How do diabetic foot ulcers start?
Diabetic foot ulcers typically start as a result of a combination of factors, including poor circulation in the feet, nerve damage (neuropathy), and high blood sugar levels. These factors can lead to the development of sores or wounds on the feet that do not heal properly. Additionally, people with diabetes may be more susceptible to foot injuries due to decreased sensation in their feet, which can make it more challenging to detect cuts, blisters, or other injuries. The most common cause of diabetic foot ulcers is a combination of peripheral neuropathy (nerve damage) and pressure. Peripheral neuropathy causes loss of sensation in the feet, so people may not feel a blister, cut, or other injury. The pressure from walking or standing can then make the wound worse. It is essential for people with diabetes to take good care of their feet and to have regular check-ups with a foot care Or General Physician specialist to minimize the risk of developing foot ulcers. This includes regular foot inspections and keeping the feet clean and dry.
Take adequate care of your foot especially if you have diabetes,to avoid further complications . You can always try to reduce the risk of getting a foot ulcer by :
Managing your diabetes. If you have diabetes you should wear appropriate footwear and never walk barefoot.
Examining your legs as well as the tops and bottoms of your feet and the areas between your toes every day. Look for any blisters, cuts, cracks, scratches, or other sores. Also check for redness, increased warmth, ingrown toenails, corns, and calluses.
Talk to your healthcare provider about ways you can stop smoking.
Manage your blood pressure.
Care for your toenails frequently. Cut your toenails after bathing, when they are soft. Cut toenails straight across and smooth with a nail file. Take care of ingrown toenails.
Exercise.
Maintain a healthy weight.

Some interesting science analyzed
BY BROOKS LEITNER
Imagine lying back in an enclosed chamber where you bask for 90 minutes in a sea of pure oxygen at pressures two to three times that felt at sea level. This is the world of hyperbaric oxygen therapy (HBOT), a technology that’s been around for decades and is now being explored as a possible treatment for Long COVID.
"The silence on the inside is deafening at first,” says John M.,* who has undergone dozens of HBOT treatments for his persistent Long COVID symptoms. Fortunately, there is a television outside the chamber in view, and it is easy to communicate with the provider if needed. While the potential protocol is still being refined, patients may undergo up to 40 HBOT sessions to address some of the most problematic, lingering symptoms of this complex condition.
HBOT is a therapeutic process that has been widely used to treat such conditions as decompression sickness in scuba divers, carbon monoxide poisoning, and diabetic foot ulcers. In HBOT, the body is exposed to 100% oxygen, a significant increase from the 21% oxygen concentration we typically breathe. The therapy takes place in an enclosed chamber where the air pressure is elevated above normal levels. The combination of high-pressure and high-oxygen conditions enhances the amount of oxygen that can reach the body's tissues. The hope is that this therapy can provide the same relief and healing to people with Long COVID that it does for those with other conditions.
According to John M., HBOT was the first treatment that helped with his sleep and reduced his heart palpitations. “At one point after hospitalization, my Long COVID symptoms were so bad that I could barely walk or talk. HBOT was a great tool that really assisted with my recovery,” he said. John added that he hopes the medical community will achieve a better understanding of how HBOT can help relieve suffering for patients with Long COVID and that more research will increase access to this innovative therapy.
Does HBOT improve Long COVID symptoms? One key observation from the work of Inderjit Singh, MBChB, an assistant professor at Yale School of Medicine (YSM) specializing in pulmonary, critical care, and sleep medicine, is that Long COVID patients often experience debilitating fatigue. Based on Dr. Singh’s previous Long COVID research, the exhaustion is thought to be linked to the muscles’ inability to efficiently extract and utilize oxygen.
To picture how HBOT might work, you can think of your muscles as engines sputtering, struggling to get the fuel they need. If oxygen is the gas that fuels the muscles, it’s as if you are trying to complete your daily routine while the gas tank is running on “empty.” By aiming to directly address this oxygen utilization impairment, HBOT may be a potential solution.
A systematic review by researchers at the China Medical University Hospital noted that HBOT could tackle another major factor in the Long COVID puzzle: oxidative stress. This relates to the body's struggle to maintain balance when harmful molecules, known as free radicals, run amok, causing chronic inflammation.
Research co-authored by Sandra K. Wainwright, MD, medical director of the Center for Hyperbaric Medicine and Wound Healing at Greenwich Hospital in Connecticut, suggests that HBOT, with its high-oxygen environment, might dampen this chronic inflammation by improving mitochondrial activity and decreasing production of harmful molecules. Other potential benefits of HBOT in the treatment of Long COVID may include restoration of oxygen to oxygen-starved tissues, reduced production of inflammatory cytokines, and increased mobilization of hematopoietic stem cells—primary cells that transform into red blood cells, white blood cells, and platelets.
