money for its shareholders as is possible.
Now, there's a hypothetical way to resolve these contradictions, a story much beloved by advocates of America's wasteful, cruel, inefficient private health industry: "If health is a "market," then a health insurer that fails to keep its customers healthy will lose those customers and thus make less for its shareholders." In this thought-experiment, Cigna will "find an equilibrium" between spending money to keep its customers healthy, thus retaining their business, and also "seeking efficiencies" to create a standard of care that's cost-effective.
But health care isn't a market. Most of us get our health-care through our employers, who offer small handful of options that nevertheless manage to be so complex in their particulars that they're impossible to directly compare, and somehow all end up not covering the things we need them for. Oh, and you can only change insurers once or twice per year, and doing so incurs savage switching costs, like losing access to your family doctor and specialists providers.
Cigna – like other health insurers – is "too big to care." It doesn't have to worry about losing your business, so it grows progressively less interested in even pretending to keep you healthy.
The most important way for an insurer to protect its profits at the expense of your health is to deny care that your doctor believes you need. Cigna has transformed itself into a care-denying assembly line.
Dr Debby Day is a Cigna whistleblower. Dr Day was a Cigna medical director, charged with reviewing denied cases, a job she held for 20 years. In 2022, she was forced out by Cigna. Writing for Propublica and The Capitol Forum, Patrick Rucker and David Armstrong tell her story, revealing the true "equilibrium" that Cigna has found:

Dr Day took her job seriously. Early in her career, she discovered a pattern of claims from doctors for an expensive therapy called intravenous immunoglobulin in cases where this made no medical sense. Dr Day reviewed the scientific literature on IVIG and developed a Cigna-wide policy for its use that saved the company millions of dollars.
This is how it's supposed to work: insurers (whether private or public) should permit all the medically necessary interventions and deny interventions that aren't supported by evidence, and they should determine the difference through internal reviewers who are treated as independent experts.
But as the competitive landscape for US healthcare dwindled – and as Cigna bought out more parts of its supply chain and merged with more of its major rivals – the company became uniquely focused on denying claims, irrespective of their medical merit.
In Dr Day's story, the turning point came when Cinga outsourced pre-approvals to registered nurses in the Philippines. Legally, a nurse can approve a claim, but only an MD can deny a claim. So Dr Day and her colleagues would have to sign off when a nurse deemed a procedure, therapy or drug to be medically unnecessary.
This is a complex determination to make, even under ideal circumstances, but Cigna's Filipino outsource partners were far from ideal. Dr Day found that nurses were "sloppy" – they'd confuse a mother with her newborn baby and deny care on that grounds, or confuse an injured hip with an injured neck and deny permission for an ultrasound. Dr Day reviewed a claim for a test that was denied because STI tests weren't "medically necessary" – but the patient's doctor had applied for a test to diagnose a toenail fungus, not an STI.
Even if the nurses' evaluations had been careful, Dr Day wanted to conduct her own, thorough investigation before overriding another doctor's judgment about the care that doctor's patient warranted. When a nurse recommended denying care "for a cancer patient or a sick baby," Dr Day would research medical guidelines, read studies and review the patient's record before signing off on the recommendation.
This was how the claims denial process is said to work, but it's not how it was supposed to work. Dr Day was markedly slower than her peers, who would "click and close" claims by pasting the nurses' own rationale for denying the claim into the relevant form, acting as a rubber-stamp rather than a skilled reviewer.
Dr Day knew she was slower than her peers. Cigna made sure of that, producing a "productivity dashboard" that scored doctors based on "handle time," which Cigna describes as the average time its doctors spend on different kinds of claims. But Dr Day and other Cigna sources say that this was a maximum, not an average – a way of disciplining doctors.
These were not long times. If a doctor asked Cigna not to discharge their patient from hospital care and a nurse denied that claim, the doctor reviewing that claim was supposed to spend not more than 4.5 minutes on their review. Other timelines were even more aggressive: many denials of prescription drugs were meant to be resolved in fewer than two minutes.
Cigna told Propublica and The Capitol Forum that its productivity scores weren't based on a simple calculation about whether its MD reviewers were hitting these brutal processing time targets, describing the scores as a proprietary mix of factors that reflected a nuanced view of care. But when Propublica and The Capitol Forum created a crude algorithm to generate scores by comparing a doctor's performance relative to the company's targets, they found the results fit very neatly into the actual scores that Cigna assigned to its docs:
The newsrooms’ formula accurately reproduced the scores of 87% of the Cigna doctors listed; the scores of all but one of the rest fell within 1 to 2 percentage points of the number generated by this formula. When asked about this formula, Cigna said it may be inaccurate but didn’t elaborate.
As Dr Day slipped lower on the productivity chart, her bosses pressured her bring her score up (Day recorded her phone calls and saved her emails, and the reporters verified them). Among other things, Dr Day's boss made it clear that her annual bonus and stock options were contingent on her making quota.
Cigna denies all of this. They smeared Dr Day as a "disgruntled former employee" (as though that has any bearing on the truthfulness of her account), and declined to explain the discrepancies between Dr Day's accusations and Cigna's bland denials.
This isn't new for Cigna. Last year, Propublica and Capitol Forum revealed the existence of an algorithmic claims denial system that allowed its doctors to bulk-deny claims in as little as 1.2 seconds:

Cigna insisted that this was a mischaracterization, saying the system existed to speed up the approval of claims, despite the first-hand accounts of Cigna's own doctors and the doctors whose care recommendations were blocked by the system. One Cigna doctor used this system to "review" and deny 60,000 claims in one month.
Beyond serving as an indictment of the US for-profit health industry, and of Cigna's business practices, this is also a cautionary tale about the idea that critical AI applications can be resolved with "humans in the loop."

