I can't believe I'm about to do this. I mean don't get too excited, it's not interesting, I'm just forced to talk about it because that's the only power I have in this stupid situation.

A couple weeks ago I was prescribed this new asthma medication, even though my problem is almost definitely from muscular-skeletal pressure but nobody can figure that out yet, so for now I just have this inhaler so I'm not constantly suffocating. The first time I took it at the recommended dosage I had a bad panic attack that took me out for an entire day. I reduced the dosage so I didn't feel dangerously insane anymore, but it still causes my fucking rosacea to go completely out of control, which is not really something I can just ignore; it drives me crazy that insurance companies just treat rosacea like it's some fancy cosmetic issue, as if it doesn't affect your entire life when your skin is visibly deteriorating at an escalating rate, but that's another story... So anyway I have a giant bag of medications that either didn't work at all or actively harmed me (my typical experience with everything) that I haven't disposed of yet, so I dug around in there for a tube of Rhofade that like I don't even know how I got it because it's the premiere celebrity-endorsed thing and it's psychotically expensive, but anyway I decided to give it another shot because I'm desperate. First couple days it worked great. I thought all my problems were solved, except that I'd have to find a way to keep paying for it. Then it seemed not so great for a couple days. Then things started to get pretty rocky. I wondered if it had to do with not being careful enough in the sun or what, but I started to worry about the medication, so I did something that will sound insane, but oh well.

Months ago I complained to my GP that I was having a hard time finding a "real dermatologist" in the city, as opposed to a salesperson for predatory beauty treatments. I've been treated pretty badly by a variety of dermos over 15 years, I often had the feeling that I was neglected because I had a medical concern as opposed to like an expensive anti-aging concern or something. Sometimes a dermo advertises themselves as a medical professional, but when you get there you realize they're a glorified beautician and you're fucked. I went to a well-reviewed practice that's now called The Dermatology Specialists several times, and every time there was a mad rush to get rid of me. The actual head of the clinic saw me a couple times, and both times he came running at me with a needle without telling me what he was doing, to try to lance a mole that I wasn't there to discuss. One of these was right over my eye and you can imagine how scary this was. I eventually realized I didn't have to take that shit and swore him off. Years later when I decided to look for a dermo in my new neighborhood, I made an appointment at a "new" place that I realized too late was the same guy; he had rebranded and expanded his thing into a giant chain that's all over the city, like half or more of the dermos in my searches are attached to his practice and it's not always obvious until the appointment is made. Fuck. I thought, maybe things will be different at this location, I'm seeing a new person and I'm the only one in the waiting room, seems pretty chill. I actually had to get a mole removed that time. I sat down with the new doctor who frantically explained what she was going to do to me before saying "OK BYE!!!" and racing out the door, which she had almost closed all the way behind her before she remembered that she still had to actually do the procedure. I couldn't believe I'd fallen for this clinic's bullshit yet again!

So that's when I talked to my GP and she wrote me a referral for what I took to be a real doctor. His office was clean, simple, and unpretentious. When I got there, an exiting patient was thanking the doctor profusely for something, like from the depths of his soul. We sat down together and he calmly denounced all the lasers and other expensive snake oil that had been upsold to me over the years. He told me to scale back to just a basic Aveeno face wash and moisturizer, and I was so relieved that he wasn't some greedy hipster asshole...however. He also told me that the active ingredient in Afrin, an OTC decongestant nasal spray, has the same effects as the top of the line rosacea medication Rhofade, and many of his patients have success just applying it to their skin. I was so impressed that he wasn't trying to sell me anything, and that he was empowering me to just take care of my own shit at home, that I believed him.

So flashing forward to the past week: I use this new inhaler that I need if I want to breathe, my face blows up like a bomb, I try Rhofade and it seems to make things worse...and then I buy some Afrin and put it on. It pretty much burned my face off my skull. My skin was purple and my face completely changed shape for more than 24 hours. I thought, at least I have this cold pack that's made for faces, for swelling from dental surgery and shit (it was recommended to me by my TMJ specialist who is also fucking with me right now but that's another story). I applied it, and it made everything a thousand times worse than it had ever been. I had to cancel all my plans. I took Tylenol, antihistamines, drank tons of water, whatever I thought would help. A colloidal oatmeal-based moisturizer kind of did something for me, but not remotely enough. It's a couple days later, now, and I'm still not completely over it, and I'm having random intense and painful flareups. I've never had exactly this problem before. And by the way "just using a moisturizer" has not helped anything at all this entire time, even though it's the advice I always get (sometimes VERY rudely) no matter what I say. Dry skin is not my problem, someone could tell just from touching it. Just being mindful of the sun and trigger foods and shit is not the answer. I know there's something else going on and like nobody cares to find out.

So anyway I'm having a followup appointment with my GP to explain all this...and in the meantime I'm going back to the fucking snake oil clinic. I just absolutely need something for right now, I don't know if it will be an antibiotic or what. I've spent years looking for a real medical dermatologist and I know I'm not going to just find one overnight, so I'm subjecting myself to more humiliation at the most convenient place, and I'll deserve whatever I get I guess. At least my appointment is with a guy I haven't seen before. Cross your fingers for me that he doesn't give me something else that just melts my face off of my face.

the jokes are funny and all but let's actually talk about the evils of american drug advertisement because I heard an ad the other day for some dermatology related drug and it was fucking ROUGH
first of all it focused entirely on physical appearances and not the actual conditions. it was like "now i can dare to wear shorts" and "i have the freedom to wear black shirts again!". and then. fucking. i know we all joke about the fucking side effects lists but immediately after that that stupid voice started listing the side effects and literally the first one it said was suicidal thoughts and actions. AND ACTIONS.
this ad is sending such a good message. hey people with skin conditions take a drug that makes you suicidal so people don't have to look at your flakey skin.
this is so fucking evil.