HBOT for Long COVID: Current and ongoing research Several small-scale reports have indicated that HBOT is safe for patients with Long COVID.
To address this question, a trial that followed the gold standard of modern medical research—a randomized, placebo-controlled, double-blind design—assigned 73 Long COVID patients to either receive 40 sessions of HBOT or a placebo of only 21% oxygen. The study observed positive changes in attention, sleep quality, pain symptoms, and energy levels among participants receiving HBOT. In a longitudinal follow-up study published in Scientific Reports, the authors at the Tel Aviv University found that clinical improvements persisted even one year after the last HBOT session was concluded. In a second study, the same authors focused on heart function, measured by an echocardiogram, and found a significant reduction in heart strain, known as global longitudinal strain, in patients who received HBOT.
In another study, 10 patients with Long COVID underwent 10 HBOT treatments over 12 consecutive days. Testing showed statistically significant improvement in fatigue and cognitive function. Meanwhile, an ongoing trial at the Karolinska Institute in Sweden has reported interim safety results wherein almost half of the Long COVID patients in the trial reported cough or chest discomfort during treatment. However, it was unclear whether HBOT exacerbated this symptom or if this adverse effect was due to the effort of participation by patients suffering from more severe Long COVID symptoms.
Is HBOT currently available as a treatment for Long COVID? For HBOT to become a mainstream treatment option for Long COVID, several critical priorities must be addressed. First, there is currently no established method for tailoring HBOT dosages to individual patients, so researchers must learn more about the specific features or symptoms that indicate potential benefits from HBOT. At the same time, we need to identify factors that may be associated with any adverse outcomes of HBOT. And finally, it’s important to determine how long these potentially beneficial effects last in a larger cohort. Will just a few HBOT trials be enough to restore patients to their baseline health, or will HBOT become a recurring component of their annual treatment regimen?
For now, HBOT remains an experimental therapy—and as such is not covered by insurance. This is a huge issue for patients because the therapy is expensive. According to Dr. Wainwright, a six-week course of therapy can run around $60,000. That’s a lot to pay for a therapy that’s still being studied. In the current completed studies, different treatment frequencies and intensities have been used, but it’s unclear how the treatment conditions affect the patient’s outcome.
“I have had some patients notice improvements after only 10 or 15 treatments, whereas some others need up to 45 treatments before they notice a difference,” notes Dr. Wainwright. “I think that HBOT is offering some promising results in many patients, but it is probably a strong adjunctive treatment to the other spectrum of things Long COVID patients should be doing, like participating in an exercise, rehab, and nutritional program.”
Dr. Singh notes that “a major challenge for research is the heterogeneity of Long COVID. It is hard to determine which symptoms to treat and enroll patients into trials based on them.”
Perhaps treatments that target multiple issues caused by Long COVID, like HBOT, may help overcome this challenge.
*Not his real name.
Brooks Leitner is an MD/PhD candidate at Yale School of Medicine.
The last word from Lisa Sanders, MD: Hyperbaric oxygen therapy (HBOT) is just one of the many existing treatments that are being looked at to treat Long COVID. We see this with many new diseases—trying to use a treatment that is effective in one set of diseases to treat another. And there is reason for optimism: We know that HBOT can deliver high levels of oxygen to tissues in need of oxygen. That’s why it’s used to treat soft tissue wounds. If reduced oxygen uptake is the cause of the devastating fatigue caused by Long COVID, as is suggested by many studies, then perhaps a better delivery system will help at least some patients.

What is the best way to treat diabetes foot ulcers?
The best way to treat diabetes foot ulcers involves caring for the wound and managing diabetes properly. Here's a simplified explanation with a focus on readability:
Treating Diabetes Foot Ulcers:
Clean and Protect the Wound: Gently wash the sore with mild soap and water. Put on special cream and cover it with a clean bandage to keep it safe from germs.
Relieve Pressure: Try not to put too much weight on the sore foot. You can use special shoes, cushions, or casts to reduce the pressure. This helps the wound heal without more harm.
Control Blood Sugar: Keep your blood sugar levels in check. Follow your doctor's advice about taking medicines, eating well, and staying active to manage diabetes properly.
Take Antibiotics: If the sore gets infected, your doctor may prescribe antibiotics to fight the infection. Make sure to take them exactly as your doctor tells you.
Remove Dead Tissue: Sometimes, a healthcare professional will need to remove dead tissue from the sore to help it heal. This is called "debridement."
Hyperbaric Oxygen Therapy: In special cases, you might need to breathe pure oxygen in a chamber to speed up healing.
Visit Your Doctor: Regularly see your doctor to check how the sore is doing. They'll ensure it's getting better and not worse.
Learn About Foot Care: Get educated on how to take care of your feet. Check them every day, wear proper shoes, and keep them clean.
Change Your Habits: If you smoke, quitting can make a big difference. Smoking harms blood flow, making it harder for the sore to heal.