In this world, the AI is an assistant to the human. For example, a radiologist might have an AI double-check their assessments of chest X-rays, and revisit those X-rays where the AI's assessment didn't match their own. This robot-assisted-human configuration is called a "centaur."
In reality, "human in the loop" is almost always a reverse-centaur. If the hospital buys an AI, fires half its radiologists and orders the remainder to review the AI's superhuman assessments of chest X-rays, that's not an AI assisted radiologist, that's a radiologist-assisted AI. Accuracy goes down, but so do costs. That's the bet that AI investors are making.
Many AI applications turn out not to even be "AI" – they're just low-waged workers in an overseas call-center pretending to be an algorithm (some Indian techies joke that AI stands for "absent Indians"). That was the case with Amazon's Grab and Go stores where, supposedly, AI-enabled cameras counted up all the things you put in your shopping basket and automatically billed you for them. In reality, the cameras were connected to Indian call-centers where low-waged workers made those assessments:

Over the past three decades, the growth of cheap telecommunications and logistics systems let corporations outsource customer service to low-waged offshore workers. The corporations used the excuse that these subcontractors were far from the firm and its customers to deny them any agency, giving them rigid scripts and procedures to follow.
This was a very usefully dysfunctional system. As a customer with a complaint, you would call the customer service line, wait for a long time on hold, spend an interminable time working through a proscribed claims-handling process with a rep who was prohibited from diverging from that process. That process nearly always ended with you being told that nothing could be done.
At that point, a large number of customers would have given up on getting a refund, exchange or credit. The money paid out to the few customers who were stubborn or angry enough to karen their way to a supervisor and get something out of the company amounted to pennies, relative to the sums the company reaped by ripping off the rest.
The Amazon Grab and Go workers were humans in robot suits, but these customer service reps were robots in human suits. The software told them what to say, and they said it, and all they were allowed to say was what appeared on their screens. They were reverse centaurs, serving as the human faces of the intransigent robots programmed by monopolists that were too big to care.
AI is the final stage of this progression: robots without the human suits. The AI turns its "human in the loop" into a "moral crumple zone," which Madeleine Clare Elish describes as "a component that bears the brunt of the moral and legal responsibilities when the overall system malfunctions":

The Filipino nurses in the Cigna system are an avoidable expense. As Cigna's own dabbling in algorithmic claim-denial shows, they can be jettisoned in favor of a system that uses productivity dashboards and other bossware to push doctors to robosign hundreds or thousands of denials per day, on the pretense that these denials were "reviewed" by a licensed physician.

now! fundamentally why i will never align with this kind of perspective is that i affirm addiction as a social construct, like all so-called mental illnesses, & the psychiatric institution which invents & reifies them as a fucking sham.
answer quickly:
what substances is it possible for one to become addicted to? does this include caffeine? why or why not?
is the claim of sugar addiction legitimate or anti-fat pseudoscience? what, if anything, differentiates this from other addiction science?
what is the harm of the so-called opioid epidemic: access to a safe supply of narcotics, or the lack thereof?
can an autistic person who eats the same dinner every night, for example, be said to be “psychologically addicted” to it if they have a meltdown & subsequent ongoing distress + disinterest in food when it is discontinued?
can you be addicted to psychiatric medication? immunosuppressants? why or why not?
my point is less that these behaviors are not indicative of addiction but rather that that wouldn’t inherently make them harmful. fuck it, let’s take it point by point!
planning your day around drugs e.g "i'll give myself an extra half hour before heading out so i can get high first"
this whole post had me asking “literally what is the problem with this,” starting with this first bullet! why does someone need to leave for the grocery store at 5:30 instead of 6, or whatever? and the other recurring theme: what happens if you replace “drugs” with “pain management”? (chronic pain is not the only valid reason to get high—all reasons for drug use are equally value-neutral—but it certainly still is one.) “i’ll give myself an extra half hour before heading out for my pain management to start working” is the kind of calculation familiar to most people with chronic pain. “stop and take one day a week without pain management” is not a test of whether you “need help,” it’s torture.
now, disregarding one’s priorities or commitments to other people in favor of drugs can happen, & in many circumstances it’s harmful to the other people impacted. that’s not what was said here, & stopping that behavior does not require getting sober.
rapidly switching emotions around drugs. you love them but you hate that you love them so much. you hate the way you feel on them but you hate being sober. feeling guilty after using even when you didn't give a crap beforehand.
do you know what else i love but hate that i love, what else i hate using? my fucking bed. three years ago, my mobility scooter. this is not a logical argument, this is a bullshit argument. my feelings about something do not inherently reflect its harm to others – or to myself, even, though i firmly argue for the right to make “self-harmful” decisions regardless.
you know what people hate being on but hate worse being off? the vast fucking majority of medications.
why might a drug user start to feel guilty when they previously didn’t? being shamed by friends, family, or a fucking tumblr post; surpassing a constructed threshold of “acceptable” use they didn’t know they’d internalized; experiencing new or greater access issues; beginning to probe their morality around drugs & unpack things they were taught; experiencing consequences of criminalization; getting triggered.
caring less about spending money. if you are budgeting for drugs like they are food, you are likely prioritizing them more than is healthy.
“if you are budgeting for pain management like it’s as important as food, you are likely prioritizing it more than is healthy.” health is absolutely useless as a value for me anyway, but: the food’s no good if i’m too nauseous or too dead to eat it.
prioritizing drugs over other people’s financial needs is harmful! this wouldn’t happen if food & drugs were provided to people; some people wouldn’t need as many drugs if their needs were met otherwise; people’s needs being met shouldn’t be dependent on their parent / partner / self not using drugs; this harm is not what the bullet says.
getting high to do household chores and other unpleasant things because it would suck less and be more bearable on drugs