what’s also so fucked up about going to the dermatologist, and i’m sure it’s not like this for most men, is that my dermatological concerns are assumed immediately to be cosmetic and not medical. like i’ve been to two dermatologists in my life, both have been very flippant about moles/marks that i’ve been worried about, and both have instead offered me treatment for my acne and scarring on my back, including injecting an acne scar with a steroid. and when i told one of them that i didn’t want to take an oral medicine, and that i’m skeptical of a lot of skincare treatments, that i’ve used benzoyl peroxide religiously to no effect, and that i wasn’t sure i wanted to use clindamycin every day because of all the side effects, and that i’m not so sure my acne has bacterial causes/is caused by bad hygiene rather than hormonal and dietary causes... he said “well then what do you expect. what do you want me to do about it.” um, examine my moles, like i said i was here for? the abundance of which i know statistically puts me at higher risk of skin cancer? and it’s like, got it, you think my acne is unsightly, and you don’t know how to react to a female patient who really doesn’t care much about that and instead just wants to make sure that her skin issues don’t indicate more serious underlying health issues.
and so even with my health anxiety, the two doctors i’m most avoidant of are the dermatologist and the gynecologist. and i know that i need to persevere and try to find female doctors in my network who will listen to my concerns. but instead all the gynecologist offices around her are now advertising labiaplasty and all kinds of cosmetic procedures and elective fertility treatments that are more profitable for them than making sure women are hormonally healthy, have access to whatever birth control methods they desire, and figuring out and treating the causes of unexplained pain… and dermatologists care more about selling you an elaborate skincare routine than making sure you’re cancer free, especially if you’re a woman. and as a feminist i know how super super important it is to go to the gynecologist and get screened and all that, but my first experience was so uncomfortable and humiliating that i haven’t been able to bring myself to go back.
i think that especially because the fields of dermatology and gynecology are becoming increasingly image-focused, it is absolutely a feminist issue that there should be greater separation of cosmetic and medical care in these offices, because i know i’m not the only woman who avoids them for this reason (and other doctors, dentistry is another example i can think of where image/having straight white teeth is becoming prioritized, although i think that is less sex-specific in its impacts), and it upsets me that i am avoiding important cancer screening because my experiences in these offices have been so hostile, but i really can’t bring myself to make these appointments. truly, i should not have to be inundated with suggestions to mutilate my genitals in the waiting room of a doctor that i am going to to make sure that my pelvic pain isn’t cause for medical concern. i should not be treated like an unsightly ogre whose skin issues must be due to uncleanliness and personal failing, even though scientific studies now point to acne being genetic and hormonal above all else, when i’ve stated that the reason i’m in the office is because of a completely unrelated skin concern. i should be able to say no to being advertised cosmetic treatments when i’m seeking medical help. the assumption shouldn’t be that, because i’m a woman, i’m more concerned about my appearance than my health.

GRWM is one of the most popular fashion trends on TikTok, as this acronym in English means “get ready with me”, which translates to “vístete conmigo” in Spanish.
To this end, GRWM's trend on TikTok generally consists of recording oneself while one is putting on makeup, dressing, or showing a skin care routine. So I'm sure you've ever seen a video with this type of GRWM trend on TikTok.
However, now the GRWM trend on TikTok is being taken to another level, I tell you:
It turns out that a couple of now ex-boyfriends are taking GRWM to another level on TikTok.
Well, it turns out that these young people made a GRWM video on TikTok for nothing more or less than to end their relationship.
Said video on TikTok that was titled “GRWM to leave my two-year relationship,” you can see the girl doing her makeup routine while she exposes her breakup.
Same video in which both talk about why they have decided to end their two-year relationship, revealing that they were both unfaithful in the relationship.
To this they explain how they came to this decision in the relationship, however, despite the controversy, in the end it turns out to be part of a humorous video on Tiktok, as it clarifies that their video “GRWM to leave my two-year relationship " is a lie.
Personally, I think these videos are quite entertaining, and you can watch them while you get ready or do any other activity, or just to distract yourself for a while.

hi! im studying medicine(gonna be an intern in summer this year) and ive been wanting to be an internal med or dermatology resident in the uk for a while now. do you have any ideas/advice? why did you choose anesthesiology? should i just start with plab or get a phd first and then start residency? sorry if those are a lot of questions 😅😅
Hi! apologies for the amount of time it took me to reply to this
As I'm a UK graduate I don't know loads about starting in the UK as an international graduate but I'll try my best to answer your questions
To train in dermatology in the UK you have to have done internal medical training first where you rotate through different medical specialties, then apply to a specific one after 2 or 3 years. I'd recommend looking at these two websites [1, 2] for information on requirements for application etc.
There are sometimes individual non-training posts in specific specialties which can be good for international graduates to get used to working in the NHS, there's lots in internal medicine (that's what I'm currently doing) but there are some specific to dermatology - look on the NHS jobs website, individual hospital trusts advertise these throughout the year
PhDs are not necessary for working in medicine in the UK, most people never do them, and those that do it tends to be after several years of training, but if you've done one then it will make you look good at interview for jobs. It depends on how soon you want to get to the UK, but it's definitely possible for you to get a job here without a PhD
I want to do anaesthetics because I just really clicked with it. The practical side of it suits me, the possibility of training in intensive care as well (and loads of other subspecialties) and still doing the diagnostic part of things, being able to see immediate effects on physiology with drugs, also the fact that it is a very training focussed specialty so as a consultant you're doing lots of direct supervision of trainees and also as a trainee you get immediate and ongoing feedback. There's probably loads of other reasons that I just can't articulate right now but basically I just love it