Remember, early and careful treatment of Diabetes Foot Ulcers is crucial. Always follow your doctor's advice, and if you have any concerns or problems, don't wait – seek help promptly. Properly managing diabetes is key to preventing future ulcers and keeping your feet healthy.

Hi , I am Nesma From Gaza ,I apologize for what I am going to ask. I am with a heavy heart and exhausted. I want you to help me spread our story and the link to our campaign. I have tried a lot, but I am not getting support, and unfortunately the situation has become difficult after my father and brother were injured, and my father’s health condition has become devastated. I want you to help me spread our story.
Please, my father needs treatment. He is slowly dying before our eyes
https://gofund.me/5ab6d135
You are my only hope of survival
My campaign has been previously verified
@\sayruq @\nabulsi @\el-shab-hussein
I do not know whether I will survive or die in this war, but know that your help for me will contribute to saving my family from death.
Nesma, thank you so much for reaching out!
Everyone, Nesma was on the verge of graduating from university while living with her 12 members family in Gaza City when the bombardment and occupation began. They once lived in a spacious house in a nice neighborhood peacefully with their cat and bird, but now are now a family of mostly children sheltering in a tent in Rafah with no water, food, electricity or healthcare. They had to flee their on foot while being hunted by Israeli soldiers while her brother and father were severly injured. Her father suffers from diabetes, high blood pressure, and heart problems (all of which require regular intense treatment) and her brother has been disabled from walking. If that is not enough, her father was arrested in Khan Yunis where he was tortured, starvedm and abused to the point that he can no longer move. She has also lost one of her brothers, and desperately wants to save the lives of her other siblings and parents. They have nothing and live in abject misery, and as /@buttercuparry mentioned in a post, they are threatened not just by famine, a lack of healthcare and medicine, bombings, shellings, israeli soldiers and the ever incrasing heat, but also poisonous SNAKES. Their fundraiser is stalling at $65,273 of $80,000 goal, so please donate, follow her tumblr, click on the links below to learn more and share to everyone you can! They ask only for your most basic humanity in these apocalyptic conditions so they may restart their lives together without more loss in their family.

Greater Good
@whumpril Day 5: Reckless
CW: exhaustion, self-blame, difficulty breathing, mentions of being choked
continuation of yesterday's entry: Aftershock
Medic was unconscious for less than a minute. It felt like no time at all for them, but it was evidently enough time for half the medical tent to have panic attacks and the acting head of medicine to take a personal interest (or more accurately, personal frustration) in their case.
"Oh good, you're back with us." the Medical Director huffed when Medic's eyes fluttered open, flinching as the Director rubbed some sickly sweet gel on their gums less than gently. They were lying on the cot Whumpee and Caretaker had previously been occupying. They were both still nearby. Someone had found a chair for Whumpee to sit in at Medic's feet, and Caretaker was looking worryingly at Medic over Director's shoulder.
"What happened?"
Medic weakly pushed Director's hand away from them and licked away the gel in their mouth. Their voice was raspy suddenly, and lying down was making breathing harder than before. But the ceiling of the tent was still swaying above them so Medic didn't risk trying to sit up.
"Low blood sugar?"
"Among other things." Director growled "You're blood oxygen is even lower, and I'm betting those bruises on your neck have something to do with it. Drink this."
They shoved an apple juice into Medic's hand and left their line of sight. Caretaker glared after Director before helping Medic to sit up so they could drink the juice. Swallowing the liquid was difficult, and Medic had to take tiny sips to tolerate the pain of it.
"I thought I told you to take a break after that incident." Director began to scold Medic as they drank. "What happened?"
"I was going to," Medic rasped "But Whumpee walked in right afterwards and..." They trailed off, not wanting, or feeling the need to continue. Whumpee gave Medic an apologetic smile and reached out to pat their foot comfortingly. They opened their mouth as though they were going to say something but Director plowed on before they got the chance.
"And when was the last time you ate? You're not diabetic or hypoglycemic, so you shouldn't be having a low blood sugar unless you're starving."
"I had one of the protein bars the volunteers passed out."
That seemed to be the last straw for Director.
"That was the last time you ate!" They yelled. "That was over a day ago! Medic! How could you be so reckless and irresponsible! People's lives were in your hands you idiot! You can't help anyone else if you're not taking care of yourself. What if you passed out while treating someone! What if you passed out while doing that surgery on that asshole's wife! He would have actually killed you, and you would have deserved it! -"
It sounded like Director was going to continue but Caretaker abruptly interrupted. Standing up so fast Medic got dizzy again, and placing a hand firmly on Director's chest.
"Stop it!"
In all the years Medic had known Director, they had never heard anyone speak that way to them. Director was the kind of person who commanded respect, even if they didn't entirely deserve it. Medic should have known they'd meet their match with Caretaker though. They were always looking for a fight, one way or another.