feeling anxious or restless while sober, not knowing what to do with oneself, feeling lost or ungrounded.
again just. what’s the problem with that. so what if being sober sucks or is boring or stressful or demanding. so what if someone decides to deal with that sober or decides to use more because of that. who gives a shit.
thinking about doing drugs constantly even while sober. maybe it's the first thing you think of when you wake up. maybe when you're bored or otherwise have free time, drugs are one of the first things you can think of to occupy yourself with.

again like. what is the problem with doing drugs because you’re bored. why do i need to occupy myself, what, fucking productively?
going to work or school while under the influence, especially if it happens regularly and if you're seeing your performance suffer as a result.
what’s wrong with going to school high. derailing a class discussion is a dick move, maybe, but that’s not inherent to being high. work & performance are both very broad terms – a surgeon or someone operating heavy machinery not being sober is putting others at risk of harm in a way a cashier is not.
the idea of taking a 'tolerance break' sounds good to you until it's actually break time, at which point you can come up with 20 very reasonable sounding points to explain why it wouldn't benefit you actually and you should just keep doing drugs regardless.
y’all think this is incredibly circular logic too right? “drugs are bad, so telling yourself drugs are not bad is proof that they’re bad.” took me right back to the sunday school classroom and i wish i was fucking exaggerating. it’s an argument founded upon the inherent wrongness of trusting yourself – what you want to do must be wrong because you want it. this is one of the points that’s a more solid indicator of, like, “congrats! you’re now in circumstances doctors are salivating to psychiatrize as XYZ Use Disorder,” but that doesn’t make it any less nonsense as a moral argument.
even if you succeed at quitting the drug, you keep your dealer's number on your phone "just in case"
so what. what’s wrong with giving yourself the continual autonomy to choose whether or not to do drugs. what’s wrong with quitting drugs for a while and starting using again.
you pretend to be sober when you aren't. you worry about other people noticing how much time you spend high. you make efforts to hide your drug use or minimize how much other people think you're using. you're scared of other people's judgement if they were to find out.
this one might be the most ludicrous to me, which is really saying something. “if other people being bigoted towards drug users makes you pretend to use less than you do, that’s your fault & not theirs.” cool! thanks for the quick heads up to not believe a word you say!
you have mood swings laced with self-hatred, regret, financial worries, and guilt. these mood swings are then very quickly wiped away by feelings of "but it doesn't matter, i can do what i want, and clearly i'm doing just fine while using drugs frequently". news flash, if you are rapidly switching between feeling numb-ok and hating yourself more than anything because of your drug use, you are mentally ill.
again, “the norm knows you better than you know yourself, you can’t listen to yourself, the body is wrong, wanting is wrong, pleasure is wrong, you are wrong wrong wrong.” but god, what a beautiful example of how oppression is psychiatrized: it’s not enough for the oppression to have worked, the system must then convince us that the effects of it working are our own fault. it’s not enough to just kill us with us fully aware of the knife, it’s gotta convince us we’re bleeding out for no reason. if you want any moments of pleasure during your miserable godforsaken little life you’d better put your nose back on the goddamn grindstone and repent. everything around you for your entire life has told you to hate yourself for your drug use but if the combined force of that violence works you are mentally ill, and that is the worst crime of all.
according to this post, when is it okay to use drugs, then? well, not planned into your day, and not at work or school, but not when you’re bored or have been thinking about it too much, and not if anyone who’d judge you or you don’t trust knowing you’re high or you just don’t want knowing is around, and not if you don’t want to quit, but also not if you’ve quit already. you have to hate your drug use otherwise that’s proof it’s attacked your ability to care but hating your drug use is proof you should stop. #JustSayNo