"They don't deserve to be talked to like that. We've just had an earthquake. Their friend was missing, and they've been working none stop without sleep for two days. Of courses Medic isn't thinking straight. It took them nearly being killed to get offered a break for God's sake! And no one has tried to provide the doctors food other than a few protein bars? Not to mention relief isn't due for another day. If anything you're being the reckless one. You're people are going to start dropping like flies soon if you keep this treatment up."
Medic's heart broke for Caretaker in that moment. As tough as their love could be at times, they were the most loyal friend Medic had. They were always the first to come to Medic's defense, no matter the situation. After two days of being blamed for everything under the sun (including in at least one circumstance, the sun itself), it was nice that someone was finally trying to stand up for them. But in this instance, it was an ill-founded defense.
"Caretaker," Medic reached out and tugged at Caretaker's wrist before they could continue admonishing Director. "It's not Director's fault. We were given breaks, I just skipped them."
Everyone stared at Medic with wide eyed expressions. Whumpee seemed to be coming down from their adrenaline and looked too tired to be anything more than surprised by Medic's admission. Director looked like they were going to start yelling again if they weren't already leaning away from Caretaker like a scared animal. Probably a good idea since Caretaker looked like they were going to hit which ever one of the two spoke first. Luckily for the both of them, it was Whumpee.
"Why?"
Medic took as deep a breathe as they could manage and trained their eyes on Whumpee because they didn't want to look at the other two.
"I wasn't going to be able to sleep with you unaccounted for, and I wasn't allowed to join the search parties during my breaks. All I could do to distract myself was to keep busy. And people just kept coming in. And the injuries kept getting worse. They needed as many hands as they could get. So I just kept telling myself it was for the greater good."
Medic cut themselves off. Their tired, oxygen deprived brain hadn't comprehended their words until it was too late. Quickly they snapped their head back to Caretaker and Director. Director looked, surprisingly, somewhat sympathetic to Medic's explanation. It was impossible to tell Caretaker's expression however as they had buried their face in their hands. Medic was certain though that they were both thinking about the same thing. The last thing Caretaker had yelled at them before marching out of the tent to go searching for Whumpee two days before.
God damn you and your greater good mentality! If that's how you feel, you better save every God damn person you touch until Whumpee is found!
"Oh Medic" Caretaker mumbled into their hands. It sounded like an apology. A moment later they lowered their hands and turned away Medic, not quickly enough to hide the tears in their eyes.
"I think we can agree Medic can't work in their condition." They told Director in a much softer voice than they'd addressed them in previously. "I'm going to take them both home to rest. When will you need Medic back here?"
As Director and Caretaker began to discuss what Caretaker should be doing to care for their friends, Medic watched Whumpee, who seemed to be getting a little more color in their face, even as their eyes were drooping and they were listing to the side, loosing themselves to exhaustion. Medic could relate. Now that it seemed final that they were not going to be working anymore today, they could also feel their eyes getting heavy, and all the pain they had been ignoring for the past few days was seeping in. They knew Caretaker was probably going to yell at them again when they were feeling better, and there was a strong possibility that Director was going to discipline them once this was all over, but all they could feel now was relief.

“Hi, My name is Hala, I'm 18 years old, I live with my family in Gaza. I have finished my high school in 2023 with a grade of 94.9 in the scientific stream and began studies at the university,
which was destroyed at the beginning of the war,Unfortunately, my dreams and ambitions were completely destroyed.
We have all been displaced more than seven times to seek safety , We escaped death several times،When we were displaced to the south, this did not mean that we fled to safety, but rather the opposite. Everywhere around us there was bombing all around us and fire belts all the time. And also the malnutrition we suffer from. We have been here for 6 months, haven’t eaten something healthy.
All had dreams but they were completely destroyed after October 7th.My father was snacks merchant and his store was completely burned when we fled south to find safety, My uncle Was working in a hotel Which was completely destroyed and has no source of income left now , My grandfather and grandmother are diabetics and unable to walk. When we were displaced, they found it very difficult to move. They need wheelchairs, which is impossible for them to have now, and they need a lot of medicines that are indispensable, but we had to replace them with very low quality and perhaps inappropriate alternative medicines. We cannot afford to pay the money to get out of this devastating war. We have survived death several times and have been displaced several other times from one place to another to search for safety. We now do not have enough to pay the money to leave Gaza.
My grandfather's condition deteriorated during the war. He was very tired, and we discovered that he had water in his lungs, water in his foot, and swelling in his feet. There is no necessary treatment for him, and there are no highly qualified doctors and no hospitals. Now, I only want to save my grandfather's life as quickly as possible.
Why did we think of this fund raising?
This is the only way to survive as we have to pay $5000 for each person to cross borders to safety.
Wish you could help us.
I wish you help me and save us.”
